Abstract

Background: Malignant pleural effusion (MPE) has a variable survival rate and prognosis. The LENT score is one method for assessing survival rates in patients with MPE. This study aimed to investigate the LENT score as a prognostic factor for overall survival (OS) and progression-free survival (PFS) of patients with MPE at a tertiary hospital in West Sumatera. Methods: This study was an observational analytic study involving several tertiary hospitals in West Sumatera with a minimum observation period of 2 years. Data were collected from medical records. We used Kaplan Meier analysis to assess OS and PFS. Results: A total of 198 subjects met the inclusion criteria. Most MPE patients in this study were aged ≥60 years, male, smokers, pleural fluid lactate dehydrogenase value <1500, ECOG 1, serum NLR value <9, and high-risk cancer, namely lung cancer. The distribution of LENT scores for MPE patients was evenly distributed among the low, medium, and high-risk groups. Kaplan Meier analysis showed that the median OS based on LENT scores were 804 days, 275 days, and 161 days, respectively (log-rank test p = 0.000). The median PFS based on LENT scores were 715 days, 202 days, and 106 days, respectively (log-rank test p=0.000). The OS and PFS findings are longer than previous studies. Conclusion: Based on LENT scores, overall survival and progression-free survival MPE patients at tertiary hospitals in West Sumatera have a better prognosis compared to previous studies.

Abstract

Background: SARS-CoV-2 can infect anyone, but people with HIV have underlying conditions or comorbidities that can make them seriously ill if infected with SARS-CoV-2. HIV attacks and destroys the immune system delays the response of specific antibodies, and even causes failure to thrive, resulting in a long course of the disease. This case report aimed to describe 2 cases of HIV patients co-infected with SARS-CoV-2 with delayed viral clearance. Case presentation: There are two HIV patients with co-infection with SARS-CoV-2. The first patient, a 32-year-old man with COVID-19 and HIV-AIDS, was referred from a regional hospital after being treated for 10 days due to clinical deterioration. Physical examination showed that the patient's general condition was moderately ill, and other vital signs were within normal limits. Oral candidiasis was seen in the patient's mouth, crackles were found in both lung fields, and epigastric tenderness was found on abdominal examination. The patient tested positive for COVID-19 based on the results of an antigen swab examination from the previous hospital. The second patient, a 26-year-old man, came with complaints of intermittent fever 1 week before entering the hospital. Complaints began with the body feeling weak and coughing without phlegm 2 weeks ago. The patient tested positive for COVID-19 based on the results of an antigen swab examination. The delayed viral clearance of SARS-CoV-2 in the two patients was possibly caused by the impaired immune response due to HIV infection, as shown by the presence of lymphopenia and decreased CD4+. Conclusion: ARV use can suppress HIV viral load and increase immunity so that can help viral clearance of SARS-CoV-2.

Abstract

Background: Inflammatory process in COVID-19 can increase inflammatory markers such as C-reactive protein (CRP), procalcitonin (PCT), and interleukin 6 (IL-6). The level of C-reactive protein describes the severity of viral infection. Several studies had been conducted to investigate the link between C-reactive protein level and the severity of COVID-19. The purpose of this study is to identify differences in C-reactive protein levels based on clinical degrees and outcomes of COVID-19 patients treated at RSUP by Dr. M. Jamil Padang.Methods: This is a retrospective cohort study that analyzed all COVID-19 patients treated at RSUP Dr. M. Djamil Padang. This study lasted from December 1st, 2021 and June 1st, 2022. The data was analyzed using univariate, bivariate, and confounding analysis. Bivariate analysis explored differences in C-reactive protein levels in clinical severity and patient outcomes for COVID-19. The Kruskal-Wallis test determined the difference between the CRP level and clinical severity, while the Mann-Whitney test determined the difference between the CRP level and length of stay and final hospitalization status. Confounding test was performed using multiple linear regression tests.Results: Majority of participants were women (51.0%) with range of age between 50–59 years (28.0%) and suffered from hypertension (46.0%). Less than half of them had secondary infection (49.0%). Majority of them had a critical clinical severity (75.0%) and length of stay ≤14 days (77.0%) and more than half were deceased (65.0%). C-reactive protein levels were higher in patients with critical clinical degrees (89.00 mg/L) compared to moderate (37.50 mg/L) and severe (23.00 mg/L), C-reactive protein levels in patients with long hospitalization ≤14 days (97.00 mg/L) was higher than >14 days (88.50 mg/L), and C-reactive protein levels were higher in patients who died (93.00 mg/L) than those who survived (68.00 mg/L).Conclusion: C-reactive protein levels differed significantly based on clinical severity, length of stay and end of stay status of COVID-19 patients.

