Abstract

The aim of the research was to analyze the factors that affect participation of Independent Midwives Practice in the National Health Insurance in Semarang City. The research design used analytic observational research. The subjects of the study were 73 IMP and taken by simple random sampling. The data collected using questionnaires. Data were analyzed using Chi Square and multiple logistic regression. The results showed the IMP participation on JKN were 52.1%. There were some relationship between socialization, resources, attitudes and bureaucracy. Attitude and resources had influence with participation on National Health services. IMP who had good attitude had opportunities participate 6 times compare with who did not participate. Resources BPM good opportunity to participate JKN 4.9 times than did not participate.  Midwives need to prepare a good attitude and its resources and cooperation partners being able to support and provide IMP to be able to follow the program of National Health Insurance.

Abstract

The purpose of research was to analyze differences in body weight gain and development of infants aged 3-6 months. Type of study was pre-experimental research with Posttest Only Design. The population were infants aged 3-6 months who visited Ananda Home Birth in Ambarawa. The subjects were SPA treatment group and control respectively 17 babies by simple random sampling. Data collection used SPA treatment group 4 times for 8 weeks . Before and after measured weight gain and development with KPSP . Data were analyzed with the Mann Whitney test. The results mean score KPSP development of infants before and after the treatment group SPA was 8,12 into 9,59. The mean weight gain infants before and after the SPA was 5705 grams to 7188 grams. There was no differencess in development progress after the SPA between the treatment group and the control group and there were any differences the increase in weight gain between treatment and control groups (p value = 0.018 ). It is important for midwives to develop midwifery care to infants and toddlers with holistic care which one of them with a baby SPA.

Abstract

The aim of this research is to evaluate the effectiveness of early ambulation on post partum hemorhage in independent of midwifery practice. The research was a quasi-experimental with two group post-test research design. The number of samples was 30 subjects. The early ambulation programs consists of lay aside - sit down position, and lay aside-sit down-stand up position for 1 to 2 hours. Post partum hemorhage was measured by counting the amount of bleeding during post partum. Afterwards, the data were analyzed using Mann Whitney test. The results showed that the average amount of bleeding within the experimental group decreased 6.33 cc after the early ambulation with lay aside - sit down position in the first and second hours as well as in the control group which declined 9.66 cc after the early ambulation lay aside-sit down-stand up position 1 hour to 2 hours. However, there was no significant diference between post partum hemorhage in the experimetal and the control group (z=1.131, p0.05). It can be concluded that either early ambulation with lay aside - sit down position or lay aside-sit down-stand up position for 1 hour to 2 can decrease post partum hemorhage.

Abstract

Perineal injury is one of the most common causes of first birth and results in perineal tears, which categorized into four degrees, including stage I, stage II, stage 3 III, and stage 4 IV. The study aimed to examine the effect of the casumba turate extract ointment (Carthamus tinctorius L) on improving the perineal wound healing among Wistar (Rattus norvegicus). An experimental laboratory test with a randomized control trial in the post-test only with control group design was applied in this study. Thirty-two samples comprised of experimental group (n=16) and control group (n=16) were involved in this study. REEDA scores using the Mann Whitney test analysis were used to determine the progress of perineal wound healing. The results showed that there is a significant difference in mean score between intervention and control group after implementation of the casumba turate (p <0.05). In conclusion, casumba turate extract 5% ointment has a positive impact on the wound healing process for perineal of Wistar (Rattus norvegicus). Keywords: casumba turate, perineal wounds, Wistar

Abstract

Women are more at risk to stress in pregnancy than during the puerperium. One method of coping with stress is through physical exercise. Maryam Exercise can be used as an alternative to the development of pregnancy exercises in Indonesia because it covers the holistic aspects and easy to do. It is a combination of basic pregnancy exercises, Islamic prayer (Salat) movements and dhikr.  The purpose of this study was to examine the impact of Maryam exercise on the stress levels and cortisol serum levels among primiparous pregnant women. This study used a quasi-experimental study design, pre-test and post-test with non-equivalent control group. In the third trimester of 40 primiparous women  were randomly assigned into two groups such the experimental group (n=20) and the control group (n=20). The experimental group received the Maryam exercise while the control group received the standard antenatal exercise. There was a significant difference in stress levels between the intervention group and the control group before and after receiving the intervention (p <0.05). The cortisol levels also showed the significance difference between the intervention group and the control group before and after receiving the intervention (p <0.05). The findings of this study suggest that Maryam Exercise has a positive impact on stress levels and cortisol serum levels of primiparous pregnant women.

