Abstract

From normal epithelial cells to invasive cervical cancer take a long time. It needs a step-by-step process. Normal cervical cells may be infected with the high-risk human papillomavirus (HPV). The infection may persist or be cleared. The HPV persistence may change normal cells to invasive cancer. Recent findings showed that the intrauterine device (IUD) might have a protective effect on cervical cancer. Inflammation in the cervix induces the immune response that may alter normal cells' progression to cervical cancer. This article will explain the change from normal cells to invasive cervical cancer, the role of the IUD in cervical cancer prevention.

Abstract

BACKGROUND: Diabetic micro and macroangiophathy lead to the incident of diabetic foot ulcers characterized by an increased number of circulating endothelial cells (CECs) and decreased function of endothelial progenitor cells (EPCs). This fact is correlated with ischemia and diabetic wound healing failure. Aloe vera gel is known to be able to stimulate vascular endothelial growth factor (VEGF) expression and activity by enhancing nitric oxide (NO) production as a result of nitric oxide synthase (NOS) enzyme activity. Aloe vera is a potential target to enhancing angiogenesis in wound healing.OBJECTIVE: The objective of this study was to explore the major role of Aloe vera gel in wound healing of diabetic ulcers by increasing the level of EPCs, VEGF, and endothelial nitric oxide synthase (eNOS), as well as by reducing the level of CECs involved in angiogenesis process of diabetic ulcers healing.METHODS: The experimental groups was divided into five subgroups consisting of non diabetic wistar rats, diabetic rats without oral administration of aloe gel, and treatment subgroup (diabetic rats) with 30, 60 and 120 mg/day of aloe gel doses for 14 days. All subgroups were wounded and daily observation was done on the wounds areas. Measurement of the number of EPCs (CD34), and CECs (CD45 and CD146) was done by flowcytometry, followed by measurement of VEGF and eNOS expression on dermal tissue by immunohistochemical method on day 0 and day 14 after treatment. The quantitative data were analyzed by One-Way ANOVA and Linear Regression, with a cofidence interval 5% and significance level (p<0.05) using SPSS 16 software to compare the difference and correlation between wound diameters, number of EPCs and CECs as well as the levels of VEGF and eNOS.RESULTS: The results of this study showed that aloe gel oral treatment in diabetic wistar rats was able to accelerate the wound healing process. It was shown by significant reduction of wound diameter (0.27±0.02); the increased number of CECs (0.42±0.57), respectively (p<0.05). On the other hand, the wound diameter and eNOS indicators showed significant differences at the dose of 60 mg, while the number of EPCs and CECs and the level of VEGF showed significantly different results at a dose of 120 mg. Aloe gel oral therapy showed a positive indication of wound healing acceleration at the optimum dose range 60-120 mg a day.CONCLUSIONS: Aloe gel is potential to be a herbal therapy candidate for diabetic wound healing through enhancing EPCs homing, decreasing the CECs number, and stimulating the increase of VEGF and eNOS levels,hence proving to be a dominant factor in the angiogenesis process.KEYWORDS: aloe gel, diabetes, wound healing, angiogenesis

Abstract

Background. Atherogenesiswas initiated by cholesterol deposits on foam cell in sub intimae of blood vessel stress oxidation. Atherogenesis in non- hypercolesterolemiausually undergoes an increased level of Lp-PLA2. It, therefore, needs to evaluate the role of Lp-PLA2in the foam cell formation. Aims. To explain the role of Lp-PLA2 in the foam cell forming, to correlate the level of Lp-PLA2 ,MDA, F2-Isp in aorta with foam cell number (FCs) as well as to correlate the level of Lp-PLA2 , MDA, F2-Isp contents in serumwith their contents in aorta and the correlation with FCs.Methods. 30 rats aged 2 months, with their weight averaging from 150-200g,were divided into the control group and the treatment one where the latterwas fed hyperlidemia for 2,8 and 12 weeks. The measurement level of LDL-C,MDA, F2-isp and Lp-PLA2in serum was performedas well as aorta and FCs. Data was analysed using anova, t-Test, path analysis and correlation. Results. Research result indicated that: (1) The level of MDA(a), F2-Isp(s) and Lp-PLA2(s) positively correlated with FCs, Lp-PLA(s)having the highestcorrelation value. (2) Lp-PLA2(a),MDA(s) and F2-Isp(s,a)did not correlate withFCs. (3) There was a positive correlation between Lp-PLA2 with MDA andF2-Isp in both serum and aorta.Conclusion. The enzyme of Lp-PLA2 acts as an activator in forming thefoam cell with stimulated stress oxidation.