Abstract

Liver injury can be caused by coronavirus disease 2019 (COVID-19), characterized by increased levels of transaminase enzymes (aspartate aminotransferase/AST and alanine aminotransferase/ALT). Objective: To determined the correlation between transaminase enzyme levels and the severity of COVID-19 in Dr. M. Djamil Central Public Hospital, Padang. Methods: This was an observational analytic study with a cross-sectional design. Data were obtained randomly from medical records of confirmed COVID-19 patients hospitalized in Dr. M. Djamil Hospital from June to August 2021, with moderate, severe, and critical severity based on Indonesian Ministry of Health criteria. Data were analyzed using the Chi-square test to examine the distribution and correlation between variables and a simple logistic regression test to assess the level of risk between variables. Results: 57,80% had abnormal AST, and 44,95% had abnormal ALT. The median AST was highest in the critical group, while the median ALT was highest in the severe group. The distribution of sex and age group were significantly different based on AST and ALT levels, and the distribution of patients with coronary heart disease was significantly different based on ALT levels. Transaminase enzyme levels had a significant correlation with the severity of COVID-19 (p-value = 0,00). Patients with critical degrees had the highest risk for abnormal AST and severe degrees had the highest risk for abnormal ALT. Conclusion: Transaminase enzymes have a significant correlation with the severity of COVID-19.Keywords:  COVID-19, liver injury, transaminases, disease severity

Abstract

Asma merupakan penyakit inflamasi kronis pada saluran pernapasan yang masih menjadi salah satu masalah kesehatan serius secara global, terutama ketika terjadi eksaserbasi akut yang mengancam jiwa. Penelitian ini bertujuan untuk mengkaji penatalaksanaan asma persisten sedang dalam kondisi eksaserbasi akut berat yang dialami oleh seorang pasien dewasa. Pendekatan yang digunakan adalah studi kasus dengan pasien yang mengalami penurunan kesadaran akibat eksaserbasi asma dan dirawat di ICU Rumah Sakit Umum Pusat Dr. M. Djamil Padang. Metode penatalaksanaan mencakup pemberian oksigen dosis tinggi melalui High Flow Nasal Canula (HFNC), bronkodilator, kortikosteroid, dan antibiotik untuk mengatasi infeksi penyerta. Hasil penelitian menunjukkan bahwa tindakan ini berhasil meningkatkan saturasi oksigen, mengurangi gejala sesak napas, dan memperbaiki fungsi paru secara signifikan dalam 48 jam pertama perawatan. Pasien menunjukkan pemulihan yang baik tanpa komplikasi serius. Kesimpulan penelitian ini menekankan pentingnya penatalaksanaan cepat dan tepat pada eksaserbasi asma berat guna mencegah kondisi yang lebih fatal dan meningkatkan kualitas hidup pasien.

Abstract

Leprosy is a chronic infectious disease caused by Mycobacterium leprae that attacks several parts of the body, especially the upper respiratory tract, skin, and peripheral nerves. Indonesia is the third country with the most leprosy in the world. This study aims to determine the profile of leprosy patients at the Dermatology and Venereology Polyclinic Dr. M. Djamil Padang Hospital in 2018-2021. This research is descriptive retrospective with total sampling technique using secondary data in the form of medical records. The sample of this study were all leprosy patients in 2018-2021 who were fulfilled inclusion criteria, with a total of 39 patients. The results showed that the most age group was >14 years (100%), the most gender was male (74.36%), the most residence was Padang (33.33%), and almost all leprosy patients (92.31%) had no history of household contact with leprosy patients. Most of the leprosy patients were of the multibacillary type (74.36%) with positive smear results (46.15%), without leprosy reaction (61.54%), and the most disability level that occurred was grade 0 disability (48.72%). The conclusion of this study is that most cases of leprosy at the Dermatology and Venereology Polyclinic RSUP Dr. M. Djamil Padang in 2018-2021 aged >14 years, male, having address in Padang City, had no history household contact, multibacillary type, positive smear result, without reaction, and level 0 disability.

