Abstract

This reforestation assistance program aims to improve the welfare of the people of Barugbug Village, Padarincang District, through environmental conservation efforts that involve the active participation of residents. This reforestation activity does not only focus on tree planting, but also on counseling and environmental education to increase residents' awareness of the importance of preserving nature. The results of this program are expected to provide long-term benefits, such as improved air quality, psychological well-being of the community, and sustainability of the local ecosystem. This program is also expected to be a model that can be replicated in other villages in an effort to create a cool, healthy and sustainable environment.

Abstract

Penelitian ini bertujuan untuk mengevaluasi tingkat ketepatan pengkodean diagnosa dan tindakan pada kasus persalinan dengan sectio caesarea di RS PKU Muhammadiyah Kutowinangun. Rekam medis merupakan komponen krusial dalam manajemen data klinis dan pelayanan kesehatan, serta penting dalam aspek administratif seperti penagihan biaya. Pengkodean yang tepat, sesuai dengan standar seperti ICD-10, sangat diperlukan untuk menghasilkan data yang berkualitas dan menghindari kerugian finansial bagi rumah sakit. Metode penelitian yang digunakan adalah deskriptif kuantitatif, dengan mengambil sampel sebanyak 56 berkas rekam medis menggunakan teknik random sampling. Instrumen penelitian berupa check-list digunakan untuk menilai ketepatan kode diagnosa dan tindakan. Data dianalisis menggunakan statistik deskriptif untuk menghitung persentase ketepatan dan ketidaktepatan pengkodean. Hasil penelitian menunjukkan bahwa dari 56 berkas rekam medis yang dianalisis, tingkat ketepatan pengkodean diagnosa adalah 55,35%, dengan 32,14% kasus menggunakan kode diagnosa yang tidak tepat, dan 12,5% tidak dapat dinilai kode diagnosanya. Sedangkan untuk pengkodean tindakan, tingkat ketepatan mencapai 71,42%, dengan 28,57% kasus menggunakan kode tindakan yang tidak tepat. Simpulan dari penelitian ini adalah bahwa meskipun terdapat tingkat ketepatan yang cukup baik dalam pengkodean tindakan, terdapat tantangan dalam pengkodean diagnosa yang memerlukan perbaikan. Ketidaktepatan dalam pengkodean diagnosa dapat berdampak pada manajemen data klinis dan aspek finansial rumah sakit. Oleh karena itu, disarankan untuk meningkatkan pelatihan SDM dalam penggunaan panduan klasifikasi terbaru serta meningkatkan kejelasan dan kelengkapan rekam medis guna memastikan ketepatan pengkodean.

Abstract

This research was conducted considering that there are still many problems regarding the completeness of medical records, especially for BPJS patients. The number of incomplete filling of medical records and the inaccuracies in the medical record completeness analysis report. The inaccuracy of the data causes miscommunication between doctors and medical record officers. Also, notes about the incompleteness of medical records that doctors need to complete are still done conventionally so they are prone to being lost and unreadable. This study aims to design an electronic medical record information system design to support the reporting of medical record completeness. The research method used is qualitative and data obtained from observations, interviews, literature studies, and software development with Extreme Programming method. This research produces a system that can help officers and doctors to access data on the results of BPJS patient medical record analysis, including information about incompleteness. This can prevent data inaccuracies and ensure officers and doctors know the latest information about the completeness of medical records as a requirement for BPJS claims. This system can also help improve the performance of officers in processing medical record analysis data and produce reports that are more accurate and efficient than the system used previously.

Abstract

The implementation of information release in medical summary requests at Arcamanik Hospital Bandung currently does not implement an electronic-based medical record management system so that the handling of information requests becomes inefficient, and increases the risk of security and privacy of patient data. Therefore, this study aims to implement electronic-based medical record governance in the release of medical record information using the agile method. The agile method is used as an option to solve problems in improving the medical record management system, this method provides flexibility and adaptability in system design, so that it can adjust to user requests. Agile stages include system requirements analysis, design, coding and black box testing. Data was collected through field studies and literature studies related to the implementation of electronic medical record governance. As a result, applying the agile method in this study can help and be an effective solution to improve the quality of medical record management, optimize the handling of information requests, increase the accuracy and security of information, and increase user satisfaction, so that the implementation of electronic-based medical record governance with the agile method can be used as an alternative for health agencies in managing medical records.

Abstract

Limited knowledge Diseases and medical conditions are constantly evolving, and sometimes physicians may not update their knowledge of recent changes in ICD-10 (International Statistical Classification of Diseases) code classifications or other coding systems in use. This may lead to errors or discrepancies in code assignment, the aim of which is to provide global standards in the documentation and reporting of health events. The ICD coding system is essential in the healthcare field because it allows medical professionals to effectively record, analyze, analyze patient data, as well as create global epidemiological statistics. Based on research, creating solutions to problems that occur. Therefore, researchers created an information system with the aim of making it easier for medical records officers to record and determine the 5 major diseases which will later be reported to the Head of Medical Records, such as manual searches, increasing officer workload, and entering data into Microsoft Excel to create a list. the top three diseases are still a long process two of the many drawbacks of indexing. The method proposed to overcome this problem is to apply the development method with Agile Software Development by involving key stakeholders. The features that can be implemented in this software include forms for collecting patient data. This system helps in efficient management of medical files by facilitating simple access to patient data and reducing the chances of human error and loss of information.   Keywords: Information system design; medical record reporting; software development using the Agile Software Development method

