Abstract

Introduction: Gastrointestinal endoscopy in children requires more than the technical expertise needed to pass an endoscope through a smaller digestive tract lumen. Children require more careful attention to patient preparation and sedation compared with adults. The incidence of various disease varies through childhood and differs from the adult. Aim of this study is to elucidate profile esophagoduodenoscopic dan colonoscopic procedures in diagnosing gastrointestinal tract disorder in children in Soetomo Hospital, including role of nursing team role these procedures.Method: This study is a descriptive study which is done in Gastroenterlology Division Soetomo Hospital in October 2009–March 2012 for patients undergoing esophagoduodenoskopy and colonoscopy. Age, sex, indication, preparation, sedation, results, complication data were collected. Data from pathological examination were also collected. Data analysis were done descriptively such as mean, median and percentage, and presented in tables.Result: There were 114 patients undergo esophagoduodenoscopy procedure and 7 patients undergo colonoscopy procedures. The most indication for esophagoduodenoscopy was recurrent abdominal pain (71.1%) and for colonoscopy was chronic diarrhea (42.8%). Nill by mouth for 6 hours is preparation for esophagoduodenoscopy and for colonoscopy needs bowel preparation a day before. Bowel preparation is using Biphosphate Sodium Phosphate oral and enema. Most of all sedation was done by anasthesiologist. Nursing team role were start from preparation, relaxing patient, and monitoring during and after procedure. Chronic gastroduodenitis was found on 110 (96.4%) cases, 15 cases of it accompanied nodularity. Seventy cases (64.9%) from 114 cases of biopsy result were Helicobacter pylore positive, and 21 cases of it has Barret's esophagus. Colonoscopy procedure results 5 (71.4%) cases were colitis, and 3 of them are amoebic colitis. Discussion: Diagnosis of sructural and histological abnormalities could be establish with endoscopic procedure. This is an important role in diagnosing gastrointestinal disorder in children and nursing team have important role in it.

Abstract

Introduction: Ileostomy is needed as an important part of the management of several congenital disorders of the intestine. One of the indication of ileostomy in children is short bowel syndrome, which is caused by intestinal atresia such as ileal atresia and intussusception. Ileostomy is a surgery procedure which consists of creating holes on abdominal walls to release stool. The objective of this study was to determine the description of pediatric patients who underwent ileostomy in Dr. Soetomo General Hospital Surabaya. Methods: This was a retrospective descriptive study on medical records in the Central Medical Record Department of Pediatrics and General Surgery Dr. Soetomo General Hospital Surabaya from January 2017 to December 2018. Results: There were 26 pediatric patients who underwent ileostomy in Dr. Soetomo General Hospital Surabaya. Samples from male patients consisted of 57.7%, while female patients consisted of 42.3% with the most common age group was neonates with 57.7%, infants 30.8%, and toddlers 11.5%. There were 30.7% patients with 2.0-2.9 kg body weight, 27% with 3.0-3.9 kg body weight, and 42.3% with more than 4 kg body weight. The outcome showed that repair was needed, or the patient was discharged with periodic follow-up after ileostomy. Conclusion: Ileostomy was performed mostly in neonates with average weight within normal body mass index (BMI) and the highest incidence of pediatric patient with ileum stoma was in pediatric patients.

