Abstract

Purpose: Stunting is a serious public health issue in Indonesia, particularly in the Mentaya Hilir Selatan Subdistrict, Kotawaringin Timur Regency. This study aims to explore the role of Community-Based Organizations (CBO) in efforts to address stunting in the region. Methods: This research uses a qualitative case study approach with data collection techniques including in-depth interviews with 33 informants, observations, and document analysis. The study subjects consist of midwives, community health volunteers, members of the Family, Welfare Movement (TP-PKK), family planning cadres, mothers in the community, and program managers at the subdistrict/village level. Data were analyzed using content and thematic analysis methods. Results: The findings show that CBOs in Mentaya Hilir Selatan play a crucial role in handling stunting through various programs such as Pos Gizi Bunting , Posyandu, Aksi Cegah Stunting (ACS), Dahsyat (Dapur Sehat Atasi Stunting), and the Family Assistance Team (TPK). These programs involve activities such as nutrition education, cooking demonstrations, toddler weighing, and home visits to monitor the condition of pregnant women and young children. Conclusion: Community-based approaches through CBOs have proven effective in addressing stunting issues in Mentaya Hilir Selatan. The success of these programs is supported by good collaboration between various sectors, active community participation, and support from the government and related institutions.

Abstract

Purpose: Delays in improving public health are closely related to the population's social and economic issues. These delays are also linked to various obstacles in implementing programs that have been proven effective in other countries. Some improvements in overcoming these barriers have occurred related to the support for field activities. This research characterizes the five organizational elements used to implement essential health programs in Indonesia. Methods: We used the five organizational elements framework, which emphasizes the roles of service delivery, frontline workers, support systems, and program managers. Findings: The clarity in contracts for program managers is often seen as a clarity in administrative functions rather than a focus on problem-solving functions within the community. This understanding is crucial for devising effective strategies for program implementation. Moreover, there is an urgent need to strengthen the support system functions that assist frontline workers, a critical step in ensuring the smooth execution of health programs. Conclusion: The success of health programs is intricately tied to the effectiveness of program managers. It is not just a role but a crucial responsibility for community health centers (Puskesmas) to adopt an internal worker contract approach for program managers. Furthermore, program managers should operate under a contract as leaders for frontline workers and support systems, underscoring their pivotal role in the process.

Abstract

Purpose: This study aims to assess the prevalence and determinants of malnutrition among children under five in Ethiopia, with evidence from the 2019 Ethiopian demographic and health survey as evidence. Methods: The data were analyzed from the 2019 EMDHS, and 3680 (939 urban and 2,741 rural) children under 5 years old were considered in this analysis. The data was extracted, edited, and analyzed using SPSS Version 26. Binary logistic regression models were used to identify the determinants of children's malnutrition. Results: The prevalence of stunting, wasting, and underweight was 31.7%, 9.4%, and 17%, respectively. About 24.5% of children were both stunted and wasted. Among the factors considered in this study, residence region, breastfeeding, electricity, mothers’ education level, household wealth index, child sex, and water and toilet facilities were significantly associated with under-five malnutrition in Ethiopia. Conclusion: Malnutrition among under-five children is one of Ethiopia's public health problems. Therefore, the influence of these factors should be considered in developing an evidence-based health policy and implementing interventions in the healthcare system that target stunting, wasting, and underweight, which can lead to better outcomes for children under five in Ethiopia.

Abstract

Purpose: This study aims to identify psychosocial hazards by examining the history of physical and non-physical violence, as well as the concerns experienced by residents enrolled in the specialist medical education program at Dr. Sardjito General Hospital. Methods: This research employs a descriptive-analytical design using a cross-sectional approach, in which data are collected at a single point in time. The sampling technique used is proportional stratified random sampling, where the sample is drawn in proportion to the size of each stratum within the population. Results: The majority of respondents were over 30 years old (56.03%), male (58.87%), and held a bachelor's degree (87.94%). The most commonly reported psychosocial hazard among respondents at Dr. Sardjito General Hospital was verbal non-physical violence. A significant relationship was found between demographic characteristics—specifically age—and the history of physical violence among specialist medical education students. Additionally, a relationship was observed between work shift patterns and the history of non-physical violence. Furthermore, non-physical violence was found to be associated with anxiety among the specialist medical education students at Dr. Sardjito General Hospital. Conclusion: Health organizations must establish and prioritize policies against workplace violence through educational training programs and safety protocols.

Abstract

Purpose: This study aims to examine whether primary health care (PHC) services in Indonesian cities are utilized exclusively by the poor or also by the non-poor. If both groups access PHC, it may indicate that the system promotes equity by providing inclusive, non-discriminatory services. Methods: This study analyzed the utilization of primary health care (PHC) among individuals aged 15 years and older in 13 major cities in Indonesia, using data from the Indonesian Family Life Survey 5 (IFLS5), conducted in 2014. The equity effect was assessed by comparing the proportion of PHC users between poor and non-poor population groups. Results: The proportion of the population utilizing primary health care (PHC) increased by 26% in the past four weeks. Among the five major urban areas in Indonesia, Jakarta demonstrated the most equitable utilization of PHC services between poor and non-poor groups. In Jakarta, the poor were more likely to use PHC services, with an odds ratio (OR) of 1.3 (95% confidence interval [CI]: 1.04–1.83). Conclusion: Jakarta demonstrates the most equitable access to health services between poor and non-poor populations among major Indonesian cities. These findings align with primary health care (PHC) reform efforts and support the policy objective of improving access to health facilities for the poor.

