Abstract

Komunikasi terapeutik medik adalah proses interaksi tenaga medik dan pasien secara individual dengan tujuan membantu pasien merasa nyaman dan tidak takut untuk mengungkapkan perasaan dan pikiran mereka. Hanya saja, ada indikasi komunikasi terapeutik medik di RSUD Dr. Pirngadi Kota Medan belum efektif. Salah satu penyebabnya adalah tingkat pengetahuan dan keterampilan yang rendah dari dokter mengenai cara menyamankan dan meyakinkan pasien. Agar dokter dapat meningkatkan keterampilannya mengenai bagaimana memprioritaskan kenyamanan dan keyakinan pasien saat berkomunikasi dengan pasien, maka diperlukan pelatihan mendengar aktif, mendengar dengan empati, komunikasi persuasif, dan komunikasi asertif. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan pengetahuan dan keterampilan dokter mengenai cara menyamankan dan meyakinkan pasien berobat di rumah sakit. Pelatihan ini menggunakan metode ceramah, games, dan role play. Peserta pelatihan yang mengikuti sampai akhir hanya 12 orang. Berdasarkan evaluasi pelatihan dengan pre-test dan post-test, pengetahuan peserta tentang cara menyamankan dan meyakinkan pasien berobat di rumah sakit RSUD Dr. Pirngadi Kota Medan meningkat dari 0% menjadi 83,3%. Berdasarkan hasil evaluasi pelatihan dengan simulasi, seluruh peserta mendapatkan nilai 70 ke atas. Disimpulkan bahwa pelatihan komunikasi terapeutik medik yang telah dilakukan dapat meningkatkan pengetahuan dan keterampilan dokter mengenai cara menyamankan dan meyakinkan pasien berobat di rumah sakit RSUD Dr. Pirngadi Kota Medan. Diharapkan pelatihan ini dapat dilanjutkan secara mandiri oleh mitra dan didukung oleh pihak terkait, serta ke depan pelatihan serupa dapat diperluas ke rumah sakit lain dengan pemantauan berkelanjutan.

Abstract

Overdimension and overloading (ODOL) practices in coal transport trucks in Indonesia contribute to increased Vehicle Operating Costs (VOC), energy inefficiency, and accelerated road infrastructure deterioration. Although the HDM-4 model has been widely applied for operational cost estimation, most previous studies rely on default aerodynamic parameters that do not adequately represent geometric changes caused by excessive loading. This study aims to develop a more accurate VOC estimation by integrating Computational Fluid Dynamics (CFD) simulations with calibration of the HDM-4 Level II model under progressive overload conditions. The analysis was conducted on open-body trucks with a 1.2 axle configuration across five loading scenarios ranging from 8 to 12 tonnes. Actual aerodynamic parameters, including frontal area (AF) and drag coefficient (Cd), were obtained through CFD simulations and incorporated into the HDM-4 model to estimate fuel, lubricant, and tyre costs. The results indicate that increasing the payload from 8 to 12 tonnes enlarges the frontal area from 4.08 m² to 4.69 m² and raises the drag coefficient from 0.73 to 0.87, thereby increasing aerodynamic resistance and engine power demand. Economically, this condition leads to a 13.06% increase in fuel costs and a 3.55% increase in tyre costs compared with the standard load condition. These findings demonstrate that integrating actual aerodynamic parameters into the HDM-4 framework produces VOC estimates that are more sensitive to variations in loading configuration. The proposed approach provides methodological contributions to evaluating the economic impacts of ODOL practices and supports the strengthening of overload control policies.   ABSTRAK (Bahasa Indonesia) Praktik Overdimension dan Overloading (ODOL) pada truk angkutan batubara di Indonesia berkontribusi terhadap peningkatan Biaya Operasi Kendaraan (BOK), pemborosan energi, dan percepatan kerusakan infrastruktur jalan. Meskipun model HDM-4 telah banyak digunakan untuk estimasi biaya operasional, sebagian besar studi masih mengandalkan parameter aerodinamika default yang belum merepresentasikan perubahan geometri akibat muatan berlebih. Penelitian ini bertujuan untuk mengembangkan estimasi BOK yang lebih akurat melalui integrasi simulasi Computational Fluid Dynamics (CFD) dengan kalibrasi model HDM-4 Level II pada kondisi overload progresif. Analisis dilakukan pada truk bak terbuka konfigurasi sumbu 1.2 dengan lima skenario muatan antara 8 hingga 12 ton. Parameter aerodinamika aktual berupa luas area frontal (AF) dan koefisien hambatan (Cd) diperoleh melalui simulasi CFD dan digunakan sebagai input kalibrasi dalam model HDM-4 untuk menghitung biaya bahan bakar, pelumas, dan ban. Hasil menunjukkan bahwa peningkatan muatan dari 8 menjadi 12 ton meningkatkan AF dari 4,08 m² menjadi 4,69 m² dan menaikkan Cd dari 0,73 menjadi 0,87, yang memperbesar hambatan aerodinamis dan kebutuhan daya mesin. Secara ekonomi, kondisi tersebut meningkatkan biaya bahan bakar sebesar 13,06% dan biaya ban sebesar 3,55% dibandingkan muatan standar. Temuan ini menunjukkan bahwa integrasi parameter aerodinamika aktual ke dalam HDM-4 menghasilkan estimasi BOK yang lebih sensitif terhadap perubahan konfigurasi muatan. Pendekatan ini memberikan kontribusi metodologis dalam evaluasi dampak ekonomi praktik ODOL serta mendukung penguatan kebijakan pengendalian muatan berlebih.

