Abstract

Background: A policy restricting the use of vancomycin was a pilot project for the implementation of the Antimicrobial Resistance Control Program (ARCP) in RSUD Nusa Tenggara Barat in 2018. There were three phases of the ARCP in hospitals, namely preparation, execution, and monitoring. This research aimed to evaluate the quality of vancomycin prescribing before, during, and after the implementation of ARCP in 2017–2019. Method: Descriptive analysis was conducted of all the vancomycin prescriptions before, during, and after the implementation of ARCP in 2018. All medical records of cases requiring vancomycin prescriptions during the research period was evaluated: patient characteristics, culture test results, and clinical diagnoses. The quality of vancomycin prescriptions, which was expressed as follows: “Vancomycin prescriptions are intended only for infections caused by Gram-positive pathogens, particularly Staphylococcus aureus, that are resistant to methicillin (MRSA), Enterococcus sp (Vancomycin-sensitive enterococci)” was evaluated. Results: There were sixty-one cases of vancomycin prescribing; 21 female and 40 male patients, with a mean age of 23 years (0–82 years). Overall, there were 41 positive cultures, 5 negative cultures, and 15 no-cultures. The diagnoses were moderate to severe infections: sepsis, pneumonia, post-surgery infections, CNS infections, low birth weight, septic shocks, and chronic obstructive lung diseases. The urinary tract infection was present as a mild infection. The prescription quality indicators ‘vancomycin is prescribed only for moderate to severe infections’ and ‘prescribed only for Gram-positive pathogens’ altogether increased. Conclusion: There was improvement in the vancomycin prescription quality after the implementation of the vancomycin restriction policy.

Abstract

Background: Negative pressure room is recommended for the treatment of COVID-19 patients. Aim this study to describe physical measurement analysis of isolation room Universitas Mataram Teaching Hospital. Methods: Newly developed negative pressure isolation room was physical measure using following instruments: anemometer, moisture meter, hygrometer and pressure gauge.  Results: This study showed physical measurement as follow: 1) ACH (air change per hour) 23.3 / hour [minimum: 12+ ACH]; 2) the difference in pressure gradient between the inpatient room and anteroom -30 Pa [minimum -15 Pa]; 3) the mean of air temperature 24.8°C [21-24]; 4) air humidity 58% [maximum 65%] and 5) concrete moisture 22.45%. Conclusion: The COVID-19 isolation room at the Universitas Mataram Teaching Hospital meets the standard criteria.

Abstract

Antisipasi dalam menghadapi ancaman EREIDs perlu dilakukan sebagai upaya pengurangan dampak buruk bagi penduduk. Sebagai tempat berkumpulnya orang banyak dalam jangka waktu lama, kapal penumpang merupakan salah satu tempat berisiko menjadi hotspot penularan EREIDs yang efektif. Simulasi kesiapsiagaan EREIDs bertujuan untuk penyajian model EREIDs dipelabuhan dan diskusi perbaikan model EREIDs. Pelaksanaan simulasi dilakukan secara hybrid mengundang Pelindo, KSOP, BKK dan operator kapal dengan materi: kesan data kesiapsiagaan EREIDs di Lembar, diskusi tupoksi masing-masing stakeholder dan menggali saran perbaikan kedepannya. Penyajian model EREIDs meliputi: 1) pengendalian teknis (1.1. fasilitas sarpras, 1.2. modifikasi sarpras dan 1.3 penyediaan instrument keselamatan), 2) pengendalian administratif (2.1. penyediaan SOP keselamatan, 2.2 Pelaksana SOP Keselamatan dan 2.3 Pengawasan pelaksanaan SOP keselamatan) dan 3) penggunaan alat pelindung diri (3.1. penyedia APD dan 3.2. Edukasi penggunaan APD). Secara kualitatif didapatkan bahwa peran utama stakeholder masing-masing: 1.1. Pelindo; 1.2. BKK; 1.3. Pelindo; 2.1. BKK; 2.2. Pelindo, Operator Kapal, KSOP; 2.3. BKK, KSOP; 3.1. Pelindo, BKK; 3.2. BKK. Adapun tantangan kesiapsiagaan EREIDs di Pelabuhan dan Kapal adalah koordinasi antar stakeholder, permintaan catatan kejadian, dan rekapitulasi insiden yang masih harus diselaraskan kembali. Dapat disimpulkan bahwa peningkatan kesiapsiagaan EREIDs dipelabuhan dan kapal merupakan penyiapan lintas sector dan lintas bidang intra dan inter stakeholder. Semua stakeholder memiliki peran krusial dalam mencegah masuknya wabah dari luar pulau dan mengirim wabah keluar pulau.