Abstract

Background. Hypercholesterolemia is a major classic risk factor for cardiovascular disease, but there are 35%-40% cases of cardiovascular patients have a normal cholesterol levels. Lp-PLA2 is an enzyme that produced and secreted by macrophages as a response to the lipid peroxide formation, especially the platelet activating factor compound and phosphocholine peroxide. Lp-PLA2 is correlated with classic risk factor of cardiovascular disease, although that correlation with number of foam cell at early stage of atherosclerosis is not clear yet. This study aims to determine whether Lp-PLA2 levels correlated with classic risk factors of atherosclerosis and the number of foam cell, and the role of Lp-PLA2 enzyme in foam cells formation.Methods. This study observes the change of Lp-PLA2, F2-Isp, MDA, TC, LDL, HDL levels in rat serum at 3 levels of early atherogenesis, Ath-I, Ath-II and Ath-III were made on the number of foam cells. The number of cells was observed on all aortic cross sectional surfaces, using the Oil-Red-O staining. The LDL-C content was measure using the Fiedwall formula, MDA content was measure by using TBA-test, the observe of F2-isoprostane and Lp-PLA2 followed the procedure Elisa methods. Results. Anova test results among the 3 initial atherosclerotic levels showed a very significant difference (p<0.01) on Lp-PLA2 plasma content. The LSD test results represented an increase in Lp-PLA2 enzyme levels significantly since AthII stage. Path analysis refers that correlation value between the Lp-PLA2and the number of foam cell (r=0.48) were higher than that of the LDL (r=0.42), was neither correlated with MDA nor F2-Isp, the highest correlation occurred between Lp-PLA2 and LDL compared to the others parameters (r = 0.58). Path analysis also showed no correlation between cell numbers with MDA and F2-Isp, but LDL levels are correlated significantly with of oxidative stress markers MDA levels (r = 0.32) and correlated very significantly with F2-Isp (r = 0.69).Conclussion. It can be concluded that elevated levels of Lp-PLA2 increase atherogenecity of LDL, due to increased inflammation, stress oxidation and elevated levels of Lp-PLA2 itself, wich are interconnected with proatherogenic loops.

Abstract

Introduction: Endometrial cancer is a malignant tumor of primary endometrial epithelium that placed as the sixth most common cancer in women worldwide. The exact cause of it is still unknown although there are several risk factors has been well studied and the incidence rate is increasing every year. The aim of the study is to describe the profile of endometrial cancer patients in third referral hospital in Surabaya based on known risk factors.Methods: We collected data retrospectively through the medical record of outpatients with endometrial cancer diagnosis from January-December 2016. A total of 120 patients were collected and only 95 patients who meet the inclusion criteria. Data about age, age of menarche, status of menopause, parity, Body Mass Index (BMI), history of disease, and family history of cancer were extracted then presented descriptively.Results: From 95 patients, the largest distribution were age 51-60 years old (47,37%), and was diagnosed mostly in women with parity ≤2 (47,37%), postmenopausal status (61,05%), age of menarche ≥ 12 (85,26%), and normal weight (35,79%). Most of patients didn’t have either any history of disease (57,89%) or family history of cancer (89,47%).Conclusion: Most of the endometrial cancer patients in Dr. Soetomo General hospital Surabaya were women aged 51-60 years old with postmenopausal status, age of menarche ≥ 12 years old, number of parities ≤2, and normal weight. The majority of them were also found to have no history of disease and family history of cancer.

