Abstract

Kader kesehatan adalah anggota masyarakat yang dipilih dan dilatih untuk membantu pelaksanaan berbagai program kesehatan, termasuk pencegahan stunting. Meskipun terdapat banyak program peningkatan kapasitas untuk kader, upaya yang secara khusus menargetkan pencegahan stunting selama periode kehamilan dan mengintegrasikan tiga level pencegahan masih terbatas. Tujuan dari kegiatan pengabdian masyarakat ini adalah untuk menilai dampak intervensi keperawatan berbasis komunitas dengan menggunakan metode pembelajaran kooperatif dan kolaboratif yang diintegrasikan dengan tiga level pencegahan terhadap pengetahuan, sikap, dan efikasi diri kader kesehatan dalam mencegah stunting selama kehamilan. Metode yang digunakan adalah pembelajaran kooperatif dan kolaboratif yang diintegrasikan dengan tiga level pencegahan, melibatkan 15 kader kesehatan di Kecamatan Karawaci, Kota Tangerang. Efektivitas diukur dengan menggunakan tiga jenis kuesioner (pengetahuan, sikap, dan efikasi diri), dan data dianalisis dengan menggunakan pendekatan kualitatif deskriptif dan kuantitatif dengan menggunakan uji-t sampel berpasangan. Hasil penelitian menunjukkan adanya pengaruh yang signifikan dari intervensi terhadap pengetahuan, sikap, dan efikasi diri kader kesehatan dalam pencegahan stunting, dengan nilai p < 0,05 untuk semua variabel. Temuan ini menggarisbawahi pentingnya pelatihan berkelanjutan bagi kader kesehatan, terutama dalam mengatasi tantangan stunting selama periode kehamilan.

Abstract

Objective: to examine the relationship between psychological changes and sleep quality with the incidence of hypertension in the elderly.Method: This cross-sectional study, conducted with a purposive sample of 126 elderly participants, assessed psychological changes using the Depression, Anxiety, and Stress Scale (DASS-42) questionnaire and sleep quality using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Blood pressure was measured using a sphygmomanometer to assess hypertension. Data analysis included independent t-tests and chi-square tests, with logistic regression used to identify predictors of hypertension. Results: The independent t-test revealed significant differences in stress levels between participants with and without hypertension (t = -6.614, p = 0.020). Similarly, significant differences in depression levels were observed between these two groups (t = -0.044, p = 0.014). Elderly individuals with poor sleep quality had a 5 times higher risk of experiencing hypertension (OR = 4.625, p = 0.008). Multivariate analysis indicated that age was a predictor of hypertension incidence (β = 0.208, p = 0.004), and sleep quality was also a predictor (β = -1.501, p = 0.023). Discussion: The findings suggest that psychological factors and sleep quality influence hypertension development in the elderly, likely through physiological mechanisms such as heightened sympathetic activity and vascular stress. These results highlight the importance of addressing both psychological health and sleep quality in managing hypertension risk. Conclusion: Sleep quality is a predictor of hypertension incidence in the elderly. Those with poor sleep quality have a 5 times greater risk of developing hypertension compared to those with good sleep quality. Keywords: elderly, hypertension, psychological changes, sleep quality

Abstract

Gangguan jiwa merupakan penyakit kronis yang menyebabkan disabilitas pada kehidupan. Tanda dan gejala yang ada, mengakibatkan penurunan fungsi kehidupan tidak hanya fisik, tetapi juga aspek kehidupan yang lain sehingga terjadi penurunan produktifitas dan kemandirian. Pengobatan dan perawatan jangka panjang, pada orang dengan gangguan jiwa, dibutuhkan keluarga atau caregiver melakukan pendampingan. Sehingga proses pemulihan pasien gangguan jiwa tidak hanya intervensi untuk pasien, tetapi untuk keluarga (caregiver). Program ini memiliki tujuan untuk meningkatkan kemandirian dan produktivitas pasien melalui intervensi pasien dan keluarga pada pemulihan di rehabilitasi sosial Unit Informasi Layanan Sosial (UILS) Kemayoran. Metode yang dilakukan adalah dengan pemberian terapi psikoedukasi keluarga, terapi okupasi pada pasien dengan ODGJ. Kegiatan ini dilakukan pada 20 keluarga dan 20 pasien yang berada di rehabilitasi sosial UILS Kemayoran. Kegiatan ini telah meningkatkan pengetahuan keluarga mengenai gangguan jiwa, cara merawat hingga manajemen stres untuk caregiver dan dibekali buku saku yang berisikan topik tersebut. Peningkatan keterampilan pasien dengan terapi okupasi yaitu menanam dan berjualan, seluruh pasien telah dapat mengikuti kegiatan ini dan mencapai indikator penilaian. Paket intervensi dengan melibatkan caregiver dan pasien diharapkan dapat menurunkan angka kekambuhan, meningkatkan harga diri yang positif, dukungan keluarga, kemandirian, produktifitas di fase pemulihan melalui program rehabilitasi, untuk menyiapkan pasien kembali kedalam masyarakat.