Abstract

Objectives: Diagnosing uterine rupture in placenta accreta spectrum (PAS) can be challenging, often confirmed only during surgery after the rupture has occurred. This poses significant risks of morbidity and mortality for both the mother and neonates. Therefore, it is crucial for obstetricians to understand the clinical signs of uterine rupture in PAS cases. We aim to present four cases of grade 3a PAS and their associated maternal and neonatal outcomes related to spontaneous uterine rupture. Case management: Four cases of spontaneous uterine rupture attributed to FIGO grading 3A PAS occurred at Wahidin Sudirohusodo Hospital, a tertiary healthcare center in Makassar, South Sulawesi, Indonesia. All patients were multigravida with prior cesarean sections, presented at preterm gestational ages, and had histologically confirmed PAS. They were referred from secondary healthcare facilities. In two cases, spontaneous uterine rupture was identified prior to surgery, with the patients presenting in hypovolemic shock and intrauterine fetal demise. In the other two cases, the rupture occurred during laparotomy before any uterine procedure, and the neonates were delivered alive. During the emergency laparotomy, uterine ruptures were identified at the sites of placental attachment. In one case, an additional rupture was observed on the posterior uterine wall. Management included emergency classical cesarean                        section               followed       by        supracervical hysterectomy,                    postoperative           ICU admission, and massive blood transfusion. All mothers were discharged in stable condition. Conclusion: Spontaneous uterine rupture in PAS cases is associated with increased maternal morbidity as well as neonatal mortality and morbidity. Abstract

ABSTRACT The growth of passenger volume at JB Soedirman Airport is influenced by various socio-economic factors, including population size, Gross Regional Domestic Product (GRDP) per capita, the number of domestic tourists, trade facilities, and umrah agencies. This study aims to analyze the influence of socio-economic factors on the number of passengers at JB Soedirman Airport during the 20212023 period. A quantitative approach with a descriptive method was employed, focusing on the analysis of socio-economic factors affecting passenger volume using secondary data from the Central Statistics Agency (BPS) for 20212023. Multiple linear regression analysis was used to measure the relationship between passenger volume and five independent variables: population size, GRDP per capita, the number of domestic tourists, trade facilities, and the number of umrah agencies. The results of the study indicate that all independent variables, namely population (X₁), GRDP per capita (X₂), number of domestic tourists (X₃), number of trade facilities (X₄), and number of Umrah bureaus (X₅), have a significant effect on the number of passengers (Y) at JB Soedirman Airport. The multiple linear regression model obtained is: Y = -326.256 + 0.000465X₁ + 0.0000064X₂ + 0.000089X₃ + 0.069X₄ + 36.514X₅. This model has been tested using the t test, F test, and coefficient of determination. The test shows that all variables have a significant effect partially (Sig. < 0.05), while the F test shows a significant effect simultaneously (Sig. = 0.005). The coefficient of determination (R²) value of 0.811 indicates that the model is able to explain the variation in the number of passengers by 0.811, while it is explained by other factors outside the model.
