Abstract

Kecemasan yang muncul pada pasien yang akan dilaksanakan tindakan Primary PCI serta kondisi ICU sebagai faktor pendukung tentu dapat mempengaruhi life satification serta mempengaruhi kondisi fisiolgis tubuh salah satunya adalah heart rate yang tidak stabil. Tujuan dari penelitian ini adalah mengembangkan modul terapi kutan untuk mengatasi kecemasan dan stabilisasi heart rate pasien Pre PCI. Desain penelitian ini menggunakan Research and Development yang dilakukan melalui tahap I adalah literature review dan penentuan isu strategis tahap II adalah focus group discussion, konsultasi pakar dan uji validitas serta reliabilitas. Populasi dan subjek dalam penelitian tahap 1 sejumlah 10 responden perawat ICU. Variabel yang diteliti adalah pengembangan modul terapi kutan dan SPO terapi kutan yang dilakukan uji ekspertisi, validitas dan reliabilitas menggunakan instrumen validitas dan reliabilitas dalam bentuk kuesioner. Analisa untuk uji validitas dan reliabilitas menggunakan CVI dan statistik Multirater Fleiss Kappa. Hasil penelitian menujukkan tahap 1 telah dilakukan literatur review dari 12 artikel terpilih dari 24.339 artikel periode 2018-2024. Hasil FGD dan konsul pakar kemudian dilakukan uji validitas CVI oleh 10 perawat ICU pada pengembangan modul secara keseluruhan dengan nilai S-CVI 0.0,88 dan SPO secara keseluruhan dengan nilai 0,940. Hasil uji reliabilitas dengan Multirater Fleiss Kappa dengan nilai 0,262 untuk modul dan 0.270 untuk SPO. Pengembangan modul dan SPO menghasilkan tatalaksana kecemasan dan stabilisasi heart rate pasien pre PCI di Ruang ICU RSI Sultan Agung Semarang.

Abstract

Stroke merupakan penyebab kematian nomor.dua serta penyebab kecacatan utama diseluruh dunia. Pengenalan dan pengambilan keputusan terhadap Stroke dapat ditingkatkan dengan menggunakan BEFAST (Balance, Eyes, Face, Arm, Speech, Time) sebagai instrumen penilaian awal pada tahap prehospital. Tujuan penelitian ini adalah untuk mengetahui efektivitas penggunaan BEFAST sebagai deteksi dini stroke di prehospital. Desain penelitian ini menggunakan Systematic Review, yang diambil dari PubMed, Proquest, Science Direct, dan Google Scholar dari tahun 2019 sampai dengan 2024. Pencarian artikel inimenggunakan kata kunci Stroke OR cerebra arterial disease AND prehospital detection AND BEFAST. Desain penelitian menggunakan Preferred Reporting Items for Systemic Review and Meta-Analysis (PRISMA) Guidelines. Dari 217 artikel yang ditemukan, 5 artikel yang di review dalam penilitian ini. 5 artikel tersebut (100%) mendukung penggunaan BEFAST sebagai pengenalan awal stroke prehospital dengan sensitivitas dan spesifisitas yang tinggi. Penggunaan BEFAST merupakan aspek objektif untuk deteksi dini stroke prehospital, persiapan aktivasi kode stroke di unit gawat darurat, sehingga dapat mengurangi risiko kematian dan kecacatan.

Abstract

Introduction: Nursing diagnoses are essential in Intensive Care Units (ICUs) for guiding decisions, prioritizing care, and improving outcomes. However, inconsistent terminology and documentation burdens hinder their effective implementation and evaluation. The aimed of this study was to examine the application of nursing diagnoses in ICUs, exploring their relationships with clinical indicators, patient outcomes, and the impact of standardized diagnostic frameworks on nursing practice. Methods: This scoping review followed PRISMA-ScR guidelines We searched PubMed, CINAHL, Scopus, and Web of Science from 2019 to 2024. Studies examining nursing diagnoses in ICU settings were included. The reviewers independently screened 527 records, with nineteen full-text articles assessed for eligibility, resulting in thirteen included studies. Results: Thirteen studies with diverse methodologies from multiple countries were analyzed. Four key themes emerged: 1) Clinical indicators linked to specific nursing diagnoses, particularly respiratory conditions where impaired spontaneous ventilation was associated with dyspnea, fever, fatigue, and cough; 2) Implementation processes for nursing diagnoses, including electronic health records and nurse-delivered thoracic ultrasound that enhanced clinical decision-making; 3) Diagnostic validation and prevalence studies highlighting the importance of standardized frameworks for accurate diagnosis; and 4) Interventions that improved nursing practice and patient outcomes, including the institutionalization of nursing processes and development of electronic care plans. Conclusion: Standardized nursing diagnoses optimize ICU patient care by providing a structured approach to assessment, intervention planning, and outcome evaluation. Technology integration and consistent terminology enhance diagnostic accuracy and intervention effectiveness. Future research should focus on validating these frameworks across diverse ICU settings and patient populations to establish best practices for implementation.

