Abstract

BACKGROUND: The search for novel antidiabetic agents that simultaneously lower blood glucose and protect pancreatic β-cells is crucial. While Eruca sativa is known for its antidiabetic properties, the specific contribution of its solvent fractions to different therapeutic targets remains poorly understood. Therefore, this study was conducted to identify how different fractions selectively target glycemic control or pancreatic islet integrity.METHODS: Twenty-five male Wistar rats were rendered diabetic by a single intraperitoneal injection of 45 mg/kg streptozotocin. Diabetic animals were divided into five groups (n=5): diabetic control, positive control (0.45 mg/kg glibenclamide), and three treatment groups receiving n-hexane, ethyl acetate, or aqueous fractions of 400 mg/kgBW E. sativa for 14 days. Phytochemical screening was performed to identify bioactive profiles. Blood glucose was monitored periodically, and pancreatic tissues were assessed using a histopathological scoring system (0–4) and islet area measurement.RESULTS: Aqueous fraction showed the most potent antihyperglycemic activity, significantly reducing blood glucose (208.8±36.02 mg/dL) compared to the diabetic control (419.6± 117.11 mg/dL). Conversely, the n-hexane fraction provided superior pancreatic protection, maintaining the highest islet area (19,109.81±7,549.98 µm²) and the best histopathological score (2.8±0.8) among all treatment groups. Phytochemical screening revealed a distinct distribution of compounds, with flavonoids concentrated in the aqueous fraction and terpenoids in the n-hexane fraction.CONCLUSION: This study demonstrates that therapeutic efficacy of E. sativa is fraction-specific, driven by its distinct phytochemical profiles. The aqueous fraction is the most effective for rapid glycemic control, significantly reducing blood glucose levels. Conversely, the n-hexane fraction provides superior pancreatic protection, as evidenced by the highest islet area and improved histopathological scores. These findings suggest that E. sativa possesses a dual-target potential; while the aqueous fraction excels in antihyperglycemic action, the n-hexane fraction is more potent for pancreatic islet preservation. Abstract

Introduction: The increasing elderly population in Indonesia presents significant challenges in managing degenerative diseases, particularly in primary health care facilities with limited resources. This study aims to explore the role of health workers in managing degenerative diseases in the elderly at the Negeri Lama Community Health Center. Methods: This research employed a qualitative approach with a case study design. Data were collected through in-depth interviews with seven informants (elderly cluster officers, non-communicable disease program holders, doctors, pharmacists, and nutritionists), observation, and documentation. Data analysis used thematic analysis with source triangulation, method triangulation, member check, and audit trail to ensure data validity. Results: Health workers performed curative roles through integrated medical services including comprehensive examinations, diagnosis, medication provision (amlodipine, metformin, glimepiride, simvastatin, allopurinol), pharmaceutical and nutritional counseling, and referral systems. Preventive roles were implemented through routine health screenings using E-pus and P-care applications, counseling on healthy lifestyles, education on balanced nutrition, and lifestyle modifications. Challenges included complex disease complications (multimorbidity), unpredictable drug shortages from the District Health Office, limited diagnostic facilities, declining memory and cognitive function in the elderly, low health literacy, irregular visits, unstable internet networks, poor road conditions, flooding, geographical distance, and economic constraints. Communication patterns applied were patient-centered with specific adjustments for the elderly including louder volume, slower speech, simple language, and family involvement in the education process. Interprofessional collaboration involved medical records staff, elderly cluster officers, pharmacists, and nutritionists working according to standard operating procedures. Discussion: The findings align with previous research emphasizing comprehensive geriatric assessment, the effectiveness of pharmacist counseling in improving medication adherence, the importance of family support in chronic disease management, and the impact of geographical and economic barriers on healthcare access. The study highlights the need for integrated approaches addressing both clinical and social determinants of health in elderly care. Conclusion: Health workers at the Negeri Lama Community Health Center have implemented comprehensive curative and preventive roles despite various challenges. Improvements are needed in logistics management systems, development of educational media, strengthening interprofessional collaboration, innovation in education methods, and family and community support to optimize health services for the elderly with degenerative diseases.