Abstract

Background: Tuberculosis (TB) remains a major global health concern, particularly in developing countries. Stunting, a manifestation of chronic malnutrition, is prevalent in these regions and is increasingly recognized as a significant risk factor for TB. This systematic review aims to comprehensively analyze the impact of stunting on TB susceptibility and treatment outcomes. Methods: A systematic search of PubMed and ScienceDirect databases was conducted for studies published in the last 10 years, focusing on the relationship between stunting and TB. The PRISMA guidelines were followed for article selection and data extraction. Results: The review identified a significant association between stunting and increased TB risk. Stunted individuals exhibit impaired immune responses, making them more susceptible to TB infection. Moreover, stunting negatively affects TB treatment outcomes, including increased treatment duration, higher relapse rates, and greater mortality risk. Conclusion: Stunting is a critical determinant of TB susceptibility and treatment outcomes. Addressing stunting through comprehensive nutritional interventions is crucial not only for reducing the burden of malnutrition but also for enhancing TB prevention and control efforts.

Abstract

Background: Tuberculosis (TB) and obesity are significant global health concerns with potentially complex interactions. Obesity, through its effects on metabolism, inflammation, and the immune system, may influence TB susceptibility, progression, and treatment outcomes. This systematic review aims to analyze the published literature on the relationship between obesity and TB, focusing on the underlying mechanisms. Methods: A systematic search of PubMed, Science Direct, and Google Scholar was conducted for articles published in the last 10 years. The search strategy included keywords such as "tuberculosis," "TB," "obesity," and "BMI." Articles were selected using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) method. Results: The review identified 11 studies that met the inclusion criteria. The studies revealed a complex relationship between obesity and TB, with nutritional status, immunity, and diabetes mellitus (DM) playing key roles. Obesity can alter the immune response to TB, potentially increasing the risk of disease and affecting treatment efficacy. Conclusion: The relationship between obesity and TB is multifaceted, with obesity potentially influencing both disease susceptibility and outcomes. Further research is needed to fully elucidate the underlying mechanisms and to develop targeted interventions for individuals with both obesity and TB.

Abstract

Background: Tuberculosis (TB) remains a major global health problem, with anemia being a frequent comorbidity. Anemia in TB is multifaceted, with iron deficiency and anemia of inflammation (AI) being the most common types. This systematic review aims to synthesize the evidence on iron deficiency and AI in TB, their prevalence, impact on outcomes, and management strategies. Methods: A systematic search of PubMed and ScienceDirect databases was conducted for articles published in the last 10 years. Observational studies examining the prevalence, types, and impact of anemia on TB outcomes were included. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed. Results: The review included 7 studies involving 1,133 participants. Anemia prevalence ranged from 61% to 89% in TB patients. AI was the predominant type, with iron deficiency also prevalent. Anemia was associated with increased mortality, delayed sputum culture conversion, and impaired TB treatment response. Conclusion: Anemia, primarily AI and iron deficiency, is highly prevalent in TB and negatively impacts treatment outcomes and survival. Effective management of anemia is crucial for improving TB outcomes. Further research is needed to optimize diagnostic and treatment strategies for iron deficiency and AI in TB.

Abstract

Background: Lung cancer frequently coexists with chronic obstructive pulmonary disease (COPD), particularly among smokers. The presence of both conditions can complicate diagnosis and lead to poorer outcomes. This case report presents a patient with lung cancer and concurrent COPD, highlighting the diagnostic and management challenges. Case presentation: A 60-year-old male presented with a history of smoking, progressive dyspnea, and a recent diagnosis of right lung cancer (T4N3M1c, stage IV B). He also exhibited symptoms suggestive of COPD, such as chronic cough and expectoration. Spirometry confirmed moderate restriction and severe obstruction, consistent with COPD GOLD 3. The patient was managed with both lung cancer treatment and COPD therapy. Conclusion: This case underscores the importance of considering COPD in patients with lung cancer, especially those with a history of smoking. Early diagnosis of both conditions is crucial for optimizing treatment strategies and improving patient outcomes.
