Abstract

Desa-desa di Kabupaten Malinau, Provinsi Kalimantan Utara mempunyai sumber dana untuk pembangunan kesehatan bersumber APBD dan APBN, namun masalah kesehatan di Kabupaten Malinau masih tinggi. Pada tahun 2015, AKB yaitu 25 per 1000 kelahiran hidup dan AKI yaitu 229 per 100.000 kelahiran hidup. Tujuan penelitian adalah untuk menganalisis kebijakan dana desa untuk pembangunan kesehatan di Kabupaten Malinau dari aspek konten, konteks, proses dan aktor.  Desain penelitian kualitatif dengan pendekatan studi kasus di 4 desa. Penelitian dilakukan dengan wawancara mendalam terhadap 26 responden yang terlibat dalam kebijakan dana desa. Penelitian dilaksanakan pada bulan Oktober tahun 2016 sampai januari tahun 2017.Kondisi geografis, mata pencaharian, kekerabatan dan status desa memengaruhi perspektif masyarakat dalam melaksanakan pembangunan kesehatan. Tidak adanya regulasi dan petunjuk teknis mengenai pelaksanaan pembangunan kesehatan di desa menyebabkan ketimpangan pembangunan kesehatan di Kabupaten Malinau. Tenaga kesehatan harus mampu mengidentifikasi dan merumuskan masalah kesehatan di desa. Prinsip swakelola dalam pelaksanaan kebijakan dana desa meningkatkan ekonomi masyarakat dan mengurangi pengangguran. Kebijakan dana desa telah dilaksanakan di Kabupaten Malinau, tetapi pemanfaatan untuk pembangunan kesehatan belum optimal. Diperlukan advokasi kepada pemerintah pusat untuk membuat regulasi alokasi dana desa untuk kesehatan. Tenaga kesehatan harus pro aktif dalam proses penyusunan kebijakan untuk mengungkit pembangunan kesehatan di desa.Kata Kunci : dana desa, Kabupaten Malinau,  segitiga analisis kebijakan

Abstract

Revitalisasi Puskesmas merupakan upaya Kementerian Kesehatan RI untuk meningkatkan kinerja Puskesmas melalui berbagai perubahan dalam penyelenggaraan Puskesmas. Perubahan-perubahan tersebut sejalan dengan semangat reformasi pembangunan khususnya reformasi otonomi daerah. Penelitian ini bertujuan untuk menganalisis manajemen perubahan oleh individu Puskesmas. Metode Desain penelitian yang dipergunakan adalah kualitatif dengan paradigma konstruktivism dan strategi studi kasus. Metode analisis yang digunakan adalah tematik. Subyek penelitian ini adalah kepala dinas kesehatan dan kepala Puskesmas. Hasil model manajemen perubahan ADKAR mendukung hasil yang menunjukkan bahwa konstruk perubahan individu Puskesmas didasari oleh tahap-tahap yang harus berurutan. Simpulan Perubahan oleh individu dalam revitalisasi Puskesmas tidak terbangun secara utuh sehingga memengaruhi pencapaian perubahan yang diharapkan.Kata kunci : ADKAR,manajemen perubahan, revitalisasi Puskesmas

Abstract

Declined in nutritional status of hospitalized Chronic Renal Failure (CRF) patients were due to nutrient intake inadequate which required special attention from the Hospital Nutrition Service Team. This study analyzed the nutritional service on hospitalized CRF patients in Dr. Hasan Sadikin Hospital, Bandung using Health Technology Assessment. The study was conducted in 2014. The study design mixed method with concurrent embedded strategies. The hospital management, nutritionist and hospitalized CRF patients were qualitative subjects, selected using purposive sampling. Quantitative subjects selected using total sampling were 25 patients. Food weighing were used to measure food waste. The nutritional services technology aspects especially nutritional care using PAGT has not been entirely done. Nutritional monitoring and evaluation have not been well programmed and scheduled. The collaboration of nutritional care team have not been optimal. The food service mechanism are quite good while its portions are standardized yet. According to patient’s point of view towards food service quality, the food have less variety and taste. Total of cost damage from food waste are Rp.18.099,00 in a year. The lack of technological and organizational aspects can reduce the food service quality. According to the patient, the lack of food service quality will increase the food waste and total cost damage.

