Abstract

The long waiting time for medication services at the Outpatient Pharmacy Depot of Mokopido Tolitoli Regional Hospital has become a major issue in improving the quality of hospital services, as it indicates inefficiencies in the human resources (HR) management of pharmacists. This study aims to evaluate HR governance at the pharmacy depot to improve the efficiency of drug services. Identified problems include workload imbalance, patient waiting times that far exceed standards, and increasing public complaints about the lengthy service process. This study used a qualitative approach with in-depth interviews, observation, and document analysis as data collection techniques. Research participants included hospital leaders, medical support officers, pharmacy installation managers, as well as pharmacists, pharmaceutical technicians, and outpatients. The results of the study indicate that HR governance is not yet optimal. Staff needs planning is not based on workload analysis, task division is still rigid, rotation mechanisms are unstructured, competency training is limited, and the performance evaluation system is not integrated with quantitative indicators such as prescription service waiting times. These conditions indicate a gap between managerial policies and operational needs in the field. Based on these findings, improving HR governance is a strategic step to increase service efficiency. The practical implications of this research emphasize the importance of workload-based HR planning, task flexibility and rotation, and the development of measurable training and performance evaluation to support continuous improvement.

Abstract

Emergency Department (ED) services are crucial as they directly impact patient survival. A decline in ED visits at Permata Depok Hospital in 2024 and early 2025 has raised concerns, particularly related to patient complaints shared via social media. This study aims to analyse the influence of healthcare workers’ competence, doctor consultation time, and facility availability on patient complaints in the hospital’s ED. This is a quantitative study with a cross-sectional design, involving 93 respondents selected using the Slovin formula from a population of 1,180 patients. Data were collected through a Likert-scale questionnaire and analysed using chi-square tests and logistic regression. The results show that all three variables significantly affect patient complaints (p-value < 0.05), with facility availability especially basic sanitation like soap, tissue, and clean water emerging as the most dominant factor. Managerial implications include the need to prioritize supporting facilities, implement SOPs and regular sanitation audits, adopt rapid reporting systems using QR codes, and make data-driven decisions through satisfaction surveys and WASH-FIT audits. Future research is recommended to expand variables, apply mixed-methods approaches, conduct inter-hospital comparisons, and monitor intervention outcomes.

Abstract

This study aims to analyze the training model for Medical Laboratory Technology Analysts (ATLM) in improving staff professionalism at the Mawar Clinical Laboratory in Bandung City. The research employed a qualitative case study design with data collected through in-depth interviews, observations, and documentation reviews. Data analysis was conducted using the ADDIE model—Analysis, Design, Development, Implementation, and Evaluation—supported by thematic coding assisted by NVivo software to enhance analytical rigor and validity. The findings indicate that the analysis phase identified the need to strengthen technical competencies, SOP comprehension, time management, and soft skills among ATLM staff. In the design phase, a competency-based training curriculum was developed according to laboratory standards. During the development phase, training modules, media, and evaluation instruments were produced and validated. Implementation was carried out through blended learning, hands-on practice, and direct supervision. The evaluation demonstrated improvements in ATLM professionalism, particularly in accuracy, discipline, SOP compliance, service quality, and work ethics. Overall, the ADDIE based training model proved relevant and effective in enhancing laboratory staff professionalism and is recommended for sustainable competency development within Mawar Clinical Laboratory.

Abstract

This study aims to analyze the implementation of training programs in improving hospitality services at RSUD dr. R. Goeteng Taroenadibrata Purbalingga. The study is motivated by the need to enhance hospital service quality, which should not only focus on medical aspects but also on non-medical aspects such as friendliness, empathy, and effective communication with patients and their families. A qualitative descriptive approach was employed involving 13 informants consisting of hospital employees, health workers, and non-health staff. Data were collected through in-depth interviews, observation, and documentation, and were analyzed using the interactive model of Miles and Huberman, which includes data reduction, data display, and conclusion drawing. The findings indicate that hospitality training has been conducted regularly, covering interpersonal communication, service excellence, and the development of patient-friendly attitudes. The implementation of the training has increased employees’ awareness of patient-oriented services. However, several challenges remain, including limited training duration, lecture-dominated learning methods, unequal training opportunities for all staff including outsourced workers, and limited post-training evaluation to measure behavioral changes. Therefore, this study recommends improving interactive training design, expanding participant coverage, and strengthening continuous evaluation to ensure sustainable improvements in hospital hospitality services.

Abstract

Digital transformation in higher education requires institutions to adopt intelligent technologies that support more accurate and data-driven decision-making. This study investigates the integration of Artificial Intelligence (AI) into Business Intelligence (BI) systems within Indonesian universities using the Technology Acceptance Model (TAM) as the theoretical framework. A mixed-methods approach was employed, involving surveys (n = 75) and interviews with academic leaders and information technology (IT) staff. The results show that AI-enhanced BI systems significantly improve decision-making effectiveness, particularly in academic planning and administrative efficiency. Regression analysis revealed that perceived usefulness and perceived ease of use explained 58% of the variance in decision effectiveness, while all variables combined (including management support and digital literacy) accounted for 69%. These findings validate the TAM in the context of AI-based decision systems in education. This study contributes both theoretically and practically by offering evidence-based recommendations for strengthening data-driven culture and institutional readiness for adopting intelligent information systems.

