Abstract

Background: Hypoxic conditions in preeclampsia cause oxidative stress in the placenta. Oxidative stress triggers endothelial dysfunction resulting in changes in placental parenchyma and vasculature, turn leads to clinical manifestations in maternal and neonatal. Previous studies reported that preeclampsia placental tissue increased the expression of the oxidative stress marker 8OH2dG.Objective: This study aimed to observe the expression of the marker 8OH2dG on the incidence ofpreeclampsia and its association with neonatal outcome. Methods: This study design was case-control, a number of 44 pregnant women participated in this study. The case group was preeclampsia women based on American College of Obstetricians and Gynecologist (ACOG) 2013 criteria and normotensive women as control group. The immunohistochemical was perform in placental tissue for expression of 8OH2dG in trophoblast nuclei. The results were positive when the trophoblast cell nuclei were stained ≥5% with moderate and strong intensity; and negative if stained weakly <5%. Bivariate analysis was performed to determine the association of 8OH2dG expression with preeclampsia and neonatal outcome. Results: This study reported the frequency of trophoblast nuclei with 8OH2dG positive staining was higher in the preeclampsia group than the control group. Expression of 8OH2dG positive staining also increased the risk of low birth weight and asphyxia. Conclusion: The results of this study indicate that the marker 8OH2dG is associated with preeclampsia and adversely neonatal outcome. Keywords: 8OH2dG, preeclampsia, oxidative stress, neonatal outcome

Abstract

Background: Pre-eclampsia (PE) is a hypertensive disorder in pregnancy and a significant cause of maternal and perinatal mortality and morbidity. Failure of spiral artery remodeling due to abnormal apoptosis, triggers disturbances in the mother and the baby’s growth. This study aimed to identify the profile of apoptotic marker genes and histopathological features of the placenta in pregnancies with pre-eclampsia.Methods: This study had used case-control method. Samples were taken from normal pregnancies (n=25) and pregnant women with pre-eclampsia (n=25) using a purposive sampling method from several hospitals in Jambi. qRT-PCR was used to examine apoptotic gene expression from placental tissue and hematoxyline eosin staining to view the placenta’s microscopic appearance. The targeted genes were BCL2-associated X (BAX) and B-cell lymphoma 2 (BCL-2). Histopathological changes of the placenta observed were syncytial node, cytotrophoblast, villous edema, hypervascularization, fibrosis stroma, atherosis, infarction, and thrombosis. Results: Relative BAX genes expression were increased once in placenta pre-eclampsia compared to controls, but not statistically significant (p-value>0.05). There was no difference between the decline of BCL-2 gene expression in pre-eclampsia placenta compared to the control (p-value >0.05). Histopathological changes in the placenta were syncytial node and cytotrophoblast (25 of 25), villous edema (19 of 25), hypervascularization (24 of 25), fibrosis stroma (22 of 25), atherosis (12 of 25), infarction (17 of 25), and thrombosis (24 of 25).Conclusion: The expression of BAX genes in pre-eclampsia tends to increase compared to normal pregnancy, and the expression of BCL-2 decreases.  The histopathological features of pre-eclampsia pregnancy placenta are mostly syncytial nodes, cytotrophoblasts, stromal fibrosis, and thrombosis.

Abstract

Background: Diabetic nephropathy (DN) is one of the frequent complications of type II diabetes mellitus (T2DM) in Jambi province. Controlling blood glucose and blood pressure does not guarantee DN prevention, since genetic factors may also contribute to this disease. Multi-ethnic studies showed that one of the strongest genetic factors associated with DN was single nucleotide polymorphism rs4343 of angiotensin-converting enzyme (ACE) gene. Study regarding phenotype-genotype association of ACE rs4343 and DN has not yet been performed in Jambi Province, which is dominated by Malay ethnicity. This study was conducted to reveal the association between ACE rs4343 and the risk of DN in the Jambi-Malay population.Materials and methods: This was a cross-sectional study involving 75 subjects (44 with DN and 31 without DN) who suffered from T2DM and hypertension. DN was defined as albumin to creatinine ratio (ACR) ≥30 mg/g. Genotyping was performed with one-step tetra amplification refractory mutation system-polymerase chain reaction (PCR) using specific primer for ACE rs4343. Bivariate and multivariate analyses were performed to analyze the genetic risk for DN.Results: The bivariate analysis showed the proportion of DN subjects was higher than non-DN within the AG genotype (11:1) than within the AA (33:30) genotype. This difference was statistically significant (p=0.012; OR (95% CI): 10.00 (1.22-82.15)). Multivariate analysis showed that AG genotype (p=0.047; OR (95% CI): 10.04 (1.03-97.31)) and uncontrolled blood pressure (p=0.001; OR (95% CI): 6.72 (2.08-21.71)) were the risk factors of DN in the Jambi-Malay population. Conclusion: Polymorphism of ACE rs4343 is a risk factor of DN in the Jambi-Malay Population. Keywords: rs4343, angiotensin-converting enzyme gene, diabetic nephropathy, Malay, Jambi
