Abstract

Kusta adalah penyakit kronis, disebabkan oleh Mycobacterium leprae dan menyebabkan cacat jika tidak dilakukan perawatan diri. Kabupaten Tuban merupakan daerah di Jawa Timur dengan kasus kusta dan cacat tingkat 2 yang tinggi. Tujuan penelitian ini adalah untuk mengetahui gambaran dan faktor-faktor yang berhubungan dengan perilaku perawatan diri eks penderita kusta yang tinggal di Unit Pelaksana Teknis Rehabilitasi Sosial Eks-Penderita Kusta Nganget, Tuban. Penelitian ini menggunakan desain cross-sectional. Populasinya adalah seluruh eks-penderita kusta di Unit Pelaksana Teknis Rehabilitasi Sosial Eks-Penderita Kusta Nganget dan semuanya diikutkan dalam penelitian sebagai sampel. Pengambilan data dilakukan pada 14-17 Desember, 2011. Hasil menunjukkan bahwa umur rata-rata eks-penderita kusta adalah 56,08 tahun dan telah menderita cacat selama 24,54 tahun. Kebanyakan dari responden menderita jenis Multi Baciller dan memiliki tingkat cacat 2, serta 61,8% dari responden selalu melakukan perawatan diri yang sesuai dengan jenis kecacatannya. Sebagian besar dari responden memiliki pengetahuan dan sikap yang baik tentang perawatan diri. Seluruhnya melaporkan bahwa mereka mendapatkan dukungan yang baik dari petugas kesehatan tetapi tidak baik dalam dukungan keluarga. Unit Rehabilitasi Sosial ini memiliki poliklinik tetapi tidak memiliki kelompok perawatan diri dan responden tidak mendapatkan alat pelindung diri. Hasil dari penelitian menunjukkan bahwa tidak ada hubungan yang signifikan antara perilaku perawatan diri dengan umur, jenis kelamin, lama cacat, dan pengetahuan maupun sikap tentang perawatan diri. Tidak ada hubungan yang signifikan juga ditemukan antara perilaku perawatan diri dengan ketersediaan alat pelindung diri dan dukungan dari keluarga responden. Sebagian besar responden telah melakukan perawatan diri yang baik untuk meminimalisasi tingkat kecacatan lebih lanjut. Semua faktor yang diteliti tidak ada yang berhubungan secara signifikan dengan perilaku perawatan diri eks penderita kusta. Petugas kesehatan diharapkan dapat mempertahankan dan meningkatkan pelayanan kesehatan, terutama dalam pemberian informasi mengenai perawatan diri.

Abstract

Background : Social independence in women, climate change, and stunting are interconnected issues through a series of pathways that can create a cycle of vulnerability and adverse health impacts on children, especially in developing countries. Objective : The aims of this study are to explore the impact of women’s social independence and climate factors on stunting in children under 5 years of age in East Java Province, Indonesia. Research Methods/ Implementation Methods : This study used secondary data from a nationally representative 2023 Indonesian Health Survey sample. Data from 3,928 children under five and their parents were analysed. Descriptive analysis and multivariable logistic regression adjusting for complex sampling design were used to examine the risks of contributing factors, controlling for potential confounders. Results : Children of mothers who had medium and higher social independence showed a reduced risk of stunting incidence, compared to children of mothers who had lower social independence. Stunting decreased by 21.61% for 1°C increase in average temperature. Meanwhile, stunting increased by 12% for 1% increase in average humidity, 0.4% for1 mm increase in average rainfall, and 1% for1 hour increase in sunshine duration. Increased stunting risks were also associated with poor environmental sanitation, diarrheal diseases, and older age. Conclusion/Lesson Learned : The study indicates that women’s social independence can reduce stunting incidence among children under five in Indonesia. While decreased temperature and increased rainfall, humidity and duration of sunshine could bring more stunting incidence in the study population. These findings highlight the importance of developing integrative preventive strategies to reduce stunting among Indonesian children under-5.

