Abstract

Childbirth is a maternal experience that often results in trauma for the mother. Pain before and after delivery is one of the factors that makes the mother uncomfortable. The mother's discomfort will make the mother stressed and will not make the mother comfortable with her new assignment. Foot massage is one method that can be applied to postpartum mothers to reduce the pain and comfort experienced by most mothers in the early postnatal period. Comfort during postpartum will improve the mother's quality of life. The location of the service activity was at the Nining Independent Midwife Practice, Deli Serdang Regency with a total of 14 participants. The target of this activity is postpartum mothers and mothers' companions during treatment. The activity methods used are lectures, role plays and simulations. The activity was carried out twice, namely on 16 and 23 May 2024. The activity consisted of preparation, implementation and evaluation. The results of this service show that most of the participants were >35 years old, female and had a high school education. There is a change in knowledge regarding optimizing foot massage to reduce pain and comfort after childbirth. Before being given health education, the majority of participants had knowledge, namely less than 78.57% and those who had sufficient knowledge were 21.42%. After being given education, the majority of participants had good knowledge about foot massage (57.14%) and 4 participants (28.57%) had good knowledge about foot massage. The presentation of increased knowledge after being given health education was 78.57%. It is hoped that foot massage education can be optimally implemented for postpartum mothers during the postpartum period so that it can help mothers reduce the pain and discomfort that occurs during the postpartum care period.

Abstract

Background: Adamantiades–Behçet is a genetically determined disorder with a probable environmental triggering factor and is an inflammatory disease representing vasculitis. Chronic relapsing progressive course, Dapson prevents problems in other organs. Case Report: A 31-year-old woman with chief complaints of a progressive painful ulcer on her genitalia in the last month, covered by pus and with a bad odor, and also recurrent tongue sores that healed spontaneously. Multiple partner sexual intercourse, joint pain, and an eye complaint were denied. Her husband has no history of previous sexually transmitted diseases. The labia majora region showed a solitary ulcer, sharply marginated, and covered with pus. The Haemophilus ducreyi, pathergy, VDRL, TPHA, and rapid test HIV all came back negative. Enterococcus faecalis, found in bacterial culture, is sensitive to Penicillin and glycopeptide. Histopathologically recited vasculitis with thrombus in the blood vessel marks Behcet disease. The patient was given Mefenamic Acid (3x500mg), doxycicline (2x100 mg), and a wet dressing. The ulcer decreased in size after 3 weeks of treatment. Discussion: Adamantiades-Behçett disease is characterized by chronic, recurrent oral aphthous ulcers, genital ulcers, skin lesions, ocular lesions, and arthritis or arthropathy. Viral and bacterial infections have been implicated in initiating immunopathologic pathways. The pathergy test is pathognomonic, but it is not specific and often gives a negative result. Histopathological examination is still the best option to establish the diagnosis.

Abstract

Background: Vitiligo is a depigmentation disorder characterized by the progressive loss of melanocytes from the epidermis and hair follicles. Vitiligo affects all age groups and is defined by acquired, idiopathic, progressive, different-sized, circumscribed, milky white depigmentation. Characteristics and clinical patterns of vitiligo will also help to understand the pathogenesis of the disease. Purpose: The aim of this study is to evaluate the characteristics profile and clinical features of vitiligo patients. Methods: The research material was taken from the medical record of vitiligo patients in the Division of Cosmetics Outpatient Clinic Dermatovenerelogy Dr. Soetomo General Hospital Surabaya from 2018-2020. Result: This study was obtained by a number of new vitiligo patients 115 (0.3%) of the total number of new outpatient clinic visits of Dermatovenereology Dr. Soetomo General Hospital Surabaya. The majority of the gender is female (53.9%), late adolescence (26.96%), students (25.22%), and the onset of vitiligo 1-5 years (30.43%). The most common risk factors were idiopathic (51.3%) and clinical types are non-segmental vitiligo (53.04%). Conclusion: Vitiligo predominantly affects in females with active age groups of 17-25 years old, and students.

