Abstract

Antenatal Care is one of the pillars supporting safe motherhood in efforts to reduce maternal and fetal morbidity and mortality. WHO estimates that 500,000 pregnant women die from pregnancy-related complications. There are factors that can influence Antenatal Care visits, including the role of health workers and family support. This study aims to analyze the role of health workers and family support in increasing pregnant women's interest in antenatal care visits at the Kuta Alam Community Health Center. This type of research is quantitative analysis using a cross sectional design. The population in this study is mothers who have given birth and had babies during the period September 2020-September 2023, totaling 250 respondents. The results obtained were significant (p=0.002) and there was a relationship between Health Facilities and Antenatal Care visits. The results obtained were significant (p=0.001) and the variables that had an influence in increasing pregnant women's interest in ANC visits at the Kuta Alam Community Health Center were the Health Facilities variable with p value 0.001, OR = 9.333 (95% CI = 3.085) meaning that respondents who stated health facilities had 9.333 times the odds compared to respondents who stated health workers were provided with a B coefficient value of 2.413.

Abstract

Diabetes melitus tipe II merupakan penyakit metabolik yang ditandai dengan peningkatan kadar glukosa darah akibat gangguan sekresi atau kerja insulin, dan merupakan salah satu faktor risiko utama terjadinya stroke. Kadar glukosa darah yang tinggi dalam jangka waktu lama dapat menyebabkan gangguan pada pembuluh darah serta memengaruhi aliran darah ke otak, sehingga meningkatkan kemungkinan terjadinya stroke. Penelitian ini bertujuan untuk mengetahui prevalensi stroke pada pasien diabetes melitus tipe II di Rumah Sakit Royal Prima Ayahanda tahun 2022–2024 berdasarkan usia dan jenis kelamin. Penelitian ini menggunakan metode deskriptif retrospektif dengan pengambilan data dari rekam medis pasien. Jumlah sampel penelitian sebanyak 86 pasien yang ditentukan menggunakan rumus Slovin. Data yang dikumpulkan meliputi usia, jenis kelamin, diagnosis diabetes melitus tipe II, dan jenis stroke. Analisis data dilakukan secara deskriptif menggunakan program SPSS dan disajikan dalam bentuk tabel distribusi frekuensi. Hasil penelitian menunjukkan bahwa mayoritas pasien berusia 61–70 tahun sebanyak 36 orang (41,9%) dan berjenis kelamin perempuan sebanyak 57 orang (66,3%). Seluruh pasien (100%) terdiagnosis diabetes melitus tipe II. Berdasarkan jenis stroke, sebanyak 21 orang (24,4%) mengalami stroke iskemik, 1 orang (1,2%) mengalami stroke hemoragik, dan 64 orang (74,4%) tidak mengalami stroke. Hasil ini menunjukkan bahwa stroke iskemik merupakan jenis stroke yang paling banyak terjadi pada pasien diabetes melitus tipe II, sedangkan stroke hemoragik jarang ditemukan.

Abstract

Latar Belakang: Komunikasi terapeutik perawat merupakan komponen kritis dalam pemberian asuhan keperawatan berkualitas yang berpengaruh langsung terhadap kepuasan pasien. Tujuan: Penelitian ini bertujuan menganalisis hubungan komunikasi terapeutik perawat terhadap kepuasan pasien rawat inap di RS Hermina Medan tahun 2025. Metode: Penelitian kuantitatif dengan desain cross-sectional dilakukan pada 100 responden yang dipilih menggunakan consecutive sampling. Data dikumpulkan melalui kuesioner tervalidasi dan dianalisis menggunakan uji Chi-Square serta regresi logistik berganda. Hasil: Mayoritas responden menilai komunikasi terapeutik perawat dalam kategori baik (57%) dengan tingkat kepuasan pasien cukup puas (53%). Analisis bivariat menunjukkan hubungan signifikan antara komunikasi terapeutik dengan kepuasan pasien (p<0,001; OR=15,802; 95% CI: 5,592-44,627). Fase kerja merupakan determinan paling dominan (?=0,367; p=0,002), diikuti fase terminasi (?=0,257; p=0,035). Simpulan: Komunikasi terapeutik perawat berpengaruh signifikan terhadap kepuasan pasien, dengan fase kerja dan terminasi sebagai determinan utama. Saran: agar rumah sakit memperkuat program pelatihan komunikasi terapeutik berbasis bukti, khususnya pada fase kerja dan terminasi, serta melakukan evaluasi berkala sebagai bagian dari indikator mutu layanan keperawatan.

