Abstract

Kapitasi Berbasis Kinerja (KBK) merupakan mekanisme pembayaran kapitasi pada Fasilitas Kesehatan Tingkat Pertama (FKTP) yang disesuaikan dengan capaian indikator kinerja pelayanan kesehatan. Penelitian ini bertujuan untuk mengevaluasi pelaksanaan KBK pada FKTP di Kabupaten Konawe Utara berdasarkan capaian Angka Kontak (AK), Rasio Rujukan Non-Spesialistik (RRNS), dan Rasio Peserta Prolanis Terkendali (RPPT). Penelitian menggunakan pendekatan kuantitatif deskriptif dengan populasi sekaligus sampel berupa seluruh FKTP yang bekerja sama dengan BPJS Kesehatan dan memenuhi kriteria inklusi. Data dikumpulkan pada Oktober 2025 menggunakan data sekunder dari laporan KBK BPJS Kesehatan, yang didukung oleh wawancara terbatas dan observasi. Analisis data dilakukan secara deskriptif melalui perhitungan nilai rata-rata dan persentase capaian indikator. Hasil penelitian menunjukkan bahwa rata-rata capaian AK sebesar 34,79‰, masih jauh di bawah target nasional ≥150‰. RRNS tercatat sebesar 4,08%, melebihi standar ≤2%, sementara RPPT sangat rendah, yaitu 0,12%, dan tidak ada FKTP yang mencapai target ≥5%. Temuan ini menunjukkan bahwa pelaksanaan KBK di Kabupaten Konawe Utara belum optimal, sehingga diperlukan penguatan sistem monitoring dan pencatatan layanan, peningkatan kapasitas sumber daya manusia, serta optimalisasi program promotif dan preventif di tingkat FKTP.

Abstract

Kebijakan Pembayaran Kapitasi Berbasis Kinerja (KBK) diterapkan untuk meningkatkan mutu layanan FKTP melalui penyesuaian kapitasi berdasarkan Angka Kontak (AK), Rasio Rujukan Non Spesialistik (RRNS), dan Rasio Peserta Prolanis Terkendali (RPPT). Namun, capaian indikator di lapangan masih berfluktuasi dan berpotensi menurunkan persentase pembayaran kapitasi. Penelitian ini bertujuan menggambarkan implementasi KBK di Puskesmas Sampara Kabupaten Konawe tahun 2025 berdasarkan capaian indikator utama dan hambatan pelaksanaannya. Penelitian menggunakan desain kuantitatif deskriptif. Populasi adalah seluruh FKTP mitra BPJS Kesehatan di Kabupaten Konawe tahun 2025, dengan sampel diambil menggunakan total sampling pada FKTP yang memiliki data kinerja lengkap. Variabel meliputi AK, RRNS, dan RPPT. Data dikumpulkan dari laporan indikator KBK BPJS Kesehatan, kuesioner terstruktur kepada pengelola FKTP, serta wawancara terbatas; dianalisis secara deskriptif menggunakan Excel dan SPSS. Hasil menunjukkan rata-rata AK 4,71% (tertinggi Desember 6,71%; terendah Agustus 3,65%), RRNS tahunan 17,54% (20 rujukan dari 114 kasus), dan RPPT belum mencapai target (DM 0,91%; HT 2,12%). Total capaian KBK 95,42% dengan penyesuaian kapitasi minor, namun terjadi penyesuaian signifikan pada April dan Agustus. Implementasi KBK tergolong cukup baik, tetapi penguatan Prolanis, konsistensi layanan, dan kualitas pencatatan perlu ditingkatkan agar kapitasi optimal berkelanjutan.

Abstract

Digital health menjadi salah satu strategi penting dalam meningkatkan akses dan mutu pelayanan kesehatan, khususnya di negara kepulauan seperti Indonesia. Penelitian ini bertujuan untuk menganalisis peran digital health dalam meningkatkan akses dan mutu pelayanan kesehatan berdasarkan tinjauan literatur. Metode penelitian menggunakan literature review terhadap artikel ilmiah dan dokumen relevan yang membahas telemedicine, aplikasi kesehatan digital, rekam medis elektronik, Big Data, dan transformasi digital di sektor kesehatan. Hasil tinjauan menunjukkan bahwa digital health, khususnya telemedicine, berkontribusi signifikan dalam memperluas akses pelayanan kesehatan bagi masyarakat di wilayah rural dan kepulauan melalui kemudahan konsultasi jarak jauh, efisiensi waktu, dan pengurangan biaya. Selain itu, penerapan rekam medis elektronik dan pemanfaatan Big Data berperan dalam meningkatkan mutu pelayanan melalui integrasi data pasien, peningkatan akurasi diagnosis, serta pengembangan pengobatan berbasis data. Namun, implementasi digital health masih menghadapi tantangan berupa keterbatasan infrastruktur teknologi, kesenjangan literasi digital, serta isu regulasi dan perlindungan data kesehatan. Secara keseluruhan, digital health memiliki potensi besar sebagai instrumen strategis dalam transformasi sistem pelayanan kesehatan yang lebih merata dan berkelanjutan di Indonesia.