Abstract

.  Induction of labor is high all over the world. Pharmacological and mechanical labor induction is one of the most frequent medical actions in midwifery services across the world. This systematic review aims to analyze the effectiveness of acupressure and acupuncture on the induction of spontaneous labor. Articles were retrieved from PubMed, Google Scholar, and ScienceDirect until March 3rd, 2020. Only articles published from 2010 to 2020 were included. Keywords used consisted of “acupuncture,” “acupressure for induction of labor,” “acupressure for initiation of labor,” “term pregnancy”.  Articles were filtered using inclusion criteria. The literature search resulted in 1.365 articles. A total of 9 articles (5 acupressure and 4 acupuncture studies) were included for review. The total number of participants was 1,656 people. One study suggested that acupressure was effective in increasing cervical maturation. Two studies (1 acupressure and 1 acupuncture) stated that it was raised the frequency of initiation of spontaneous labor. One study showed that acupuncture reduced the need for medical induction. Other studies showed no significant effect. Overall, studies demonstrated a positive effect of acupressure and acupuncture on cervical maturation and increasing the frequency of initiations of spontaneous labor. Besides, it may reduce the need for medical induction. Further investigations are needed

Abstract

Menyusui merupakan proses alamiah yang akan di hadapi oleh seorang ibu, namun bila tidak dipersiapkan  sejak awal dapat menimbulkan beberapa  masalah seperti payudara bengkak, puting susu lecet. Tujuan pengabdian ini adalah mengajarkan kepada kader dan ibu hamil  pencegahan terjadinya engorgement dengan pelatihan, pendampingan ibu hamil dan menyusui dalam mempersiapkan pemberian ASI. Langkah kegiatan  adalah memberikan pelatihan  kepada  40 kader kesehatan dan ibu hamil, tentang ASI ekslusif, masalah-masalah menyusui, cara mengatasi engorgement, pada pertemuan selanjutnya dilakukan praktik kompres daun dadap serep dan kompres hangat payudara. Kegiatan dilakukan di RW II dan III Kelurahan Gedawang  bulan Agustus sd Oktober 2018. Hasil kegiatan skor rata-rata pengetahuan ibu sebelum diberikan pelatihan tentang ASI ekslusif 65, skor masalah menyusui 68, cara mengatasi engorgement 65 dan kompres daun dadap serep dan kompres hangat dingin 61. Setelah pelatihan selesai diberikan post test  terjadi kenaikan skor pengetahuan ASI ekslusif menjadi 83, masalah menyusui 82,  cara mengatasi engorgement 80, dan kompres daun dadap serep dan kompres hangat dingin skore 78.  Diharapkan  kader di wilayah kelurahan Gedawang  dapat mendampingi ibu nifas dalam memberikan ASI ekslusif, di wilayahnya serta ibu hamil dapat mempersiapkan sedini mungkin dalam memberikan ASI dan cara-cara mengatasi masalah menyusui.