Abstract

Effective mitigation strategies are necessary because high noise levels in maritime work situations frequently represent serious threats to hearing health. Acoustic panels and sound absorbers are examples of sound-dampening materials that are used to lower noise levels; nevertheless, it is unclear how these materials affect employees' awareness and attitudes. The purpose of this study is to investigate how the usage of sound-absorbing materials affects the attitudes and understanding of marine personnel about noise-induced hearing loss (NIHL). The study also covers ship personnel' attitudes toward NIHLs, their understanding of NIHLs, and their hazards. The strategies for increasing ship employees' understanding of and attitudes toward NIHL, the impact of sound-dampening materials on their knowledge and attitudes toward NIHL, and the effectiveness of NIHL awareness and training programs are also covered. While the majority of marine workers are knowledgeable that noise can lead to hearing loss, the review's findings show that their understanding of the causes and long-term consequences of NIHL is insufficient. Diverse perspectives exist regarding prevention, and several employees find it awkward to wear hearing protection. Sound-absorbing materials facilitate better communication and teamwork among employees, which can raise satisfaction at work and safety awareness while lessening the disruption caused by excessive noise. Moreover, employing hearing protection devices (HPPs) can raise staff members' awareness of NIHL and motivate them to utilize HPPs more proactively. These results provide recommendations for more potent preventive measures as well as significant insights into occupational safety and health regulations in the maritime sector.

Abstract

Tonsillitis is a disease that is often found in society and can affect all age groups. However, the most common occurrence is in the children's. Tonsillitis can occur acutely, chronically or chronic acute exacerbation, depending on the duration and frequency of infection. The prevalence of tonsillitis globally shows a fairly high number, especially in developing countries. There are several risk factors that play a role in the high prevalence of tonsillitis in Indonesia including air pollution, poor sanitation, and exposure to infection in dense environments. Public awareness is considered lacking in early detection and treatment of tonsillitis often causes delays in treatment. The purpose of this health education is to provide understanding and increase the knowledge of the community visiting the polyclinic at the NTB Provincial Hospital regarding tonsillitis, its risk factors, symptoms, and how to prevent it. In this educational activity, participants were very enthusiastic about following each explanation of the material presented. Some of the education participants also asked questions about tonsillitis and its complications during the discussion session.This can be used as a benchmark as well as a reference and evaluation material for implementing similar activities in the future.

Abstract

Kesenjangan akses layanan kesehatan di wilayah pedesaan Indonesia, khususnya di daerah pembangunan pariwisata strategis seperti KEK Mandalika, menjadi tantangan serius. Di Desa Kuta, Lombok Tengah, keterbatasan fasilitas kesehatan spesialis seperti poli kandungan, mata, dan THT menyebabkan skrining dan diagnosis dini tidak optimal, yang berdampak pada peningkatan risiko kebutaan, gangguan pendengaran, serta kematian ibu dan bayi. Untuk menjawab permasalahan tersebut, Tim Bantuan Medis (TBM) Bumi Gora Fakultas Kedokteran Universitas Mataram melaksanakan kegiatan bakti sosial pemeriksaan kesehatan gratis bekerja sama dengan ITDC Mandalika dan Puskesmas setempat. Kegiatan ini mencakup pemeriksaan kesehatan umum dan pelayanan poli spesialis, serta edukasi kesehatan kelautan. Sebanyak 100 peserta berpartisipasi, dengan rincian pemeriksaan meliputi poli umum, kandungan, mata, dan THT. Hasil kegiatan menunjukkan peningkatan akses masyarakat terhadap layanan kesehatan, serta kontribusi positif dari kolaborasi lintas sektor dalam mendukung pendekatan Health in All Policies. Kegiatan ini juga memperkuat literasi kesehatan masyarakat dan menjadi model praktik pengabdian berbasis komunitas.

Abstract

Nasopharyngeal cancer is one of the most common types of cancer in Indonesia, especially head and neck cancer. One of the etiology of this cancer is infection with Epstein-Barr virus. Early diagnosis is difficult to establish because the early signs and symptoms of nasopharyngeal carcinoma are not specific. Examination of latent membrane protein-1 (LMP1) oncogene expression and BCL2-associated X (BAX) gene expression proved to be useful in the identification of nasopharyngeal cancer. LMP1 oncogene is an oncogene that functions as a tumor necrosis factor receptor (TNFR), so that apoptosis does not occur and promotes invasion and metastasis. The BAX gene is a pro-apoptotic gene that inhibits cancer development, but its regulation will be downregulated by LMP1 because LMP can protect B-cells from apoptosis.

Abstract

Until now, ENT disease is still a health problem that needs special attention, especially in coastal areas. This can have an impact on various sectors of life both socially, economically, schools and also the quality of life, so it is very necessary to make efforts to improve health status through easy and affordable access to health services. Coinciding with the commemoration of World Cancer Day, ENT Specialists gathered in PERHATI-KL Cab. NTB carried out health social service activities in the western coastal area of ​​Lombok Island. The purpose of this activity is to provide ear health services to the Coastal Communities in Tanjung Karang which include health services in the ENT sector, namely ear cleaning, ENT and head and neck examinations, medication and audiometric examinations. This Social Service activity was attended by approximately 36 people from the community around the Tanjung Karang coastal area. The most dominant health problem was cerumen obturans (36.1%) which occurred in both children and adults, followed by CSOM, Rhinitis, Tinnitus, Pharyngitis, Otitis Externa and others within normal limits. The community is very enthusiastic and welcomes this Social Service activity, because they get direct health services from ENT-KL Specialists and can also consult about various kinds of complaints in the ENT field. The data obtained in this activity can be used as a general description of the health status of coastal communities in the west of Lombok Island.