Abstract

Prevalensi malnutrisi pasien rawat inap di rumah sakit cukup tinggi. Pelayanan gizi  diharapkan dapat mengatasi terjadinya malnutrisi di rumah sakit. Penelitian ini bertujuan untuk melakukan analisis terhadap kebijakan outsourcing penyelenggaraan makan pasien di Rumah Sakit  Umum Sumedang. Desain penelitian adalah mixed method dengan strategi concurrent embedded. Pengambilan sampel kualitatif dilakukan secara purposive sampling. Subjek penelitian adalah tim pelayanan gizi rumah sakit. Sampel kuantitatif  menggunakan stratified simple random sampling, diperoleh jumlah sampel 154 pasien. Pengukuran status gizi pasien menggunakan subjective global assessment. Penelitian dilakukan pada bulan November sampai Desember 2012. Mekanisme penyediaan makan cukup  terstruktur dan jelas, meskipun masih ada Standar Operasional Prosedur (SOP) yang belum dimiliki katering seperti SOP untuk menu pasien anak-anak,  penyiapan logistik dan membersihkan peralatan makan pasien.  Beban kerja karyawan katering sangat tinggi dan rangkap tugas. Tim Panitia Asuhan Gizi (PAG) khususnya perawat dan petugas gizi dapat bekerja lebih baik, namun belum memiliki perencanaan stratejik untuk pelayanan gizi. Kapasitas tim PAG dalam pelayanan gizi belum optimal. Terdapat peningkatan gizi buruk dari 1,9% sebelum perawatan menjadi 13% setelah perawatan. Simpulan, penyelenggaraan makan dengan sistem outsourcing belum berdampak pada peningkatan status gizi pasien, namun mekanisme layanan asuhan gizi dan penyediaan makan menjadi lebih jelas dan lebih baik. Nutritional services can overcome such problem. This study aimed to perform a policy analysis of food service outsourcing in Sumedang District Hospital. The design of this study was a mixed method using a concurrent embedded strategy. The purposive sampling was chosen for the qualitative study in which the hospital nutritionist team members became the subjects. The sampling for the quantitative study was performed using the stratified simple random sampling. About 154 patients were selected as subjects. The subjective global assesment was used as the instrument. The study was conducted from November to December  2012.  The food service mechanism was well structured and clear even though there were missing Standard Operating Procedures (SOPs) such as SOPs for pediatric patient menus, logistic preparation, and patient meal equipment cleaning. Catering employees had a very high workload and were multitasking. The Nutrition Care Committee Team may work better. There was no strategic plan for nutrition services. The capacity of the team was not optimum in terms of nutritional care. The proportion of malnutrition increased from 1.9% to 13% after hospitalization when compared to the pre-hospitalization condition. In conclusion, the outsourcing system for food services has no impact on patients’  nutrition status; however, it is apparent that a clear and better mechanism of nutrition care and food provision is needed.Key words: Food services, outsourcing, policy analysis DOI: 10.15395/mkb.v46n4.343

