Abstract

The prevalence of diabetes mellitus and hypertension in Indonesia is increasing both in low- and high-income communities. Therefore, health awareness should be started from a younger age for the prospective future generation. This study, included in a community engagement activity, aims to assess the health profile, in terms of blood pressure (BP) and total cholesterol, of the students (by cross-sectional method) and teachers (by cohort method) of a private madrasah in the rural area of Kecamatan Ciparay, Kabupaten Bandung, West Java. 54 students and 10 teachers participated in this study. Results revealed that the students' average systolic and diastolic BP were 120.23 mmHg and 78.67 mmHg, respectively. The average systolic BP of the teachers in August 2021, January 2022, and August 2022 was 136.8 mmHg, 128.1 mmHg, and 131.9 mmHg, respectively. The average diastolic BP of the teachers in August 2021, January 2022, and August 2022 were 91.6 mmHg, 87.3 mmHg, and 87.1 mmHg, respectively. Moreover, the average cholesterol levels of the students (148.38 mg/dL) and the teachers (190.81 mg/dL) were categorized as normal. Taken together, the health profile of the students and the teachers in Madrasah Al Ihsan is within the normal range. This activity gets positive support from the school and needs sustainability.

Abstract

Kerusakan ginjal akut merupakan penyakit yang berisiko tinggi menyebabkan pasien kritis hingga kematian, membutuhkan diagnosis yang cepat dalam penanganannya. Saat ini pendeteksian penyakit kerusakan ginjal akut masih terbatas dan belum dilakukan secara umum untuk dijadikan standar biomarker yang cepat dan tepat. Neutrophil gelatinase-associated lipocalin (NGAL) merupakan protein lipocalin 25 kDa yang terdapat pada sel epitel ginjal dan hati. NGAL dapat merespon langsung terjadinya kerusakan pada sel tubular epitel, ditandai dengan meningkatnya NGAL dalam urin dan plasma, sehingga NGAL dapat dijadikan penanda kerusakan ginjal akut dengan cepat dengan sensitivitas dan spesifisitas tinggi. Metode yang digunakan dalam review artikel ini menggunakan penelusuran melalui PubMed. Tujuan review artikel ini adalah untuk menunjukkan bahwa NGAL dapat dijadikan penanda gagal ginjal akut yang cepat dan sensitif.Kata Kunci: Kerusakan ginjal akut; biomarker NGAL; pengujian NGAL; sensitivitas; selektfitas.

Abstract

Berbagai efek farmakologis telah dikaji terhadap ekstrak dan islolat dari Acalypha Indica L. terutama terkait dengan aktifitas anti kanker. Matrix metalloproteinases MMP9 merupakan contoh yang sangat baik dari kelompok proteinase terpenting yang terlibat dalam sel metastatic/kanker. MMP9 telah dikaitkan dengan berbagai macam penyakit, termasuk sistem pernapasan, sistem autoimun, dan bahkan kanker. Maka dari itu, makalah ini bertujuan untuk mendapatkan senyawa pemandu dari beberapa fitokimia Acalypha Indica L. yang bertanggung jawab dalam migrasi sel dengan menggunakan penambatan molekul dan dinamika molekul. Ligan ditambatkan ke situs aktif struktur kristal MMP9 (PDB ID: 5UE4) menggunakan aplikasi AutoDock Release 4.2.6. Proses penambatan molekul divalidasi dengan melakukan penambatan ulang ligan alami pada sisi aktif MMP9. Selanjutnya, menggunakan GROMACS dilakukan simulasi dinamika molekul (MD) ditandem dengan algoritma gmx_MMGBSA dan gmx_MMGBSA_ana untuk menghitung energi ikatan dan kontribusi residu terhadap terhadap kestabilan kompleks. Hasil menunjukkan hanya satu ligan dari 36 ligan yang dievaluasi tidak mampu berikatan dengan sisi aktif MMP9, dan enam dari ligan mampu mengikatnya lebih negatif daripada ligan alami. Kompleks dengan ligan S54 dan S56 yang memiliki energi paling negatif tetapi dari sisi parameter lainnya, kompleks S56 paling stabil. Dapat disimpulkan, g-sitosterol asetat (S56) memiliki potensi untuk berfungsi sebagai senyawa pemandu untuk MMP9