Abstract

Background: Patient safety is a primary priority in healthcare services; however, adverse events persist and are often associated with nurse performance and weak supervision. This study aimed to analyze the influence of supervision by the Head of Medical Services and the performance of inpatient ward nurses on patient safety at Aek Kanopan Regional General Hospital, North Labuhanbatu Regency. Methods: This quantitative study employed a cross-sectional design. The study population comprised all 120 implementing nurses in the inpatient wards, who were also recruited as the total sample. Data were collected using a structured questionnaire measuring nurse perceptions of supervision, performance, and patient safety implementation. Data analysis included univariate and bivariate analyses using the Chi-Square test. Results: The majority of respondents rated supervision as good (84.2%), nurse performance as good (84.2%), and patient safety as good (84.2%). Bivariate analysis demonstrated a significant association between supervision and patient safety (p=0.000) and between nurse performance and patient safety (p=0.000). Conclusion: This study concludes that effective supervision by department heads and optimal nurse performance contribute significantly to improved patient safety. Hospital management is advised to strengthen supervisory capacity and invest in continuing professional development programs for nurses.

Abstract

Hospital management, leadership, and governance have an important role in ensuring the successful implementation of hospital accreditation policies. This study aims to analyze the influence of leadership and management governance on the implementation of accreditation policies at the Medan City Air Force Hospital. This study uses a mixed-methods approach with a sequential explanatory design, in which quantitative research is conducted first, followed by qualitative analysis to deepen the research findings. The quantitative sample comprised 80 health workers and managerial staff, selected using the total sampling technique. At the same time, qualitative data were obtained through purposive sampling involving five key informants: the hospital head, the hospital secretary, and the head of the installation. Data were collected through structured questionnaires with a Likert scale, in-depth interviews, observations, and document review. Quantitative data were analyzed using descriptive statistics and multiple linear regression, while qualitative data were analyzed using thematic analysis. The results showed that leadership had a significant effect on the implementation of hospital accreditation policies (β = 0.228; p < 0.05). Management governance also exerted a significant influence on the implementation of accreditation policies (β = 0.716; p < 0.05). These findings show that effective leadership and good management governance are important factors in strengthening the hospital's quality management system and ensuring the sustainability of accreditation implementation.

Abstract

Diabetic food complications are among the most serious challenges faced by elderly individuals with type 2 diabetes mellitus (T2DM), often leading to reduced mobility, diminished quality of life, and a heightened risk of ulceration. This study evaluated the effectiveness of diabetic foot exercise as a non-pharmacological intervention to improve foot health in older T2DM patients. Using a quasi-xperimental pre-test - post-test design, participants at Pratama Prima Melati Clinic were assigned to either an intervention group or a control group. The control group received standard care, while the intervention group performed structured diabetic foot exercises three times per week for four weeks. Foot health outcomes, including circulation, sensitivity, and range of motion, were assessed using standardized clinical tests before and during the intervention. Results showed that the intervention group demonstrated significant improvements in circulation, neuropathic symptoms, and joint flexibility compared to the control group (p < 0.05). These findings suggest that diabetic foot exercise is an effective strategy to enhance foot health and potentially prevent complications in elderly T2DM patients. Incorporating such exercise programs into primary healthcare practice is strongly recommended to support comprehensive diabetes control management.

Abstract

Latar belakang: Individu yang terkena hipertensi membutuhkan pelayanan kesehatan yang berkesinambungan, terintegrasi, dan berkualitas untuk mengendalikan tekanan darah serta mengurangi risiko komplikasi yang serius. Oleh karena itu, kualitas pelayanan kesehatan menjadi faktor penentu utama dalam keberhasilan pengelolaan penyakit dan tingkat kepuasan pasien. Penelitian ini bertujuan untuk mengetahui pengaruh kualitas pelayanan kesehatan terhadap kepuasan pasien hipertensi. Metode: Penelitian ini menggunakan pendekatan cross-sectional. Populasi penelitian adalah seluruh pasien hipertensi yang menerima layanan rawat jalan di Puskesmas Labuhan Bilik pada bulan Mei 2023, dengan jumlah 200 orang. Sampel ditentukan menggunakan metode total sampling, sehingga diperoleh 200 responden. Data dikumpulkan menggunakan kuesioner terstruktur. Analisis data dilakukan dengan uji chi-square menggunakan aplikasi SPSS versi 26. Hasil: Hasil penelitian menunjukkan bahwa kehandalan (p= <0,001), daya tanggap (p = <0,001), kepedulian (p = <0,001), jaminan (p = <0,001) berpengaruh signifikan terhadap kepuasan pasien hipertensi, sedangkan variabel bukti fisik tidak berhubungan secara signifikan terhadap kepuasan pasein (p = 0,155). Kesimpulan: Kehandalan yang kurang baik, daya tanggap yang kurang baik, kepedulian dan jaminan yang kurang baik berhubungan signifikan terhadap kepuasan pasien.