Abstract

Technology is currently very rapidly developing in all fields including the health sector, one of which is a hospital in a medical record agency, namely the claiming of BPJS internal medicine polyclinic patients who have not been computerized. The results of research conducted by the author found several problems, namely incomplete internal medicine polyclinic patient data, unfilled entry and exit dates, vacant diagnoses and anamnesis. So that it can affect the claiming of BPJS polyclinic internal medicine patient files, therefore, an information system is needed to support the completeness of BPJS patient medical record files so that claims are made quickly and accurately. Methods for collecting research data used include literature studies, interviews, and observations. The design of the information system uses the V model with the stages of Verification, Validation, Coding. System development consists of Flowmap, DFD, and ERD. then implemented using microsoft visual studio 2010 and database using microsoft access 2021. The results of designing an information system using the methods that have been chosen can help the performance of officers facilitate claiming and support the completeness of the medical record files of BPJS internal medicine polyclinic patients for accurate claiming.

Abstract

Rapidly developing technology can support various human activities or performance, one of which is the performance of hospital employees who work on emergency patient medical record installations which are not yet fully optimized and computerized. Various problems were found, such as recording BPJS claims reports which were still manual and only relied on programs such as Microsoft Excel, errors in making Patient Eligibility Letters (SEP) which resulted in data being delayed so that the BPJS claims process was hampered or claims were pending. Therefore, an information system is needed that is useful for making the performance of medical records employees more effective in making BPJS claim verification reports for emergency room patients more easily, quickly and accurately. The development method used by researchers in this research is the Agile method, while the research data collection method includes observation, interviews, documentation with descriptive analysis. From the results of the design, an information system design was produced with various methods chosen to streamline the performance of medical records employees in preparing BPJS claim verification reports for emergency room patients, thereby producing accurate and complete data. Able to optimize services and reduce data errors that result in delayed claims, maintain the confidentiality of claims data and also control the completeness of files and improve the quality of service at the hospital.

Abstract

With a focus and in-depth analysis of the case findings from research conducted at Hospital X, it was found that in the process of submitting claims for national health insurance incomplete patient data often hinders the claim submission process. Delays in claim submission can impact the hospital's finances and the quality of services. Therefore, the completeness of patient data is a crucial factor in the claim submission process. The purpose of this research is to design a confirmation menu to support decision-making for national health insurance claims using the extreme programming method. The main focus of this research is to develop a confirmation menu in the system to assist staff in addressing incomplete patient data. This design is built using the extreme programming method, which is considered more flexible, simple, and easy to adapt to frequently changing requests. As a result of this research, the application of the extreme programming method in system design can assist the staff involved in the claim process to confirm and communicate when patient data is incomplete. Additionally, this system can improve performance indexes and minimize hospital losses, thereby continuously enhancing excellent service delivery.

Abstract

Medical records are data about the history of patients who receive treatment at health care institutions. Along with the development of technology, most healthcare services in Indonesia have now switched from paper-based medical records to digital ones to speed up healthcare services. Despite the good impact of digital medical records, there are various problems, especially for handling huge medical record data. Efficient and effective processing of medical record data is essential to improve the quality of health services. This research utilizes inpatient medical record data into several groups by using the data mining clustering method using the k-means clustering algorithm to handle very large and complicated data by grouping inpatient data into several clusters. Data mining k-means clustering can help organize and analyze medical record data more effectively, so as to improve service quality. The results obtained from this study are inpatient data divided into 4 clusters with 2 variables, namely age category and diagnosis. The results of this analysis obtained data from each cluster, namely cluster 0 disease categories of arthritis, asthma, and cancer patients with adolescent and elderly age categories, cluster 1 disease categories of diabetes, hypertension, and obesity patients with adolescent and elderly age categories, cluster 2 disease categories of diabetes, hypertension, and obesity patients with adult age categories, cluster 3 disease categories of arthritis, asthma, and cancer patients with adult age categories, with a total of 10,000 patients. The findings of this study provide information about the spread of diseases based on the age range of patients, which can be the basis for minimizing the spread of diseases based on age range.

Abstract

The rapid development of technology in this era has prompted various sectors to undergo digitalization, including hospitals. The primary objective of this research is to create a web-based traffic accident information system that will enhance the efficiency of processing and reporting traffic accident patient data. Data was collected through interviews, observations, and literature studies using a descriptive qualitative approach. The software development method employed is extreme programming. The system is built using the Laravel v10 framework with the PHP v8.1 programming language, complemented by MySQL v8 as its database. The results of the study at Hospital X indicate that traffic accident data reporting is still conducted manually using Microsoft Excel. This research aims to provide a reference for healthcare facilities, especially hospitals, to develop such a system. The information system is built and tested using black-box testing. Based on the functional aspects tested, the system operates effectively. It can be concluded that the system is suitable for use. Thus, medical record officers no longer need to sort emergency department patient data by admission date, diagnosis, and external cause, thereby facilitating the aggregation process and producing more comprehensive information.