Abstract

Background: The challenge of fatigue significantly affects children undergoing cancer treatment, impacting their overall well-being and quality of life. Unlike their healthy counterparts, pediatric cancer patients grapple with the demanding impacts of chemotherapy, leading to anemia and nutritional deficiencies that exacerbate fatigue. Beyond fatigue, chemotherapy alters appetite, nutrient utilization, and mental function. Fatigue's multidimensional impact, marked by reduced energy and increased need for rest, can detrimentally affect the quality of life for pediatric patients. Understanding these challenges is essential for devising tailored strategies that comprehensively address the unique needs of children undergoing cancer treatment, enhancing their overall well-being and quality of life.Objective: This research aims were to determine the relationship between anemia and nutritional status with the occurrence of fatigue in children with cancer undergoing chemotherapy at Dharmais Cancer Hospital in Jakarta.Methods: An analytical cross-sectional study conducted in August 2022 included a sample of 24 children selected through accidental sampling. Demographic and disease characteristics, such as age, gender, cancer type, cancer stage, and treatment duration, were collected through a questionnaire. Anemia was determined by hemoglobin levels (<12 mg/dl), and nutritional status was assessed using Body Mass Index. Fatigue was measured with tailored instruments for different age groups: the Parent Fatigue Scale (PFS) for children under 7 years and the Child Fatigue Scale (CFS) for those aged 7 years and older. Statistical analyses included univariate assessment, bivariate analysis with the chi-square test, and logistic regression.Results: Demographic analysis revealed a predominantly male sample (70.8%) with a significant adolescent presence (41.7%) and a prevalence of leukemia (37.5%). Most participants were in early cancer stages (58.3%), undergoing treatment for over a year (62.5%). Anemia was common (54.2%), and 58.3% were underweight. High fatigue prevalence was observed (62.5%). Chi-square analysis found no significant gender or cancer stage differences in fatigue. Age and treatment duration showed no significant associations with fatigue. Logistic regression revealed a significant association between anemia and fatigue in the first model (p = 0.022, Exp. B = 0.104). The second model, incorporating nutritional status, showed a non-significant relationship with anemia but a notable association with normal nutritional status and fatigue (p = 0.022, Exp. 0.059). This underscores the crucial role of nutritional interventions in mitigating fatigue.Conclusion: This study highlights a significant relationship between anemia and nutritional status with fatigue in children undergoing chemotherapy. These findings underscore the importance of considering anemia and nutritional interventions when providing care to pediatric cancer patients undergoing chemotherapy. Nurses play a vital role in addressing these concerns and enhancing the overall well-being of children with cancer.

Abstract

Aims: Tuberculosis (TB) remains a critical global health issue, characterized by significant morbidity and mortality rates. Recent research highlights the potential of social support interventions to enhance TB treatment outcomes and improve patient quality of life. This study examines the positive effects of social support interventions on TB patients throughout their treatment process. Method: Utilizing a traditional review approach, this study applies thematic analysis to synthesize findings from the literature. The review encompasses 17 articles selected from reputable databases, including PubMed, Wiley Online Library, ProQuest, and Google Scholar, based on predefined inclusion criteria. These articles underwent quality assessment using the Critical Appraisal Skills Programme (CASP) tool. The PRISMA flowchart provided transparency in the selection process. Results: Key themes identified through the analysis include treatment adherence, psychological well-being, motivation to recover, health-related quality of life, prevention of TB transmission, successful TB treatment, and self-efficacy to recover. Conclusion: The findings suggest that social support interventions can play a crucial role in addressing the persistent challenges of TB management globally.

Abstract

Perubahan pada aspek perilaku, kognitif, peraasan dan emosi mengakibatkan pasien dengan gangguan jiwa membutuhkan orang lain untuk merawat dan membantu memenuhi kebutuhan, yang disebut dengan caregiver dan mayoritas berasal dari keluarga. Proses perawatan yang panjang yang dilakukan oleh caregiver menimbulkan suatu adanya beban dalam merawat, yang disebut dengan caregiver strain. Caregiver strain dapat berdampak pada psikologis caregiver sehingga berdampak dalam kualitas merawat pasien sampai dengan kegagalan dalam merawat pasien. Masalah yang dihadapi oleh caregiver membutuhkan intervensi, oleh sebab itu penelitian ini bertujuan untuk mengetahui pengaruh pemberian psikoedukasi keluarga terhadap caregiver strain. Penelitian ini menggunakan metode kuasi eksperimen dengan analisis paired t test. Instrument yang digunakan yaitu modified caregiver strain index (MCSI) dengan sampel 20 keluarga pasien rehabilitasi sosial Unit Informasi Layanan Sosial (UILS) Jakarta Pusat, pengambilan sampel dengan teknik total sampling. Hasil menunjukan adanya perbedaan signifikan pada caregiver strain sebelum dan sesudah intervensi. Rata-rata perbedaan skor sebelum intervensi adalah M = 14,45 (SD = 1,27), setelah intervensi adalah M = 10,95 (SD = 1,27), dengan perbedaan rerata M = 3,50 (SE = 0,29), dengan interval kepercayaan 95% berkisar antara 2,90 hingga 4,10. Nilai t sebesar 12,25, (df) 19. Nilai signifikansi (p) yang diperoleh adalah < 0,001, menunjukkan bahwa perbedaan ini sangat signifikan secara statistik. Temuan ini menunjukkan bahwa psikoedukasi keluarga efektif dalam menurunkan caregiver strain.