Abstract

Pharmaceutical services in hospitals must comply with the standards set forth in Minister of Health Regulation No. 72 of 2016 to ensure the safe, effective, and efficient use of medications. One crucial aspect of clinical pharmacy services is evaluating the appropriateness of drug use in accordance with the National Formulary (Fornas), particularly in antihypertensive therapy, which is widely used in clinical practice. This study aims to evaluate the conformity level of antihypertensive drug use with Fornas at Bandung-Based General Hospital and analyze its economic impact. This research is a retrospective descriptive study that collects data on antihypertensive drug use from various service units from August to October 2024. The analysis was conducted by calculating the percentage of conformity based on the number of drug units used compared to the Fornas list, as well as assessing the economic value of the drugs utilized. The results showed a very high adherence rate to Fornas, with the highest conformity percentage reaching 99.88% in September 2024 and the lowest at 99.81% in August 2024, surpassing the minimum compliance standard of 80% set by the Hospital Minimum Service Standards. These findings reflect the effectiveness of the National Formulary policy and the hospital's commitment to rational therapy. Collaboration among pharmacists, doctors, and the medical team, along with continuous education, is necessary to ensure safe, regulatory-compliant, and efficient therapy.

Abstract

Allium ascalonicum (shallot) is a medicinal plant with promising potential in the management of type 2 diabetes mellitus (T2DM). This review summarizes the phytochemical constituents, mechanisms of action, and existing research gaps that warrant further exploration for targeted drug development. A systematic literature search was performed using PubMed, ScienceDirect, and Google Scholar to identify relevant studies published between 2013 and 2023. Only full-text original research articles in English were included, while reviews and incomplete studies were excluded. The search strategy employed the keywords: “Allium ascalonicum OR shallot AND α-glucosidase.” The findings indicate that A. ascalonicum demonstrates significant antidiabetic activity across in vitro, in vivo, and limited human studies, primarily through inhibition of the α-glucosidase enzyme. This activity is largely attributed to quercetin and other flavonoid compounds, which play an essential role in reducing blood glucose levels and mitigating oxidative stress. Collectively, the evidence highlights the therapeutic potential of A. ascalonicum as a natural α-glucosidase inhibitor. However, further clinical investigations and comprehensive toxicity assessments are required to establish its efficacy and safety as a standardized herbal medicine and future phytopharmaceutical candidate for diabetes management.

Abstract

Single chain fragment variable (scFv) merupakan antibodi rekombinan yang terdiri dari bagian fragmen antigen-binding (Fab) antibodi dengan ukuran sangat kecil sekitar 26 -27 kDa. Secara umum scFv bersifat stabil ,dikode oleh satu gen sehingga akan mempermudah modifikasi genetik. Dibandingkan dengan antibodi lengkap, scFv yang memiliki aktivitas pengikatan antigen yang serupa memiliki kelebihan lain diantaranya adalah ukurannya yang kecil, penetrasi ke pembuluh darah dan jaringan yang lebih baik, serta imunogenisitas yang lebih rendah. Oleh karena itu, scFv memiliki potensi yang sangat baik untuk digunakan sebagai senyawa pemandu diagnosis maupun terapi penyakit. Hasil dari berbagai penelitian menunjukkan scFv anti-HER2 (Human Epidermal Growth Factor Receptor 2) dapat digunakan sebagai pemandu pada diagnosis kanker payudara HER2+ baik menggunakan Magnetic Resonance Imaging (MRI) ataupun positron-emission tomography (PET). Pada terapi kanker payudara bertarget reseptor HER2, scFv anti HER2 juga memiliki potensi digunakan sebagai pemandu untuk imunotoksin, antibody drug conjugate (ADC), prodrug, dan pretargeted radioimmunotherapy (PRIT).

