Abstract

Highlights: 1. The major risk factors for type I endometrial cancer, namely age and obesity, play a major role in the increase in this case and are thought to be related to the grade in these cancer patients.2. Type I endometrial cancer patients at Dr. Soetomo General Academic Hospital 2019-2020 are dominated by patients diagnosed at the age of 56-65 years, having an overweight body mass index (23-24.9 kg/m2).3. The patient's age and body mass index did not correlate with endometrial cancer type I grade. Abstract Background: The increase in obesity and life expectancy has contributed to type I endometrial cancer cases worldwide. Increased risk factors play a role in the increase of these cases. Objectives: To determine the relationship between age and BMI with the cancer grade. Material and Method: This research was a cross-sectional study. The data were obtained from the patient’s medical records. The sampling technique was total sampling. The analysis used was Spearman Rho correlation test for the relationship between age and BMI with cancer grade. Results: This study recorded 54 patients with type I endometrial cancer in Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, from 2019 to 2020. The patients were from the age group 56-65 years 25 patients (46.30%), 46-55 years 17 patients (31.48%), 36-45 years 8 patients (14.81%), 26-35 years 2 patients (3.7%), and >65 years 2 patients (3.7%). For BMI, the patients were overweight (23-24.9 kg/m2) 21 patients (38.89%), normal (18.5-22.9 kg/m2) 14 patients (25.93%), obesity (25-29.9 kg/m2) 12 patients (22.22%), and obesity II (≥30 kg/m2) 7 patients (12.96%). For grade, grade III were 22 patients (40.75%), grade II  20 patients (37.04%), and grade I 12 patients (22.22%). There was a weak, insignificant positive correlation between age with grade (ρ=0.116, 0.405>α=0.05) and a weak, insignificant negative correlation between BMI with grade (ρ=-0.206, 0.135>α=0.05). Conclusion: A total of 54 patients with type I endometrial cancer at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, from 2019-2020 aged 56-65 years and overweight (23-24.9 kg/m2) with grade III cancer. The patient's age and BMI did not correlate with the patient's grade.

Abstract

Twin pregnancy with Hydatidiform Mole is a rare case, only about 1 in 22.000 to 100.000 pregnancies. Molar pregnancy with triplet pregnancy is even rarer, which is only 6 cases reported and mostly occurred in women who received therapy for infertility. Management in cases of twin pregnancy with hydatidiform mole is dilemmatic both for the patient and physician. We present a case of 29 years old woman with her first pregnancy of twin fetuses complicated with hydatidiform mole. Diagnostic approaches were made mainly by ultrasound examination, continued with laboratory and radiology examinations which some did not performed due to worsened vaginal bleeding. This case ended with premature labor with the result of delivery of two babies and placenta with vesicles which is a characteristic of hydatidiform mole.

Abstract

Background: In 2019 there were 13,078 cases of cervical cancer in East Java, and in 2018 the prevalence reached 2.2 per 1,000 population. However, the coverage of HPV (Human Papilloma Virus) vaccination as primary prevention of cervical cancer in Indonesia is still low. The purpose of this study was to analyze the influencing factors and the most influencing factors for female midwifery students in Human Papilloma Virus vaccination. Subjects and Method: This was an observational analytic study with a cross sectional design. The study was conducted in East Java. The study population was midwifery students. A sample of 252 midwifery students in East Java was selected by consecutive sampling. The dependent variable was HPV vaccination. The independent variables are attitudes and family support. The data were collected by online questionnaire form and analyzed by a multiple logistic regression. Results: Family support increased the likelihood of HPV vaccination in midwifery students (OR= 5.03; CI = 1.83 to 13.76; p= 0.002). Conclusion: Family support increased the likelihood of HPV vaccination in midwifery students.