Abstract

Objective: This study aimed to ascertain the prevalence of frailty and associated psychological factors among hemodialysis patients. Methods: A cross-sectional study was conducted employing a convenience sample of CKD patients undergoing hemodialysis at a Jakarta hospital. A total of 151 participants were included in this study. Instruments included demographic and clinical characteristics. The Tilburg Frailty Indicator (TFI) assessed frailty, while the Self-Rating Depression Scale (SDS) and Self-Rating Anxiety Scale (SAS) evaluated psychological status. Data analysis encompassed univariate descriptive statistics, t-test, and Pearson correlation to identify the relationship among variables. A multivariate regression analysis studied frailty index associations with demographic, clinical, and psychological factors. Results: Results: This study indicates that the prevalence of frailty, assessed by the Tilburg Frailty Index (TFI), is 53.6%, primarily evident in the physical aspect. Positive correlations are found between frailty and age, unemployment, higher CCI, and a history of diabetes. Anxiety is correlated with female gender and a history of diabetes, while depression correlates with age, female gender, higher CCI, and a history of diabetes. Conclusion: The study highlights the complexity of frailty among hemodialysis patients. Significant correlations emerged, associating frailty with factors such as age, unemployment, comorbidity, and a history of diabetes. Frailty was linked to psychological aspects, specifically anxiety and depression. Tailored interventions are recommended, focusing on early identification, psychological support, and a holistic care approach. Educational initiatives for healthcare providers are crucial to enhance awareness and improve the quality of care for this vulnerable population, leading to better health outcomes.

Abstract

Aims: Chronic Kidney Disease (CKD) poses a significant global health challenge, with an increasing number of kidney transplantations in Indonesia. The success of kidney transplants hinges on adherence to immunosuppressant medications to prevent organ rejection. Non-adherence among post-transplant patients can lead to infections, graft loss, or a return to dialysis. This study evaluated the impact of the TEMAN SETIA program on adherence to immunosuppressant medications among post-kidney transplant patients at Dr. Cipto Mangunkusumo National Referral Hospital (RSCM) in Jakarta. Methods: A pre-post-test design with a control group was utilized. The sample included 28 post-transplant patients meeting inclusion criteria. The TEMAN SETIA program provided patient mentoring to improve adherence. Adherence was assessed before the intervention, one month after, and two weeks post-intervention using a validated questionnaire. Data were analysed using t-tests and repeated measures ANOVA. Results: The control and intervention groups were comparable in demographic and clinical characteristics, though the intervention group had more non-working participants. The program significantly improved adherence (Cohen’s d = 1.97) and knowledge (Cohen’s d = 1.99). While family support increased in both groups, differences were not significant (Cohen’s d = 1.02). A significant group-by-time interaction (F (1.246, 26) = 7.394, p = 0.007, η² = 0.221) demonstrated greater adherence improvements over time in the intervention group. Discussion: The findings demonstrate that the TEMAN SETIA program is an effective intervention for enhancing medication adherence and knowledge among post-transplant patients. The structured mentoring approach provided patients with the necessary support and education to improve their adherence behaviors. Although family support increased in both groups, the changes were not statistically significant, highlighting the need for further exploration of the role of familial involvement in adherence programs Conclusion: The TEMAN SETIA program effectively enhanced medication adherence and knowledge in post-transplant patients. This structured intervention offers a promising strategy to improve outcomes and reduce risks of organ rejection.

Abstract

Stress and fatigue are common complaints among hemodialysis patients, significantly affecting their quality of life during long-term therapy. This study aimed to evaluate the effectiveness of a modified Mindfulness-Based Stress Reduction (MBSR) intervention on stress and fatigue in patients undergoing hemodialysis. The study adopted a quasi-experimental pretest-posttest control group design and employed simple random sampling. A total of 32 respondents participated, with 16 in each group. Stress was measured using the Perceived Stress Scale (PSS-10) and fatigue using the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F 40). The modified MBSR intervention was conducted for 30 minutes per session. Data were analyzed using paired t-tests and independent t-tests. The results showed significant differences in stress (MD = -13.12, t = -4.748, p < 0.001) and fatigue levels (MD = 7.34, t = -3.865, p = 0.001) between the intervention and control groups. These findings indicate that the modified MBSR therapy was effective in reducing stress and fatigue, with a p-value < 0.05 indicating statistical significance. Therefore, this modified MBSR therapy is a promising non-pharmacological nursing intervention that can be integrated into nursing care for chronic kidney failure patients experiencing stress and fatigue during hemodialysis.