Abstract

AbstrakStatus gizi adalah kondisi fisiologis tubuh terkait konsumsi makanan dan penggunaan zat gizi oleh tubuh. Hingga sekarang, masalah gizi masih umum terjadi terutama di negara berkembang. Status gizi pada siswa sekolah dasar penting karena dapat mempengaruhi kognitif dan capaian pembelajaran siswa. Salah satu akar masalah gizi adalah kemiskinan yang terkait dengan sosiodemografi yang meliputi status sosial dan ekonomi. Tujuan penelitian ini adalah untuk menganalisis hubungan karakteristik sosiodemografi dan status gizi siswa sekolah dasar di Desa Wokam dan Desa Karangguli, Kabupaten Kepualaun Aru, Maluku. Penelitian ini menggunakan rancangan cross-sectional dan jenis penelitian ini adalah analitik observasional. Penilaian status gizi menggunakan grafik berat badan terhadap tinggi badan dengan kriteria Waterlow. Data mengenai sosiodemografi dikumpulkan dengan wawancara. Hubungan antara karakteristik sosiodemografi dan status gizi dinilai dengan uji statistik chi-square. Dari 106 sampel, 73 siswa (68,9%) memiliki status gizi normal dan 33 siswa (31,1%) memiliki status gizi kurang. Uji statistik tidak menunjukkan hubungan yang signifikan antara karakteristik sosiodemografi dan status gizi di Desa Wokam dan Karangguli, Kabupaten Kepulauan Aru. Kata kunci  : hubungan, siswa sekolah dasar, sosiodemografi, status gizi AbstractNutritional status is a physiological condition of the body related to food consumption and requirements of the body. Until now, malnutrition is a common problem in developing countries. Nutritional status in school age children is important because it can affect cognitive ability and student achievement. One of the root problems of malnutrition is poverty which is related to sociodemographic including social and economic status. The purpose of this study is to analyze the correlation between sociodemographic characteristics and nutritional status in elementary school children in Wokam and Karangguli Village, Aru Islands Regency, Maluku. This study used cross-sectional study design and the type of this study is analytical observational. Nutritional status was assessed using weight to stature growth chart with Waterlow criteria. Data about sociodemographic were collected by interview. Correlation between sociodemographic characteristics and nutritional status were analyzed using chi-square test. From 106 samples, 73 students (68,9%) have normal nutritional status and 33 students (31,1%) were wasted. No significant correlation was found between sociodemographic characteristics and nutritional status in Wokam and Karangguli Village, Aru Islands Regency. Keywords:     correlation, elementary school students, nutritional status, sociodemographic

Abstract

Diarrhea is an endemic disease in Indonesia and has the potential to cause outbreaks that are often accompanied by death, even the mortality rate during outbreaks in 2017, amounting to 1.97%, had not yet reached the expected target by the government, which is less than 1%. This study aims to determine the distribution of sex, age, nutritional status based on weight/age, and how to leave the hospital for chronic diarrhea patients and to know the correlation between nutritional status based on weight/age and death cases of chronic diarrhea in children under five years old treated at RSUD Dr. Soetomo in the period 2015-2019. This research was a retrospective analytical and descriptive study using secondary data from a patient's electronic medical record. Results showed that out of 45 chronic diarrhea patients, 28 (62,2%) were male, 16 (35,6%) children were in the 0 - 5 months age group, 18 (40,0%) children had good nutritional status, 39 (86,7%) children were discharged from the hospital, and there was a significant correlation between nutritional status based on weight/age and death cases of chronic diarrhea in children (p = 0,017). It could be concluded that the majority of children with chronic diarrhea were male, were in the 0 - 5 months age group, had good nutritional status, and were discharged from the hospital which meant that their condition had improved or cured and there was a significant correlation between nutritional status based on weight/age with death cases of chronic diarrhea in children.

Abstract

Diarrhea is a disease that still be a health problem in the world, including Indonesia. The number of cases of diarrhea in children under five in East Java had increased in prevalence from 6.6% in 2013 to 10.7% in 2018. Deaths in diarrhea usually occurred as a result of severe dehydration which 70% -80% of them were children under-5 years. This study aimed to know the characteristics of children under-5 years who had acute diarrhea at Tanah Kali Kedinding Primary Health Care in September 2018 - January 2019. This type of research was a descriptive study, where the results was only a picture, using a cross-sectional approach. Statistical analysis was performed to describe the characteristics of each patient by using a frequency descriptive test. The results of this study found that in September 2018 - January 2019, of 116 children under-5 years suffering from acute diarrhea in the Tanah Kali Kedinding Health Care, Surabaya, most of them were females, within 1-2 years, had exclusive breastfeeding, had parents with the last education graduated from high school and family income under minimum of wage in Surabaya. It is hoped that this research will become a source of information for the Government and Primary Health Care to reduce the incidence of acute diarrhea.

