Abstract

Economic burden analysis of measles outbreak in the Special Region of Yogyakarta 2015Purpose This study aimed to analyze the economic burden due to outbreaks of measles in the household sector in Yogyakarta.Methods This research was a descriptive study with survey design, with data collected of direct and indirect costs for patients exposed to measles, by conducting interviews of 177 respondents using a questionnaire. Secondary data was compared in the form of outbreaks of measles in 2014 and 2015, to research in February-March, 2016.Results The number of outbreaks of measles patients who visited health facilities reached 163 patients (92.09%). Total medical costs were IDR 17.982.000, non-medical costs IDR 7.804.900, so the total direct cost of IDR was 25.786.900 (average IDR 145. 700). Total indirect costs were IDR 11.720.000 (average IDR 156. 300). The economic burden of measles outbreak was IDR 37.506.900 (average IDR 211. 900). Generally, the cost that is paid out of pocket amounted to 6.32% of household income, not yet considered catastrophic. Regression analysis showed the duration of illness averaged more than 10 days (p = 0.002 with some status of hospitalization (p = 0.000).Conclusions The economic burden of outbreaks of measles in the province in 2015 was IDR 37.506.900, including direct and indirect costs. Medical costs are the dominant component of direct costs. The factors that were related to the economic burden are the duration of illness of more than 10 days and inpatient status. There needs to be an advocacy to the community about the use of health care and prevention of measles by way of measles immunization.

Abstract

Predictors of tuberculosis recurrence in the district of Bantul PurposeThe purpose of this research was to identify associated factors of TB recurrence. MethodsThe study design was a retrospective cohort. We  used TB cases registered in Bantul regency data in 2003-2014 that consisted of 3418 patients. Sampling was conducted by non-random methods with purposive sampling based on inclusion criteria of 904 samples. The variables measured were: age, sex, drug swallowing supervisor, health service facility, socioeconomic and sputum conversion. We used the Cox proportional hazard model for data analysis to determine the rate of occurrence and timing of the occurrence of an event. ResultsMale sex (HR = 0.91; p-value = 0.17) age> 40 years (HR = 1.03; p-value = 0.59), low income (HR = 0.89; p-value = 0.11) were not predictors of TB recurrence, while hospital treatment (HR = 0.81; p-value = 0.00), no sputum conversion (HR = 0.58; p-value = 0.00), treatment failure (HR = 0.49; 95% CI: 0.37-0.64), and treatment of less than 6 months (HR = 0.008; 95% CI: 0.00-0.07) were predictors of TB recurrence. There were 56 (6.18%) of 904 patients who experienced relapse after cured or failed treatment. The average patient relapsed after 17 months of completion of the first treatment. ConclusionHealth facilities, sputum conversion, end-of-treatment status and duration of treatment are pulmonary TB recurrence factors, and as a result can be used to predict TB recurrence in the district of Bantul.

Abstract

Quality of life among people living with HIV/AIDS based on criteria diagnosis and other factors in Surakarta PurposeThis study aimed to determine the quality of life among people living with HIV/AIDS based on the criteria for diagnosis and other factors.MethodsThis study was conducted in the VCT clinic hospital of Dr. Moewardi. The population was HIV-positive patients with antiretroviral therapy. Data collection conducted through medical records and interview to patients. ResultsOut of a total of 89 respondents, 66.29% were males and 71.91% were aged between 26-45 years. We found significant correlations for diagnosis of HIV/AIDS, opportunistic infections, time since HIV diagnosis, duration of ARV therapy, social support, modes of transport, sex, age, and marital status with the quality of life. Multivariate analysis obtained by each variable showed the strongest association with the quality of life was time since diagnosis, social support and duration of ARV therapy. ConclusionThe quality of life was better for those who have been diagnosed with HIV/AIDS ≥ 32 months, with social support, and who have been undergoing antiretroviral therapy ≥ 29 months. Improved counseling in the early days of ARV therapy is necessary to always maintain the treatment and provide support for their social life.

Abstract

Risk factors for loss to follow up of antiretroviral therapy in HIV patientsPurposeThis study aimed to determine risk factors for loss to follow-up of antiretroviral therapy among HIV-infected patients in Dr. Sardjito Yogyakarta, 2011-2014.MethodsA retrospective cohort study was conducted involving 499 HIV patients. Observations were conducted for four years using medical records. Data analysis was performed using Kaplan-Meier and Cox proportional hazards regression tests.ResultsThere were 190 loss to follow-up patients. Risk factors for loss to follow-up of ARV therapy were: a student (AHR = 2.42; 95% CI = 1.20-4.89), the distance ≥ 10 km (AHR = 1.58; 95% CI = 1:09 to 2:31), using health insurance (AHR = 1.67; 95% CI = 1:11 to 2:51) and homosexual as a protective factor of loss to follow-up of antiretroviral therapy (HR = 0:49; 95% CI = 0.30-0.80).ConclusionBeing a college student, the distance between home and ARV service ≥10 km and using health insurance were the risk factors for loss to follow-up of ARV treatment. Adherence counseling for students, cooperation with the drug taking supervisor and decentralization ARV service, as well as effective and efficient services for patients who use health insurance need to be strengthened.