Abstract

HIGHLIGHTS This study aimed to identify the characteristics of Gestational Trophoblastic Neoplasia (GTN). GTN is chemosensitive, but without appropriate therapy and follow-up, GTN will develop into complications and fatalities.   ABSTRACT Objective: Gestational Trophoblastic Neoplasia (GTN) is a pregnancy-related malignancy due to abnormal proliferation of trophoblastic tissue. This study aimed to identify the characteristics of patients with GTN to help diagnose cases of GTN earlier and provide better treatment. Materials and Methods: This was a descriptive retrospective study on medical records of patients with GTN in Dr. Soetomo General Academic Hospital Surabaya, Indonesia, during the period of January 2018 to December 2020 with a total sampling technique. There were 41 patients with GTN included as study subjects.  Results: Forty-one cases of GTN met the inclusion criteria out of the fifty medical records collected. The majority of patients aged 21 – 30 years old (34%) and had parity status without data (42%). Regarding the clinical profile based on prognostic factors, the predominant patients (71%) also had no data about the time interval between the end of the last pregnancy and the first time diagnosed by GTN, Those with more than 100,000 mIU/ml of beta-hCG levels were 32%, and those without metastases were 41.5%. Most patients belonged to the low-risk group (49%) and received chemotherapy (71%) with the MTX LD regimen (69%).  Conclusion: GTN occurred predominantly in reproductive women that belonged to the low-risk group. Furthermore, chemotherapy is one of the chosen therapy for those patients.

Abstract

HIGHLIGHTS Squamous cell carcinoma majorly covered in histopathologic of the records, while adenosquamous followed second. Most of the subjects were referral patients to Dr. Soetomo General Academic Hospital. They were mostly originated from Java outside Surabaya.   ABSTRACT Objective: To identify the distribution of age, histopathology type, clinical stage, treatment type, parity, first complaint, and referral origin of geriatric cervical cancer patients at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. Materials and Methods: This study was a retrospective study observing data from medical records and presented the data descriptively. The medical records were taken from Oncology Outpatient Clinic in Dr. Soetomo Hospital, from 2020-2021, covering geriatric patients aged more than 55 years old. The data were screened and processed. Results: At Dr. Soetomo Hospital, in 2020-2021 there were 228 cervical cancer patients. From 176 patient data that met the inclusion criteria, the average age was 65.38 ± 4.86 years, with 4 types of histopathology dominated by squamous cell carcinoma (82.39%), adenocarcinoma (11.93%), adenoquamous (3.41%) and others (4%), divided into 8 clinical stages and dominated by stages IIIB (77.27%), IIB (15.9%), IVB (2.84%), IIIA and IB had same number (1.14%), 1A (0.57%) and no cases of IIA were found. The treatments were dominated by chemotherapy (86.36%), radiation therapy (7.38%), no treatment (3.41%), hysterectomy (1.7%), while for conization and palliative therapy each in 1 case (0.57%). Most experienced 3 parity (29.5%), followed by 4 parity (17.61%), >5 (13.07%), 5 (10.23%), 1 (6.82%) and no parity (2.27%). The three first complaints were. vaginal bleeding (82.38%), vaginal discharge (46.02%), and pain (82.38%), and the patients were dominated by referrals from Java Island other than Surabaya City (78.40%), outside Surabaya in Java Island as many as 36 referrals (20.45%) and outside Java Island 2 referrals (1.14%). Conclusion: There were 176 geriatric patients with cervical cancer at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, in the 2020-2021 period, dominated by age 56-65 years, the histopathology type of squamous cell carcinoma, stage IIIB patients, most received therapy was chemotherapy, most were multiparous with 3 parities, the majority experienced complaints of vaginal bleeding when diagnosed with cervical cancer, and were dominated by referrals from Java Island outside Surabaya.

