Abstract

Indonesia gets good sun exposure every year. Sunlight is a major source of vitamin D., but studies show that tropical countries still have a high prevalence of vitamin D deficiency. Geriatrics and obesity are some of the factors that can lead to vitamin D deficiency. Lack of information and knowledge about the effects of vitamin D deficiency, concerns about skin cancer from sunburn, and some current trends that want white skin cause them to limit sun exposure. Based on the high prevalence of geriatric and obesity data in Indonesia and the decline of functional status in geriatrics, which is a person's ability to perform daily physical activity, thus limiting himself to outdoor physical activities to expose the sun. Researchers conducted research to see the knowledge of sun exposure. This study was conducted to analyze vitamin D deficiency risk factors by the cross-sectional method to 88 obesity geriatrics and 88 non-obesity geriatrics based on knowledge of sun exposure. Test the risk factor aspects of knowledge using the odds ratio. The result of the odds ratio test of the knowledge aspect is 0.725, so it can be concluded that good knowledge of sun exposure has the same risk in both groups of respondents (obesity and non-obesity).

Abstract

Drug services in Public Healthcare are an important component whose availability influences the success of health efforts. District / City Health Office is a regional government work unit in the health sector, one of the tasks and authorities of which is to provide and manage public medicines for the district/city area. Effective pharmaceutical management in health service facilities is very important for patient welfare so risks must be identified and controlled. One of the efforts made by the City Health Office is to procure drugs using other methods that are legal according to government procurement of goods/services besides e-purchasing, which is called non-e-purchasing of drugs. Risks in the pharmaceutical supply chain are related to product discontinuity, product shortages, poor performance, patient safety, expense errors, and technological errors, all of which can result in system disruption. This study aims to determine the suitability of the plan with the realization of e- purchasing of drugs, identify problems that hamper the implementation of e-purchasing of drugs, determine the impact of obstacles on procurement, cost efficiency of drug procurement and determine the potential savings of drug procurement by e-purchasing for the period 2015 to 2019 by using the FMEA method, the e-purchasing data was collected at the Wonokromo Public Health Center in Surabaya. Study Failure Mode Effect (FMEA) in the procurement of drugs and UHC health supplies in Surabaya Wonokromo Health Center for the period of 2015-2019, it can be concluded that the absorption of capitation funds is more considered using the availability variable compared to the uptake of funds based on the rupiah value because the availability of more guarantees the need for treatment in the sustainable health centre. In this case, the perception of procurement actors in the analysis of UHC procurement process obstacles there are 41 obstacles, the role of E-purchasing is a very helpful system even though the implementation has not been perfect.

Abstract

The primary source of Vitamin D is sunlight, which plays a role in improving respiratory health. The level of knowledge and attitudes regarding sun exposure can affect daily lifestyle, thus affecting lung function. The research design was cross-sectional and, from April to June 2018, was conducted in Rungkut District, Surabaya. The construction workers were divided into with and without respiratory problems. The variables were knowledge and attitudes towards sun exposure related to vitamin D deficiency risk, with data collection using a questionnaire. The result showed that the level of knowledge (98.31%) and attitude (100%) was low. There was no significant difference in the level of knowledge p-value (0.161 > 0.05) between with and without respiratory problems. The correlation test of knowledge and attitudes towards sun exposure related to the risk of vitamin D deficiency cannot be carried out because there was no excellent attitude. Knowledge and attitudes towards sun exposure related to the risk of vitamin D deficiency tend to be low in construction workers. However, knowledge and attitude were not the main factors affecting lung function.

Abstract

Treatment of asthma exacerbation is necessary to get attention to get effective and efficient treatment. The first line of treatment for asthma exacerbation is nebulation salbutamol, but some asthma treatments require additional combinations of ipratropium. know the cost-effectiveness with the use of a combination of salbutamol and ipratropium bromide with salbutamol alone to improve asthma symptoms and length of treatment in patients with asthma attacks. The study design was retrospective from medical record data of asthma patients in a hospital in Surabaya in the period from January-October 2017. Dependent variables were cost and clinical outcome (duration of treatment and loss of breathless symptoms). Data analysis used was ACER calculation and different test. The subjects were 64 people, consisting of 23 people in group A (salbutamol-ipratropium) and 41 people in group B (salbutamol). Group A was more cost-effective in length of treatment and trade-off in reducing asthma symptoms than in group B. There was no significant difference between costs (p(0.816)> 0.05), length of treatment (p=0.386)> 0.05)) and improvement of asthma symptoms (p (0.676)> 0.05). Therefore, further research was needed on the level o f other asthma attacks and other cost/outcome components.

Abstract

The Department of Health estimates that asthma including 10 major causes of morbidity and mortality in the hospital. Asthma is a chronic respiratory disease that is becoming a serious health problem in many countries around the world. Drugs used in the treatment of asthma exacerbations is salbutamol and aminophylline. Drugs such as salbutamol and aminophylline can cause ADR (Adverse Drug Reaction) in the form of tremor with a sign involuntary shaking part of the hand. This research was conducted at the RSAL Dr. Oepomo to sample Aminofilin and in hospitals Seowandi to sample Salbutamol. The method used was quasi experimental. This study was conducted to compare the incidence of tremor in both asthma drug that is often used for the treatment of asthma such as Salbutamol and Aminofilin. The results of this study indicate there are differences in the incidence rate of ADR-related tremor between salbutamol and Aminofilin. Knowing the difference in incidence rate of tremor between aminophylline and salbutamol can assist in the selection of treatment which safer to avoid the effects of tremors that can occur from the use of of drugs asthma exacerbations.