Abstract

Background: According to the Ministry of Health, 91% of active tuberculosis (TB) cases in Indonesia occur in crowded and slum areas. The Healthy Homes Program under the Healthy Indonesia Indicators (2010) aims to improve housing conditions in such areas to meet health standards. Malang District, with the second-highest housing density in East Java, ranks fifth in TB cases, suggesting residential exposure as a contributing factor. Objectives: This study aims to map trends and examine the correlation between healthy home coverage and TB incidence in the Malang District Health Center Working Area 2019-2021. Methods: The study was conducted in January-April 2023 at the Malang District Health Office with a unit of analysis of 39 health centers. The variables of this study were the coverage of healthy homes and the incidence of BTA-positive pulmonary TB. The study used a mixed ecological study and Pearson correlation test with secondary data from the Malang District Health Office and BPS. Results: TB incidence showed a decreasing trend alongside increasing healthy home coverage. Nine health centers remained in high-incidence zones, and five exhibited poor housing coverage. Correlation analysis revealed a weak, negative relationship (r = -0.084). High TB incidence was still found in areas meeting healthy home criteria. Conclusions: The incidence rate of BTA-positive pulmonary TB in the working area of the Malang District Health Center showed a downward trend for 3 years. Future research using individual data and other specific risk factors that may influence TB incidence is recommended.

Abstract

Surgery is the most performed procedure to treat brain tumors. However, information about the survival of brain tumor patients after undergoing surgery is still limited in Indonesia. This study aimed to determine 1 year survival in brain tumor patients after undergoing surgical procedures and factors associated with patient survival. A retrospective cohort study was conducted in March until April 2023 with subjects of 46 brain tumor patients who were registered at the Gatot Soebroto Army Hospital and underwent surgical procedures in 2018 until 2022. Data on respondent characteristics (gender and age), clinical conditions (tumor location, stage, and tumor classification), date of surgery, and date of death were obtained from medical records. Kaplan Meier was used to determine the 1 year survival of brain tumor patients after surgery, while Cox Proportional Hazard Regression was used to determine factors associated with its survival. Kaplan Meier analysis showed that 1 year survival in post-operative brain tumor patients reached 80.5 percent. Secondary tumor was known to increase the risk of death in post-operative brain tumor patients (aHR 11.432; 95 percent CI 1.394 until 93.767). There was no significant relationship found between sex, age, tumor location, and tumor stage with brain tumor patients’s survival (p-value more than 0.05). Identification of tumor classification needs to be done correctly and as early as possible so that the patient's prognosis becomes better. The therapeutic approach needs to pay attention to the condition of each patient, especially in cases of secondary brain tumors.

Abstract

Purpose: The burden of COVID-19 threatened the health system and reversed gains in healthcare services. It led to significant disruptions in access and delivery of maternal healthcare. In consequence, the Maternal Mortality Rate (MMR) in Ngawi Regency is 276.9 per 100,000 live births, surpassing the MMR for East Java, and the coverage of maternal health indicators has declined during the pandemic. This study aims to investigate the effect of the COVID-19 pandemic on maternal healthcare in Ngawi Regency, Indonesia. Methods: An Interrupted Time-Series (ITS) design was used to assess the impact of the COVID-19 outbreak on the outcome variable, the number of women who utilized maternal healthcare services. Data were collected from January 1st, 2019, to December 31st, 2021. An interrupted-time series analysis was conducted using an Autoregressive Integrated Moving Average (ARIMA) model. Results: A significant decline of 54 women (95% CI -80.45 to -27.06) in fourth maternal healthcare utilization at pandemic onset. The following reductions were also observed in health-facility delivery and postnatal care utilization: 66 women (95% CI: -112.04 to -20.51) and 106 women (95% CI: -184.60 to -26.49); however, no significant changes in first antenatal care or obstetric-complication treatment at pandemic onset. The statistically significant reductions in fourth antenatal care, health-facility delivery, and postnatal care utilization were 3 (95% CI: -5.14 to -1.67), 7 (95% CI: -11.66 to -1.85), and 11 (95% CI: -15.50 to -5.51) women, respectively. Conclusion: The COVID-19 pandemic has disrupted the fourth antenatal care, health facility delivery, and postpartum care in Ngawi Regency. The disruption due to mobility restrictions, a shift in focus and health resources towards combating COVID-19, and a decrease in the frequency of antenatal care visits.

Abstract

The increasing prevalence of smoking among women globally has become a critical public health issue. World Health Organization (WHO) data shows 20 percent of women use tobacco. Women smokers face an increased risk of various diseases. Spatial studies need to be conducted as a basis for planning and providing targeted interventions to reduce the spread of diseases caused by smoking. The study aimed to determine the spatial description of smoking among women in Indonesia. This study analyzed the 2017 IDHS data and used an ecological study design with a descriptive approach and a spatial analysis model. The results are Papua was the province with the highest percentage of women smoking with no education and with the lowest de jure wealth quintile. Provinces using the Internet almost daily were Banten and East Java, and the province with women unemployed in the past 12 months before the survey was North Sulawesi. Based on the results, there are five provinces in Indonesia with a high percentage of smoking among women, assessed by education, internet access, wealth quintile, and non-working women. This study concludes that there is a need to implement smoke-free zones (SFZs), also conduct communication, information, and education (IEC) through mass media.