Abstract

Cancer patients in Indonesia experience mild to severe depression around 34.4% which can affect their quality of life. The study aimed to determine the distribution of depression in cervical cancer patients, the level of depression that predominated in cervical cancer patients, and the relationship between the level of depression in cervical cancer and the influencing factors in cervical cancer patients RSUD Dr. Soetomo. The research design used descriptive-analytic with a research population of 2845 people. The research sample was determined using a descriptive formula and was taken in September 2021 from outpatients at the Poli Onkologi Satu Atap (POSA), who was diagnosed with cervical cancer were 45 people by distributing questionnaires containing BDI-II and Holmes and Rahe Scale instruments. The sampling technique used non-probability sampling with the purposive sampling technique. Analytical descriptive data analysis using Chi-square test. The results showed that the majority had mild depression as many 21 respondents (46.7%), followed by minimal depression as many 19 respondents (42.2%), moderate depression as many three respondents (6.7%), and severe depression as many two respondents (4.4%). The relationship between the level of depression with influencing factors was found to be significant in the number of marriages. The second marriage does not always show better results than the first marriage, the fear of repeated marriage failures and the emergence of conflicts in the future can potentially lead to depression. Women with second marriages have higher rates of depression. In the relationship between depression and other factors, namely age, last education, marital status, monthly income, cervical cancer stage, family history of cervical cancer, and duration of diagnosis, did not have a significant relationship.

Abstract

Wanita hamil merupakan populasi yang rentan terdampak penyakit COVID-19. Kehamilan dengan COVID-19 memiliki peningkatan risiko untuk terjadi penyulit seperti persalinan preterm. Data per Januari 2021 di RSUD dr. Soetomo Surabaya, menunjukkan kasus kehamilan dengan COVID-19 telah mencapai 140 kasus, di mana 85% adalah warga Surabaya. Tujuan kegiatan ini adalah untuk mengetahui tingkat pengetahun masyarakat terkait COVID-19 pada kehamilan di Kota Surabaya. Metode yang digunakan dalam kegiatan ini adalah penyuluhan digital menggunakan Whatsapp secara individual dilengkapi video kepada kelompok ibu hamil di wilayah kerja Puskesmas Mulyorejo Kota Surabaya. Video berisi materi mengenai kesehatan ibu hamil dan janin di masa pandemi COVID-19, protokol Kesehatan untuk ibu hamil, serta cara deteksi dini COVID-19 pada kehamilan dan kegawatannya. Selanjutnya dilakukan intervensi secara daring dengan peserta mengisi google form pre-test dan post-test sebagai indikator peningkatan wawasan peserta. Hasil dari kegiatan pelaksanaan ini adalah terdapat pengaruh peningkatan pengetahuan ibu hamil mengenai pentingnya menjaga kesehatan kehamilan di masa pandemi COVID-19.