Abstract

Highlights: 1. Given the suboptimal implementation of clinical practice guidelines at Mandau District General Hospital, it is important to conduct research on potential obstacles to this problem. 2. The findings of this study can encourage hospitals to develop and apply effective clinical practice guidelines while also addressing the obstacles to their implementation. Abstract Clinical practice guidelines are procedures followed by physicians to optimize healthcare services and achieve more effective outcomes. Conducted in October 2022, this study aimed to analyze the factors that hinder the implementation of clinical practice guidelines among physicians at Mandau District General Hospital, Bengkalis, Indonesia. This study used an analytical survey design and a cross-sectional approach. This study included all 85 individuals who were employed as general practitioners at the Mandau District General Hospital. The data were subjected to univariate, bivariate, and multivariate analyses. The independent t-test was used in the data analysis, with a value of p<0.05 considered significant. The results showed that there was an association between the implementation of clinical practice guidelines and several variables, including the levels of ability in recognizing and explaining clinical pictures of diseases (p=0.008), diagnosing diseases (p=0.004), carrying out initial management (p=0.000), and carrying out independent and directed management (p=0.001). Upon conducting the follow-up analysis on logistic regression, it was found that the variables that were associated with the implementation of clinical practice guidelines were the ability to carry out initial management (p=0.000, OR=65.512, 95% CI=22.048–98.835). In conclusion, physicians' insufficient ability to perform initial management hinders the implementation of clinical practice guidelines. This study suggests incorporating the findings as input in the evaluation and development of training for physicians on the implementation of clinical practice guidelines.

Highlights: This article presents an understudied topic of nursing workloads in a hemodialysis unit and concludes that directly and indirectly productive activities occupied the majority of the nurses' working time. An analysis using the Workload Indicators of Staffing Need produced a general report that may prompt re-evaluation of nursing staff requirements and policy-making for workload distribution. Abstract The number of hemodialysis patients continues to increase almost every month, while the number of nurses in the Hemodialysis Unit at Rasyida Renal Specialist Hospital, Medan, Indonesia, remains constant. This situation has increased the nurses' workload and responsibilities. Therefore, it is necessary to examine the balance between the workload and the number of required nursing staff in the Hemodialysis Unit at Rasyida Renal Specialist Hospital. This study aimed to identify the optimal number of nursing staff required in the Hemodialysis Unit at the hospital in 2022. This study used a mixed-methods approach using the Workload Indicators of Staffing Need (WISN) method. This study included 44 nurses working in the Hemodialysis Unit at Rasyida Renal Specialist Hospital. Primary data included questionnaires, observations, and interviews during working hours, while secondary data included document reviews and staffing data. WISN calculations showed that the Hemodialysis Unit required a total of 65 nursing staff. In other words, an addition of 21 nursing staff was needed to meet the current workload. In conclusion, the current human resources available are not sufficient, and the workload is beyond the capacity of the nursing staff at the hospital.

Abstract

Background: Triple‑negative breast cancer (TNBC) is an aggressive subtype of breast malignancy with limited targeted therapy options, making adherence to systemic therapy critical for improving survival outcomes. This study aimed to determine the sociodemographic, clinical, and psychosocial factors influencing adherence to systemic therapy among patients with TNBC, especially in resource-constrained settings.Method: A cross-sectional observational study was conducted among 105 female TNBC patients undergoing systemic therapy at the Dharmais Cancer Hospital. Adherence was defined as completion of ≥ 85% of planned systemic therapy cycles without unjustified delay or omission. Data were collected from medical records and structured questionnaires, covering sociodemographic, clinical, and psychosocial characteristics, including psychological distress measured using the Depression Anxiety Stress Scale-10 (DASS-10). Bivariate and multivariate logistic regression analyses were performed to identify factors associated with non-adherence.Results: Median patient age was 48 years (range 25–76), and the majority worked as housewives (58.1%). Most had completed high school education (51.5%), and nearly half earned less than IDR 3 million monthly (48.6%). Clinically, most patients were diagnosed at stage III (40.9%) or IV (38.1%), with comorbidities present in 41.9%, predominantly hypertension (19.1%). While overall adherence to systemic therapy was relatively high, logistic regression analysis identified significant predictors of non-adherence. Comorbid conditions (p 0.01; OR 5.7; 95% CI: 1.94–16.02) and severe levels of anxiety/depression based on DASS-10 scores (p = 0.015; OR 5.65; 95% CI: 1.08–29.42) remained independently associated with lower adherence (p 0.05), underscoring the importance of psychological and clinical support to improve treatment compliance.Conclusion: Comorbid conditions and psychological distress significantly contribute to non-adherence among TNBC patients. Integrating psycho-oncology support and comorbidity management into routine cancer care may enhance treatment adherence and outcomes.