Abstract

Purpose: One of the key barriers to exclusive breastfeeding is the increasing number of mothers who work outside the home. This study aimed to analyze the relationship between maternal employment and exclusive breastfeeding in Indonesia using data from the Indonesian Family Life Survey (IFLS) 4 and 5. Methods: This study employed an analytic observational design with a cross-sectional approach. The sample consisted of women aged 15–49 years who were never married and currently breastfeeding. Data from IFLS 4 and 5 were analyzed using Chi-square tests and logistic regression to examine the relationship between maternal employment status and exclusive breastfeeding. Results: The percentage of exclusive breastfeeding increased by approximately 10% between 2007 and 2014 (from 10.4% to 22%). During the same period, the percentage of working mothers also increased by about 20% (from 70.3% to 95.2%). In 2007, the highest proportion of breastfeeding working mothers was among private employees (13.5%), while in 2014, it was among those working in informal agricultural jobs (39.3%). There was no statistically significant relationship between maternal employment status in general and exclusive breastfeeding (p>0.05). However, there was a significant association between knowledge of exclusive breastfeeding and exclusive breastfeeding practices (p<0.05). Conclusion: Exclusive breastfeeding rates in Indonesia are affected by maternal knowledge about exclusive breastfeeding.

Abstract

Purpose: This study aims to analyze cigarette expenditures among poor households and examine whether economic status, chronic diseases, health insurance ownership, and demographic factors influence these expenditures. Methods: This study employs a quantitative research design with a cross-sectional approach, utilizing secondary data from the Indonesia Family Life Survey (IFLS) conducted in 2007 and 2014. The criteria for the respondents are poor families, smokers, and age > 15 years old, with a sample of 1,243 respondents (2007) and 1,549 respondents (2014). Results: In 2007, cigarette expenditure was influenced by low economic status (OR = 3.34), interest in health insurance (OR = 1.10), age, sex, and place of residence, while a history of chronic disease and education level had no effect. In 2014, economic status and health-related factors no longer had a significant impact; however, age (OR = 2.45) and gender (OR = 0.54) remained influential, with no effect observed from education or residence. Conclusion: Economic status and demographic factors are key determinants of high cigarette expenditures among poor families in Indonesia. Monthly spending on cigarettes remains relatively high, and many poor households still lack health insurance. The government is expected to strengthen awareness and policies related to smoking, particularly those concerning health and health insurance for the poor.

Abstract

Background: Midwifery educational institutions have an important role in the scoring power of professional andqualified midwives. To be able to produce a professional midwife as expected, then it takes a model of effective andinnovative learning.Objective: To know the effectiveess of reflective learning on Student Academy Midwifery Yogyakarta.Method: This type of research survey with descriptive study. The research at Academy of Midwifery in Yogyakarta.Sample student research level I, II, and III. Sampling using a minimum sample size. Variable independent variable research, namely is the reflective learning. The dependent variable is the attitude of professional midwiferystudents.Result: The result showed statistical significance (P <0.05) between the independent variable (reflective learning)with the dependent variable (professional attitude) with a prevalence ratio 2.13 95% CI (1.62 to 2.78).Conclusion: Students who did reflective learning finely will have a chance to do a good professional attitude.Keywords: Reflective learning, professional attitude, midwifery student

Abstract

There are several adolescent girls who have experienced anemia in villages around Puskesmas Mlati II, especially at the junior high school level, 21.03% of a total of 233 students in January 2020, which is before the COVID-19 pandemic. The prevalence of anemia is expected to increase after the pandemic. The main factors are the habit of not eating breakfast, poor diet, and lack of adolescent health services. The poverty factor in the family and the irregular distribution of blood supplement tablets are the drivers of the problem of anemia. As for the prevention of the problem of anemia of adolescent girls in the villages around the Puskesmas Mlati II area, the PKPR and nutritionist plan to establish adolescent posyandu in each hamlet. It is hoped that the initiation of the adolescent posyandu will not only carry out posyandu with five tables, but can also include other additional programs such as educational webinars, online peer education, shared breakfast practices, and permaculture nutrition garden practices. The purpose of this activity is to create an adolescent posyandu as one of the efforts of adolescent girls' health services in preventing anemia. The results of this activity are in the form of a description of the urgency of adolescent posyandu for the health of adolescent girls and the initiation of the adolescent posyandu itself, such as situation analysis of the problem of anemia, planning of program implementation, including the formation of adolescent cadres, and preparing a draft evaluation document.

Abstract

This study aimed to determine validity and reliability of the new developed Stunting Early Detection Kit (SEDEK). This study was a cross-sectional study. A total sample of 30 children under two years from a Posyandu in Seyegan, Yogyakarta, was involved in the study. Every child was measured using SEDEK and infantometer-WHO Length for Age Growth Chart as the gold standard to find out its validity. Validity was determined by sensitivity, specificity, and predictive value. Reliability in this study was measured by intra-rater reliability, by comparing the first measurement and the second measurement from a rater using SEDEK. The intra-rater reliability determined using the Intraclass Correlation Coefficient (ICC). The results showed no significant difference between length measurements using SEDEK and the gold standards (p>0.05). The SEDEK sensitivity was 80%, specificity 85%, positive predictive value 72.7%, negative predictive value 89.5%. SEDEK reliability is demonstrated by ICC of 0.781. However, this study suggest that the current SEDEK version has not met the required sensitivity and positive predictivevalue of more than 80% so that it can be used as a detection tool. Further research is needed to improve the quality of SEDEK so that the SEDEK improved version can be used at the community-based health facilities level.
