Abstract

Based on the community service team's observations, the patient registration process at Prima Husada Clinic still uses manual recording through a register book. This condition results in long registration times because officers have to search for patient data one by one. This community service activity aims to provide assistance to medical records officers in analyzing the needs of a Clinical Management Information System (CMI) in the patient registration section. The activity was carried out for three days with three participants, consisting of two registration officers and one head of medical records. The implementation method included socialization, interactive discussions, and practice in preparing a patient registration flowchart. The evaluation results showed an increase in participant knowledge of 40%, with an average pretest score of 50.00 and a posttest score of 90.00. This activity is expected to help officers understand and analyze the needs of a Clinical Management Information System (SIM Klinik) so that the system designed is in accordance with the clinic's operational needs.

Abstract

Program pengabdian masyarakat ini bertujuan meningkatkan pemahaman serta keterampilan petugas rekam medis Klinik Prima Husada terkait tata cara peminjaman dan pengembalian berkas rekam medis, sekaligus membantu penyusunan dokumen standar operasional prosedur (SOP). Metode kegiatan meliputi ceramah interaktif, tanya jawab, demonstrasi, serta praktik penyusunan SOP. Kegiatan berlangsung dalam tiga sesi, melibatkan tiga orang petugas rekam medis. Hasil pelaksanaan menunjukkan adanya peningkatan pengetahuan sebesar 35% dan keterampilan petugas dalam penyusunan SOP. Kendala yang muncul terutama pada keterbatasan waktu dan proses legalisasi SOP, namun komunikasi dan dukungan manajemen Klinik Prima Husada berperan besar dalam kelancaran kegiatan. Kesimpulannya, kegiatan ini berdampak positif terhadap peningkatan mutu pengelolaan rekam medis di Klinik Prima Husada.

Abstract

Digital transformation is still an ongoing process in health service delivery to improve operational performance and service quality. However, BPJS Kesehatan patient referrals are still trapped in administrative bottlenecks. At Mulia Medika Clinic, staff used to handle BPJS patient referrals manually, so that records were prone to errors and delays in obtaining information. We designed, implemented, and tested a desktop-based BPJS referral information system using Python and Tkinter for clinic operations. The development process followed the Waterfall methodology, which consisted of requirements analysis, system architecture design using Context Diagrams, Data Flow Diagrams, Entity-Relationship Diagrams as well as Flowcharts followed by implementation and black-box testing validation. The system will manage patient records, referral processing as well as user administration. Automated features include generating referral letters and producing reports. Testing has proven that this system is accurate and efficient—the manual workload has reduced, data traceability has improved, and continuity in the referral service has been maintained. Results prove operational readiness for clinic deployment to enhance administrative efficiency and precision of the referral data. Currently, it runs standalone without real-time database synchronization; hence workflow integration cannot be achieved. Future versions should have direct connections with both clinic management and BPJS databases to allow seamless data exchange without manual synchronization

Abstract

This study designs and develops an information system with a centralized database for recapitulating pending inpatient claims at Hospital X Bandung. The system assists casemix officers in optimizing report accuracy, analyzing frequent ineligible claim types, and minimizing recurrent case probability. Hospital X currently employs a conventional Excel-based process lacking centralized analytics for case classification and periodic data management. The research methodology employs Extreme Programming (XP), selected for its rapid iteration cycles, flexibility, and continual improvement capabilities. Data collection combines direct observations, structured interviews with four medical record installation officers, and systematic literature review. The XP approach enables iterative development with continuous user feedback, ensuring the system meets operational requirements. Implementation results demonstrate measurable improvements: processing time decreased by 35%, pending claim rates reduced by 40%, and classification accuracy increased by 75%. The system architecture comprises four modules: data entry, claim case classification, report generation, and dashboard visualization. Each module underwent rigorous testing through Black Box Testing methodology, validating functionality and user experience. Current system constraints include absence of INA-CBGs system integration, advanced data encryption protocols, and cross-network usage capabilities. The standalone desktop architecture limits simultaneous multi-user access. Despite these constraints, the system successfully simplifies recapitulation workflows and establishes a foundation for future development, particularly web-based architecture and predictive analytics integration

Abstract

Medical action consent forms are known to be legal documents in the delivery of healthcare services. However, when they are not well completed, it affects the quality of services and the accountability of an institution. Recurring problems in documentation processes include incomplete data entry, late submissions, and substandard recording; all these increase risks for medical errors and administrative complications. A web-based information system was developed to address these systematic deficiencies using Rapid Application Development (RAD) methodology. Field observations and structured interviews with healthcare personnel were used as research tools in the design of this system. The development process continued through four iterative phases: requirements planning, user-centered design, system construction, and deployment evaluation. This platform comprises automated validation protocols that prevent incomplete submissions from being accepted into the database until all required fields have been completed by the user; real-time alert mechanisms for missing data fields within a submission; and streamlined interfaces optimized for clinical workflows. Functional testing shows statistically significant improvements in documentation efficiency and completion rates among medical staff documented by the system compared to those not documented by it. The system permits real-time documentation of procedures with lower error margins compared to existing methods plus shorter processing times than existing methods. This study found that RAD-based solutions present feasible approaches toward improving accuracy and speed in medical consent documentation while simultaneously achieving legal compliance and patient safety goals within hospitals. The platform uses technology-driven quality assurance mechanisms to solve longstanding challenges in documentation