Abstract

Tujuan: Mengetahui dan membandingkan sensitivitas, spesifisitas, Positive Predictive Value, dan Negative Predictive Value antara HE4, CA-125, serta kombinasi HE4 dan CA-125 sebagai tumor marker pada pasien kanker ovarium tipe epitel. Mengetahui adanya korelasi antara kadar HE4 dengan kadar CA-125 serum pada pasien tumor ovarium.Metode penelitian dan tempat: Penelitian ini merupakan studi observasional analitik dengan cross-sectional, dilaksanakan di poli, ruang kandungan, dan Laboratorium Patologi Klinik RSU dr. Soetomo, sert laboratorium Prodia. Sampel dan alur penelitian: Terdapat 53 pasien dengan tumor ovarium yang dioperasi di RSU dr. Soetomo antara Februari sampai Mei 2011 yang memenuhi kriteria inklusi dan ekslusi. Terhadap masing-masing pasien dilakukan pemeriksaan HE4 dan CA-125 pada 1 hari sebelum operasi. Setelah hasil PA selesai, didapatkan 11 pasien yang mengalami drop out, 42 pasien lainnya kami masukkan dalam analisa penelitian dengan rincian 17 pasien kanker ovarium tipe epitel dan 25 pasien dengan tumor jinak ovarium. Peneliti berusaha mengetahui dan membandingkan sensitivitas, spesifisitas, Positive Predictive Value, dan Negative Predictive Value antara HE4, CA-125, serta kombinasi HE4 dan CA-125 sebagai tumor marker pasien kanker ovarium tipe epitel.Hasil penelitian: Analisis statistik Mc Nemar menunjukan kadar CA-125 serum memiliki perbedaan yang bermakna dengan hasil PA (p = 0,004), sedangkan kadar HE4 serum (p = 1,000) dan kombinasi kadar HE4 & CA-125 serum (p = 0,344) tidak memiliki perbedaan yang bermakna dengan hasil PA dalam menentukan diagnosis kanker ovarium. Spesifisitas kombinasi HE4 dan CA-125 (88,00%) lebih baik dari HE4 (80,00%) maupun CA-125 (20,00%). Sensitivitas HE4 (76,47%) lebih baik dari CA-125 (70,59%) maupun kombinasi HE4 dan CA-125 (58,82%). Positive predictive value kombinasi HE4 dan CA-125 (76,92%) lebih baik dari HE4 (72,22%) maupun CA-125 (37,50%). Negative predictive value HE4 (83,33%) lebih baik dari kombinasi HE4 dan CA-125 (75,86%) maupun CA-125 (50,00%). Uji korelasi dari Pearson menilai semua pasien sampel tumor ovarium mendapatkan adanya korelasi kadar HE4 serum dengan CA-125 serum, nilai r = 0,453 (p = 0,003).Kesimpulan: Kombinasi HE4 dan CA-125 memiliki nilai diagnostik lebih tinggi dibandingkan dengan HE4 tunggal dan CA-125 tunggal sebagai tumor marker kanker ovarium tipe epitel. Terdapat korelasi antara kadar CA-125 serum dengan kadar HE4 serum pada pasien tumor ovarium, walaupun tidak ada hubungan patofisiologi antara kedua tumor marker tersebut.Kata kunci: HE4, CA-125, sensitivitas, spesifisitas, positive predictive value, negative predictive value, kanker ovarium tipe epitel, tumor jinak ovarium

