Abstract

Hilibala Community Health Center is the largest contributor to diarrhea cases, namely 662 cases. One of the causes is poor water quality. Therefore, this study aims to analyze the effect of health promotion in improving water quality to prevent diarrhea. The location of this research was carried out in the working area of the Hilibala Community Health Center, South Nias Regency. The research will start in June-August 2023. The sample in this study was 99 families and was determined using a purposive sampling formula. There were 6 informants for this research. The data was analyzed univariately, bivariately, and multivariately as well as by theme analysis. The results of the research show that there is an influence of advocacy, partnership, and community empowerment on the role of families in improving water quality as an effort to prevent diarrhea at the Hilibala Community Health Center, South Nias Regency, with the results of statistical tests obtaining a significant p-value for each, namely 0.000 < 0.05. The variable that influences this is the empowerment variable with a p-value of 0.000, OR = 9.333 (95% CI = 3.085), while the strategy used to improve water quality as an effort to prevent diarrhea at the Hilibala Health Center, South Nias Regency is to empower the community, such as working together to find clean water locations, digging wells and maintaining the cleanliness and quality of the water. This research concludes that the right strategy is to increase empowerment as a health promotion strategy in improving water quality to prevent diarrhea at the Hilibala Community Health Center, South Nias Regency. Suggestions for community health centers are to carry out situation analysis, outreach, cadre formation and intervention, and evaluation. The results of community empowerment activities with character building have a good impact, namely that the majority of people have increased understanding. The quality of life of the community will also improve as seen from the reduced number of diarrhea incidents.

Abstract

The National Health Insurance (JKN) program is a government initiative aimed at supporting comprehensive health development to ensure that all Indonesians have the right to good health. According to data obtained from BPJS Kesehatan and RS Murni Teguh Medan, the average waiting time for polyclinic services exceeds the national quality indicators, which requires outpatient waiting times should not exceed 60 minutes. This study aims to analyze the challenges in implementing the JKN program, specifically related to patient admission, medical data processing, reporting, and funding at RS Murni Teguh Medan. This is a qualitative descriptive research involving three key informants (Persons-in-Charge or PICs) and six supporting staff members from the Registration, Medical Records, and JKN Billing units. The research identified several challenges: Man: The registration unit's human resources do not meet the required qualifications, as staff should ideally have a medical records background, resulting in reporting issues. Materials: Patient files are incomplete for administrative processes. Machine: The applications used, such as MTHIS (Hospital Management Information System), Mobile JKN, V-Claim, DIVA, and Online Pharmacy, face operational issues, leading to inefficient medical data processing and reporting. Methods: Although procedures are aligned with the SOPs, issues remain include patient overcrowding at registration, outpatient service waiting times exceeding 60 minutes, issues with registration in executive polyclinics, self-registration issues at APM, service penalties, and semi-manual medical data processing and reporting. Markets: Patients have limited knowledge of using Mobile JKN application, and pending claims are still prevalent. Money: Prolonged use of outdated electronic meda reduces systems performance.

Abstract

Needs planning is an activity to determine the quantity and period of procurement of pharmaceuticals, medical devices and disposable medical materials in accordance with the results of selection activities to ensure that the criteria for the right type, right quantity, right time and efficiency are met. The purpose of the research is to analysed drug planning with ABC critical index method at the Pharmacy Installation of Royal Prima Marelan Hospital. Type This study is a combination of qualitative and quantitative research at the Pharmacy Installation at Hospital.Royal Prima Marelan. The qualitative sample of the study was 3 people and the data source was the drug needs plan. The research instrument used a questionnaire. The results of this study with ABC analysis of the critical index value of group A usage were 30 items (51%), group B obtained 23 items (39%) and group C obtained 8 items (9%). Based on the calculation with ABC analysis of the critical index of investment value, group B obtained 2 items (3%) of the total items and group C obtained 57 items (97%) of the total items in the Pharmacy Installation. The results of qualitative research on drug planning at the Pharmacy Installation of Hospital. It is necessary to prepare and evaluate a drug requirement plan (RKO) which is carried out every month as a basis for planning drug requirements and/or can use methods to analysed drug requirements that are efficient, effective and appropriate.