Abstract

Highlights: These cases of maternal deaths caused by COVID-19 infections illustrated the significant risk factors for maternal mortality during the early phases of the pandemic, while studies had not extensively reported this. COVID-19 infections increase the risk of maternal and neonatal mortality, with infants having a lower chance of survival even if they are delivered. Respiratory support, antiviral medications, antibiotics, anticoagulants, and supportive care are the primary treatments for severe COVID-19 in pregnancy. AbstractThis article presents seven cases of maternal deaths attributed to COVID-19 during the first year of the pandemic wave. These cases provide insights into the natural progression of COVID-19 in pregnant women who were not vaccinated. This study showed that COVID-19 significantly increased maternal and neonatal mortality and morbidity. All of the patients exhibited symptoms of fever, cough, and dyspnea upon admission to the hospital. They were admitted with elevated respiratory rates (26–32 times/minute) and low oxygen saturation (<95%). Four patients had obesity, while one patient had pregestational diabetes. The COVID-19 diagnosis was established using a rapid antibody or antigen test and chest X-ray, which indicated pneumonia. Medical interventions administered to the patients included antiviral therapy (5 patients), antibiotics (6 patients), and anticoagulants (4 patients). From a total of five babies delivered, four babies were delivered via cesarean section. Two babies were not delivered due to previability and maternal deaths before delivery. The patients passed away within 3–10 days of hospital admission. In conclusion, adequate and early intervention and management of pregnant women infected with COVID-19 are crucial in preventing maternal and neonatal deaths, especially in unvaccinated women.

Abstract

Cervical cancer is the most common malignancy affecting women in developing countries, including Indonesia. In Indonesia, the incidence of cervical cancer is the type of cancer with the highest incidence rate after breast cancer in women. Women who are diagnosed with cancer not only experience the physical burden of illness and treatment, but also have psychological and social needs, including feelings of anxiety, anxiety, uncertainty, and so on. Even though they suffer from a dangerous disease they never give up hope to continue to survive. Moreover, they are more motivated to seek additional health information. This study aims to find out the characteristics of cervical cancer sufferers in seeking additional health information. This study uses a descriptive quantitative method with a purposive sampling technique. Based on this study, most of the cervical cancer patients who came to RSUD Dr. Soetomo were 50-59 years old, with a moderate level of education, and a housewife. Based on the characteristics when seeking additional health information, most of them seek additional information by using oral media. Those who use internet media mostly visit media that has low credibility. After getting the information they needed they felt relieved.

Abstract

Agaricus blazei Murill has been known as a mushroom with medicinal properties, such as its efficacy in maintaining the immune system and other metabolic processes. The main polysaccharide found in A. blazei, is b-glucan. This study aims to quantify and compare b-glucan content in A. blazei compared with the medicinal mushrooms Ganoderma sp. and Pleurotus ostreatus. ?-glucan was extracted from each species using an alkaloid extraction method. The b-glucan content was determined using UV-Vis Spectrophotometer at a wavelength of 220 nm. The data showed that A. blazei contained the highest level of b-glucan, 6.99% (w/w), while the other mushrooms contained less than 2 %. This study obtained that A. blazei contain high levels of b-glucan compared with Ganoderma sp. and Pleurotus ostreatus. Thus, A. blazei has the potential as medicine, especially to maintain the balance of the immune system. To keep the body healthy and balance immune system patient can consume Agaricus blazei Muril.

AbstractIntroduction: Endometrial cancer is one of the most common cancers that affect the female genitalia. This study aimed to find the profile of endometrial cancer patients in Dr. Soetomo General Academic Hospital, Surabaya, from January 2019 to December 2020. Methods: Data were collected from medical records of endometrial cancer patients in Dr. Soetomo General Academic Hospital, Surabaya, from January 2019 to December 2020. There were 154 endometrial cancer patients, and 66 met the inclusion and exclusion criteria. Forty-one of those patients also had curettage grades. Data were extracted from the medical records of those patients.Results: There were 66 samples used in this study. The most common age group of patients was the age group of 51-60 years old (40.91%). The highest parity rate was nullipara (28.79%). The highest body mass index (BMI) group was the normal BMI group (42.42%). The most common type of endometrial cancer was type 1, with endometrioid adenocarcinoma (93.94%). The most common stage of endometrial cancer was stage II (36.36%). The similarity between curettage and post-operative grade was 70.73%. Patients with a history of diabetes mellitus were 6.06%, and patients with a history of hypertension were 12.12%.Conclusion: Most of the endometrial cancer patients in Dr. Soetomo General Academic Hospital, Surabaya, were women in the age group of 51-60 years old, nullipara, with normal BMI, had type 1 endometrial cancer, and on stage II. Most patients had similar curettage and post-operative grades, and only a minority had diabetes mellitus and hypertension histories.