Abstract

Background: After a history of exposure to the suspected material and a thorough physical examination, the diagnosis of occupational contact dermatitis (OCD) is commonly suspected. During the initial consultation, essential information involving the above is frequently neglected. Purpose: To evaluate the clinical manifestations of OCD cases in health workers and analyze the factors that influence OCD cases in health workers at the isolation ward of Dr. Soetomo Surabaya Hospital during the COVID-19 pandemic. Methods: The sample selection was done through purposive sampling. Accessible population that met the inclusion criteria and did not have exclusion criteria were included as research samples. The target population was healthcare workers in the Special Isolation Ward of Dr. Soetomo Hospital Surabaya. The accessible population was healthcare workers who performed hand hygiene activities and used Personal Protective Equipment (PPE) in the Special Isolation Ward of Dr. Soetomo Hospital Surabaya. The research subjects were healthcare workers who worked in the Special Isolation Ward. Result: The research results showed a strong and significant correlation between atopic history and the occurrence of contact dermatitis due to poor skin barrier function and decreased tolerance. The study also found that there was no significant correlation between atopic history and the severity level of OCD. There was a significant difference in transepidermal water loss (TEWL) values in the lesion and non-lesion areas, both on the palmar and dorsal aspects of the hand. Moisturizer is an important component in managing hand dermatitis as it can repair and maintain the skin's protective barrier. Conclusion:

Abstract

Background: Xerosis cutis presents in more than 50% of the elderly population and is currently the most common complaint related to a skin condition in the elderly population. However, many of them are still uninformed of this complaint. Education plays an important role in the management of xerosis cutis, such as how to identify and avoid triggering factors and how to break the itch-scratch cycle. Purpose: This study aims to evaluate the level of knowledge on xerosis cutis in the elderly population after receiving health education. Methods: This observational study involved 71 subjects who met the inclusion criteria of being elderly and were willing to participate in this study. The data were collected using questionnaires. The level of knowledge was assessed before and after the health education around xerosis cutis. Result: Eleven (15.49%) male and 60 (84.51%) female subjects participated in this study. There was a significant difference mean scores between the pre-test and post-test after health education (p= <0.001). The means of total scores of the pre-test and post-test were 11.72 ± 1.475 and 12.58 ± 1.662. The questionnaire consisted of  chapters regarding skin changes in the elderly, risk factors for xerosis cutis, and management of xerosis cutis. A significant difference in score between the pre-test and post-test was found in all the chapters (p=0.046, p=0.002, and p=0.006). Conclusion: Following health education, there was an improvement in the elderly's level of knowledge on xerosis cutis.

Abstract

Background: Psoriasis vulgaris can be diagnosed clinically. A biopsy is usually used to confirm non-classic cases. The findings of the histopathology feature are not usually present, thus clinical relevance is needed to confirm the diagnosis. Purpose: To describe the histopathology findings from tissue samples of histopathologically confirmed psoriasis vulgaris reported in the Department of Pathology and Anatomy of Dr. Soetomo General Hospital. Methods: A descriptive-retrospective study of pathology-ascertained tissue samples of histopathologically confirmed psoriasis vulgaris was reported in the Department of Pathology and Anatomy from patients who were also previously diagnosed clinically with psoriasis vulgaris in the Dermatology and Venereology Outpatient Unit of Dr. Soetomo General Academic Hospital over a period of 2 years. Morphological parameters were observed after histopathology sections were stained with hematoxylin and eosin. Result: Thirty-three tissue samples were examined. Parakeratosis (86%) and hypogranulosis (70.3%) were the most observed findings. Club-shaped rete ridges, suprapapillary plate thinning, and spongiform pustules of Kogoj were the least observed findings (each by 2.7%). Three tissue samples (8.1%) showed only parakeratosis. Only 1 tissue sample (2.7%) showed 4 features (absence of the granular layer, parakeratosis, microabscess of Munro, and spongiform pustules of Kogoj). No tissue sample showed every histopathology finding of psoriasis vulgaris. Conclusion: All of the histopathology features of psoriasis vulgaris are rarely found in one tissue section. Albeit histopathology is believed to be the gold standard diagnosis of psoriasis vulgaris, relevance with clinical findings is still mandatory to support the diagnosis.

