Abstract

Risk factors of death due to dengue hemorrhagic fever in a tertiary public teaching hospital of YogyakartaPurposeWe examined risk factors of dengue hemorrhagic fever death in Dr. Sardjito Hospital.MethodWe conducted a case control study from patient medical records and interviews with parents.ResultsWe found 29 deaths and 58 patients who survived. The probability of death among obese children was 6 times higher than non obese children and the probability of death in children with prolonged shock was 12 times higher than children without prolonged shock. Other variables were family occupation, family income, residential zones, transportation, treatment financing, accuracy of diagnosis in previous health facilities, and fluid resuscitation before being referred had no significant relationship with dengue mortality.ConclusionObesity and prolonged shock were risk factors of dengue hemorrhagic fever death in children. Improved education to parents about high risk of shock syndrome among patients is needed especially for obese children. Further studies related to social determinants in dengue hemorrhagic fever death are also necessary.

Abstract

What do MPH student theses tell us? Experience from Universitas Gadjah MadaPuposeThis study aimed to evaluate MPH professional performance through thesis writing. MethodWe review 103 MPH student theses in health policy and management tract from data library of Faculty of Medicine Universitas Gadjah Mada since 2013 to 2015. We extract topic and reference from each thesis with a questionnaire.ResultsSeventy percent of students choose subject relevant to health policy and management tract. Sixty percent of theses are located in quadrant 3 because they have relevant topic but weak on use of appropriate reference. Only several journals in health policy and management are cited in theses such as health policy and planning, health policy, BMC health services research and human resources for health. More references related to other multidisciplines and published over 5 years. ConclusionMPH students are in need of clear understanding of the domain and scope of work they are interested in. Their theses showed limited evidence about references focus that has practical relevance to their future work or to the job they currently have.

Abstract

Tujuan: Tanggal 17 Mei 2017 Dinas Kesehatan Kabupaten Sleman menerima laporan dari Rumah Sakit Islam Yogyakarta adanya pasien yang mengalami gejala mual, muntah, diare, lemas, nyeri perut dari PT. X Indonesia sebanyak 36 orang. Penyelidikan dilakukan untuk mengkonfirmasi KLB, menentukan faktor risiko dan upaya pengendalian. Metode: Penyelidikan ini menggunakan cohort retrospektif. Kasus adalah karyawan PT. X Indonesia yang mengkonsumsi makan siang tanggal 16 Mei 2017 pukul 12:00 WIB dan mengalami dua atau lebih gejala; diare atau mual atau keringat dingin atu pusing disertai gejala lainnya mulai tanggal 16-18 Mei 2017 setelah mengkonsumsi makan siang. Penemuan kasus secara active case finding dan pengumpulan data melalui wawancara di PT X Indonesia dan observasi. Sampel makanan dan fases dilakukan pemeriksaan mikrobiologi. Hasil: Penyelidikan menemukan 218 kasus keracunan makanan. Kasus terbanyak pada laki-laki (73,36%) dengan dominasi umur <20 tahun (80%). Gejala yang dirasakan diare (80,73%), mual (59,17%),  dan keringat dingin(50,45%). Keracunan makan terjadi 16-18 Mei 2017 dan puncaknya 17 Mei 2017 pukul 04.01-08.00 WIB. Penularan keracunanan makanan terjadi secara common soure. Analisis menunjukkan bahwa makanan penyebab KLB yaitu ayam bumbu bali (RR=4,02; p= 0,000; CI= 1,0-3,1). Hasil observasi diketahui suplayer ayam merupakan suplayer baru. Dan hasil pemeriksaan laboratorium ayam bumbu bali positif Stapyloccocus aureus. Simpulan: Terjadi KLB keracunaan makanan di PT X Indonesia 16-18 Mei 2017 disebabkan oleh ayam bumbu bali yang terkontaminasi oleh bakteri Stapyloccocus aureus. Proses memasak, kebersihan dan penyimpanan makanan pada suhu ruangan menjadi penyetus keracuanan makanan. Rekomdasi yang dapat dilakukan dengan monitoring berkala dalam proses pembuatan makanan.

