Abstract

Hypertension or high blood pressure is one of the significant health problems in the world, including in Indonesia. The prevalence of hypertension in Lima Puluh Kota Regency in 2022 reached 15% or 10,688 out of 70,250 people who took blood pressure measurements. However, only 627 people (11.8%) took blood pressure measurements and received hypertension services according to standards. Therefore, it is necessary to increase public knowledge about hypertension. This service activity was carried out in September 2023 through the emo-demo method, which expects health promotion efforts through an emotional approach. The service activities were carried out on 37 community members, and it was found that there was an increase in the average score of community knowledge from 51.56 to 90.63. An increase can follow this increase in community knowledge, attitudes, and actions toward efforts to prevent and control hypertension, including the prevention of complications of hypertension in the community.

Abstract

Outpatient pharmacy services in a hospital shows the quality of health service to the community. Data from the internal quality assurance report of Hospital ”X” in Padang showed that only 93.76% of non-compounded medication and 94.6% of compounded medication recipes met the waiting time standard. This study aimed to analyze the causes of the long waiting time for outpatient pharmacy services at Hospital “X”. The type of this research was a mix-method sequential type explanatory, conducted from February until May 2022. The quantitative design was a cross-sectional study of 141 for non-concoction and 35 for compounded medication recipes determined by quota sampling. Qualitative research was conducted on seven informants by purposive sampling. Observation, in-depth interviews, and document review collected data. Results showed that the average waiting time was 84.19 (standard ≤30) minutes for non-concoction and 164.58 (standard ≤60) minutes for compounded medication recipes. The causes of the long waiting time were a need for more staff, prescriptions outside the hospital formulary, equipment errors, and medicines delivery delays. There should be a job redesign by rearranging the shift picket for pharmacy staff, regular equipment maintenance, and updating standard operating procedures related to medicines delivery to the patients.

Abstract

Kasus TB anak di Kota Padang mengalami kenaikan. Untuk menekan laju pertumbuhan dan morbiditas, peran petugas kesehatan dalam penemuan kasus dan pengobatan pasien sangat dibutuhkan. Tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang berhubungan dengan kejadian tuberkulosis pada anak di Kota Padang Tahun 2022. Penelitian menggunakan desain studi case control dengan mencocokan umur dan jenis kelamin. Penelitian dilakukan di Puskesmas Pegambiran, Andalas, dan Anak Air pada bulan Maret-April 2023. Populasi yang digunakan adalah pasien yang tercatat di formulir TB 06 tahun 2022 dengan 80 sampel. Teknik pengambilan sampel kasus menggunakan Simple Random Sampling. Analisis data yang digunakan adalah analisis univariat dan bivariat. Dari hasil uji bivariat terdapat varibel yang memiliki hubungan dengan kejadian tuberkulosis pada anak yaitu riwayat kontak, pendapatan orang tua, dan keberadaan perokok dirumah. Disarankan Dinas Kesehatan Kota Padang dan Puskesmas dapat meningkatkan pelaksaan skrining TB dalam penemuan kasus baru dan mengoptimalkan pemberian TPT.

Abstract

The low achievement of hypertension services in the work area of the Padangsidimpuan City Health Office needs to be analyzed on the level of knowledge and attitude of patients, the availability of facilities and infrastructure, and the support of health workers. System components are also analyzed to find out how input, process and output elements are in hypertension services. The research method combines quantitative and qualitative methods sequentially. A sample of 107 respondents was taken proportionally random sampling in the hypertensive population aged ≥ 15 years. The results of the study revealed that 42.1% of patients had low knowledge, 62.6% of patients had a negative attitude, and 57% of the support of health workers was negative. In the system analysis, (input elements) are known to be inadequate in the availability of human resources and the availability of funds. It was concluded that public attitudes, support of health workers, human resources and inadequate budgets have a relationship with low hypertension service achievement. The author's recommendations include the importance of improving public attitudes through more optimal education. The role of the government must be more decisive on the availability of human resources and consistently provide support for the hypertension program budget.

Abstract

The prevalence of stunting under five in the working area of the Air Child Health Center in 2020 was 16.3%. This number is the highest compared to all community health centers in Padang City and has not yet reached the national stunting rate target of 14%. The purpose of this activity is to increase the knowledge of mothers in the working area of the Air Child Health Center about the eating behavior of infants and toddlers and to improve the skills of cadres to conduct health education. The community empowerment method used is Emotional Demonstration (Emo-Demo) training on infant and toddler eating behavior, which consists of demonstration, practice, discussion, and question and answer. The activity was carried out on Wednesday, September 29, 2021 at the Jazirah Nur Mosque, Padang City. The training was given to 29 participants, consisting of 4 pregnant women, 16 mothers with toddlers, 8 posyandu cadres, and 1 community Health centers officer. The training materials consist of 4 Emo-Demo modules developed by The Global Alliance for Improved Nutrition (GAIN), namely: 1) infant and child feeding schedule, 2) mother and child bonding, 3) stacking blocks, and 4) random snacks. The result of the activity in general is an increase in mothers' knowledge of the eating behavior of infants and toddlers. This can be seen from the 5 randomly selected participants who were able to correctly answer the questions from the informants. In addition, there was an increase in the skills of posyandu cadres in implementing the Emo-Demo technique. This can be seen from all cadres who are able to practice Emo-Demo without needing to be guided by a facilitator after the training.

