Intestinal Malrotation with Midgut Volvulus Comorbidity with Bronchopneumonia: A Case Report Fatmah Sindi; Yusri Dianne Jurnalis; Jon Effendi

Abstract

Background: Intestinal malrotation is a congenital anomaly resulting from abnormal rotation and fixation of an imperfect midgut during embryonic development. This study aims to describe the diagnosis and management of intestinal malrotation and its comorbid diseases in children. Case presentation: A boy, aged 9 months, was referred from a regional hospital with complaints of repeated vomiting accompanied by a distended stomach and no bowel movements for 4 days. Based on the birth history, the patient was born with vacuum assistance for the indication of prolonged phase II. The results of the physical examination showed that the patient was seriously ill, with compos mentis, pulse rate 130 x/minute, respiratory rate 34x/minute, blood pressure 90/60 mmHg, temperature 37.6ºC, 99% saturation with the help of O2 nasal canule 2L/minute. There are increased bowel sounds on abdominal examination. Blood examination showed an increase in leukocytes (10.850/mm3). The chest X-ray showed lateral bronchopneumonia, and the abdominal X-ray showed low-lying obstructive ileus. The patient was diagnosed with obstructive ileus due to volvulus, moderate dehydration, comorbid with bronchopneumonia, and global developmental delay. Conclusion: Management of intestinal malrotation with volvulus in this patient includes general improvement and rehydration, Ladd's procedure for definitive correction of the cause. Management of comorbid global developmental delay is carried out after the emergency is resolved.

Abstract

Background: Human Cytomegalovirus (CMV) is a Beta-Herpes virus that leads to congenital infection in 0.4% to 2,3 % of all newborns. Diagnosis of CMV infection includes the culture of the nasopharynx, saliva, and urine. Serologic tests are also beneficial for CMV diagnosis. IgM CMV-specific antibodies can be monitored. Biliary sludge is a mixture of particulate solids that have precipitated from bile. Laparoscopic cholecystectomy offers the most definitive therapy for biliary sludge. Case presentation: A 2-month-old boy complained yellow appearance since the age of 1 week ago. Looks pale. Defecation was sometimes colored putty since the age of 2 weeks. Blood laboratory examination with hemoglobin 8,7 gr/dL, Prothrombin time, activated partial thromboplastin time, and liver function were increased. Bilirubin, alkaline phosphatase, and Gamma GT were increased. Two-phase and abdominal USG were biliary atresia. PCR CMV examination with urine and blood sample was positive. Head USG was consistent with CMV infection. Cholangiography showed sludge bile with liver cirrhosis. Therapy gansiklovir IV for 14 days then gansiklovir per oral for 6 months. Conclusion: Cholangiography is the definitive diagnosis for biliary sludge. CMV is a congenital infection that causes asymptomatic to multiple severe symptoms, so it is necessary to monitor children for adherence to treatment, improve nutritional status and reduce morbidity and mortality rates.

Abstract

Background: The incidence of corrosive ingestion is high and largely unreported in developing countries due to lack of parental supervision and careless storing of these chemicals at home. Case presentation: A 5 years, 1-month-old boy came to the outpatient clinic department of pediatric Dr. M. Djamil General Hospital with a chief complaint of recurrent vomit since 6 weeks before admission. There was a history of ingested accumulator water 1,5 months before admission. The patient vomits every day, especially after eating and drinking. Conclusion: The most common symptoms are vomiting, feeding problems in oral intake, and rapid weight loss. The clinical diagnosis of prepyloric stricture is based on the history of the disease, physical examination, and supporting the investigation. Treatment of gastric outlet obstruction caused by corrosive ingestion should be treated surgically.

Abstract

Background: Abdominal pain was one of the most complaints in children. Chronic appendicitis was the common cause of recurrent abdominal pain. The incidence of this case was 1,5% of all appendicitis cases. The manifestation was not specific, and one of the treatment options was surgical with laparotomy or laparoscopy methods. Case presentation: A 10-year-old boy came to the outpatient clinic of gastroenterohepatology with a diagnosis of abdominal pain caused by chronic appendicitis differential diagnosis of chronic gastritis with abdominal pain, especially the pit of the stomach moving to the right waist since 3 months ago. Physical examination of the patient showed the general condition was moderate, conscious, and normal vital signs. Abdominal distension was absent, tenderness in the lower right abdomen was present, and the pain was present in the lower right abdomen. There was contralateral tenderness in the left lower abdomen. An appendicogram examination shows chronic appendicitis. The patient was consulted for pediatric surgery for appendectomy and later controlled to a surgical outpatient clinic after discharge. Conclusion: Chronic appendicitis usually shows unspecific manifestation and physical examination. Early diagnosis and surgical treatment are options to treat appendicitis in children.

Abstract

Background: Choledochal cysts (CC) are rare congenital anomalies of the biliary tract. The classic triad of jaundice, abdominal pain, and right upper quadrant mass are rare symptoms, mainly in children and adolescents. This study aimed to present the diagnosis and management of choledochal cyst cases in a baby girl. Case presentation: A 1-year-old girl, domiciled outside the city of Padang, was brought to the hospital by her parents with complaints of repeated vomiting for 1 day before being admitted to the hospital. This complaint has been accompanied by yellowish eyes and body, pale bowel movements, and dark brown urine since 3 months ago. The patient was never brought to the doctor for treatment. Since 1 month ago, the patient often feels pain in the abdomen and has a lump in the upper right abdomen. On the abdomen area, there was no distention, hepar was ¼-1/4 sharp edge, tenderness at the epicondrium area, no rebound tenderness, turgor was decreased, and no palpable mass. Abdominal ultrasonography presented a choledochal duct cyst with biliary obstruction. Abdominal CT scan with contrast revealed intrahepatic and extrahepatic biliaryectasis et causa suggestive of the choledochal cyst (Todani IVA) and hepatomegaly. The patient was consulted by the pediatric surgery division for further management and was advised to undergo surgery with the Roux En-y procedure. Follow-up after the patient underwent surgery showed clinical improvement. Symptoms of jaundice in the eyes and skin begin to decrease, as brownish stools and light brown urine. Conclusion: Surgical treatment with the Roux-en-Y procedure is the definitive treatment for choledochal cysts in this patient.