Abstract

Hospital mortality data management directly influences service quality and strategic decision-making capabilities. While electronic medical records have become standard practice in healthcare facilities, existing systems frequently lack the adaptive features necessary for efficient mortality reporting. We address these limitations through the development of a web-based mortality reporting system using Extreme Programming (XP) methodology and the Laravel Framework. The development process engaged two primary user groups: nurses responsible for data entry and medical record officers managing validation and reporting. Black-box testing validated system functionality against operational requirements. Performance evaluation revealed substantial operational gains: patient data entry time decreased from 7-10 minutes to 2-3 minutes, duplicate validation transitioned from manual verification to automated real-time checking, and quarterly report generation reduced from 2-3 hours to 10-20 seconds. The XP methodology, particularly pair programming practices, accelerated defect resolution and enhanced system responsiveness to user feedback. These results demonstrate that iterative development approaches combined with modern web frameworks can fundamentally transform healthcare data management workflows, replacing error-prone manual processes with reliable, efficient digital solutions.

Abstract

Inpatient medical record review is a major activity in ensuring the quality, completeness, and consistency of hospital healthcare documentation. Besides ensuring compliance with documentation standards, the review process also ensures that clinical services are accurately recorded for reference in future medical, legal, and administrative matters. This study aims to design a web-based information system that will facilitate structured, efficient, and integrated review workflows between medical record officers and other related units. The development of the system uses the Agile method which prioritizes flexibility and fast adaptation to user feedback so that improvements can be made iteratively. The prototype created can manage data input on reviews, generate summary results as well as reduce human errors usually found in manual entries. The hospital currently uses Google Spreadsheets and Google Forms which are tools with too many columns and prone to errors hence making tracking time and data inefficient. Usability testing using the System Usability Scale (SUS) obtained an average score of 68.13 which is categorized as “acceptable” usability indicating that the system is easy to use and feasible for real implementation. This system is then a basic solution for workflow simplification, validation accuracy improvement, and increasing documentation reliability of medical records in hospital environments.

Abstract

Hospital X as one of the referral health facilities in the Cimahi area and its surroundings faces significant challenges in health claims management. This semi-digital process can result in significant financial losses and high operational costs. This research aims to build an application for the health claim verification process with the results of analysis at X Hospital. Developed a web-based health claims verification application with Agile methods that integrates ICD-10 standards and artificial intelligence (AI) to automate document checks, reduce human error, and speed up the verification process. The research uses agile methods with system development through Scrum. Key frameworks include Flask-Python (backend), Bootstrap 5 (frontend), and SQLite/PostgreSQL (database). Data is collected through interviews with medical staff and observation of existing verification processes. Implementation during 4 sprints resulted in a reduction in verification time from 3-5 days to <24 hours. The claim reduction is rejected from 8% to 2.5% through automatic validation. Multi-role system with 4 separate dashboards (admin, medical staff, verifier, participant). Pre-screening AI integration detects 92% of nominal anomalies of claims. This app is proven to improve verification accuracy and faster time efficiency. The success of the implementation of Agile is demonstrated by the ability to adapt to 12 changes in the requirements of the development time.

Abstract

Cocoa farming in Bulungan Regency, particularly in Sajau Village, plays a vital role in supporting the rural economy. However, the management of harvest data still faces challenges, especially due to manual record-keeping, which tends to be inefficient. This study aims to develop a QR Code–based information system called QR Kakao that assists farmers and facilitators in managing and accessing harvest data more quickly, accurately, and in a structured manner. The research employed a qualitative approach through field observations, interviews with farmers and stakeholders, and literature studies. Meanwhile, the software development process adopted the Waterfall model, consisting of requirement analysis, system design, implementation using the Laravel framework, testing, development, and maintenance. The QR Kakao system successfully provides core features such as generating and scanning QR Codes containing information such as farmer identity, farm location, harvest dates, and harvest yield. The implementation of this system offers several benefits, including easier access to harvest information, improved efficiency in data management, and encouragement of digitalization within smallholder cocoa farming. With the support of PT Pesona Khatulistiwa Nusantara, QR Kakao is expected to contribute to a more modern, transparent, and sustainable cocoa farming management system in Sajau Village.

Abstract

The patient registration process at Klinik Gigi drg. Miranti P. Darmawati is still carried out manually, often causing long queues, data entry errors, and service delays. This study aims to develop an automated outpatient registration system to improve service effectiveness. The research applies the Waterfall method, starting from needs analysis, system design, implementation, and testing. Data were collected through observations of the registration process and interviews with clinic staff. The results show that the automation system can speed up the registration process, reduce data entry errors, and increase satisfaction for both patients and staff. It is recommended that the system be further developed with integration to electronic medical records and a schedule reminder feature to achieve more optimal healthcare services.