Abstract

Latar Belakang: Complementary and Alternative Medicine merupakan salah satu strategi penatalaksanaan non-farmakologi. Salah satu konsepnya dikenal dengan istilah self-healing. Deep breathing dan afirmasi positif merupakan  metode self-healing yang paling sederhana, lebih mudah diaplikasikan secara mandiri, praktis untuk dilakukan setiap saat, tidak memerlukan instruktur dan tempat khusus yang kondusif sehingga relevan diterapkan pada pasien dengan status sosial ekonomi menengah ke bawah. Oleh karena itu, penelitian ini dilakukan untuk membandingkan pengaruh kedua metode tersebut terhadap penurunan tekanan darah pada pasien hipertensi esensial di Kabupaten Kulon Progo.Metode: Penelitian ini merupakan penelitian eksperimental analitik dengan rancangan eksperimen terkontrol acak. Melibatkan 96 pasien hipertensi esensial yang dibagi secara acak ke dalam dua kelompok intervensi yaitu deep breathing dan afirmasi positif. Kelompok DB diminta untuk melakukan pernafasan perut selama kurang lebih 10 menit, sedangkan kelompok AP diminta untuk mengucapkan salah satu kalimat afirmasi positif yang telah ditentukan secara berulang-ulang dan ritmis di dalam hati selama kurang lebih 10 menit. Pengukuran tekanan darah dilakukan sebelum, segera setelah, dan 15 menit setelah intervensi. Perbedaan rerata tekanan darah diuji menggunakan paired t-test dan unpaired t-test sedangkan kekuatan efek size diuji dengan logistic regression.Hasil: Deep breathing dan afirmasi positif dapat menurunkan tekanan darah sistolik (11,9±10,9 dan 13,6±10,5) dan diastolik (8,3±8,4 dan 6,0±8,6) pada pasien hipertensi esensial (p value 0,000; std mean diff>0,7). Rerata tekanan darah pada pengukuran post1 dan post2 tidak berbeda (CI 95%:0,06-4,91). Penurunan tekanan darah pada kedua kelompok tidak berbeda (p value>0,1). Kesimpulan: Efektivitas deep breathing dan afirmasi positif dalam menurunkan tekanan darah adalah sama. Penurunan tekanan darah kedua metode tersebut stabil hingga 15 menit setelah intervensi. Oleh karena itu, perlu diagendakan kegiatan relaksasi rutin untuk pasien hipertensi minimal pada kelompok pasien yang sudah terorganisir seperti Kelompok Hipertensi Prolanis di Puskesmas.

Abstract

Physical activity and screen based activity with adolescent sleep patterns in Wates PurposeThe purpose of this study was to explore the relationship between physical activity and screen based activity with sleep patterns in adolescents.MethodsA cross-sectional study was conducted among junior high school students in August 2016. Samples were 536 respondents selected using stratified random sampling. Data were collected through interviews with a structured questionnaire, and measurements of height and waist circumference. Data analyses used chi-square, mann-whitney and logistic regression tests to explore factors associated with sleep pattern.ResultsOut off 536 respondents, poor sleep quality (34.5%) and short sleep duration (45.3%) happened among female adolescents. There was a significant correlation between screen based activity with sleep quality in adolescents. Physical activity had a significant relationship with sleep duration.ConclusionPhysical activity and SBA was associated with adolescent sleep patterns. Appropriate educational promotions by the Kulon Progo education authorities should be conducted to provide information about the importance of adequate sleep.

Abstract

Implementation of the smoke-free neighborhood program in YogyakartaPurposeThis study aimed to identify the successfulness of smoke-free neighborhood and to understand the relationship between the householders' knowledge and household smoking status. MethodsAn observational analytic study was conducted using cross-sectional surveys. Chi-square, Fisher exact and logistic regression tests were used to analyze the correlation between variables. ResultsFrom those numbers, qualified households which fulfill the program criteria were 76.1% and 75.9%, respectively. To be more specific, in smoke-free neighborhood, about 56.8% householders were under smoking status and 43.2% householders were identified as ex- or non-smoker. Bivariable analysis showed that there was a significant correlation between educational background, age, knowledge, and perception of householder with smoking status. The last model of multivariable analysis showed that knowledge, and age of householders was related to household smoking status. ConclusionThis study shows that smoke-free neighborhood program has not yet affected passive smoker protection significantly. A more specific controlling program to improve the knowledge especially for elderly (> 45 years old) has to be increased in smoke-free neighborhood of Yogyakarta.