Abstract

Penelitian ini mengevaluasi penerapan Kapitasi Berbasis Kinerja (KBK) di Fasilitas Kesehatan Tingkat Pertama (FKTP) di Kabupaten Kolaka Timur, Indonesia. Skema KBK, sebagai kebijakan utama dalam program Jaminan Kesehatan Nasional (JKN), menyesuaikan pembayaran fasilitas berdasarkan pencapaian tiga indikator layanan utama: Angka Kontak (AK), Rasio Rujukan Non-Spesialistik (RRNS), dan Rasio Peserta Prolanis Terkendali (RPPT). Latar belakang menggarisbawahi peran kunci FKTP sebagai gatekeeper sistem dan penggunaan KBK untuk menjamin mutu layanan serta efisiensi keuangan. Penelitian menggunakan pendekatan deskriptif kuantitatif dengan metode total sampling terhadap seluruh FKTP yang memenuhi kriteria. Data dari Januari dan Agustus 2025, yang mencakup laporan kinerja sekunder, kuesioner, dan wawancara, dianalisis secara deskriptif. Hasil penelitian menunjukkan kinerja yang beragam. RRNS (0,54%) berhasil dikendalikan, mengindikasikan fungsi gatekeeper yang efektif. Namun, tantangan besar teridentifikasi: AK (70,54‰) masih jauh di bawah target 150‰, mencerminkan rendahnya akses layanan atau pencatatan yang kurang optimal, dan RPPT (1,08%) sangat di bawah target 5%, menandakan pengelolaan penyakit kronis yang belum optimal. Pembahasan menghubungkan temuan ini dengan masalah sistemik seperti beban peserta yang tinggi, keterbatasan sumber daya manusia, serta kelemahan dalam pencatatan data dan keterlibatan masyarakat dalam Prolanis. Disimpulkan bahwa meskipun fungsi rujukan berjalan efektif, diperlukan peningkatan signifikan dalam aksesibilitas layanan dan perawatan kronis. Rekomendasi yang diajukan meliputi penguatan sistem pencatatan layanan, peningkatan monitoring rutin multipihak, optimalisasi kegiatan Prolanis berbasis komunitas, serta peningkatan kapasitas tenaga kesehatan untuk memperbaiki hasil pencapaian KBK secara keseluruhan.

Abstract

The digital transformation of health information systems (HIS) is a critical yet persistently challenging endeavor, often failing to deliver promised value due to a narrow, techno-centric focus. This study argues that successful transformation is fundamentally a socio-technical co-optimization problem, requiring simultaneous attention to interdependent technical, organizational, and human dimensions. Through an integrative literature review and conceptual framework analysis, we identify and categorize a multidimensional landscape of barriers, including legacy system inertia, interoperability gaps, fragmented governance, workflow misalignment, clinician resistance, and cultural friction. These challenges are not isolated but are dynamically interconnected, as visualized in a network dependency model, where a flaw in one dimension exacerbates problems in another, creating cycles of failure. To navigate this complexity, we propose a actionable socio-technical framework that translates theory into practice. The framework prescribes integrated strategic enablers: leadership must champion iterative, user-centered design and inclusive governance; technical strategy must adopt open standards and modular architecture; and a foundational culture of psychological safety and continuous learning must be fostered. The findings underscore that sustainable transformation is not a technology installation project but a long-term organizational evolution, demanding a balanced portfolio of initiatives to cultivate an adaptive learning healthcare ecosystem.

Abstract

Introduction: Stunting is a chronic nutritional problem affecting children and remains a major public health issue in Indonesia. The prevalence of stunting in North Konawe Regency was 21.6% in 2022 and increased to 25.6% in 2023. One of the policy efforts implemented by the North Konawe Regency Government is the URC-KISS program (Rapid Response Unit for Extreme Poverty, Inflation, and Stunting). However, the effectiveness of this policy has not yet been evaluated. This study aimed to evaluate the input components of the URC-KISS policy in efforts to reduce stunting rates. Method: This study used a qualitative approach with a case study design. A total of twelve informants were recruited, consisting of two key informants and ten supporting informants. Informants were selected using snowball sampling techniques. Data were analyzed manually through data reduction, data display, and conclusion drawing. Result: The findings show that program inputs include financing provided directly by the local government, adequate human resources with appropriate competencies, and infrastructure such as operational vehicles and anthropometric measurement tools. In addition, program implementation is supported by official guidelines for stunting reduction in North Konawe Regency. Conclusion: The URC-KISS program is supported by adequate inputs, including funding, human resources, infrastructure, and implementation guidelines, which contribute to efforts to reduce stunting rates in North Konawe Regency.

