Abstract

Background: The prevalence of prediabetic state in population is such a hard project to be confirmed within an increased number of obese population.Aim: The study aims to define the prevalence of prediabetes among obese young women (body mass index, BMI >25 kg/m2) in Denpasar.Methods: Research variables namely physical activity, food pattern, anthropometric values for identify level of obesity (light BMI 25-30, severe BMI >30 kg/m2), tipe of obesity (perifer obesity waist circumference <80 cm,  and central obesity waist circumference ≥80 cm), as well as fasting blood glucose (normal <100 mg/dL, prediabetes -125 mg/dL).Results: the prediabetes prevalence of subjects is 38,1%. All subjects 42 (100%) have light daily physical activity, with high mean of cholesterol intake (more than 200mg) and low of PUFA intake (less than 10% of fat intake). Pre-diabetic subjects have higher waist circumference (visceral fat) than subjects with normal blood glucose (p=0.023). In contrary, that was no relationship between subcutaneous fat (bicep and tricep skinfold) to prediabetic state(p >0,05). Central obesity was significant OR 1,76 (1,33-2,33) risk of prediabetes compared to peripheral obesity (p= 0,023). However, there was not significant risk (OR 2,67 (0,738-9,63) (p= 0,130)) of severe obesity to be prediabetes compared to light obesity. Latar Belakang: Peningkatan prevalensi obesitas bersamaan dengan prevalensi diabetes melitus tipe 2. Prevalensi prediabetes sangat susah dideteksi pada suatu populasi.Tujuan: untuk mengetahui prevalensi dan faktor penentu prediabetes pada wanita muda obese (indeks massa tubuh, IMT >25) di Denpasar.Metode: Penelitian ini termasuk studi cross-sectional dengan variabel penelitian, meliputi aktivitas fisik, pola makan, antropometri pada tingkat obesitas (obesitas ringan IMT 25-30 kg/m2, obesitas berat IMT>30 kg/m2) dan jenis obesitas (obesitas perifer lingkar perut <80 cm, obesitas sentral lingkar perut >80 cm), serta kadar glukosa darah puasa (normal <100 mg/dL, prediabetes -125 mg/dL).Hasil dan Simpulan: prevalensi prediabetes dari subjek adalah 38,1%. Semua subjek 42 (100%) memiliki aktivitas fisik harian yang ringan, dengan rata-rata asupan kolesterol yang tinggi (>200mg) dan asupan PUFA (pure unsaturated fatty-acid) yang rendah (<10% dari asupan lemak). Subjek dengan prediabetes mempunyai lingkar perut (lemak visceral) lebih besar dibandingkan dengan subjek dengan kadar gua darah normal (p=0,023). Sebaliknya, tidak didapat hubungan antara lemak subkutan (lipatan kulit bisep dan trisep) dengan kejadian prediabetes (p>0,05). Obesitas sentral bermakna berisiko menderita prediabetes OR 1,76 (1,33-2,33) dibandingkan dengan obesitas perifer (p= 0,023). Namun, tidak ada risiko yang signifikan [OR 2,67; (0,738-9,63) (p= 0,130)] obesitas berat menjadi prediabetes dibandingkan dengan obesitas ringan.

Abstract

Background and purpose: The prevalence of stunting among under-five children in Indonesia and also in Bali is high. Studies on risk factors of stunting have been widely conducted in Indonesia, however association between stunting and diet pattern is still inconsistent. The aim of this study is to examine association of supplementary feeding pattern with stunting among children aged 1-3 years. Methods: A case control study was conducted in Bangli District. A total of 48 cases and 48 controls were selected to participate in the study. Age and sex variables between cases and controls were matched. Cases and controls were selected using a systematic random sampling method from 26 health post registers in Kintamani I Public Health Centre between November and December 2016. Data were collected in March 2017 by interviewing the mother at the health post. Data were analysed using bivariate and multivariate analysis. A logictic regression was performed to calculate adjusted odd ratio (AOR). Results: Cases and controls were comparable for age (p=0.773), sex (p=0.219), mother’s education (p=0.673) and history of infectious diseases (p=0.584). Cases and controls differed in several variables: frequency, variability and type of supplementary feeding (p=0.002, <0.001 and <0.001), family income (p=0.038), poor personal hygiene (p<0.001), environmental sanitation (p=0.022) and access to clean water (p<0.001). Our analysis showed that several variables were associated with stunting among children aged 1-3 years, which included lack of supplementary feeding variability (AOR=12.45; 95%CI: 2.25-69.71), poor personal hygiene (AOR=3.52; 95%CI: 1.03-12.03), and poor access to clean water (AOR=6.49; 95%CI: 1.61-26.19). Other variables included supplementary feeding initiation, frequency of supplementary feeding, consistency or type of supplementary feeding, sex, and family income were not associated with stunting among children aged 1-3 years. Conclusions: Variability of supplementary feeding, personal hygiene, and access to clean water were all associated with stunting among children aged 1-3 years.