Abstract

Objective: To determine the unit cost of lithotripsy for treatment of urinary bladder calculus at Vita Insani Hospital Pematangsiantar. Material & Methods: This study is a descriptive quantitative study, in which the data was obtained from the finance division of Vita Insani Hospital. The research object is the unit cost of accommodation that occurs in patients with urinary bladder calculus with lithotripsy cases from January 2020 to December 2020. Results: The unit cost of lithotripsy for urinary bladder calculus services at Vita Insani Hospital Pematangsiantar which is calculated using the activity-based costing (ABC) method is Rp 4.167.009. There is a difference in cost among the ABC method, hospital calculations, and INA CBG’s rates with the following details: the amount calculated for the hospital is Rp 4.996.718, while the calculation using ABC method is Rp 4.167.009; the difference in costs is Rp 829.709. INA CBG’s rates Rp 6.137.500, and compared to calculation of hospital, the difference cost is Rp 1.140.782. Compared to the INA CBG’s rates, to the unit cost of the ABC method, there is a difference, amounting Rp 1.970.491. Conclusion: The results of unit cost lithotripsy for treatment of urinary bladder calculus through the ABC method are lower compared to hospital calculation and INA CBG’s rates.

Abstract

An important indicator to determine the quality of service in a hospital is about the completeness of the hospital's medical record files. There are many problems arise due to incomplete medical record files and do not recorded neatly and regularly. The incident is due to lack of medical team awareness in recording all information or actions that would be given to the patient. The aim of this study is to analyse the effect of Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. This study uses quantitative research method which is analytical in nature. The total sampling of this study is 115 active nurses at RSU ESHMUN Medan. The result of the study informs that there are very strong and significant effect among Job Demand-Control-Support and Protection Motivation on compliance in filling out Medical Record files at RSU ESHMUN Medan 2023. The variable that has the most influence on Obedience with filling in inpatient medical record files at RSU ESHMUN in 2023 is the Protection Motivation variable. It is recommended to RSU ESHMUN Medan to improve the administration pattern in order to all of nurses have the same job demands and receive the same support

Abstract

Antenatal Care is one of the pillars supporting safe motherhood in efforts to reduce maternal and fetal morbidity and mortality. WHO estimates that 500,000 pregnant women die from pregnancy-related complications. There are factors that can influence Antenatal Care visits, including the role of health workers and family support. This study aims to analyze the role of health workers and family support in increasing pregnant women's interest in antenatal care visits at the Kuta Alam Community Health Center. This type of research is quantitative analysis using a cross sectional design. The population in this study is mothers who have given birth and had babies during the period September 2020-September 2023, totaling 250 respondents. The results obtained were significant (p=0.002) and there was a relationship between Health Facilities and Antenatal Care visits. The results obtained were significant (p=0.001) and the variables that had an influence in increasing pregnant women's interest in ANC visits at the Kuta Alam Community Health Center were the Health Facilities variable with p value 0.001, OR = 9.333 (95% CI = 3.085) meaning that respondents who stated health facilities had 9.333 times the odds compared to respondents who stated health workers were provided with a B coefficient value of 2.413.

Abstract

Diabetes melitus tipe II merupakan penyakit metabolik yang ditandai dengan peningkatan kadar glukosa darah akibat gangguan sekresi atau kerja insulin, dan merupakan salah satu faktor risiko utama terjadinya stroke. Kadar glukosa darah yang tinggi dalam jangka waktu lama dapat menyebabkan gangguan pada pembuluh darah serta memengaruhi aliran darah ke otak, sehingga meningkatkan kemungkinan terjadinya stroke. Penelitian ini bertujuan untuk mengetahui prevalensi stroke pada pasien diabetes melitus tipe II di Rumah Sakit Royal Prima Ayahanda tahun 2022–2024 berdasarkan usia dan jenis kelamin. Penelitian ini menggunakan metode deskriptif retrospektif dengan pengambilan data dari rekam medis pasien. Jumlah sampel penelitian sebanyak 86 pasien yang ditentukan menggunakan rumus Slovin. Data yang dikumpulkan meliputi usia, jenis kelamin, diagnosis diabetes melitus tipe II, dan jenis stroke. Analisis data dilakukan secara deskriptif menggunakan program SPSS dan disajikan dalam bentuk tabel distribusi frekuensi. Hasil penelitian menunjukkan bahwa mayoritas pasien berusia 61–70 tahun sebanyak 36 orang (41,9%) dan berjenis kelamin perempuan sebanyak 57 orang (66,3%). Seluruh pasien (100%) terdiagnosis diabetes melitus tipe II. Berdasarkan jenis stroke, sebanyak 21 orang (24,4%) mengalami stroke iskemik, 1 orang (1,2%) mengalami stroke hemoragik, dan 64 orang (74,4%) tidak mengalami stroke. Hasil ini menunjukkan bahwa stroke iskemik merupakan jenis stroke yang paling banyak terjadi pada pasien diabetes melitus tipe II, sedangkan stroke hemoragik jarang ditemukan.