Abstract

Background: Contact dermatitis is an inflammatory skin response that can be caused by several things. This inflammatory reaction is most often caused by external compounds that become allergens or irritants to the body. Purpose: To evaluate the clinical profile of contact dermatitis patients at the allergy-immunology division of dermatology and venereology outpatient clinic of Dr. Soetomo general academic hospital. Methods: The sampling technique used in this study is a total sampling technique and was processed with Excel descriptively. Result: The most frequent age group diagnosed with contact dermatitis was the 20–45-year-old age group (45.5%). The most frequent gender was female (72.2%). The most frequent occupation was private employee (30.8%). The most frequent suspected causal factor was cosmetics (35.2%). Itching was the most common type of patient complaint (44.1%). Face was the most frequent location (30.8%). Erythematous macules were the most frequent clinical manifestation (45.9%). The diagnosis of contact dermatitis was divided into allergic contact dermatitis ACD (60.4%) and irritant contact dermatitis ICD (39.6%). The most frequent topical therapy was corticosteroid (53.2%). Meanwhile, the most systemic therapy is antihistamine (73.8%). Conclusion: ACD is more prevalent than ICD and is more prevalent in women between the ages of 20 and 45. The most prevalent occupation distribution is among private employees. Cosmetics are the most frequently suspected cause factor. Itch is the most common patient complaint. The face is where contact dermatitis occurs most frequently. Erythematous macules are the most frequent clinical symptom. Corticosteroids and antihistamines are the most commonly given medication.

Abstract

Background: Melasma is a skin pigmentation disorder characterized by brownish hyperpigmented lesions with symmetrical distribution and irregular borders. It becomes a cosmetic problem that causes a negative impact on quality of life and a lack of confidence in patients. The etiology of melasma is not completely understood, but several factors that trigger melasma can be explained. Yet, the profile and triggering factors associated with the prevalence of melasma patients in Surabaya are still unexplored. Purpose: To determine the profile and triggering factors of melasma patients in the Dermatology and Venereology Outpatient Unit of Dr. Soetomo General Academic Hospital Surabaya from January to December 2019. Methods: A retrospective descriptive study using data from medical records with a total sampling technique. Result: In this study, most of the melasma patients were female (97.5%), dominated by the age group 46-55 years (49.4%), and worked as housewives (40.74%). The triggering factors found in patients were sun exposure (96.3%), cosmetics (67.9%), hormonal (49.4%), and family history (22.2%). The physical examinations of melasma lesions showed that they were mostly located in the malar area (72.8%), light brown colored (74.7%), and 2.0 – 4.0 cm in size (54.3%). Wood's lamp examination showed that most of the lesions had firm and indistinct borders (55.6%). Conclusion: The triggering factors of melasma are multifactorial, but we suspect that sun exposure plays an important role in affecting melasma.

Abstract

Background: One of the most prevalent skin conditions worldwide is acne vulgaris (AV). Even mild forms of AV could have an adverse impact on psychological wellbeing. Purpose: To explore the profile of mild AV patients in a tertiary care facility in Surabaya, Indonesia. Method: This was a descriptive study conducted for eight weeks. Data regarding the patient's identity, history, skin type, and complaints related to AV or other complaints that arose were noted on the sample's medical record and collected for analysis. The Lehmann classification was used to assess the AV degree. Result: A total of 34 patients were included. Gender equality was similar in our study, and late adolescence (17-25 years) is the most dominant age group (73.5%). Genetics plays the biggest precipitating factor (73.5%), followed by stress (70.6%). Most patients had an oily skin type (91.2%) with Fitzpatrick's phototype 4 (85.3%). Comedone is the most prevalent lesion in our study (median = 10, range = 4-20). Conclusion: Most patients with mild AV had an oily skin type, and late adolescent is most dominant age group.

Abstract

Background: Atopic dermatitis (AD) is a chronic inflammatory skin disorder that typically manifests in childhood and has a diverse etiology. In the United States, the prevalence of AD among adults was 4.9%, while in Japan it was 2.1%. The majority of people with AD have dry skin. Hanifin-Rajka criteria were the first to be established and are frequently used as diagnostic standards in research. Purpose: The purpose of this study is to determine the Hanifin-Rajka criteria and skin hydration in adult patients with mild-moderate atopic dermatitis at Dr. Soetomo General Academic Hospital, Surabaya March-May 2022.  Methods: This cross-sectional descriptive study included adult patients with mild to moderate AD according to Hanifin-Rajka aged 18-64 years who visited the Dermatology and Venereology Outpatient Dr. Soetomo General Academic Hospital Surabaya between March and May 2022. Skin hydration was measured using a corneometer on the volar side of the left forearm in the skin area.  Result: Based on the major criteria, all participants had pruritus, typical morphology and distribution, and chronic or chronically recurrent dermatitis. As for the minor criteria, more varied results were obtained, one of which was xerosis, which was experienced by all participants. The skin hydration shows that the mean value for skin hydration was 39.8 ± 12.1. The lowest skin hydration was 21.4 and the highest was 87.4, both found in subjects with mild AD. Conclusion: ..