Abstract

Voluntary work behavior that goes beyond formal employee responsibilities (OCB) cannot develop optimally without attention to Quality of Work Life (QWL). QWL reflects the organization's efforts in creating a work environment that supports the physical, psychological, and social well-being of employees, which ultimately impacts on improving individual and organizational performance. The formulation of the problem of this research is how does Quality of Work Life (QWL) influence employee performance through Organizational Citizenship Behavior (OCB) at Royal Prima Hospital in 2025? This study uses a quantitative approach. The results of the study There is no significant influence between the variable of tenure and performance at Royal Prima Hospital Medan. There is a significant influence between the variable of Quality of Work Life (QWL) and performance at Royal Prima Hospital Medan. There is a significant influence between the variable of Organizational Citizenship Behavior (OCB) and performance at Royal Prima Hospital Medan. There is a significant influence between the variable of Leadership and performance at Royal Prima Hospital Medan. There is a significant influence between the variable of Satisfaction and performance at Royal Prima Hospital Medan. There is a significant influence between the variable of encouragement and performance at Royal Prima Hospital Medan. The most dominant variable with the greatest influence on performance is motivation because it has an Exp B value of 7.348.

Abstract

Hospital occupational health programs have not been fully implemented in hospitals such as evaluation of the work environment, documentation of special and periodic health checks, the risk of occupational diseases (PAK). This type of research is qualitative with 10 informants selected purposively. on the input component: HR K3, the budget, and safety facilities provided are still lacking. In the process component: Risk management, hospital safety and security efforts, occupational health services, and B3 management are good. There are not enough fire prevention and control facilities. Infrastructure management is good with water sources, backup electricity, and periodic maintenance. Management of medical equipment and disaster preparedness is good. In the output component: Supervision is carried out by internal hospitals cases of work accidents are still found, and there is still a shortage of human resources, budget, and K3 facilities. It is recommended to the management of the hospital to add human resources in the field of work safety according to standard.

Abstract

Background: Acute Respiratory Infections (ARI) remain a significant global health burden with high morbidity, particularly in developing nations. Modern lifestyle shifts—characterised by smoking habits, poor dietary patterns, physical inactivity, and compromised sleep quality—alongside environmental pollution, are hypothesised to contribute to susceptibility to these infections. Objective: This study aims to analyse the association between lifestyle factors (smoking, diet, physical activity, sleep quality) and environmental pollution exposure with the incidence of ARI among patients at the Mayen Primary Clinic in 2023–2024. Methods: This study employed an analytic observational design with a cross-sectional approach. The research was conducted at the Mayen Primary Clinic, Medan, involving 82 respondents selected via consecutive sampling. Data were collected using medical records and validated lifestyle questionnaires, followed by analysis using the Chi-Square test. Results: The findings revealed that the majority of respondents were suffering from ARI (85.4%). Bivariate analysis demonstrated significant correlations between ARI incidence and smoking habits (p<0.001), dietary patterns (p=0.001), sleep quality (p<0.001), and air pollution exposure (p<0.001). However, no significant association was observed between physical activity and ARI incidence (p=0.601). Conclusion: Unhealthy lifestyles and environmental pollution are primary determinants of ARI incidence in primary care settings, necessitating health interventions focused on behavioural modification and environmental control.

Abstract

This study aimed to identify factors associated with fatigue among employees of PT. XYZ in Medan, a futures brokerage firm where preliminary observations indicated a high prevalence of exhaustion and decreased alertness among employees. This study aimed to identify factors associated with fatigue. A descriptive analytic study with a cross-sectional design was conducted using a total population sample of 45 employees. Data were collected on workload (measured as %CVL), work duration, work environment (assessed through a questionnaire), and occupational fatigue (measured using the IFRC questionnaire). Univariate, bivariate (Chi-square), and multivariate statistical analyses were performed. The results showed that 33.3% of participants experienced fatigue. Bivariate analysis demonstrated statistically significant associations between fatigue and workload (p=0.015), work duration (p=0.001), and work environment (p=0.006). Multivariate analysis further quantified these relationships, identifying the work environment as the most dominant risk factor (OR=3.18), followed by work duration (OR=2.4) and heavy workload (OR=2.4). In conclusion, an unsafe work environment, long work duration, and heavy workload are significant and quantifiable risk factors for occupational fatigue, with the work environment serving as the strongest predictor.
