Abstract

Kesalahan pemberian obat (medication error) masih menjadi masalah utama dalam keselamatan pasien. Data World Health Organization (WHO) dan Joint Commission International (JCI) menunjukkan bahwa kesalahan pengobatan masih sering terjadi secara global. Di Indonesia, medication error menempati urutan pertama insiden keselamatan pasien sebesar 24,8% dengan total 7.400 kasus, di mana 2,3% berakibat fatal. Kesalahan yang paling sering terjadi meliputi salah dosis (40,9%), salah obat (16%), dan salah rute (9,5%). Kondisi ini menunjukkan pentingnya penerapan prinsip 8 benar pemberian obat, yang sangat dipengaruhi oleh tingkat pengetahuan perawat. Penelitian ini bertujuan untuk mengetahui hubungan antara pengetahuan perawat dengan penerapan prinsip 8 benar pemberian obat di ruang rawat inap RSUD Jenderal Ahmad Yani Metro. Penelitian ini menggunakan metode kuantitatif dengan desain cross-sectional. Sampel penelitian berjumlah 71 perawat yang dipilih menggunakan teknik stratified random sampling. Instrumen penelitian berupa kuesioner pengetahuan (16 item) dan kuesioner penerapan prinsip 8 benar (20 item) yang telah diuji validitas dan reliabilitas. Analisis data dilakukan secara univariat dan bivariat menggunakan uji Gamma. Hasil penelitian menunjukkan bahwa mayoritas perawat memiliki tingkat pengetahuan baik sebanyak 43 responden (60,6%) dan penerapan prinsip 8 benar dalam kategori baik sebanyak 34 responden (47,9%). Hasil uji statistik menunjukkan nilai p-value = 0,001 (<0,05), yang menandakan terdapat hubungan signifikan antara pengetahuan perawat dengan penerapan prinsip 8 benar pemberian obat. Disimpulkan bahwa pengetahuan perawat berperan penting dalam meningkatkan kepatuhan terhadap prinsip 8 benar. Disarankan agar rumah sakit meningkatkan pelatihan berkelanjutan, supervisi klinis, dan integrasi prinsip 8 benar dalam standar operasional prosedur guna meningkatkan keselamatan pasien.

Abstract

The work motivation of implementing nurses is an important factor in determining the quality of nursing services in hospitals. However, problems are still found in the form of low work motivation characterized by a lack of discipline, work enthusiasm, appreciation, and unequal distribution of tasks. This condition is thought to be related to the suboptimal implementation of the ward head management function, including planning, organizing, directing, and controlling. This study aims to determine the relationship between the implementation of the ward head management function and the work motivation of implementing nurses in the Inpatient Ward of the Bandar Lampung Army Health Detachment Hospital. This study used a quantitative, correlative analytical design with a cross-sectional approach. The population and sample of the study amounted to 60 implementing nurses using a total sampling technique. Data collection was carried out using a questionnaire in September 2025, then analyzed univariately and bivariately. The results of the statistical test showed a p value = 0.007 (p 0.05), which indicates a significant relationship between the ward head management function and the work motivation of implementing nurses. Thus, it can be concluded that the effective implementation of ward head management functions is closely related to increased work motivation among nurses. Therefore, nurses are expected to continue to improve their work motivation through competency development, strengthening teamwork, and active participation in nursing activities

Abstract

The International Diabetes Federation (IDF) in 2021 recorded 19.47 million people with diabetes mellitus in Indonesia with a prevalence of 10.6% and is predicted to increase to 28.57 million in 2045. The number of elderly people with diabetes at the Bumidaya Health Center has increased from 2022, namely 125 people to 146 people 2023. The risk factor for most diabetics is the elderly, one of the government's efforts to improve the health of the  with the elderly Posyandu program, but the activity rate of the elderly visit to the Bumidaya Puskesmas is still low, which is below 50%, which means that this figure is still below the Ministry of Health standard with a minimum of 8 attendances or 66% in one year. Education and family support are needed to improve the quality of life of the elderly. The purpose of the study was to determine the relationship between education and family support with visiting elderly diabetes mellitus at the Bumidaya Health Center. Type of quantitative research using cross sectional method. The study population was elderly with a history of diabetes mellitus. The number of research samples was 102 respondents taken by purposive sampling technique. Research time October 10-24, 2024. Collecting data on education and family support using a questionnaire, data on the activity of elderly visits with the elderly Posyandu register book. Bivariate analysis using Chi square and Mann whitney tests. Based on statistical tests obtained p-value 0.539> α (0.05) there is no relationship between education and elderly visits and p-value 0.025 < α (0.05) there is a relationship between family support and elderly visits. It is hoped that health workers will improve health services, the elderly family always provides support by accompanying and motivating the elderly to maintain health and routinely come to the Elderly Posyandu.

Abstract

Background: Hospital readmission is a key indicator of the quality and continuity of patient care. In Indonesia, readmission rates have been reported at approximately 8.8%, contributing to increased healthcare costs ranging from 104% to 113% of initial treatment expenses. Effective discharge planning is essential to ensure continuity of care and reduce the likelihood of avoidable readmissions.Objective: This study aimed to examine the relationship between the implementation of discharge planning and hospital readmission risk, as measured using the LACE index.Methods: A correlational study with a cross-sectional design was conducted among 100 patients admitted to adult inpatient wards. Participants were selected using proportionate stratified random sampling. Inclusion criteria included patients who were conscious, had limb disorders, chronic illnesses, or long-term wound care needs, and required assistance with daily activities upon discharge. Data were collected using a validated discharge planning questionnaire and the LACE index to assess readmission risk. Statistical analysis was performed using the Gamma test to determine the association between variables, with significance set at p < 0.05.Results: The findings showed that 57% of respondents had a moderate risk of readmission based on the LACE index, while 48% experienced a high level of discharge planning implementation. A statistically significant relationship was found between discharge planning implementation and readmission risk (p = 0.001), indicating that better discharge planning is associated with lower risk of hospital readmission.Conclusions: The study highlights the critical role of comprehensive discharge planning in reducing hospital readmissions. Early and continuous implementation of discharge planning from admission through discharge can enhance continuity of care and improve the efficiency of nursing services. These findings provide important implications for hospital management in strengthening discharge planning practices as part of quality improvement strategies.