Abstract

In advanced ovarian cancer, the main goal of surgery is complete debulking with no visible residual tumor, and predicting it before surgery has become a goal that still unresolved. The ability in advanced ovarian cancer surgery varied between healthcare facilities. Recent discovery put forward the role of fatty acid as one of the main source of energy for ovarian cancer aggresiveness and the role of fatty acid binding protein (FABP) escpecially FABP4 expression as an important biomarker for predicting residual disease in ovarian cancer. There are no data about FABP4 serum in ovarian cancer and it’s role in predicting the extensiveness of advanced stage ovarian cancer. To evaluate correlation between FABP4 serum level with PIV Fagotti Score and Peritoneal Carcinomatosis Index (PCI) in advanced stage ovarian cancer. Analytic observational study. Among 28 subjects, most age groups (46.4%) are 40–45 years of age, 46.4% with normal body mass index, 71.4% with ovarian cancer stage IIIC, 75% with high grade serous adenocarcinoma, average ascites volume 3232.1 ± 2006.9 mL, median CA125 serum 1094 u/mL (12–19425 u/mL). The rate of optimal and complete debulking is 53.5% from 28 subjects. Strong expression of FABP4 on 42.9% subjects. MeanFABP serum 69.6 ± 51.4 ng/mL. Mean PCI score is 14.5 (3–29), mean PIV score is 6 (2–12). There is a significant correlation between FABP4 expression and FABP4 serum level (p<0.05) with moderate power (r=0.55), and a significant correlation with moderate power (r=0.421) between FABP4 serum level with PCI and PIV score. There is a significant correlation with moderate power between rising FABP4 serum and the extent of peritoneal carcinomatosis evaluated by PCI and PIV Fagotti Score in advanced stage ovarian cancer.ABSTRAKPrediksi resektabilitas pada kanker ovarium epitel masih merupakan masalah penting yang belum terpecahkan. Penelitian-penelitian mutakhir mengemukakan peran dari asam lemak sebagai salah satu sumber energi penting bagi progresivitas kanker ovarium. FABP4 sebagai salah satu protein yang berperan dalam transportasi asam lemak merupakan salah satu biomarker yang dijumpai pada kanker ovarium dengan residu tumor. Akan tetapi, penelitian mengenai kadar serum FABP4 pada stadium lanjut belum ada. Selain itu, apakah PCI dan PIV skor Fagotti mempunyai hubungan dengan kadar serum FABP4 pada kanker ovarium stadium lanjut belum pernah diteliti. Penelitian ini bertujuan untuk menilai korelasi antara PIV skor Fagotti dan PCI dengan kadar serum FABP4 pada penderita kanker ovarium stadium lanjut. Observasional analitik digunakan dalam penelitian ini, di mana peneliti ingin melihat korelasi antara nilai skor PIV skor Fagotti dan PCI dengan kadar serum FABP4. Subjek penelitian berjumlah 28 orang dengan karakteristik: kelompok usia terbanyak 40–45 tahun (46,4%); indeks massa tubuh normal (46,4%); stadium FIGO IIIC (71,4%); high grade serous ovarian cancer (75%); jumlah rerata asites 3232,1 ± 2006,9 mL; median kadar serum CA125 1094 u/mL (12–19425 u/mL); 53,5% subjek dapat dilakukan sitoreduksi optimal dan komplit. Ekspresi FABP4 yang kuat terjadi pada 42,9% subjek. Rerata kadar serum FABP4 69,6 ± 51,4 ng/mL. Korelasi signifikan antara ekspresi FABP4 dengan kadar serum FABP4 pada penelitian ini (p<0,05), dengan tingkat korelasi sedang (r=0,55). Rerata PCI total 14,5. Rerata PIV adalah 6 dengan nilai minimum 2 dan maksimum 12.Didapatkan korelasi signifikan (p<0,05) antara kadar serum FABP4 dengan penilaian PCI dan PIV dengan tingkat korelasi sedang (r=0,421, r=0,458). Pada penelitian ini ditemukan korelasi signifikan dengan kekuatan korelasi sedang antara peningkatan skor PIC dan PIV skor Fagotti dengan peningkatan kadar serum FABP4 pada pasien kanker ovarium stadium lanjut.

Abstract

Background: Endometrial cancer is the sixth most common malignancy incidence in women worldwide. The surgical staging in endometrial cancer management such as bilateral salpingooophorectomy and lymphadenectomy is not a routine procedure for all endometrial cancer patients. Therefore, a biomarker to estimate the presence of endometrial cancer metastases to adnexa and lymph nodes is needed. This study aims to find out the level of cancer antigen 125 (CA-125) in endometrial cancer endometrioid type and the relationship between the level of CA-125 and the incidence of adnexal metastases, lymph node metastases, clinical stage, and myometrial invasion.Methods: This observational analytic study used a direct observational design. The in-vitro quantitative examination was performed to measure CA-125 levels using CA-125 Advia Centaur CA-125 Kit. Fischer exact test used to analyzed CA-125 levels with the subject characteristics. All the statistical analyses were performed using SPSS v. 21.0.Results: Of the 30 research subjects with the characteristics of the most age groups of 51–60 years (60%), with the most multiparous parity (73.3%), grade III (43.3%), myometrial invasion > 50% by 66.7%, early-stage (63.3%), the incidence of adnexal metastases (30%), and the incidence of lymph node (LN) metastases (10%), a significant relationship between CA-125 levels and lymph node (LN) metastases was found with a p = 0.041 and the phi association of 0.016 and between CA-125 levels and adnexal metastases with a p = 0.035 and the phi association of 0.026. There was also a significant relationship between CA-125 levels and the clinical stage (advanced stage) with p = 0.004 and the phi association of 0.002. There is no significant relationship with a p = 0.702 and a phi association value of -0.098.Conclusions: CA-125 can be used as a tumor marker to predict the presence of extrauterine metastases (adnexa & lymph nodes).
