Abstract

Limited knowledge Diseases and medical conditions are constantly evolving, and sometimes physicians may not update their knowledge of recent changes in ICD-10 (International Statistical Classification of Diseases) code classifications or other coding systems in use. This may lead to errors or discrepancies in code assignment, the aim of which is to provide global standards in the documentation and reporting of health events. The ICD coding system is essential in the healthcare field because it allows medical professionals to effectively record, analyze, analyze patient data, as well as create global epidemiological statistics. Based on research, creating solutions to problems that occur. Therefore, researchers created an information system with the aim of making it easier for medical records officers to record and determine the 5 major diseases which will later be reported to the Head of Medical Records, such as manual searches, increasing officer workload, and entering data into Microsoft Excel to create a list. the top three diseases are still a long process two of the many drawbacks of indexing. The method proposed to overcome this problem is to apply the development method with Agile Software Development by involving key stakeholders. The features that can be implemented in this software include forms for collecting patient data. This system helps in efficient management of medical files by facilitating simple access to patient data and reducing the chances of human error and loss of information.   Keywords: Information system design; medical record reporting; software development using the Agile Software Development method

Abstract

Technology is currently very rapidly developing in all fields including the health sector, one of which is a hospital in a medical record agency, namely the claiming of BPJS internal medicine polyclinic patients who have not been computerized. The results of research conducted by the author found several problems, namely incomplete internal medicine polyclinic patient data, unfilled entry and exit dates, vacant diagnoses and anamnesis. So that it can affect the claiming of BPJS polyclinic internal medicine patient files, therefore, an information system is needed to support the completeness of BPJS patient medical record files so that claims are made quickly and accurately. Methods for collecting research data used include literature studies, interviews, and observations. The design of the information system uses the V model with the stages of Verification, Validation, Coding. System development consists of Flowmap, DFD, and ERD. then implemented using microsoft visual studio 2010 and database using microsoft access 2021. The results of designing an information system using the methods that have been chosen can help the performance of officers facilitate claiming and support the completeness of the medical record files of BPJS internal medicine polyclinic patients for accurate claiming.

Abstract

Rapidly developing technology can support various human activities or performance, one of which is the performance of hospital employees who work on emergency patient medical record installations which are not yet fully optimized and computerized. Various problems were found, such as recording BPJS claims reports which were still manual and only relied on programs such as Microsoft Excel, errors in making Patient Eligibility Letters (SEP) which resulted in data being delayed so that the BPJS claims process was hampered or claims were pending. Therefore, an information system is needed that is useful for making the performance of medical records employees more effective in making BPJS claim verification reports for emergency room patients more easily, quickly and accurately. The development method used by researchers in this research is the Agile method, while the research data collection method includes observation, interviews, documentation with descriptive analysis. From the results of the design, an information system design was produced with various methods chosen to streamline the performance of medical records employees in preparing BPJS claim verification reports for emergency room patients, thereby producing accurate and complete data. Able to optimize services and reduce data errors that result in delayed claims, maintain the confidentiality of claims data and also control the completeness of files and improve the quality of service at the hospital.

Abstract

With a focus and in-depth analysis of the case findings from research conducted at Hospital X, it was found that in the process of submitting claims for national health insurance incomplete patient data often hinders the claim submission process. Delays in claim submission can impact the hospital's finances and the quality of services. Therefore, the completeness of patient data is a crucial factor in the claim submission process. The purpose of this research is to design a confirmation menu to support decision-making for national health insurance claims using the extreme programming method. The main focus of this research is to develop a confirmation menu in the system to assist staff in addressing incomplete patient data. This design is built using the extreme programming method, which is considered more flexible, simple, and easy to adapt to frequently changing requests. As a result of this research, the application of the extreme programming method in system design can assist the staff involved in the claim process to confirm and communicate when patient data is incomplete. Additionally, this system can improve performance indexes and minimize hospital losses, thereby continuously enhancing excellent service delivery.

Abstract

Medical records are data about the history of patients who receive treatment at health care institutions. Along with the development of technology, most healthcare services in Indonesia have now switched from paper-based medical records to digital ones to speed up healthcare services. Despite the good impact of digital medical records, there are various problems, especially for handling huge medical record data. Efficient and effective processing of medical record data is essential to improve the quality of health services. This research utilizes inpatient medical record data into several groups by using the data mining clustering method using the k-means clustering algorithm to handle very large and complicated data by grouping inpatient data into several clusters. Data mining k-means clustering can help organize and analyze medical record data more effectively, so as to improve service quality. The results obtained from this study are inpatient data divided into 4 clusters with 2 variables, namely age category and diagnosis. The results of this analysis obtained data from each cluster, namely cluster 0 disease categories of arthritis, asthma, and cancer patients with adolescent and elderly age categories, cluster 1 disease categories of diabetes, hypertension, and obesity patients with adolescent and elderly age categories, cluster 2 disease categories of diabetes, hypertension, and obesity patients with adult age categories, cluster 3 disease categories of arthritis, asthma, and cancer patients with adult age categories, with a total of 10,000 patients. The findings of this study provide information about the spread of diseases based on the age range of patients, which can be the basis for minimizing the spread of diseases based on age range.