Abstract

Diabetes melitus saat ini menjadi ancaman kesehatan global yang terus meningkat menurut penelitian epidemiologi. Meningkatnya prevalensi diabetes melitus disebabkan oleh kurangnya pengendalian penyakit dan tidak terkontrolnya gula darah sehingga dapat menimbulkan banyak komplikasi. Terjadinya kompIikasi membuat pasien harus mendapatkan obat daIam jumIah banyak (poIifarmasi). OIeh karena itu, studi literatur ini bertujuan untuk menggambarkan peran apoteker dalam memberikan pendampingan terapi diabetes melitus dengan polifarmasi untuk meningkatkan kualitas hidup pasien. PeneIitian ini menggunakan metode studi Iiteratur dengan menggunakan artikeI berbasis nasionaI dan internasionaI. ArtikeI yang digunakan berupa artikel dari tahun 2014 hingga tahun 2024. Data yang diambil memiIiki sumber dari Pubmed, NCBI, Science Direct dan GoogIe SchoIar dengan total 10 artikel yang sesuai dengan kriteria dan mengandung kata kunci yang telah ditentukan. Artikel tersebut kemudian dilakukan analisis dan interpretasi dengan cara membuat rangkuman hasiI penelitiannya. HasiI dari tinjauan pustaka ini didapatkan bahwa ada 4 bentuk peran utama yang dapat diberikan oleh seorang apoteker dalam menigkatkan kualitas hidup pasien diabetes melitus yaitu, memastikan kepatuhan pasien, memberikan edukasi & konseling, melakukan pemantauan terapi obat dan monitoring terapi pasien. Apoteker diharapkan dapat meningkatkan kualitas hidup pasien diabetes melitus melalui peran-perannya tersebut. Kata kunci: diabetes melitus, peran apoteker, peningkatan kualitas hidup

Abstract

Type 2 diabetes mellitus (T2DM) is a widespread global health issue, characterized by insulin resistance and impaired a-amylase activity—an enzyme essential for carbohydrate metabolism. Phenolic compounds derived from brown macroalgae have been identified as potential a-amylase inhibitors and are promising candidates for the development of novel antidiabetic agents. This study aimed to explore the molecular interactions between phlorotannin-derived metabolites from Sargassum sp. and the α-amylase enzyme (PDB ID: 1B2Y) through in silico approaches, including molecular docking and molecular dynamics simulations. Molecular docking was performed using AutoDock 4.2, followed by molecular dynamics (MD) simulations using GROMACS to assess the stability of the ligand–enzyme complexes. The results revealed that dieckol (S06) and 6,6′-bieckol (S07) showed the strongest binding affinity with a docking score of -9.57 and -8.95 kcal/mol, respectively and the most favorable binding free energy (ΔTOTAL -56.87 kcal/mol), suggesting its strong potential for stable interaction with the enzyme. These results highlight the potential of dieckol and 6,6′-bieckol as effective α-amylase inhibitors.

Abstract

The aim of this study was to determine the pharmacological profile, clinical effectiveness, and safety of Vocinti (vonoprazan fumarate) as a P-CAB class gastric acid secretion inhibitor therapy. The method used was a literature review with a qualitative-descriptive approach. Information was collected, manually sorted and critically reviewed to ensure its validity and relevance, in order to produce a comprehensive picture of the pharmacological aspects, clinical effectiveness, and safety of Vocinti use. Based on the available literature review, Vocinti (vonoprazan fumarate) as a P-CAB class gastric acid secretion inhibitor therapy shows a superior pharmacological profile compared to conventional PPIs. Vonoprazan works through reversible competitive inhibition of the H+/K+-ATPase enzyme, resulting in faster, stronger, and more consistent acid suppression, with a longer duration of action and independence from acid activation. Thus, vonoprazan is worthy of consideration as an alternative primary therapy for gastric acid disorders, especially in refractory patients or those requiring long-term therapy.