Abstract

Introduction: Pressure ulcers in high-risk patients are contributes to patient burden and cost financially. Identifying strategies on the prevention of high ulcers among those with high risk is necessity. Nevertheless, an intense study exploring those issues is scarce. The study aimed to analyze the prevention method and their effectiveness in pressure ulcer prevention. Method: This study used PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis) to guide the systematic review process. The article searches were conducted on Web of Science (WOS), ScienceDirect, Wiley Online Library, and Scopus using the keywords used Decubitus Ulcer, Pressure Ulcer, Management, and Prevention, and any possible combinations. Results: A total of 18 studies met the inclusion criteria for this review and show some different prevention for a pressure ulcer. The findings of the present study are foam dressings, support surfaces, repositioning, topical-based interventions, wearable patient sensors, and two distinct endotracheal tube fixations affect the incidence of pressure ulcers. Conclusions: This current review emphasized a bundle care consisted of two methods or more are effective to reduce the incidence of pressure ulcers rather than a single treatment. Nurses should consider intervention to prevent ulcers among high-risk patients by way analyses of each single treatment prior bundling. Further study investigating factors associated wound's ulcer healing is necessary.

Abstract

Introduction: Out-of-hospital cardiac arrest is a life-threatening emergency with a high mortality rate, often associated with delays in bystander response. Limited knowledge and inadequate skills in Basic Life Support among the general public remain major barriers to improving survival outcomes. Educational approaches using simple and practical media, such as CPR mats, have the potential to enhance community readiness in responding to cardiac emergencies. This community service activity aimed to improve the knowledge and skills of community members in performing BLS through the use of innovative CPR mat-based training. Methods: This community-based activity was conducted in Sukolilo Village in collaboration with the local Community Health Center (Puskesmas), involving 33 participants consisting of health cadres, PKK members (Family Welfare Movement), and community representatives. The intervention included structured education and simulation-based BLS training using a CPR mat. Program effectiveness was evaluated using pre- and post-tests, and the data were analyzed using the Wilcoxon signed-rank test. Results: Participants’ knowledge significantly improved after the intervention (p < 0.001), with the mean score increasing from 55.63 to 81.25 and a shift from predominantly poor to good knowledge levels. Practical BLS skills also showed significant improvement based on simulation assessments (p < 0.001), indicating enhanced ability to perform key procedures. Conclusion: CPR mat-based BLS education is effective in enhancing community knowledge, skills, and readiness to act as first responders in cases of cardiac arrest. This approach has strong potential to be implemented as a community-based strategy to improve early emergency response and survival outcomes. Abstract

Comfort in patients with coronary heart disease (CHD) is crucial to accelerate the healing process during their stay in intensive care units (ICUs). Factors contributing to patient comfort are essential in providing comprehensive nursing care. This study aims to investigate the relationship among variables, including patient characteristics, emotional response, healthcare service, Islamic spirituality, spiritual meaning, and comfort in patients with CHD. A cross-sectional study was conducted, involving 110 respondents in the ICUs of hospitals across Central Java Province, Indonesia, from January to May 2023. The sample was taken using purposive sampling. The data were collected using questionnaires that had been modified under a permission from the authors. SmartPLS 3 with partial least square structural equation modelling (PLS-SEM) was employed to analyze the correlation among the variables. The result showed that patient characteristics showed significant correlations with emotional response (t=2.082, p=0.038) and spiritual meaning (t=3.142, p=0.002). Similarly, support significantly correlated with emotional response (t=4.032, p<0.001) and spiritual meaning (t=2.448, p=0.015). Healthcare service exhibited significant correlations with emotional response (t=9.485, p<0.001) and spiritual meaning (t=5.249, p<0.001). Emotional response was found to correlate with Islamic spirituality and spiritual meaning (t>1.96, p<0.05), while Islamic spirituality showed a significant correlation with spiritual meaning (t=2.727, p=0.007). Finally, spiritual meaning significantly correlated with comfort (t=6.119, p<0.001). In conclusion, this study demonstrated significant correlations among variables such as patient characteristics, support, healthcare service, emotional response, Islamic spirituality and spiritual meaning to enhance patient comfort in ICUs. Policymakers should pay attention to factors affecting patient comfort during their stay in ICUs, including physical, psychospiritual, sociocultural, and environmental comfort based on self-regulation.

Abstract

Background: Traumatic brain injury (TBI) is an acute neurological disease commonly encountered in emergency rooms. Patients with traumatic brain injury face death early in their hospitalization, with mortality remaining high for several years after the injury occurs. Adequate early screening is needed to reduce the mortality rate. Methods: This study used a systematic review design. The articles selected in this study ranged from 2020 to 2025. The search was conducted using four databases: Scopus, PubMed, EBSCO, and ProQuest. The study followed the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. Results: Fifteen articles met the inclusion criteria after being evaluated for quality using the JBI Critical Appraisal Tool to determine their suitability for systematic review. Conclusion: This study shows that the use of early detection using the Early Warning Score will improve patient outcomes, even though EWS has several weaknesses when used in patients with traumatic brain injury. Abstract

Introduction: Delivering nursing care in intensive care units has numerous challenges, one of which pertains to cultural concerns. Cultural views of nurses can obstruct the provision of holistic nursing care. This study seeks to identify characteristics related to views of cultural competency among ICU nurses. Methods: This study employed a descriptive-analytic strategy utilizing a cross-sectional approach. The study sample consisted of 208 respondents, chosen using purposive sampling. Data were collected via a questionnaire and analyzed through binomial logistic regression at a significance level of p<0.05. Results: The perception of cultural competence among ICU nurses was significantly connected with age (p = 0.008), gender (p=0.038), and both career and competency level (p=0.031). Perceptions of cultural competency among ICU nurses were often inadequate. Gender was the predominant variable. Conclusions: ICU nurses with a positive sense of cultural competency influence the enhancement of nursing care implementation and quality
