Abstract

Tumor mediastinum merupakan salah satu kelainan di rongga mediastinum dengan berbagai spektrum klinis dan jenis histopatologi. Di Indonesia masih sedikit penelitian dan literatur mengenai tumor mediastinum. Khususnya di Padang, masih kekurangan karakteristik epidemiologi, klinis, dan karakteristik histopatologi dari tumor mediastinum. Penelitian ini bertujuan untuk mengetahui karakteristik dari tumor mediastinum di RSUP Dr. M. DJamil Padang. Penelitian ini merupakan penelitian deskriptif observasional dengan desain cross sectional. Data penelitian didapatkan dari rekam medis yang memenuhi kriteria sampel periode 2015 – 2019. Kriteria inklusi dari penelitian ini adalah seluruh rekam medis pasien dengan diagnosis klinis tumor mediastinum, dan kriteria eksklusinya adalah tidak lengkap umur dan jenis kelamin pasien, status merokok, lokasi tumor, gambaran klinis, serta jenis histopatologi tumor. Hasil penelitian dikelompokkan dalam tabel distribusi frekuensi. Hasil penelitian didapatkan 40 sampel yang memenuhi kriteria inklusi. Pasien tumor mediastinum kebanyakan berusia 51-60 tahun (27,5%), berjenis kelamin laki-laki (55%) dan hampir separuh pasien memiliki riwayat merokok (47,5%). Lokasi massa terbanyak pada kompartemen anterior. Pasien umumnya datang dengan keluhan sesak napas (82,5%), diikuti oleh batuk, nyeri dada dan penurunan berat badan. Jenis tumor mediastinum yang paling sering terjadi adalah tumor timus (42,5%). Kematian saat perawatan di rumah sakit mencapai 7,5%. Kesimpulan pada penelitian ini adalah tumor mediastinum lebih banyak terjadi pada pasien di atas umur 51 tahun, berjenis kelamin laki-laki, dengan tumor timus sebagai jenis histopatologi yang paling umum. Abstract

Background. Aplastic anemia is a failure of the process of formation and development of blood cells that causes a decrease or absence of hematopoietic precursor cells in the bone marrow. Although the prevalence of this disease is rare, aplastic anemia has the potential to cause death with risk factors that are easily found in everyday life and are rarely realized.Objective. To determine the profile of patients with aplastic anemia at General Hospital Dr M Djamil Padang.Methods. This is a descriptive retrospective study. The population of this study are all patients diagnosed with aplastic anemia by clinicians in the Medical Record Installation of Dr. M. Djamil Hospital, Padang, between 1 January 2018 - 31 December 2019..Results The results showed that 21.57% patients with aplastic anemia were at the age of 56-65 years and the incidence was higher in women than men. The highest risk factor was exposure to chemical and insecticides (18.62%), but 54.9% of the data did not include risk factors. 31.37% work as students. 67.65% suffered from mild aplastic anemia, 60.78% were on immunosuppressive therapy, and 65.69% had partial remission outcomes.Conclusion. The majority of patients with aplastic anemia based on age are 56-65 years and based on gender, the incidence is higher in women. The most common risk factors were exposure to chemical and insecticides. . The most common level of severity is non-severe aplastic anemia. The most common type of therapy is immunosuppressive therapy. The most common outcome is partial remission.

Abstract

Latar Belakang: Defisiensi vitamin D telah menjadi masalah kesehatan dunia dan berkembang pesat selama beberapa tahun terakhir ini. Defisiensi vitamin D merupakan salah satu faktor risiko penyakit kardiovaskular. Indeks aterogenik plasma yang dirumuskan sebagai log10(TG/HDL) merupakan penanda aterogenesitas untuk memperkirakan risiko penyakit kardiovaskular. Defisiensi vitamin D dan nilai indeks aterogenik plasma yang tinggi sering ditemukan pada penyandang obes. Objektif: Penelitian ini bertujuan untuk mengetahui korelasi antara kadar vitamin D dengan indeks aterogenik plasma pada mahasiswa penyandang obes. Metode: Penelitian ini merupakan penelitian analitik observasional dengan desain cross sectional yang dilakukan di Fakultas Kedokteran Universitas Andalas dan Laboratorium Sentral RSUP Dr. M. Djamil Padang dari bulan Agustus 2020 hingga April 2021. Subjek penelitian ini adalah 30 mahasiswa angkatan 2017-2018 penyandang obes di Fakultas Kedokteran Universitas Andalas, yang terdiri dari 16 orang laki-laki dan 14 orang perempuan. Hasil: Hasil penelitian didapatkan rerata umur subjek 19,57±0,86 tahun dan rerata IMT 32,53±4,13 kg/m2. Rerata kadar vitamin D subjek 13,90±4,10 ng/mL dan rerata indeks aterogenik plasma subjek -0,025±0,237. Analisis bivariat didapatkan korelasi negatif sangat lemah dan tidak bermakna antara kadar vitamin D dengan indeks aterogenik plasma (r= -0,129; p= 0,498). Kesimpulan: Tidak terdapat korelasi yang bermakna antara kadar vitamin D dengan indeks aterogenik plasma pada mahasiswa penyandang obes.