Abstract

ABSTRACT Medical records are one of the means to support the achievement of an administrative order in the context of efforts to improve health services in hospitals. An important thing that must be considered by medical record personnel is the accuracy in providing diagnosis and procedure codes. The research problem is the accuracy of the coding of the Diagnosis & Procedure on the medical records of inpatients at the Pariaman Regional General Hospital. The research objective was to describe the general characteristics and accuracy of disease coding based on diagnosis and service procedures as well as the impact of the accuracy of disease coding with inpatient disease diagnosis on the quality of service at the Pariaman Regional General Hospital. The research method is mixed methods. A quantitative approach to the patient's medical record file and a qualitative approach were obtained through FGDs with medical record management officers. The results showed that the accuracy of the main Diagnosis coding has shown results that exceed 80% of all medical records of patients selected as the study sample. However, the accuracy of the secondary diagnosis code is only 50%. Procedure code acceleration is still very low at only 15.4%, and secondary procedure only at 1.8%. The conclusion of the study, the accuracy of the diagnosis code and procedures is closely related to the quality of service. Inaccuracy of diagnosis codes as well as procedures will affect patient satisfaction, especially in payments that are not in accordance with the services they receive.  Keywords : Encoding, Diagnosis, Procedure, Medical records

Abstract

Stunting, a persistent global nutritional issue from inadequate care and nutritional intake, becomes evident when a child ages two. Tanah Datar recorded a 21.5% stunting rate in 2022, prompting a targeted service to enhance the literacy of pregnant women, mothers of toddlers, and cadres about the critical first 1000 days of life. Utilizing an interactive game, Emotional Demonstration, and drawing on four modules from the Global Alliance for Improved Nutrition (GAIN), the service employed printed modules and game equipment. Held on 10 June 2023 at Wali Nagari Pandai Sikek Hall, the event gathered 20 participants, including pregnant women, mothers, cadres, and X Koto Community Health Center officers. Activities commenced with a pre-test, followed by interactive game-based education, and concluded with a post-test. The community service positively contributed to increased health literacy and awareness in the first 1000 days, validated by significant T-test results (p-value = 0.001), indicating a substantial knowledge improvement post-intervention.

Abstract

Background : Quality improvement (QI) is a practice that hospitals use to improve efficiency, clinical outcomes, employee satisfaction, and patient safety. One of the QI practices is the implementation of lean principles. Lean is a set of methods intended to eliminate waste and increase process efficiency. Lean implementation can have a significant impact on operational efficiency and effectiveness. This research is aimed at analyzing services at the RS Naili DBS Outpatient Installation using lean principles. Method : Data collection was carried out through direct observation of the outpatient service process from arrival to receiving doctor consultation services. From the observations, visualization of the service process was carried out using big picture mapping and analysis of waiting time and waste was carried out using lean principles. Apart from that, in-depth interviews were conducted with selected informants, namely from within the hospital and patients as service recipients. Result : The results of the research showed that the outpatient waiting time from the time the patient contacted the registration officer until the doctor's service was completed was 1 hour 18 minutes 50 seconds. This time still exceeds the minimum service standard set by the government, namely 60 minutes. The results of observations regarding waste, found as many as 36 types of waste spread across the queuing process, registration, polyclinic waiting room and clinic room.

Abstract

The Infant Mortality Rate (IMR) in Indonesia remains above the reduction target set in the Sustainable Development Goals (SDGs) 2030 agenda. IMR serves as an important indicator of health sector quality, reflecting the success of health development. This study aims to examine the influence of maternal and infant characteristics on infant mortality through a literature review. Literature searches were conducted in four main databases: Google Scholar, Garuda Portal, PubMed, and SINTA, using relevant keywords in both Indonesian and English. Inclusion criteria comprised articles published between 2018 and 2024, written in Indonesian or English, and employing observational study designs. A total of 12 eligible articles were analyzed. Study quality was assessed using the STROBE checklist to ensure completeness and transparency in methodology and results reporting. Narrative analysis was conducted to synthesize the influence of maternal and infant characteristics on infant mortality. Results indicate that maternal factors such as gestational age under 37 weeks, fewer than four ANC visits, pregnancy complications, and high-risk maternal age, as well as infant factors including low birth weight (LBW), male sex, asphyxia, and congenital anomalies, increase the risk of infant mortality. LBW was identified as the dominant factor closely linked to prematurity. Efforts to reduce infant mortality should focus on preventing and managing LBW and optimizing pregnancy and neonatal services, especially in rural areas and high-risk groups.