Abstract

Background and purpose: The prevalence of diabetes mellitus (DM) in Indonesia is increasing, including in Buleleng District, Bali Province. Studies regarding determinants of blood glucose control in DM patients show varied findings. This study aims to examine association between blood glucose level with physical activity, compliance to medication and glycemic load among DM patients. Methods: A cross-sectional survey was conducted in Buleleng District General Hospital from March to April 2017. A total of 73 patients were concecutively recruited to participate in our study. Data were collected through interview, observation and measurement. Data collected included socio-demographic characteristics, fasting blood glucose level, 1-hour and 2-hour postprandial blood glucose levels, carbohydrate intake, body mass index, glycemic index, glycemic load, physical activity, genetic or family history, and compliance to medication. A bivariate analysis was performed to examine association between independent variables and blood glucose levels. Multivariate analysis was also conducted to calculate adjusted odd ratio using a binary logistic regression. Results: Our study found that blood glucose levels of most respondents were relatively well managed–63% for fasting, 61.6% for 1-hour postprandial, and 63% for 2-hours postprandial. Multivariate analysis revealed that fasting blood glucose level were associated with regular physical activities (AOR=74.09; 95%CI: 7.52-729.69) and compliance to medication (AOR=11.90; 95%CI: 2.24-63.29). Furthermore, 1-hour postprandial blood glucose level were associated with breakfast glycemic load (AOR=0.63; 95%CI: 0.47-0.85) and compliance to medication (AOR=27.29; 95%CI: 2.29-323.95), while 2-hour postprandial blood glucose level were associated with breakfast glycemic load (AOR=0.69; 95%CI: 0.54-0.89) and compliance to medication (AOR=19.81; 95%CI: 2.31-170.14). Conclusions: Factors associated with fasting blood glucose level were regular physical activities and adherence to medication, while 1-hour and 2-hours postprandial blood glucose levels were both influenced by breakfast glycemic load and compliance to medication.

Abstract

Background and purpose: Diabetes mellitus (DM) is a serious health problem worldwide. Local alcohol consumption is one of Balinese’s traditions considered to be associated with the high incidence of DM. This study aims to determine the pattern of alcohol consumption and risk of type 2 DM in Manggis Subdistrict, Karangasem, Bali. Methods: A case control study was conducted in 55 men with type 2 DM as cases and 55 non-diabetic men as controls. Non-diabetic men were defined with fasting glucose level (GDP) <100 mg/dl and without clinical symptoms of DM. Cases were recruited from the patient’s register at the public health centre (PHC) in Manggis Sub-District and controls were recruited from similar neighborhood with the cases and matched by age. Data were collected over May-July 2017 by households’ interview using structured questionnaires. Multivariate analysis was employed using logistic regression to identify the risk factors of type 2 DM. Results: The characteristics of cases and controls were similar in terms of age, education and physical activity, but there was a significant difference in employment. Multivariate analysis showed that heavy alcohol consumption (AOR=7.84; 95%CI: 1.46-42.28), frequent consumption of high sugar alcohol drinks (AOR=3.45; 95% CI: 1.16-10.22), history of obesity (AOR=8.82; 95% CI: 2.43-32.01), employed (AOR=5.98; 95%CI: 1.89-18.93) and frequent consumption of sweetened beverages (AOR=39,57; 95%CI: 4.00-391,8) were significantly associated with the incidence of type 2 DM. No significant association was found between the duration of alcohol consumption and the incidence of type 2 DM. Conclusions: Alcohol consumption, a history of obesity, consumption of high sugar alcohol drinks, retirement/unemployment and consumption of sweetened beverages are associated with the incidence of type 2 DM. Education regarding those risk factors should be enhanced to reduce the incidence of type 2 DM.