Abstract

Breast milk is the best food for babies for growth and development. The purpose of this study was to determine the nutritional intake and diet of mothers who provide exclusive breastfeeding. This research design was a mixed method with a sequential explanatory strategy. Quantitative research to assess food intake, while qualitative research was used to determine the dietary patterns of mothers. Quantitative samples were 44 mothers, while qualitative samples were 5 mothers, conducted by purposive sampling. Quantitative data collection by multiple 24-hour recalls for 3 days. Qualitative data collection by in-depth interviews. Quantitative analysis was conducted descriptively, while qualitative data were analyzed using content analysis. The results showed that intake of energy (36.63%) and protein (40.90%) of inadequate breastfeeding mothers (adequate if ≥ 80%). For vitamin A, vitamin B1, vitamin B2, vitamin B6, vitamin C, iron, and zinc the intake is below the RDA. Mothers who provide exclusive breastfeeding have a diet such as types of food that are not diverse, amount of food that is not certain, frequency and schedule of irregular meals. The nutritional intake and diet of breastfeeding mothers are not by guidelines for balanced nutrition. ABSTRAK ASI adalah makanan terbaik bagi bayi untuk pertumbuhan dan perkembangan. Tujuan penelitian ini untuk mengetahui asupan gizi dan pola makan ibu yang memberikan ASI eksklusif di wilayah kerja UPT Puskesmas Ibrahim Aji Kota Bandung. Desain penelitian ini mixed method dengan strategi sequential explanatory. Penelitian kuantitatif untuk melihat asupan makan sedangkan penelitian kualitatif digunakan untuk mengetahui pola makan ibu yang memberikan ASI eksklusif. Jumlah sampel kuantitatif 44 orang, sedangkan kualitatif 5 orang, dilakukan secara purvosive sampling. Pengumpulan data kuantitatif dengan cara multiple 24 hour recall selama 3 hari. Pengumpulan data kualitatif dilakukan dengan wawancara mendalam. Analisis kuantitatif dilakukan secara deskriptif, sedangkan data kualitatif dengan content analysis melalui transkripsi, koding, kategorisasi, dan pembuatan tema. Hasil penelitian menunjukkan bahwa, asupan energi (36,63%) dan protein (40,90%) ibu menyusui tidak adekuat (adekuat jika ≥ 80%). Untuk vitamin A, vitamin B1, vitamin B2, vitamin B6, vitamin C, kalsium, zat besi dan seng asupannya berada di bawah AKG. Ibu yang memberikan ASI eksklusif memiliki pola makan dengan jenis makanan yang tidak beragam, jumlah makanan yang tidak tentu, frekuensi dan jadwal makan yang tidak teratur. Asupan gizi dan pola makan belum sesuai dengan pedoman gizi seimbang. Kata kunci: asupan gizi, pola makan, ibu menyusui ASI eksklusif

Abstract

Background: Congenital heart disease (CHD) is the most frequent type of heart disease that occurs in children. Children with CHD are more vulnerable to suffer impaired nutritional status related to morbidity and mortality. The nutritional status, however, may not be influenced only by CHD, but also by other factors.. The study was conducted to identify factors related to malnutrition in children with CHDMethods: This cross-sectional study involved 86 children with CHD recruited from Dr. Hasan Sadikin General Hospital Bandung from October to November 2015. Data collected in this study were obtained by conducting anthropometric measurements, interviewed the parents (obstetric history, patient’s medical history and socioeonomic status) and from medical records (the type and time of CHD diagnosis, other diseases that accompanied the CHD). The collected data were presented by frequency distribution. Results: There were 46.5%  patients who had nutritional disturbance. Malnutrition in males and patients with Tetralogy of Fallot were higher than other groups. Almost half patients with maternal history of passive smoking during pregnancy had nutritional disturbance. The most common disease found was diarrhea followed by lower respiratory infection. Percentage of normal children in low-income parents group was lower than the high-income group. Conclusions: Nearly half of patients have nutritional disturbance. Percentage of nutritional disturbance in male was higher compared to female. Active and passive smokers, Tetralogi Fallot, diarrhea, lower respiratory infection and lower income  were factors contributed to malnutrition. [AMJ.2017;4(1):111–7]DOI: 10.15850/amj.v4n1.1031

Abstract

   Background: Maternal parenting, especially feeding practices, is very important in childhood period and may affect the child’s nutritional state. Proper nutrition affects the growth and development of children. The aim of this study is to describe parenting feeding practices in malnourished under-five children.Methods: A descriptive study on 43 mothers with malnourished under-five children aged 12−59 months was performed in Cipacing Village, Jatinangor, Sumedang during the periode of October−November 2012. Results: The composition of children based on their malnutrition status and description on good and poor parenting were presented in the result. Twenty two children (51.2%) were severely under nourished, 18 children (41.9%) were under nourished, and 3 children (7%) experienced overweight. Good maternal parenting was identified in child nurturing (86%), feeding frequency (93%), feeding style (62.8%), and situation of feeding (74.4%) while poor maternal parenting was observed in feeding time (55.8%) and types of food given (51.2%). Conclusions: Maternal parenting plays an important role in children nutrition status.Keywords: Children, feeding practice, malnutrition, maternal parentingDOI: 10.15850/amj.v2n1.435