Abstract

Background: Diarrhea is the most common cause of death in under-five children. Bloody diarrhea comprises around 10% of all cases of diarrhea and may lead to severe complications until death. This study examined the characteristics of bloody diarrhea in children under five years old in Dr. Soetomo General Hospital Surabaya from 2013 to 2017. Material and Methods: A retrospective, cross-sectional study was conducted using secondary data from Dr. Soetomo General Hospital's inpatients with bloody diarrhea from 2013 to 2017. Gender, age, nutritional status, clinical symptoms, degree of dehydration, and laboratory results were assessed, and the data were presented in percentage (%) Results: Fifty-six samples were included in this study. The main demographics were male (58,9%), aged 7-24 months (44,6%), and normal nutritional status (66,1%). Meanwhile, the most notable manifestations were stool mucous (55,3%), mild to moderate degree of dehydration (60,7%), and leukocytosis (62%). Eleven patients (39,2%) had temperatures ≥380C. Leukocytes were positive in 93.7% of the stools. Furthermore, amoeba was found in 46,8% of samples. The serum electrolyte result showed hyponatremia (18%) and hypokalaemia (15%). Conclusion: The primary demographics of bloody diarrhea in under-five children admitted to Dr. Soetomo General Hospital were males, 7-24 months of age, and with normal nutritional status. The most frequent manifestations were mucous in stool, mild to moderate dehydration, leucocytosis, as well as positive leucocytes and amoeba in the stool.

Abstract

Background: Childhood constipation is still common among children under 5 years of age. This study was conducted to determine the clinical profile of childhood constipation under 5 years of age at dr. Soetomo General Hospital, Indonesia. Methods: This retrospective cross-sectional study was performed by reviewing medical record of constipated children under 5 years of age at dr. Soetomo General Hospital Surabaya from January to December 2021. Data regarding age, gender, living place, body anthropometric (weight, height and nutritional status), sign and symptoms, and physical examination were collected for each patient. Nutritional status was determined by using WHO 2006 growth chart. We also assessed the presence of concomitant disease among those children and presented the data on the table. Results: A total of 35 subjects with constipation were included in this study. Mean (±SD) for age, body weight, length/height of these subjects were 27.09±17.71 months, 10.93±6.11 kg, 82.34±18.52 cm, respectively, with the male and female ratio of 1,67: 1. Around 18.2% of subjects presented with wasted, 18.2% with severely wasted, and 18.2% with severely stunted. Hardened stool (80%) was the most prevalent symptom, followed by straining (37.1%) and fecal impaction (22.9%). The most common concomitant diseases of childhood constipation were Hirschsprung’s disease (17.1%), congenital heart disease (11.4%), and hypothyroidism (11.4%). Conclusion: The results of this study showed that boys had a higher prevalence of constipation in children under 5 years of age, with the most prevalent clinical features being hardened stool, straining, and fecal impaction. Meanwhile, Hirschsprung’s disease, hypothyroidism, and congenital heart disease were the most concomitant disease.

Abstract

Introduction: The involvement and ability of health workers in Mandangin Island in early managing acute diarrhea in children to reduce infant mortality due to diarrhea are very important. This study aimed to determine the effect of community development Faculty of Medicine Universitas Airlangga - Dr. Soetomo General Academic Hospital Surabaya in a seminar on Early Management of Acute Diarrhea in Children using video conference to educate health care workers (HCWs) at the Mandangin Community Health Center. Material and Methods: This cross-sectional study was designed on 20 HCWs at the Mandangin community health center. Subjects were evaluated for demographic data (gender, age, occupation, last education), and the knowledge was assessed using a pre-test before the seminar was given. We used questionnaire about acute diarrhea containing 10 questions with maximum score 100. The seminar was given through video conferences with form classical lecturer and a case study on acute diarrhea, and subjects attended the seminar in Community Health Center’s Hall. We assessed knowledge after the seminar using the same questionnaire with randomization in question and answer and compared the pre-and post-test using the Wilcoxon Rank sum test. Results: Most of the subjects were midwifery, with a mean age of 32.4 ± 5.75 years old, 45% with D-3 last education and 80% were female. There was a significant difference between pre-and post-test in acute diarrhea in children (47.89±20.7 vs 64.74±22.94) (p=0.008). Conclusion: Video conference education effectively increased health care workers' knowledge about acute diarrhea in children