Abstract

Latar Belakang: Komunikasi terapeutik perawat merupakan komponen kritis dalam pemberian asuhan keperawatan berkualitas yang berpengaruh langsung terhadap kepuasan pasien. Tujuan: Penelitian ini bertujuan menganalisis hubungan komunikasi terapeutik perawat terhadap kepuasan pasien rawat inap di RS Hermina Medan tahun 2025. Metode: Penelitian kuantitatif dengan desain cross-sectional dilakukan pada 100 responden yang dipilih menggunakan consecutive sampling. Data dikumpulkan melalui kuesioner tervalidasi dan dianalisis menggunakan uji Chi-Square serta regresi logistik berganda. Hasil: Mayoritas responden menilai komunikasi terapeutik perawat dalam kategori baik (57%) dengan tingkat kepuasan pasien cukup puas (53%). Analisis bivariat menunjukkan hubungan signifikan antara komunikasi terapeutik dengan kepuasan pasien (p<0,001; OR=15,802; 95% CI: 5,592-44,627). Fase kerja merupakan determinan paling dominan (?=0,367; p=0,002), diikuti fase terminasi (?=0,257; p=0,035). Simpulan: Komunikasi terapeutik perawat berpengaruh signifikan terhadap kepuasan pasien, dengan fase kerja dan terminasi sebagai determinan utama. Saran: agar rumah sakit memperkuat program pelatihan komunikasi terapeutik berbasis bukti, khususnya pada fase kerja dan terminasi, serta melakukan evaluasi berkala sebagai bagian dari indikator mutu layanan keperawatan.

Abstract

Highlights: 1. Given the suboptimal implementation of clinical practice guidelines at Mandau District General Hospital, it is important to conduct research on potential obstacles to this problem. 2. The findings of this study can encourage hospitals to develop and apply effective clinical practice guidelines while also addressing the obstacles to their implementation. Abstract Clinical practice guidelines are procedures followed by physicians to optimize healthcare services and achieve more effective outcomes. Conducted in October 2022, this study aimed to analyze the factors that hinder the implementation of clinical practice guidelines among physicians at Mandau District General Hospital, Bengkalis, Indonesia. This study used an analytical survey design and a cross-sectional approach. This study included all 85 individuals who were employed as general practitioners at the Mandau District General Hospital. The data were subjected to univariate, bivariate, and multivariate analyses. The independent t-test was used in the data analysis, with a value of p<0.05 considered significant. The results showed that there was an association between the implementation of clinical practice guidelines and several variables, including the levels of ability in recognizing and explaining clinical pictures of diseases (p=0.008), diagnosing diseases (p=0.004), carrying out initial management (p=0.000), and carrying out independent and directed management (p=0.001). Upon conducting the follow-up analysis on logistic regression, it was found that the variables that were associated with the implementation of clinical practice guidelines were the ability to carry out initial management (p=0.000, OR=65.512, 95% CI=22.048–98.835). In conclusion, physicians' insufficient ability to perform initial management hinders the implementation of clinical practice guidelines. This study suggests incorporating the findings as input in the evaluation and development of training for physicians on the implementation of clinical practice guidelines.

Highlights: This article presents an understudied topic of nursing workloads in a hemodialysis unit and concludes that directly and indirectly productive activities occupied the majority of the nurses' working time. An analysis using the Workload Indicators of Staffing Need produced a general report that may prompt re-evaluation of nursing staff requirements and policy-making for workload distribution. Abstract The number of hemodialysis patients continues to increase almost every month, while the number of nurses in the Hemodialysis Unit at Rasyida Renal Specialist Hospital, Medan, Indonesia, remains constant. This situation has increased the nurses' workload and responsibilities. Therefore, it is necessary to examine the balance between the workload and the number of required nursing staff in the Hemodialysis Unit at Rasyida Renal Specialist Hospital. This study aimed to identify the optimal number of nursing staff required in the Hemodialysis Unit at the hospital in 2022. This study used a mixed-methods approach using the Workload Indicators of Staffing Need (WISN) method. This study included 44 nurses working in the Hemodialysis Unit at Rasyida Renal Specialist Hospital. Primary data included questionnaires, observations, and interviews during working hours, while secondary data included document reviews and staffing data. WISN calculations showed that the Hemodialysis Unit required a total of 65 nursing staff. In other words, an addition of 21 nursing staff was needed to meet the current workload. In conclusion, the current human resources available are not sufficient, and the workload is beyond the capacity of the nursing staff at the hospital.