Abstract

Background : Insomnia is a pervasive global public health concern with significant implications for productivity and overall well-being, particularly among youth. Objective : This study aimed to assess the association between mental health factors (depression, anxiety, and stress) and selected modifiable lifestyle factors (physical activity and coffee consumption) with the incidence of insomnia in an urban Indonesian setting. Research Methods/ Implementation Methods : A cross-sectional study was conducted on a sample of 111 youth in East Java, Indonesia. Data were collected using the Depression, Anxiety, and Stress Scale-21 (DASS-21) and the Insomnia Severity Index (ISI). Bivariate analysis using the Chi-Square test was performed. Results : The prevalence of insomnia in this study was 41.4% while depression, anxiety, and stress were reported at 12.6%, 23.4%, and 0.9% respectively. Anxiety was significantly associated with the incidence of insomnia. Youth classified with anxiety were associated with a twofold increase in the risk of insomnia (PR=1.916; 95%CI=1.276-2.879; p=0.009) compared to those without anxiety. No statistically significant association was observed between stress(p=0.414), depression (p=0.324), physical activity level (p=0.697), and coffee consumption status (p=0.389) with insomnia. Conclusion/Lesson Learned : The study concludes that in this population, anxiety is the most critical and isolated mental health determinant of insomnia, while behavioral factors such as physical activity and coffee consumption did not emerge as significant risk factors. These findings emphasize the necessity for targeted public health strategies focusing on anxiety screening and early management to alleviate the burden of insomnia among youth in similar urban settings.

Abstract

Background: Bojonegoro Regency is among the 10 regencies/cities with most hypertension in East Java. Similar condition occurred in Tanjungharjo Village, Bojonegoro Regency. Morbidity rate for hypertension cases at Puskesmas Tanjungharjo is ranked first among non-communicable diseases and third among 10 diseases with the most sufferers in that area. Objectives: This study aims to analyze planning and evaluation of hypertension management program using PRECEDE-PROCEED approach in Tanjungharjo Village, Bojonegoro. Research Methods: The research was conducted using mix-methods study design with primary and secondary data. Preliminary study was conducted to obtain hypertension data in Tanjungharjo Village. The research was conducted on 54 PROLANIS participants in Tanjungharjo Village. Primary and secondary data analysis was conducted using PRECEDE-PROCEED framework. Results: The results of preliminary study show that respondents’ hypertension knowledge is low (75,9%). Analysis of respondents characteristics resulted that respondents were suitable for receiving hypertension programs. Analysis using PRECEDE-PROCEED framework shows the need for wider accessibility and more attractive programs. A program called “GERINDI POTENSI” was initiated with 3 sub-programs, namely Kelas Memasak Sehat Yuk (Kemayu) aimed at increasing knowledge regarding diet and healthy cooking methods for hypertension, Senam Lansia Anti Hipertensi (Selasi) aimed at increasing physical activity for hypertension, and Buku Rekap Skrining Kesehatan (Buku Rekan) aimed to summarize health examinations results of PROLANIS participants. Conclusion: : Planning and evaluation of hypertension programs in Tanjungharjo Village using PRECEDE-PROCEED approach resulted “GERINDI POTENSI” program which is suitable with needs and priority problems. Monitoring and evaluation has been conducted with results of achieving program indicators.

Abstract

The prevalence of metabolic syndrome is increasing globally due to various factors that influence the progression of this syndrome. Identifying the factors that influence the development of metabolic syndrome is important, especially among working populations. This study aimed to explore the association between job-related stress and metabolic syndrome in mining workers, with emotional eating and mood disorders as mediating factors. An observational analytic method with a cross-sectional study design was employed. The research subjects were 131 workers from a mining contractor company in East Kalimantan, who were chosen after the research objectives were explained and their informed consent was obtained. The independent variable examined was work stress, and the dependent variable was metabolic syndrome. Emotional eating and mood disturbances were included as mediating variables in the analysis. Data were collected through measurements and questionnaire completion. The data were analyzed using path analysis techniques, and SPSS version 26 was used for statistical processing. The results showed that there was no relationship between work stress and metabolic syndrome mediated by emotional eating and mood disturbances. However, a relationship was found between work stress and the mediator variables of emotional eating and mood disturbances.