Abstract

Inflammation is caused by infectious diseases, including COVID-19. Severe inflammatory responses contribute to weak adaptive immune response, there by resulting in immune response imbalance. Circulating biomarkers can represent inflammation and immune status are potential predictors for the prognosis of COVID-19 patients. Ferritin and interleukin-6 can serve as significant biomarkers in the detection of cytokine storm, systemic inflammation, and the prognosis of COVID-19. Neutrophil-lymphocyte ratio is an independent prognostic biomarker for COVID-19 patients. The aim of this study to determine differences levels of IL-6, ferritin, and NLR in COVID-19 patients treated ICU and non-ICU. This study was comparative cross-sectional design of 62 COVID-19 patients. Research was conducted at the Central Laboratory and Medical Record of Dr. M. Djamil Padang hospital (May-September 2021). Interleukin-6 levels determined the ECLIA methods, ferritin with the ELFA methods, and NLR with calculation methods. Bivariate data were analyzed with Mann-Whitney test. The characteristics of COVID-19 patients treated in ICU: 64.5% male, average age was 53.52(10.71) years, 48.4% death, and non ICU: female 71.0%, average 42.32 (12.22) years, and recovered 100%. The most common comorbid is hypertension. There were significant differences levels of IL-6, ferritin, and NLR in ICU and non-ICU patients with COVID-19 (p<0.001).

Abstract

Sepsis is a clinical condition of potentially life-threatening organ dysfunction caused by the host's response to infection. Delayed diagnosis and treatment of sepsis lead to worsening of the disease, which can lead to circulatory collapse, multiple organ failure, and death, therefore accurate and timely diagnosis can prevent death. Procalcitonin (PCT) is the most widely used, studied, and promising marker for the diagnosis of sepsis. Recent studies have shown that procalcitonin plays an important role in the clinical diagnosis of sepsis because it can differentiate sepsis from SIRS. Most studies found an increase in the Mean Platelet Volume (MPV) in septic patients, both neonates, and adults. The MPV value can be used as a parameter to assess the prognosis of septic patients. This cross-sectional analytical study was conducted on 21 septic patients at Dr. M. Djamil Hospital, Padang from January 2020 to April 2021. Mean platelet volume values were determined using an automatic hematology analyzer based on the formula made by the device. Procalcitonin levels were measured using the enzym-linked immunofluorescent assay method. Data were analyzed by Pearson correlation test and p < 0.05 was significant. The mean age of the research subjects was 54 years and the range was 22-77 years. The number of male and female research subjects in this study was almost the same, consisting of 10 male (48%) and 11 female (52%) patients. The average MPV was 10.59(0.56) fL and the median procalcitonin was 9.35(2.08-200) ng/mL. Pearson correlation test showed that MPV was positively correlated with procalcitonin with moderate correlation (r=0.435, p=0.049). There was a moderate positive correlation between MPV and procalcitonin in septic patients.

Abstract

Latar Belakang: Coronavirus Disease 2019 (COVID-19) adalah penyakit pada saluran pernafasan yang diakibatkan oleh Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2). Virus ini dapat menginfeksi kelompok rentan, termasuk ibu hamil. Penelitian ini bertujuan untuk mengetahui gambaran profil kasus pada ibu hamil yang dirawat dengan COVID-19. Objektif: Penelitian ini bertujuan untuk mengetahui gambaran profil kasus pada ibu hamil yang dirawat dengan COVID-19 di RSUP Dr. M. Djamil Padang Periode Januari—Desember 2020. Metode: Penelitian ini menggunakan metode deskriptif dengan desain cross-sectional di RSUP Dr. M. Djamil Padang dengan jumlah sampel sebanyak 136 ibu hamil dengan COVID-19 pada tahun 2020. Data yang digunakan berupa data sekunder dari rekam medis dengan menggunakan teknik total sampling. Hasil: Ibu hamil yang terpapar COVID-19 memiliki gejala ringan (95,6%), berada pada usia tidak berisiko (83,8%),berada pada kehamilan trimester III (87,5%), dan terpapar tanpa komorbid (81,6%). Didapatkan juga peningkatan kadar NLR dan D-Dimer, banyak yang mendapat tindakan berupa sectio caesarea (64,7%), dan adanya kematian 3 ibu (2,2%). Kesimpulan: Sebagian besar ibu hamil dengan COVID-19 memiliki gejala ringan, berada pada usia tidak berisiko (usia 20—35 tahun), hamil pada trimester III, terpapar tanpa komorbid, memiliki peningkatan NLR dan D-Dimer, dilakukan tindakan sectio caesarea, dan tidak banyak terjadi kasus kematian.