Abstract

Background: Obesity has become the 5th leading cause of death worldwide in 2010. It is a combination of genetic susceptibility, increased availability of high energy food intake and decreased requirements for physical activity in modern society. The increase in calorie intake among obese people often leads to many chronic diseases. Therefore, the aim of this study was to observe calorie intake pattern among obese undergraduates.Methods: This study was conducted at Faculty of Communication Studies Universitas Padjadjaran, from September to November 2013. This cross-sectional study used anthropometric measurement based on Asian Body Mass cut-off points, which the screening was conducted on 512 undergraduate students, consisting of 103 undergraduates, overweight (50) and obese (53). From 53 undergraduates, 30 respondents were randomly selected. The respondents were then interviewed using 3x24 hours Food Recall questionnaire, conducted on two weekdays and one weekend to observe their pattern of nutritional intake. Total number of calories was categorized into few groups according to the Recommended Dietary Allowance (RDA) 2012.Results: From the thirty obese undergraduates, both male (11) and female (9) consumed more excess protein in daily life. Consumption of carbohydrate and energy was significantly normal. Common consumption of food as daily basis contained more carbohydrate then protein, fats or energy.Conclusions: Prevalence of obesity is still high. Frequent meal and high intake of carbohydrates has become factors to nutritional imbalance of obese undergraduates. Abstract

Background: Undernutrition frequently occurs in children under five. If not treated, it will cause acute health effects and affect on cognitive development, social, physical work capacity and productivity. Undernutrition  can be accompanied by the presence of infectious disease that can worsen the children’s nutritional status. This study aimed to describe the nutritional status and infectious disease of undernutrition children under five in Jatinangor Subdistrict.Methods: A qualitative study was carried out to 7 parents and undernourished children under five, in Desa Cipacing, Jatinangor. It was conducted from April to December 2012. The inclusion criterias were undernourished children under five with a history of infectious disease in the previous year, and the parents were willing to participate in this study. Exclusion criteria were parents and/or  the children who were not at home when the collection of the data was conducted.. Data collection was conducted using measurement of nutritional status, in depth interview and environmental observation. The data were presented in tables, figures and narration.Results: Three subjects with undernutrition (-3SD to -2SD) and four subjects with severe undernutrition (<-3SD). Factors affecting poor nutritional status were weight loss, no significant weight gain, diet and eating habit, and onset of disease. Commonly occurred infectious diseases were common cold, diarrhea, fever and cough. Some factors affecting infectious diseases were family member transmission, immunization, and treatment behavior.Conclusions: Poor nutritional status and infectious diseases contribute to undernutrition in children under five. DOI: 10.15850/amj.v2n3.553

Abstract

Background: Obesity has a serious impact on health, leading to not only premature mortality but also to long-term morbidity. Factors that contribute to obesity are genetic and lifestyle. This study aimed to analyze the impact of nutritional counseling on nutritional knowledge and energy intake among obese children in junior high school.Methods: This quasi-experimental study was conducted in Jatinangor in September–November 2014 using a consecutive sampling until 25 students had reached. The inclusion criteria were junior high school students with BMI z-score per age >2 obese categories. Nutritional knowledge and energy intake were measured using a 24-hour recall test, conducted before and after 2 weeks of nutritional counseling. Data were analyzed using a paired t-test.Results: The average of nutritional knowledge before counseling was 46.72±2.63 and after counseling was 65.92±2.92 (p<0.001), whereas the average in energy intake before counseling was 1769.68±113.32 and after counseling was 1397.52±114.31 (p=0.006).Conclusions: There is an increase knowledge and decrease energy intake among obese children in junior high school students in Jatinangor subdistrict after nutritional counseling. A better understanding among obese children needs to be highlighted and nutrition education and counseling should be given in schools.