Abstract

Latar belakang. Kolostrum susu sapi atau Bovine colostrum mengandung berbagai growth factor dan immune factor, salah satunya secretory IgA (sIgA) yang dengan jumlah signifikan dapat menghalangi adhesi patogen ke membrane mukosa dan menghambat kolonisasi sehingga bermanfaat untuk mengobati penyakit di saluran pencernaan. Penelitian yang telah dilakukan di Indonesia belum banyak membahas secara spesifik hubungan sIgA yang didapatkan dari kolostrum susu sapi sebagai terapi tambahan diare akut pada anak. Tujuan. Mengetahui pengaruh pemberian kolostrum susu sapi terhadap durasi diare akut dehidrasi ringan sedang dan kadar sekretori IgA pada anakMetode. Penelitian eksperimental yang dilaksanakan di Puskesmas dan Rumah Sakit di kota Padang. Penelitian dimulai dari bulan februari tahun 2022 sampai November 2022. Data terkumpul 30 sampel masing-masing pada kelompok kontrol yang mendapatkan terapi standar WHO dan kelompok intervensi yang mendapatkan terapi standar who ditambah kolostrum susu sapi. Dilakukan pengamatan terhadap durasi diare akut serta pemeriksaan terhadap kadar sIgA.Hasil. Terjadi pemendekan durasi diare secara signifikan antara kelompok intervensi dan kelompok kontrol sebesar 11,93 jam (p=0,021). Terdapat perbedaan kadar sIgA yang signifikan antara sebelum dan sesudah pemberian kolostrum susu sapi pada kelompok intervensi (p=0,003).Kesimpulan. Pemberian kolostrum susu sapi dapat memperpendek rerata durasi diare akut dan meningkatkan rerata kadar sIgA secara bermakna. Pemberian kolostrum susu sapi ini dapat disarankan sebagai terapi adjuvan dalam tatalaksana diare akut pada anak.

Abstract

Introduction. Coronavirus Disease 2019 (COVID-19) is caused by Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) which in 2020 was declared a global pandemic. In the management of COVID-19, patients are classified according to the severity. Identifying prognostic factors at an early stage helps assess severity risks. Abnormal hematological parameters were present in COVID-19 patients such as the elevation of neutrophil lymphocyte ratio (NLR) and D-dimer patients. This study aimed to determine the NLR and D-dimer levels based on the severity of COVID-19 patients. Methods. This study was an observational analytical study that used a case-control approach using 100 medical records of COVID-19 patients who were treated at Semen Padang Hospital in 2021. Consecutive sampling was used in this study. ANOVA test and the Kruskal-Wallis test were used for bivariate analysis. The results were considered significant if the p-value <0.05. Results. Characteristics of patients treated for COVID-19 were: 51% male and 49% female, mean age 57.79 (SD 13.5) years. NLR values based on the severity (mild, moderate, severe, and critical) were 3.12 (SD 2.04), 3.51 (SD 2.87), 6.89 (SD 3.6), 12.57 (SD 10.34) respectively; p<0.05. The median value of D-dimer (ng/mL) based on the severity (mild, moderate, severe, and critical) were 444, 791, 1,610, 2,135; p<0.05. The results of this study showed that there were significant differences in the NLR values and D-dimer levels based on severity. Conclusion. RNL value and D-dimer levels increase the most in the critical degree group and shows a significant relationship in COVID-19 patients.

Abstract

Pendahuluan : Diare merupakan penyebab kedua tertinggi kematian pada anak di bawah usia 5 tahun di Indonesia. Puskesmas Pauh dan Puskesmas Lubuk Buaya merupakan puskesmas tiga tertinggi angka kejadian diare pada balita di Kota Padang serta salah satu penyebab kematian balita tertinggi akibat diare sejak tahun 2018 sampai 2021. Tujuan : Penelitian ini bertujuan untuk mengetahui prevalensi dan gambaran klinis diare balita di Puskesmas Pauh dan Puskesmas Lubuk Buaya tahun 2019-2021. Metode : Metode yang digunakan pada penelitian ini adalah deskriptif retrospektif dengan teknik pengambilan sampel menggunakan Simple Random Sampling dengan memperhatikan kriteria inklusi dan eksklusi. Sampel penelitian berjumlah 218 balita dengan menggunakan data rekam medis pasien tahun 2019-2021. Hasil penelitian : Hasil penelitian ini menunjukan prevalensi kejadian diare tertinggi di Puskesmas Pauh terjadi pada tahun 2019 (6,82%) dan Puskesmas Lubuk Buaya tahun 2019 (7,74%). Rerata usia balita adalah 22,79±13,89 bulan dengan jenis kelamin terbanyak laki-laki (56,4%) dibandingkan perempuan (43,6%). Kesimpulan : Gambaran klinis yang paling banyak ditemukan dari 218 pasien yaitu demam (57,3%), diikuti dengan muntah (44,5%). Sebagian besar pasien berstatus gizi baik (67,4%), pasien dengan lama sakit 1 hari (48,6%), feses berlendir (7,8%), dan masih ditemukan pasien dengan dehidrasi ringan (1,8%) dan dehidrasi sedang (1,4%).