Abstract

Purpose: This community service activity aims to improve the performance of the immunization program through strengthening leader behaviors and management processes at Puskesmas Pujud. Design/methods/approach: This activity used a descriptive approach through community service implementation. Data were collected using observation, interviews, questionnaires, and documentation. The intervention included counseling, training, and mentoring for health workers. A total of 20 participants were involved. Findings/results: The results showed an improvement in participants’ understanding of leadership and management concepts. Team coordination improved, particularly in task distribution and communication. The implementation of management functions became more structured, especially in planning, implementation, and supervision. Furthermore, the performance of the immunization program improved, indicated by increased immunization coverage, timely implementation, and improved recording and reporting systems. Conclusions: Community service activities focusing on strengthening leadership and management processes can effectively improve the effectiveness of immunization programs at primary healthcare centers.

Abstract

Clinics are healthcare facilities that provide medical services to the community. This study aims to analyze the influence of workplace environmental health conditions, healthcare service quality, and human resource (HR) competence on patient satisfaction at Evi Primary Clinic. Despite being accredited and having a strong vision, the clinic still receives complaints regarding suboptimal services and facilities.Using a quantitative analytic correlational design with a cross-sectional approach, this study examined variable relationships at a single point in time. The population included all patients visiting the clinic within one month, with data collected through questionnaires and interviews. A total sampling technique was applied, and the data were analyzed using univariate, bivariate (Chi-Square), and multivariate (Discriminant Analysis) methods.Results show that HR competence significantly influences healthcare service quality, with a coefficient of 0.806 (p = 0.000), and also impacts patient satisfaction (coefficient 0.609; p = 0.000). Most respondents (62.8%) rated HR competence as fair, and 19.9% as good. Workplace environmental conditions and health facilities also significantly influence service quality (coefficient 0.639; p = 0.000) and patient satisfaction (coefficient 0.527; p = 0.000). Multivariate analysis further revealed that facilities affect service quality with a coefficient of 0.748.In conclusion, HR competence, healthcare facilities, and service quality all significantly affect patient satisfaction. A comprehensive approach is needed to enhance staff competence, improve infrastructure, and optimize service procedures. These strategies are essential for achieving high patient satisfaction and realizing the clinic’s mission of delivering excellent healthcare.

Abstract

This study aimed to analyze psychosocial and organizational factors influencing Personal Protective Equipment (PPE) compliance and their implications for Healthcare-Associated Infections (HAIs) among emergency department healthcare workers at RSIA Artha Mahinrus. This study used a mixed-methods approach with a sequential explanatory design, in which quantitative data were collected first, followed by qualitative data to deepen the findings. The quantitative sample comprised 16 healthcare workers (4 doctors, 4 nurses, and 8 midwives) using a total sampling approach. Data were analyzed using univariate, bivariate (Fisher's Exact Test), and multivariate (multiple linear regression) analyses. Qualitative data were obtained through in-depth interviews. The results showed that psychosocial factors (p = 0.030) and organizational factors (p = 0.012) significantly influenced PPE compliance, with organizational factors as the dominant predictor. The findings also revealed the phenomenon of clinical discretion, in which emergencies and time pressure led to intentional noncompliance with PPE protocols. In addition, a paradox was identified: high organizational involvement was associated with noncompliance due to safety complacency and perceived bureaucratic burden. These findings indicate that PPE compliance is not merely adherence to standard procedures but reflects a complex interaction between individual and organizational factors in maintaining patient and worker safety.