Abstract

Objective : This study systematically reviewed and meta-analysis the prevalence and factors associated with anxiety in pregnant women during the pandemic. Methods : We searched PubMed MEDLINE, Web of Science, Scopus, ProQuest, EBSCO, Science Direct, and Garuda journal databases in July 2021 and updated them in October 2021. All English and Bahasa journal articles from December 2019 were included in the search. We included studies that investigate factors affecting anxiety exclusively in pregnant women. The primary outcome was the prevalence ratio. The secondary outcome was the risk and protective factors as the independent variable. Joanna Briggs Institute Critical Appraisal Tools and RevMan 5.4 were used to do the analysis. Results: After screening 2082 articles, we included 21 studies with 42.177 pregnant women. The pooled prevalence of anxiety was estimated at 28% (95% CI, 23-33.3). We found that 12 of the 21 studies contributed to 8 risks and 1 protective factor significantly in the meta-analysis. Not married/divorced/widowed, monthly income < 780 USD, screen time > 3 hours/day, history of exposure to COVID-19, complications in the current pregnancy, sleep less than 7 hours per day, subjective poor sleep quality, and high perception of vulnerability were risk factors. Meanwhile, the protective factor was trust in the government's official media. Conclusion : There is a significant increase in the prevalence of maternal anxiety during the pandemic. Mental health screening during the antenatal visit must be carried out, and interventions to lower the anxiety level must be planned to prevent further harm. Metode: Pencarian dilakukan pada database jurnal PubMed MEDLINE, Web of Science, Scopus, ProQuest, EBSCO, Science Direct, dan Garuda pada Juli 2021 dan memperbaruinya pada Oktober 2021. Semua artikel jurnal berbahasa Inggris dan Indonesia dari Desember 2019 dimasukkan dalam pencarian. Kami menyertakan penelitian yang menyelidiki faktor-faktor yang mempengaruhi kecemasan secara eksklusif pada wanita hamil. Hasil utama adalah prevalensi. Hasil sekunder adalah faktor risiko dan protektif sebagai variabel independen. Joanna Briggs Institute (JBI) critical appraisal guideline digunakan untuk menilai kualitas penelitian yang disertakan. RevMan 5.4 dan Jamovi 2.0 digunakan untuk melakukan analisis kuantitatif Hasil : Setelah menyaring 2082 artikel, kami menyertakan 21 penelitian dengan 42.177 wanita hamil. Prevalensi kecemasan yang terkumpul diperkirakan sebesar 28% (95% CI, 23-33,3). Kami menemukan bahwa 12 dari 21 studi berkontribusi terhadap 8 risiko dan 1 faktor pelindung secara signifikan dalam meta-analisis. Faktor risiko yaitu kondisi tidak menikah/bercerai/janda, pendapatan bulanan < 780 USD, waktu layar > 3 jam/hari, riwayat paparan COVID-19, komplikasi pada kehamilan saat ini, tidur kurang dari 7 jam per hari, kualitas tidur subyektif buruk, dan tingginya persepsi kerentanan. Sedangkan faktor protektif adalah kepercayaan terhadap media resmi pemerintah. Kesimpulan : Terdapat peningkatan prevalensi kecemasan ibu yang signifikan selama pandemi. Skrining kesehatan mental selama kunjungan antenatal harus dilakukan, dan intervensi untuk menurunkan tingkat kecemasan harus direncanakan untuk mencegah bahaya lebih lanjut.