Abstract

Background and purpose: Foods leftover is still very common in many hospitals in Indonesia. Minimum standard of hospitals require that foods leftover of patient is not more than 20%. The aim of this study was to determine the number and cost wasted due to the foods leftover of patient. Methods: The study was cross-sectional involving 68 inpatient subjects in the 1st, 2nd and 3rd ward class and hospitalized up to 10 days and had regular food with a 10-day menu cycle. Data of foods leftover were obtained by three methods: observation, medical records and interview. Foods leftover were measured by Visual Comstock method with the scale of 6 points. Age, sex, length of stay, ward classes, and types of patients’ diseases were obtained from medical records. Patient’s perception of food appearance and taste were gained through interviews. Data were analyzed by independent sample t-test and one-way ANOVA. Results: Average number of patients’ food leftover amounted to 14.79%. Patients who left foods were mostly women, aged 50-64 years, hospitalized in the class 2nd and 3rd with a length of stay ≤5 days. Those who had good perception on appearance and taste of the food tended to leave less food leftovers. The average cost of meal wasted a day was Rp 2,939 per patient. There were significant different leftover foods according to gender, age group, length of stay, ward classes, and the patient’s perception (p<0,05), whereas there were no significant different leftover foods according to the type of diseases and the menu cycles (p>0,05). Conclusion: Average number of patients’ food leftover at Sanglah Hospital Denpasar has satisfied hospital minimum standard (less than 20%) with an average daily cost of meal wasted was Rp 2,939 per patient.

Untuk mencapai hal tersebut perlu komunikasi yang sistematis dan intensif antara pemegang kebijakan dengan masyarakat, antara organisasi PPK dengan masyarakat dan antara kelompok masyarakat itu sendiri.

Abstract

Background and purpose: Medical nutrition therapy at Sanglah Hospital, Denpasar consists of diet prescription, food labelling, food portion, and food serving. Nutrition assessment has been carried out based on the minimum service standards of Sanglah Hospital Denpasar in 2013, but agreement assessment of energy and protein contents has never been performed in the provision of medical nutrition therapy. The study was conducted to determine the compatibility of energy and protein contents of the diet prescription until the food is served in hospitalized patients who received medical nutrition therapy at Sanglah Hospital Denpasar. Methods: This is a cross-sectional study involving 50 samples of food served with prescribed diet by clinical nutrition specialists at Sanglah Hospital selected by consecutive sampling. Data were collected using observation sheets filled by surveyors and analyzed using Bland Altman Test. Results: There was compatibility of the contents of energy and protein in the provision of medical nutrition therapy of diet prescription with food labelling (energy r=0.43; p=0.04, protein r=0.48; p=0.02), food labelling with food portion (energy and protein r=1.00; p=0.00), food portion with the food serving (energy r=0.48; p=0.03, protein r=0.50; p=0.01), and from diet prescription until the food is served (energy r=0.52; p=0.04, protein r=0.57; p=0.02). Although there is a strong agreement, there is mean value difference which is clinically acceptable. Conclusion: Energy and protein content in the prescribed diet until the food is served at Sanglah Hospital were compatible. The medical nutrition therapy is appropriate and should be evaluated continuously to improve the quality of services at the tertiary level.

Abstract

Tekanan darah tinggi atau hipertensi adalah suatu kondisi dimana tekanan darah atau kekuatan darah yang mengalir pada pembuluh darah terlampau tinggi. Kualitas hidup pasien hipertensi tergantung pada tekanan darah, kerusakan organ, penyakit penyerta dan terapi yang dilakukan oleh pasien. Menilai kualitas hidup pasien hipertensi adalah masalah penting karena merupakan masalah utama bagi pasien, tenaga kesehatan, dan pemegang kebijakan. Tujuan dari studi ini untuk mengetahui gambaran kualitas hidup pasien hipertensi di Puskesmas I Denpasar Barat. Penelitian ini merupakan studi deskriptif dengan metode cross-sectional (potong lintang ). Teknik pengambilan sampel pada studi ini dilakukan secara consecutive sampling, yakni subyek penelitian diambil dari pasien yang melakukan kontrol rutin di Puskesmas I Denpasar Barat dan memenuhi kriteria penelitian hingga jumlah subyek yang diperlukan terpenuhi. Penelitian dilaksanakan pada bulan Agustus 2020 – Oktober 2020. Terkumpul sebanyak 45 sampel, dari 8 domain penilaian kualitas hidup berdasarkan kuisioner SF-36 menunjukkan bahwa seluruh domain memiliki skor rata rata kualitas hidup baik, yakni fungsi fisik (78,8), peran fisik (75), rasa nyeri (75,2), kesehatan umum (58,6), vitalitas (67,4), fungsi sosial (91,5), peran emosi (79,3), kesehatan mental (82,8). Sementara berdasarkan komponen kualitas hidup, komponen mental memiliki skor lebih tinggi dibandingkan komponen fisik dengan rata rata 80,3. Kualitas hidup paling tinggi ditemukan pada domain kesehatan mental dengan persentase responden yang memiliki kualitas hidup baik sebanyak 97,8%. Kata kunci: Hipertensi, Kualitas Hidup, SF-36