Abstract

The rapid development of technology in this era has prompted various sectors to undergo digitalization, including hospitals. The primary objective of this research is to create a web-based traffic accident information system that will enhance the efficiency of processing and reporting traffic accident patient data. Data was collected through interviews, observations, and literature studies using a descriptive qualitative approach. The software development method employed is extreme programming. The system is built using the Laravel v10 framework with the PHP v8.1 programming language, complemented by MySQL v8 as its database. The results of the study at Hospital X indicate that traffic accident data reporting is still conducted manually using Microsoft Excel. This research aims to provide a reference for healthcare facilities, especially hospitals, to develop such a system. The information system is built and tested using black-box testing. Based on the functional aspects tested, the system operates effectively. It can be concluded that the system is suitable for use. Thus, medical record officers no longer need to sort emergency department patient data by admission date, diagnosis, and external cause, thereby facilitating the aggregation process and producing more comprehensive information.

Abstract

Penelitian ini bertujuan untuk mengevaluasi tingkat kesiapan implementasi rekam medis elektronik (RME) di Klinik Mata Dr. Hasri Ainun Habibie pada tahun 2024. Metodologi penelitian ini menggunakan metode kualitatif dengan parallel implementation melalui pengumpulan data dengan observasi dan wawancara. Hasil penelitian menyatakan bahwa alur pelayanan di Klinik Mata Dr. Hasri Ainun Habibie masih harus disesuaikan dengan alur yang sudah berjalan sebelum implementasi RME. Masalah yang terjadi Sumber daya manusia di klinik, terutama DPJP, membutuhkan waktu untuk belajar RME. Mereka, seperti DPJP dan anggota staf lainnya yang sudah terbiasa dengan sistem manual, harus mulai belajar dan beradaptasi dengan fitur, alur kerja, dan logika sistem RME yang baru. Dalam kebanyakan kasus, sistem RME memiliki banyak fitur dan tugas yang kompleks, seperti manajemen data pasien, pencatatan tindakan medis, resep obat, dan integrasi dengan sistem laboratorium Sesuai dengan kebijakan Menteri Kesehatan Nomor 24 tahun 2022 tentang rekam medis, penggunaan RME dapat mengurangi penggunaan kertas, meningkatkan efisiensi, dan meningkatkan kemudahan akses informasi. Saran yang diharapkan dipelaksanan yaitu membentuk tim khusus yang bertanggung jawab untuk mengawasi dan mendukung proses implementasi RME. Tim ini memiliki kemampuan untuk memberikan bimbingan langsung kepada staf klinik dalam mengatasi masalah dan memastikan proses berjalan lancar dan memberikan pelatihan komprehensif kepada seluruh staf klinik, terutama DPJP, tentang penggunaan sistem RME.

Abstract

Abstract This system functions to record daily, monthly, and yearly censuses without changing the performance of the computer used because the data is stored using cloud storage. The purpose of this study was to determine the governance of information technology-based electronic medical records in reporting emergency department patients using the Agile method. The recording of daily, monthly, and yearly censuses in the emergency room at Hermina Arcamanik Hospital in Bandung is not optimal due to frequent problems with the local area network (LAN); computers often hang or freeze, and the system has not been updated. Currently, the recording of daily, monthly, and yearly sensors still uses the Visual Basic program, which has the concern that it requires a very large amount of memoryduring the installation process. Management of information technology-based electronic medical records in reporting emergency department patients using agile methods The agile method used in the development of this program is feature-driven development (FDD). The system that has been made is tested using the black-box testing method. This program only focuses on completing a certain program and produces medical record management for reporting emergency department patients based on information technology in order to speed up the work process and minimize the occurrence of device errors. Keywords: Agile, Black-box testing, Patient Census, Web Service Abstrak Sistem ini berfungsi untuk mencatat sensus harian, bulanan maupun tahunan dengan tidak merubah performance dari komputer yang digunakan, karena penyimpanan data menggunakan cloud storage. Tujuan penelitian ini untuk mengetahui tata kelola rekam medis elektronik berbasis teknologi informasi dalam pelaporan pasien IGD dengan metode Agile. Pencatatan sensus harian, bulanan maupun tahunan di IGD Rumah Sakit Hermina Arcamanik Bandung belum optimal, dikarenakan seringnya terjadi kendala pada jaringan Local Area Network (LAN), komputer sering mengalami hang atau freeze, dan dengan sistem yang belum diperbaharui. Pencatatan sensur harian, bulanan maupun tahunan pada saat ini masih menggunakan program Visual Basic, untuk program tersebut memiliki concern membutuhkan memory yang sangat besar pada saat proses instalasi. Tata kelola rekam medis elektronik berbasis teknologi informasi dalam pelaporan pasien IGD menggunakan metode Agile. Metode Agile pada program yang digunakan pada pengembangan program ini yaitu Feature Driven Development (FDD). Sistem yang telah dibuat diujikan menggunakan metode black-box testing. Program ini hanya berfokus untuk menyelesaikan suatu program tertentu, dan menghasilkan tata kelola rekam medis dalam pelaporan pasien IGD berbasis teknologi informasi, agar mempercepat proses pekerjaan dan meminimalisir terjadinya error device. Abstract