Abstract

Tujuan: Demam Berdarah Dengue (DBD) adalah penyakit yang disebabkan oleh virus dengue, ditularkan melalui gigitan nyamuk Aedes sp. DBD di Indonesia merupakan salah satu penyakit endemis dengan angka kesakitan yang cenderung meningkat dari tahun ke tahun. Daerah terjangkit semakin meluas mencapai 400 kabupaten/kota dari 474 kabupaten/kota di Indonesia dan sering menimbulkan KLB. Sistem surveilans DBD dilaksanakan sebagai salah satu upaya pengendalian penyakit DBD di Indonesia, salah satu hal penting dalam sistem surveilans adalah pelaporan kasus DBD. Tujuan evaluasi sistem pelaporan kasus DBD adalah untuk mengetahui hal yang berkaitan dengan sistem pelaporan kasus DBD di Kota Magelang tahun 2016. Metode: Studi evaluasi menggunakan metode deskriptif analitik, pengumpulan data primer dilakukan pada lima puskesmas dan satu RS melalui wawancara menggunakan kuesioner dan observasional pada informan sebanyak tujuh orang. Variabel yang di evaluasi berupa ketepatan dan kelengkapan laporan kasus. Hasil: Laporan DBD dikirimkan RS ke Dinas Kesehatan Kota Magelang, puskesmas mendapatkan informasi kasus DBD dari Dinas Kesehatan dan selanjutnya dilakukan penyelidikan epidemiologi. Kelengkapan laporan DBD dari RS ke Dinas Kesehatan sebesar 100% dengan ketepatan sebesar 51,28%. Keterlambatan pelaporan kasus DBD oleh RS disebabkan keterlambatan informasi diagnosis DBD dari ruang perawatan ke petugas surveilans RS, selain itu dikarenakan keterbatasan petugas surveilans RS. Laporan kasus DBD tidak ditembuskan ke puskesmas disebabkan ketidaktahuan petugas surveilans tentang alur pelaporan kasus DBD. Pelaporan kasus DBD oleh puskesmas tidak dapat dihitung baik kelengkapan maupun ketepatan, karena puskesmas hanya menerima informasi laporan kasus DBD dari RS melalui Dinas Kesehatan. Simpulan: Laporan kasus DBD di wilayah Kota Magelang hanya bersumber dari RS dan dikirimkan ke Dinas Kesehatan,  kelengkapan laporan baik sedangkan ketepatan belum optimal, petugas surveilans RS tidak mengetahui alur pelaporan kasus DBD. Studi ini merekomendasikan untuk meningkatkan pengetahuan petugas tentang sistem pelaporan kasus DBD, termasuk ketepatan dan kelengkapan serta alur pelaporan dan kasus DBD.

Abstract

Tujuan: Surveilans berbasis individu merupakan strategi dalam pelaksanaan Sistem Surveilans campak di Indonesia. Oleh karena itu, kerjasana antara Dinas Kesehatan, Puskesmas, Rumah Sakit (RS), maupun pelayanan kesehatan swasta sangat penting dalam pelaksanaannya. Berdasarkan keterangan Dinas Kesehatan Kota Magelang Peran Rumah sakit di Kota Magelang belum berjalan seperti yang diharapkan sehingga perlu dilakukan evaluasi. Evaluasi ini bertujuan untuk mengetahui gambaran pelaksanaan sistem surveilans campak, menganalisis kelemahan yang dimiliki serta hal-hal yang melatarbelakangi kelemahan tersebut, dan memberikan rekomendasi dalam rangka perbaikan sitem surveilans campak di Kota Magelang. Konten: Observasi dan wawancara dilakukan pada petugas surveilans Dinas Kesehatan dan petugas pelaporan (contact person) RS di Kota Magelang. Hasil menunjukan bahwa terdapat satu rumah sakit yang belum ada contact person dari enam rumah sakit yang ada di Kota Magelang. Hambatan yang di hadapi antara lain kurangnya pengetahuan tentang pentingnya surveilans campak dan rincian tugas seorang contact person, serta surveilans aktif yang belum berjalan semestinya. Permasalahan tersebut berdampak pada ketidakaktifan maupun keterlambatan pelaporan RS, sehingga upaya pencegahan dan pengendalian menjadi tidak optimal. Belum adanya pelatihan surveilans campak bagi petugas surveilans, tidak adanya pedoman teknis surveilans di RS, serta tidak adanya forum komunikasi antar petugas menjadi penyebab masalah yang dihadapi. Berdasarkan hal tersebut, pelatihan serta pengadaan pedoman sistem surveilans campak di RS perlu dilakukan agar setiap petugas memahami tugasnya. Advokasi untuk membangun jejaring surveilans di RS yang belum terjangkau perlu dilakukan sehingga setiap kasus dapat terdeteksi. Selain itu, penguatan surveilans aktif serta forum komunikasi penting untuk diadakan agar kerjasama dapat terjalin sehingga berdampak pada jalannya sistem surveilans campak yang optimal