Abstract

Background: Primary dysmenorrhea occurs because of the excessive amount of prostaglandins in the blood. Papaya leaf extract is considered to be able to reduce prostaglandin and menstrual pain. Objective: This study aims to prove that papaya leaf extract (Carica Papaya L.) can decrease the level of menstrual pain and prostaglandin levels in primary dysmenorrhea. Methods: A true experimental study with randomized pretest posttest control group design. There were 32 samples recruited using simple random sampling, with 16 each assigned to an experiment group and a control group. Numerical Rating Scale (NRS) was used to measure menstrual pain, and the level of prostaglandin was measured using the enzyme-linked immunosorbent assay (ELISA) method. Independent t-test and paired t-test were performed for data analysis. Results: There was statistically significant difference of menstrual pain and prostaglandin level before and after intervention with p-value 0.000 (<0.005), which indicated that papaya leaf extract had a significant effect on reducing menstrual pain. The decrease of menstrual pain in the experimental group was -3.375 and in the control group was -3.438; while the decrease of prostaglandin level in the experimental group was -56.971 and in the control group was -57.557. Conclusion: Provision of papaya leaf extract (Carica Papaya L.) significantly decreased the level of menstrual pain and prostaglandin levels in primary dysmenorrhea. Papaya leaf extract can be used as a safe and effective herbal medicine for primary dysmenorrhea which has almost the same efficacy as mefenamic acid.

Abstract

Background: Low milk production is one of the barriers to exclusive breastfeeding. Oxytocin massage is considered as an alternative treatment, which combined with lavender essential oil as an aromatherapy.Objective: This study aims to examine the effect of oxytocin massage using lavender essential oil on the increase of levels of prolactin and milk production in primiparous mothers after caesarean section.Methods: This was a quasi-experimental study with non-equivalent control group design conducted in October-December 2016 at the General Hospital of Dr.H. Soewondo Kendal, Indonesia. There were 32 recruited by consecutive sampling, divided to be intervention (16 participants) and control group (16 participants). Prolactin hormone levels were measured using Enzyme-linked immunosorbent assay (ELIZA), breast milk production was measured based on the indicators of milk volume, urination and defecation frequency and sleep duration of babies; and infant’s weight was also measured by digital scale. Data were analyzed using Mann Whitney and Wilcoxon test.Results: The mean difference of prolactin hormone level in control group was 17.82 ng / ml while mean of difference of hormone prolactin level in intervention group was 132.13 ng / ml. There were statistically significant differences between intervention and control group in prolactin levels (p-value 0.000), milk volume (p-value 0.000), infant weight (p-value 0.000), urination frequency (p-value 0.017), defecation frequency (p-value 0.002), and infant sleep duration (p-value 0.000).Conclusion: There was a significant effect of the oxytocin massage using lavender essential oil on the increase of breast milk production and prolactin levels. Therefore, oxytocin massage using lavender essential oil can be used as an alternative treatment for midwives and other health professionals in an effort to increase milk production in postpartum.

Abstract

Background: Kangaroo mother care (KMC) and lullaby music methods have been considered as the alternative treatment for vital sign changes in low birth weight infants. However, little is known about the combination of the two methods. Objective: To identify effectiveness of combinations of Kangaroo mother care and Lullaby music methods on changes in vital signs in low birth weight infants. Methods: A quasi experiment with non-equivalent control group design. This study was conducted on October– December 2016 at the General Hospital of Ambarawa and General Hospital of Ungaran, Semarang, Indonesia. There were 36 samples selected using consecutive sampling divided into three groups, namely: 1) a group of low birth weight infants with the combination of KMC and lullaby music, 2) an low birth weight infant group with the lullaby music intervention, and 3) a control group given standard care in low birth weight infants by KMC method. Paired t-test and MANOVA test were used to analyzed the data. Results: Findings revealed that there were significant differences between the combination group, lullaby music group, and control group in temperature (p=0.003), pulse (p=0.001), respiration (p=0.001), and oxygen saturation (p=0.014) with significant value of <0.05, which indicated that there was a statistically significant difference in vital sign changes among the three groups. Conclusion: The combination of KMC method and lullaby music intervention was effective on vital sign changes (temperature, pulse, respiration, and oxygen saturation) compared with the lullaby music group alone and control group with KMC method in low birth weight infants. It is suggested that the combination of KMC and lullaby music methods can be used as an alternative to improve low birth weight care for mothers in the NICU and at home and to reach the stability of the baby's vital signs.