Abstract

Breast cancer is the most common malignancy in females globally and one of the main causes of cancer death. There are several markers of cancer-related inflammation to help predict the relationship between pathologic characteristics of breast cancer. The research objective was to analyze the correlation between PLR and NLR with associated tumor size in breast cancer. This was a retrospective study with a cross-sectional design. This study used secondary data from the medical records of breast cancer patients who met the inclusion criteria at Siti Khodijah Hospital, Sidoarjo from January 2021 to March 2023. The sample size was 54 subjects, patients with breast cancer confirmed by pathology anatomy. The exclusion criteria were patients with infection, autoimmune disease, and hematology disorder. All the subjects were female. The mean age at the time of breast cancer diagnosis was 50.18±10.23 years, range of 27-80 years old. Most of the patients were over 50 years as much as 28 (51.85 %), while there were 26 (48.14%) > 50 years. The PLR range was  15.45-600.0, the NLR range was  0.58-9.98, tumor size range was 0.5-10.0 cm. Correlation between PLR and NLR with tumor size in breast cancer (p=0.351 and p=0.339). Correlation analysis showed that PLR and NLR had no significant correlation with tumor size ( r= -0.129, p=0.351 and r= -0.133, p=0.339). In conclusion, there is no significant correlation between PLR and NLR with histopathology of tumor size in patients with breast cancer.

Abstract

Tujuan : Penelitian ini bertujuan untuk melihat hubungan hidronefrosis pra operasi dengan penurunan hemoglobin pasca PCNL pada penderita batu ginjal; Metode: Penelitian ini merupakan penelitian analitik dengan pendekatan prospektif. Jumlah dari sampel dalam penelitian ini sebanyak 21 sampel di RSUP Dr. M. Djamil Padang. Analisis data menggunakan uji Chi square dimana hubungan antar variabel dikatakan signifikan jika p value < 0,05;  Hasil:  Penelitian menunjukkan bahwa  21 pasien yang dioperasi PCNL, jumlah jenis kelamin terbanyak yakni pada perempuan dan berada pada rentang usia 46-64 tahun. Penurunan hemoglobin terbanyak  berada pada kelompok <2g/dl. Kebanyakan pasien batu ginjal disertai dengan adanya hidronefrosis. Analisis statistik menunjukkan bahwa tidak ada hubungan antara hidronefrosis pra operasi dan penurunan hemoglobin setelah PCNL (p value = 0,553); Simpulan: Tidak terdapat hubungan yang bermakna antara hidronefrosis pra operasi dengan penurunan hemoglobin pasca PCNL pada penderita batu ginjal di RSUP Dr.M.Djamil Padang.

Abstract

Background: Chronic hepatitis B (CHB) is a persistent hepatitis B virus (HBV) infection and can cause progressive liver damage. This damage can be measured through a decrease in albumin levels and an increase in liver enzymes such as aspartate aminotransferase (AST/SGOT), alanine aminotransferase (ALT/SGPT), and de Ritis ratio (AST/ALT). This study aims to analyze the relationship between albumin levels and SGOT, SGPT, and de Ritis ratio in CHB patients. Methods: This cross-sectional retrospective analytical study involved 50 CHB patients diagnosed at Dr. M. Djamil General Hospital Padang between June 2022 to June 2023. Data on albumin levels, SGOT, SGPT, and de Ritis ratio were obtained from medical records. The de Ritis ratio, SGOT, and SGPT were grouped into normal and increased. Statistical analysis used an independent t-test with a significance of p < 0.05. Results: The research subjects consisted of 26 men (52%) and 24 women (48%) with an average age of 42.46 ± 13.39 years. The mean albumin level in the increased SGOT group (3.69 ± 0.78 g/dL) was significantly lower than the normal SGOT group (4.39 ± 0.61 g/dL) (p = 0.003). The mean albumin level in the increased de Ritis ratio group (3.90 ± 0.80 g/dL) was also significantly lower than the normal de Ritis ratio group (4.46 ± 0.70 g/dL) (p = 0.006). There was no significant difference between albumin levels in the normal and increased SGPT groups (p = 0.548). Conclusion: There is a significant negative relationship between albumin levels and SGOT and de Ritis ratio in CHB patients. Decreased albumin levels may be an indicator of more severe liver damage in these patients.