Abstract

Evaluation of leptospirosis control management based on one health in Boyolali DistrictPurpose: The study aims to evaluate the control of leptospirosis in Boyolali Regency. Method: This study was a descriptive observational. Samples were conducted with purposive sampling techniques for Boyolali District Health Office, Boyolali District Animal Husbandry Office, Boyolali District Agricultural Service, and 9 health centers with findings of leptospirosis cases in 2017-2018. Variables that evaluated included inputs, planning, implementation, output. Data were collected through Observational and interviews. Results: Seventy percent of health workers have received training. Rapid diagnosis tests (RDTs) are not available at the primary health care but available in the Health Office. Data collection is reported passively. Examination of leptospirosis cases in livestock if there are findings in humans. However, the main reservoir (rat) was not inspected by the Agriculture Office. Every case that found was reported to the Boyolali District Health Office and received treatment.Conclusion: Leptospirosis control in Boyolali Regency has not successful. It was depicted by cases of death a year after the adoption of the one health concept. Improving both quality of human resources on a regular basis and increasing collaboration between various sectors such as community leaders, religious leaders, the Livestock Service Office, the Agriculture Service, and other relevant agencies were needed.

Abstract

Purpose: Microscopic examination is the main diagnostic method in countries with a high TB burden. Microscopic examination has an important role in TB case detection. The purpose of this study was to examine the quality of TB microscopic sputum examination at the level of the Public Health Center of Kulon Progo District, Indonesia.  Method: This research was a cross-sectional study with 21 Health Center subjects. A Structured questionnaire is used to record HR and logistical availability. Blinded rechecking was done for nine public health centers; data were analyzed to evaluate smear quality indicators and examine the error.  Results: The majority of laboratory workers were women (88.1%), aged 36-45 years (42.86%). Most laboratory staff had 11-20 years of work of length (42.86%). Logistical availability was good, but there were shortages of stock boxes, filter paper, and lens paper. A total of 762 slides from 9 health centers were rechecked. The positivity was 1.57%, error was 1.57% (FP= 41.7% FN= 58.3% sensitivity= 75% specificity= 98.9%). Major errors found 33.37% and 66.73% minor errors from 12 errors. Quality issues found were thickness (38%), salivary specimens (27%), and size (26%).  Conclusion: The performance of TB microscopic examination at The Health Center level in Kulon Progo was quite good, but it needs efforts to get better quality sputum to improve case detection

Patients with chronic condition have a higher risk of death and disability than others without chronic condition. Meanwhile, low to middle income countries facing challenges in reducing the burden of Non-Communicable Diseases (NCDs) and controlling infectious diseases such as Human Immunodeficiency Virus, Tuberculosis and Malaria. Patients with Diabetes are more often facing infection than people without diabetes. This research aimed to evaluate the length of hospitalization and polypharmacy in patients with multimorbidity of Diabetes with TB, HIV, and Malaria. Design of this research was a retrospective cohort study. All patients diagnosed with Diabetes since 2008 were grouped according to their illnesses to examine the impact of the diseases from the clinical perspective. Data were collected and extracted from the electronic medical records of PT. Freeport Indonesia since 2008. Determination of level of multimorbidities was performed by using principal component analysis, then the clinical impact of those multimorbidities analyzed using ANOVA tests. Multimorbidities have a significant effect on length of stay (p<0.05) and polypharmacy (p<0.05). The highest need for hospitalization and medication was in the Diabetes-Malaria-Tuberculosis group compared to the other multimorbidity groups. Multimorbidities have a significant impact on length of stay and polypharmacy.

Abstract

Background: challenges to work in underdeveloped areas andpeople's expectations of the quality of healthcare requiresdoctors who have public health program managementcompetencies. Objectives: identify perception of the needs and responsibilityof medical education institution to meet the managementcompetencies. Method: a mixed method study mixed method with sequentialexploratory. Results: Communication skills and respecting the right ofothers are the most needed competencies and the less neededcompetency is become chairman/leader of an activity.Competency to manage the organization's finances have notbeen met by a medical education institution and competencywhich has been met by medical education institutions is anupdating of medical knowledge actively. Conclusion: doctors need management competencies whileon duty in underdeveloped areas, but medical educationinstitutions are not yet fully meet its responsibilities in thefulfilment of these competencies.