Abstract

Kebugaran fisik adalah kemampuan tubuh melakukan tuntutan tugasnya tanpa mengalami kelelahan yangisignifikan. Kebugaran yang baik penting dimiliki sumberidayaimanusiaiuntuk dapatibekerjaisecaraimaksimal. Penelitianiiniibertujuan untuk mengetahuiipengaruhiolahraga aerobik dan status gizi terhadap kebugaran fisik mahasiswa. Penelitian ini merupakan studi cross-sectional dengan populasi terjangkau mahasiswa program studi sarjana kedokteran Universitas Udayana angkatan 2016-2018. 260 responden dipilih dengan teknik proportional simple random sampling berdasarkan absen dari kelas yang berkaitan yaitu 86 mahasiswa dari angkatan 2016, 84 mahasiswa dari angkatan 2017, dan 90 mahasiswa dari angkatan 2018. Hubungan antar variabel dianalisis dengan ujiiChi-Square. Variabel olahraga dihitung berdasarkan kuesinoer Baecke, status gizi berdasarkan massa index tubuh, dan kebugaran berdasarkan denyut nadi. Didapatkan hubunganiyang signifikaniantara olahraga aerobik dan kebugaran fisik (pi<0,001) dan didapatkan hubungan yang tidak signifikan antar status gizi dan kebugaran fisik dimana ada 2 perbandingan pada statusigizi yaituistatus giziikurang dibandinginormal (p=0,742) dan status giziilebih dibanding normal (p=0,215) dimana status gizi normal adalah dari rentang 18,5-24,99. Dapat disimpulkan olahraga aerobik berpengaruh terhadap kebugaran fisik, sedangkan status gizi tidak. Kata Kunci: Olahraga Aerobik., Status Gizi, Kebugaran

Abstract

Definisi sehat ialah suatu keadaan sehat yang baik, baik fisik, mental maupun sosial dan bukan sekadar tidak adanya penyakit. Tujuan dari WHO ialah tercapainya tingkat kesehatan yang paling tinggi oleh semua orang. Salah satu faktor yang berhubungan dengan kesehatan adalah tingkat kebugaran jasmani. Kebugaran jasmani menurut Sutarman, adalah aspek fisik dari kebugaran yang menyeluruh (total fitness), yang memberikan kesanggupan kepada seseorang untuk menjalankan hidup yang produktif dan dapat menyesuaikan diri pada tiap-tiap pembebanan fisik (physical stress) yang layak. Kebugaran jasmani dipengaruhi oleh aktifitas fisik yang dilakukan. Penelitian ini menggunakan rancangan penelitian menggunakan metode studi analitik dengan pendekatan cross-sectional menggunakan data primer. Responden merupakan Mahasiswa Program Studi Sarjana Kedokteran dan Profesi Dokter Fakultas Kedokteran Universitas Udayana Tahun Ajaran 2019/2020. Sampel diambil dengan convenience sampling, dan terkumpul sebanyak 40 responden yang memenuhi kriteria inklusi dan ekslusi. Pada penelitian ini, responden dengan usia 21 tahun sebesar 18 mahasiswa (45%), tingkat aktivitas fisik dalam kategori rendah sampai sedang sebesar 38 mahasiswa (82,5%), tingkat kebugaran jasmani jasmani dalam kategori kurang sampai sedang sebanyak 33 mahasiswa (82,5%). Berdasarkan hasil analisis secara bivariat dari 40 responden, didapatkan p= 0.357, r=0.131. Simpulan dari penelitian ini ialah tidak ditemukan hubungan signifikan antara aktivitas fisik dengan kebugaran jasmani. Hasil penelitian ini dapat digunakan sebagai bahan dasar untuk melakukan penelitian lebih lanjut mengenai hubungan antara akvitas fisik dengan kebugaran jasmani.