Latar Belakang: Rumah sakit adalah institusi pelayanan kesehatan Review yang dilakukan dalam penelitian ini mengacu pada standar MIRM 13.4 dari Rumah Sakit X Bandung Tujuan: Tujuan dari penelitian ini adalah untuk mengetahui apakah pelaksanaan review berkas rekam medis di rumah sakit X kota bandung sudah sesuai dengan standar MIRM 13.4 Metode: Metode yang digunakan dalam penelitian ini adalah metode deskriptif dan kuantitatif. Sampel yang diambil sebanyak 84 berkas dari 561 populasi. Hasil: Hasil penelitian terdapat 24 berkas (2,85%) yang tidak tepat dalam pengembaliannya dan yang tepat sebanyak 60 berkas (71,4%). Dalam keterbacaan ada 10% yang tidak terbaca dan 90% terbaca. Secara kelengkapan ada 8 formulir yang tidak lengkap diataranya Assesment IGD, Assesment Edukasi, Lembar Penolakan, Form Edukasi, Resume, Transfer Ruangan, dan yang mecapai lengkap terdapat pada formulir Triage serta Informed Consent. Kesimpulan: Berdasarkan hasil penelitian yang telah dilakukan masih terjadi ketidaktepatan waktu sebesar 28,5%, Ketidakterbacaan pada formulir sebesar 10% dan Ketidaklengkapan sebesar 53,5% terdapat pada formulir CPPT dan Transfer Ruangan.

Abstract

Latar Belakang: Dokumen rekam medis merupakan bagian dari bukti hukum dalam undang-undang. Tenaga medis dan tenaga kesehatan lainnya wajib melengkapi rekam medis sebagai alat bukti yang baik pada saat memberikan pelayanan atau pengobatan kesehatan kepada pasien. Tujuan: Tujuan penelitian ini adalah untuk mendeskripsikan kelengkapan dokumen rekam medis pada fungsi assembling di RS AR Bunda Prabumulih tahun 2021. Metode: Metode penelitian yang digunakan adalah deskriptif, melihat hasil observasi pengembalian dokumen rekam medis bulan Juni 2021 di RS AR Bunda Prabumulih yang tidak lengkap dengan analisis kualitatif. Informasi atau responden penelitian ini adalah 2 (dua) orang petugas assembling di unit rekam medis RS AR Bunda Prabumulih. Hasil: Berdasarkan observasi dan wawancara, disimpulkan bahwa ketidaklengkapan dokumen rekam medis disebabkan oleh kurangnya disiplin dokter dan petugas ruang rawat inap dalam pengisian dokumen rekam medis, kurangnya evaluasi dokumen rekam medis dan tidak adanya reward and punishment kepada petugas untuk pengisian dokumen rekam medis dan dari sejumlah 1043 dokumen rekam medis yang belum dikembalikan sebanyak 106 dokumen rekam medis masih belum lengkap. Kesimpulan: Sesuai hasil penelitian dapat disimpulkan bahwa kurangnya kedisiplinan Dokter dan petugas ruang rawat inap untuk mengisi dokumen rekam medis, kurangnya evaluasi dari pihak manajemen rumah sakit dalam mengevaluasi dokumen-dokumen rekam medis apa saja yang kurang lengkap, terkait kurang lengkapnya dokumen rekam medis di rumah sakit AR Bunda perlu adanya reward dan punishment kepada petugas untuk mengisi dokumen rekam medis.