Abstract

Introduction: Benyamin Guluh Regional General Hospital in Kolaka Regency faces various challenges in healthcare service performance, including declining healthcare worker performance, leadership adaptability challenges, and weaknesses in strategic planning and performance measurement systems. This study aims to evaluate healthcare worker performance with a focus on the management dimension of leadership. Method: A qualitative research method with a case study design was applied, utilizing triangulation techniques from 12 informants, including the Director and Head of the Hospital Human Resources Department. Snowball sampling was used in selecting informants, with data analysis conducted inductively. Performance evaluation was based on the Malcolm Baldrige National Quality Award (MBNQA) Method. Result: The study results show that hospital leadership is effective but needs to improve adaptability in crisis situations. The existing strategic plan is clear but lacks flexibility and insufficient involvement of healthcare workers in planning. Customer focus is good, but capacity management needs improvement. The performance measurement system is effective but requires additional training. Conclusion: The need for adjustments in leadership and strategic planning to deal with crisis situations at the Benyamin Guluh Regional General Hospital, Kolaka Regency.

Abstract

Introduction: This study examines patient satisfaction levels at Kolaka Regency Community Health Centers using the American Customer Satisfaction Index (ACSI) model. The background of the study indicated patient satisfaction issues, particularly related to service responsiveness, identified through initial observations at four of the 14 health centers. The main issues included long waiting times, difficulty accessing information, and a lack of effective communication. The study aimed to evaluate patient satisfaction with a focus on the responsiveness of patient care at Kolaka Regency Community Health Centers. Method: This research employed a qualitative method with a case study design, involving 15 informants selected through purposive and snowball sampling techniques. Data collection was conducted through in-depth observation and triangulation, with data analysis using the N-VIVO application. Result: Despite positive aspects such as good communication and competent medical services, several areas still require improvement. Key challenges include long waiting times, the need for facility upgrades in some areas, and the need for efficiency in stock management and service times. Analysis using the ACSI Model revealed variations in patient satisfaction levels across various service aspects, with some community health centers performing better than others. Conclusion: Patient satisfaction levels at Kolaka Regency Community Health Centers vary and require improvement in several areas. Factors such as community health center accreditation, the number of healthcare workers, and the quality of facilities significantly influence patient satisfaction.

Abstract

Introduction: Stunting remains a major nutritional problem affecting children's growth and development and contributes to the quality of human resources. The prevalence of stunting in several community health centers in Konawe Islands Regency remains high. This study aimed to analyze the relationship between drinking water factors (source, treatment, and physical quality) and the incidence of stunting among children under five years old. Method: This study used a quantitative approach with a cross-sectional design. The population consisted of 1,125 toddlers, and 89 respondents were selected using simple random sampling. Data were analyzed using the Chi-Square test, correlation test, and binary logistic regression. Result: The Chi-Square test results showed a significant relationship between drinking water sources and stunting (p=0.000), drinking water treatment and stunting (p=0.000), and drinking water quality and stunting (p=0.000) with correlation coefficient φ = 0.396. Conclusion: Drinking water factors, including water source, treatment, and physical quality, are significantly associated with the incidence of stunting among children under five in Konawe Islands Regency.

Abstract

Introduction: Patient safety culture is an indicator of service quality and a system that makes patient care safer. The leadership style of the head of the ward is one factor that influences the implementation of a patient safety culture. Indonesia is still struggling to improve the quality of healthcare services by enhancing the patient safety culture among care professionals, including nurses. This study aims to examine the influence of the Head of Room's transactional and transformational leadership style on the implementation of a patient safety culture in the Regional Public Service Agency, Benyamin Guluh Hospital, Kolaka Regency. Method: Qantitative approach with a cross-sectional design was used, this study took place during July 2024. Involving 140 respondents, consisting of team leaders and nurses working in inpatient care. Result: The positive influence of transformational leadership style (0.332 (positive direction), T statistic 2.665 and p-value 0.008) and laizzes faire (0.309 (positive direction), T statistic 2.828 and p-value 0.005) in the implementation of patient safety culture. Conclusion: This study emphasizes the importance of a ward head's leadership style that supports a patient safety culture, particularly in reporting individual performance indicators and implementing employee work targets. It is recommended that hospitals improve the implementation of a patient safety culture by conducting continuous patient safety rounds, supervising, monitoring, and evaluating the implementation of individual performance indicators, and encouraging staff to report.