Abstract

Background: The reliability of laboratory diagnoses is significantly influenced by the performance and readiness of medical equipment. In healthcare facilities with remote geographic locations such as Wedabay Medical Center (WMC) in North Maluku, preventive maintenance of medical equipment is a crucial strategy for maintaining service quality and avoiding service disruptions due to equipment failure. Purpose: To formulate a managerial strategy for preventive maintenance to improve diagnostic reliability. Method: The study used a qualitative approach with a case study design. Data were obtained through in-depth interviews, observations, maintenance document reviews, and focus group discussions (FGDs). Informants were selected using purposive sampling from laboratory staff, medical personnel, and management. Data analysis was conducted descriptively using a reduction, presentation, and verification model, and was supported by a SWOT analysis. Results: Preventive maintenance has been implemented routinely with the support of management commitment and a regular schedule. However, challenges remain, such as limited technical resources, vendor dependence, remote location, and suboptimal documentation. Conclusion: Recommended strategies include strengthening the documentation system, optimizing vendor collaboration, and developing a more structured maintenance management system. These strategies contribute to improving diagnostic reliability and the quality of laboratory services.   Keywords: Diagnostic Reliability; Managerial Strategy; Medical Equipment; Preventive Maintenance.

Abstract

Proses discharge pasien merupakan tahapan kritis dalam pelayanan rumah sakit karena menentukan efisiensi alur pasien, kesiapan perawatan lanjutan, dan pengalaman pasien. Evaluasi di RSU San Medical Center Karawang menunjukkan rata-rata waktu discharge 143 menit, melebihi standar pelayanan minimal rumah sakit yaitu maksimal 120 menit sejak pasien dinyatakan boleh pulang sampai menerima tagihan akhir. Penelitian ini bertujuan merumuskan strategi implementasi Lean Six Sigma untuk optimalisasi proses discharge pasien. Penelitian menggunakan pendekatan kualitatif dengan desain studi kasus di RSU San Medical Center Karawang pada September-November 2025. Informan dipilih secara purposive sebanyak 10 orang, terdiri atas 2 dokter penanggung jawab pelayanan (DPJP), 2 perawat rawat inap, 2 petugas farmasi, 2 petugas administrasi/kasir, 1 manajer pelayanan keperawatan, dan 1 manajer pelayanan medis. Data diperoleh melalui wawancara semi-terstruktur, observasi proses dan telaah dokumen, kemudian dianalisis secara tematik menggunakan kerangka Define, Measure, Analyze, Improve, dan Control (DMAIC) serta pemetaan akar penyebab 5M. Temuan utama yang paling dominan adalah waiting waste akibat keterlambatan resume medis, alur serial, komunikasi manual, bottleneck farmasi, rework administrasi, dan verifikasi penjamin yang panjang. Strategi perbaikan meliputi standardisasi SOP lintas profesi, penjadwalan visit discharge, penyederhanaan resume medis, integrasi SIMRS, penerapan proses paralel, pembentukan koordinator discharge, serta pengendalian melalui indikator kinerja utama (key performance indicator/KPI) dan audit berkala. Lean Six Sigma dapat digunakan sebagai strategi manajerial lintas unit untuk meningkatkan efisiensi, konsistensi, dan mutu proses discharge pasien.

Abstract

Pendahuluan: Perhitungan biaya pelayanan kesehatan yang akurat merupakan faktor penting dalam mendukung efisiensi pengelolaan rumah sakit. Penggunaan metode perhitungan biaya tradisional dalam mengalokasikan biaya overhead sering menimbulkan distorsi biaya karena tidak mempertimbangkan konsumsi aktivitas secara spesifik. Tujuan: Untuk mengevaluasi penerapan Activity-Based Costing (ABC) dalam perhitungan overhead cost Instalasi Gawat Darurat (IGD) serta menganalisis kontribusinya terhadap peningkatan efisiensi biaya rawat jalan. Metode: Penelitian menggunakan pendekatan kualitatif dengan desain studi kasus. Data diperoleh melalui wawancara mendalam, observasi, studi dokumentasi, serta analisis dokumen keuangan rumah sakit. Analisis dilakukan melalui identifikasi aktivitas, pengelompokan aktivitas ke dalam cost pool, penentuan cost driver, pembebanan biaya menggunakan metode ABC, dan triangulasi sumber data. Hasil:Metode Activity-Based Costing mampu mengalokasikan biaya overhead secara lebih proporsional berdasarkan aktivitas yang mengonsumsi sumber daya dibandingkan metode tradisional. Informasi biaya yang dihasilkan menjadi lebih akurat sehingga mendukung pengambilan keputusan manajerial, evaluasi efisiensi penggunaan sumber daya, serta penyusunan tarif pelayanan yang lebih rasional. Implementasi metode ABC masih menghadapi kendala berupa keterbatasan sistem informasi manajemen rumah sakit, kompleksitas aktivitas pelayanan, dan kesiapan sumber daya manusia. Simpulan: Penerapan Activity-Based Costing berpotensi meningkatkan efisiensi biaya rawat jalan melalui penyediaan informasi biaya yang lebih akurat dan dapat dijadikan dasar dalam pengendalian biaya serta perencanaan keuangan rumah sakit.