Abstract

Two year survival of drug resistant tuberculosis patients in Moewardi hospital in Surakarta in 2010-2014PurposeThe purpose of this study was to determine the two year survival rate and predictor factors of mortality in drug resistant tuberculosis patients during treatment at the Moewardi Hospital in Surakarta.MethodsThis research was a retrospective cohort study of 250 drug resistant tuberculosis patients receiving treatment in the Moewardi Hospital in January 2011-September 2014. Data were analyzed using survival analysis to find factors affecting the 2 year survival. Our variables were demographic factors, disease characteristics and treatment history. We used Cox regression test with 5% significance level.Results2-year survival rates of drug resistant patients was 74.82%. age, the type of patient, HIV status, side effects of medications and culture conversion were significant to survival rate in bivariate analysis. Cox regression test showed that aged ≥ 40 years (HR 3.221; 95% CI 1.037 to 10.001) and have HIV-positive status (HR 18.086; 95% CI 1.958 to 167.073) were related with reduction of two year survival rate in drug resistant tuberculosis patient. ConclusionAge above 40 years old and HIV positive status for drug-resistant tuberculosis patients may accelerate their death. The screening of HIV in drug resistant tuberculosis patients is needed in order to increase two year survival rate of patients during treatment.

Abstract

ABSTRAKLatar belakang: Hipertensi yang dibiarkan dapat menyebabkan komplikasi terhadap jantung, otak, mata dan ginjal. Prevalensi hipertensi menurut jenis pekerjaan pada pegawai 20,6%, wiraswasta 24,7%, petani, nelayan dan buruh: 25%. Aktivitas fisik pekerja berdasarkan data Riskesdas 2013 yang masih kurang yaitu pegawai 23,3%; wiraswasta 18,3%; dan petani, nelayan, dan buruh 13,4%. Variabel luar yang mempengaruhi hipertensi yaitu faktor risiko yang dapat diubah dan tidak dapat diubah. Tujuan: untuk mengetahui pengaruh aktivitas fisik dan variabel luar terhadap hipertensi pada pekerja.Metode: Penelitian kuantitatif non eksperimen menggunakan data Riskesdas 2013 dengan rancangan cross sectional. Populasi sebanyak 1.027.763 dengan sampel yang memenuhi kriteria inklusi yaitu responden mempunyai pekerjaan utama, berusia ≥ 15 tahun, diukur tinggi badan, berat badan, lingkar perut, tekanan darah, dan tidak sedang hamil, sebanyak 371.713. Data dianalisis secara univariabel, bivariabel dan multivariabel.Hasil: Aktivitas fisik mempengaruhi kejadian hipertensi pada pekerja dengan OR sebesar 1,25 (CI 95%: 1,21-1,28), aktivitas fisik dengan melibatkan variabel luar menjadi 1,16 (CI 95%: 1,13-1,19). Variabel luar yang menjadi faktor risiko hipertensi meliputi umur, obesitas, obesitas abdominal, kebiasaan merokok setiap hari, jumlah rokok 10-20 batang/hari, kebiasaan konsumsi makanan berlemak, konsumsi sayur < 3 porsi/hari, stres, riwayat DM, gagal ginjal kronis dan batu ginjal.Kesimpulan: Aktivitas fisik berpengaruh terhadap kejadian hipertensi pada pekerja.

Abstract

Quality of life among patients with multi-drug resistant tuberculosis in the district of BanyumasPurposeThis study was conducted to investigate the quality of life among patients with multi-drug resistant tuberculosis and to examine factors associated with them. MethodsData from an in-depth interview, involved 22 patients with multi-drug resistant tuberculosis, were used in this study. ResultsThis study showed 54.5% males, 50.0% adults, 59.1% married, 22.7% divorced, 72.7% lived in rural, 95.5% had basic education, 45.5% as a private employment (before sick), 77.3% as unemployment (after sick), 72.7% low income, 54.5% in continuation phase, 95.5%  re-treatment (45% relapse and 40.9% failure cases), 90.9% had drugs side effects (mild/severe); 36.4% had comorbidity; 45.5% smokers, 31.8% falcoholics; 54.5% lack of social support; 31.8% poor and 36.4% moderate quality of life. ConclusionPatients with multi-drug resistant tuberculosis had a worse quality of life and lack social support. Health worker needs to improve their quality of life and social support through establishing a peer support groups to give counseling, information education, communication, motivation, and skills training for them.
