Abstract

Stunting in early life, particularly in the first 1000 days has longterm consequences on children’s physical growth, cognitive performance, and emotional well-being. Adequate parental and community knowledge plays a critical role in enabling early recognition, prevention, and appropriate management of factors contributing to stunting. This study aims to investigate the impact of health education on community knowledge related to stunting. A cross-sectional design was applied involving 24 adult participants (≥ 18 years) from the general public in Teluk Sasah District, Bintan Island using total sampling. The educational intervention was delivered through pamphlet distribution, health talks, and discussions with experts focusing on stunting. Pre and post-test were conducted to assess their understanding about stunting. Data were analyzed using Statistical Product and Service Solution (SPSS) Software version 16.0, employing the Shapiro-wilk for normality and the Wilcoxon signed-rank test to compare scores before and after the interventions. This study demonstrated a significant difference between pre-test and posttest (80.83 ± 9.74 vs 90.83 ± 7.76, p=0.000). Furthermore, the minimum score of post-test and pre-test also improved markedly (60.00 vs 70.00, p=0.000). This study found that the implementation of health education, including pamphlet distribution, classical lectures, and case discussions, effectively enhanced participants’ knowledge about stunting.

Abstract

Highlights: 1. The focus of this study was on the treatment methods for acute diarrhea, which have not been extensively investigated in the region studied. 2. This study can provide an overview of doctors' adherence to the available guidelines for the treatment of acute diarrhea in children aged 1–5 years in Surabaya, Indonesia. Abstract: Diarrhea is the second leading cause of death among children under five, following pneumonia. Insufficient knowledge of diarrhea and dehydration management may contribute to this high mortality rate. Implementing prompt and effective management and prevention strategies has the potential to decrease morbidity and mortality associated with diarrhea. Therefore, this cross-sectional study aimed to investigate the treatment methods used by doctors for children aged 1–5 suffering from acute diarrhea in Surabaya, Indonesia. Data were collected from doctors in Surabaya using an online form containing case-based questions. Microsoft Excel for Mac version 16.17 (Microsoft Inc., Redmont, WA, USA) was used to process the binary data by calculating frequencies and percentages with a point estimate of 95% confidence interval (CI). A descriptive method was used in the data analysis, and the findings were presented in tables. From a total of 51 respondents who participated in this study, 18 (35.29%) only administered oral rehydration therapy. In addition, 49 respondents (96.08%) prescribed zinc supplementation for ten days, while 10 respondents (19.61%) opted for antibiotics. Interestingly, 49 respondents (96.08%) recommended both breast milk and food for the patients, and nearly all respondents provided guidance to the mother or caregiver of the patient. The findings of this study suggested that the respondents primarily used rehydration therapy to treat children suffering from acute diarrhea with moderate dehydration. However, some respondents still relied on antibiotics despite the available recommendations to use oral rehydration therapy. Nearly all respondents agreed on zinc supplementation, breast milk and food provision, and advice for the mother or caregiver of the patient. While certain aspects of the disease management for diarrhea align with the recommended guidelines, there is room for improvement in promoting the exclusive use of oral rehydration therapy and reducing unnecessary antibiotic prescriptions.

Abstract

In developing countries, Norovirus is the second-leading cause of acute diarrhea, after rotavirus. The approved gold standard method for diagnosis of norovirus infection is RT-PCR. The rapid immunochromatographic test is a novel and expedient method for diagnosing norovirus that is relatively affordable. However, the use of the rapid immunochromatographic test remains controversial because of its accuracy. This study aimed to explore whether the rapid immunochromatographic test could be used for diagnosing norovirus-related diarrhea in children. Rapid immunochromatographic test (QuickNaviTM-Norovirus2) and RT-PCR on stool samples was used to diagnose norovirus. Stool samples were obtained from pediatric patients aged between 1 and 60 months who had diarrhea and were admitted to the pediatric ward at Dr. Soetomo General Hospital Surabaya, between April 2013 and March 2014. Ninety-four subjects provided stool samples that were tested using QuickNaviTM-Noro2 and RT-PCR. Using the test, 64 samples tested positive for norovirus and 30 tested negatives. The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of the rapid immunochromatographic test were consecutively 90.3%, 42.9%, 43.8%, 90%, and 58.5%. RT-PCR was used to test all samples to assess the accuracy, which showed that one from 31 samples contained the GI strain (1.1%), while 30 samples (32%) contained the GII strain. This study definitively establishes that the rapid immunochromatography test is not sufficiently accurate for use as a screening or diagnostic tool in norovirus-related diarrhea cases in children.

Abstract

Highlights: Low prevalence of giardiasis among kindergarten students. Good level of knowledge and personal hygiene practices of kindergarten children.   Abstract Introduction: Giardiasis is a gastrointestinal disease caused by the parasite Giardia lamblia and affects many individuals in developing countries. Kindergarten children are at risk of Giardia infection due to limited knowledge and inadequate personal hygiene. This study aimed to determine the prevalence of G. lamblia infection among kindergarten children on Mandangin Island, Sampang, Indonesia. Methods: A cross-sectional study was performed between January and March 2023. Stool samples and questionnaires were collected from 37 participants to obtain data on their demographics, socioeconomic status, and behavioral risk factors. Giardia infection was diagnosed through direct stool examination and rapid antigen testing. The data were analyzed statistically using the International Business Machines Corporation (IBM) Statistical Package for the Social Sciences (SPSS) version 26, with p>0.05 considered statistically non-significant. Results: A total of 37 subjects were enrolled in this study. The prevalence of giardiasis was 5.4% (n=2) among the study population. There was no significant correlation between Giardiasis infection and the risk factors examined (p>0.05). Conclusion: The low prevalence of giardiasis in this study might be related to the relatively good level of knowledge and personal hygiene practices among the kindergarten children. In addition, the majority of residents of Mandangin Island, Sampang, Indonesia, used boiled rainwater for drinking. Access to safe drinking water, along with good personal hygiene practices, especially handwashing, can reduce the risk of giardiasis.