Abstract

Highlights: 1. The focus of this study was on the treatment methods for acute diarrhea, which have not been extensively investigated in the region studied.2. This study can provide an overview of doctors' adherence to the available guidelines for the treatment of acute diarrhea in children aged 1–5 years in Surabaya, Indonesia. Abstract: Diarrhea is the second leading cause of death among children under five, following pneumonia. Insufficient knowledge of diarrhea and dehydration management may contribute to this high mortality rate. Implementing prompt and effective management and prevention strategies has the potential to decrease morbidity and mortality associated with diarrhea. Therefore, this cross-sectional study aimed to investigate the treatment methods used by doctors for children aged 1–5 suffering from acute diarrhea in Surabaya, Indonesia. Data were collected from doctors in Surabaya using an online form containing case-based questions. Microsoft Excel for Mac version 16.17 (Microsoft Inc., Redmont, WA, USA) was used to process the binary data by calculating frequencies and percentages with a point estimate of 95% confidence interval (CI). A descriptive method was used in the data analysis, and the findings were presented in tables. From a total of 51 respondents who participated in this study, 18 (35.29%) only administered oral rehydration therapy. In addition, 49 respondents (96.08%) prescribed zinc supplementation for ten days, while 10 respondents (19.61%) opted for antibiotics. Interestingly, 49 respondents (96.08%) recommended both breast milk and food for the patients, and nearly all respondents provided guidance to the mother or caregiver of the patient. The findings of this study suggested that the respondents primarily used rehydration therapy to treat children suffering from acute diarrhea with moderate dehydration. However, some respondents still relied on antibiotics despite the available recommendations to use oral rehydration therapy. Nearly all respondents agreed on zinc supplementation, breast milk and food provision, and advice for the mother or caregiver of the patient. While certain aspects of the disease management for diarrhea align with the recommended guidelines, there is room for improvement in promoting the exclusive use of oral rehydration therapy and reducing unnecessary antibiotic prescriptions.

Abstract

Functional gastrointestinal disorders are problems that are often experienced by children, especially in isolated areas. Mandangin Island is among the secluded islands located in Sampang district.. The Department of Child Health, Faculty of Medicine, Universitas Airlangga - Dr. Soetomo General Academic Hospital held a community development that aimed to increase public knowledge about functional gastrointestinal disorders in children on Mandangin Island, Sampang District. The community service was conducted using a hybrid seminar approach, where speakers participated remotely via Zoom while the local community gathered at the Village Hall on Mandangin Island.. The speakers explained the definition, important signs and symptoms, early management of Functional Gastrointestinal Disorders. All subjects who took part in community service activities were evaluated for knowledge based on pre- and post-tests, each of which consisted of 5 questions with maximum score 100. Subject’s knowledge was considered good if the score was ≥ 60. Based on our data, there were 29 people who participated in community development with the most working as fishermen and entrepreneurs. We found that 69% of the subjects had an increase in the pre-test score. At the time the pre-test was distributed, only 10.4% had good knowledge scores, while in the post-test there were 51.7% who had good knowledge. We also succeeded in making 3 health cadres caring in childhood functional gastrointestinal disorders. Community development in the form of health education for functional gastrointestinal disorders in children using video conference (ZOOM) on Mandangin Island could increase public knowledge and this activity can be implemented on other islands.
