Abstract

Abstract

Breast cancer is the most commonly diagnosed cancer and the leading cause of cancer death in women, therefore breast cancer is the most common cancer in women in most countries. Providing adequate information needed by patients can reduce anxiety and feelings of insecurity. Most of women with breast cancer stated that the information obtained was unsatisfactory. However, few studies have comprehensively addressed the information needs of women with breast cancer across all stages of the disease. This scoping review aimed to fully understand the unique information needs of breast cancer patients at various stages of the cancer continuum. A scoping review will be conducted following PRISMA guidelines. Relevant studies will be identified from databases such as PubMed, Scopus, and Taylor & Francis, EBSCOhost, and ScienceDirect. In addition, searches via Google Scholar, interpersonal network recommendations, and reference chains will also be conducted. Studies will be selected based on PCC criteria, focusing on breast cancer patients as the population, information needs as content, and at various stages of the cancer continuum as the context. Data extraction and risk of bias assessment will be performed using Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) Checklist. This review is expected to provide comprehensive information needs of breast cancer patients at various stages of the cancer continuum. The findings will be valuable for educational institutions, healthcare facilities, in designing innovative, technology-based educating programs that enhance knowledge retention, practical skills, and psychological preparedness.

Abstract

Evidence suggests that cold therapy (cold spray) may be recommended with low confidence for reducing swelling in acute soft tissue injuries among athletes. However, the effect of cold therapy on the mechanical function of muscles remains unclear. This study aims to explore the effects of cold therapy application as a treatment strategy to reduce pan in acute sport injuries. Data collection was conducted in April 2024. We present a case of an acute knee injury in a 16-year-old male during a football tournament. He reported pain with a VAS score of 8 (on a scale of 1 to 10). The patient received cold spray as first aid for his knee injury. However, reassessment revealed that the pain level did not decrease. Several factors may influence the effectiveness of cold spray in reducing pain, including the type of affected nerve fibers, duration of the intervention, history of repeated injuries, size of the injured area, and improper spraying technique. Cold spray is effective in reducing pain and inflammation in superficial tissues. However, in acute injuries involving deeper tissues, cold spray does not significantly alter the pain level. Its effectiveness in reducing inflammatory responses in deeper tissues remains uncertain and warrants further investigation. Keywords: acute pain, cold therapy, foot injury

Abstract

Background: Refractory hyperglycemia in diabetic ketoacidosis (DKA) presents a critical challenge in geriatric patients due to multifactorial physiological and clinical complexities. Early recognition and integrated management are essential, with critical care nursing as a pivotal component. Purpose: This case report aims to describe and analyze the multifactorial causes of refractory hyperglycemia in a geriatric DKA patient, highlighting the importance of early detection, comprehensive correction, and collaborative nursing roles. Method: A descriptive analytic approach was employed to examine the clinical course of a 75-year-old woman with DKA and persistent hyperglycemia. Data were collected from direct patient observation, systematic physical examinations, laboratory assessments, imaging, and multidisciplinary clinical records. Daily therapeutic interventions and patient responses were meticulously documented. Results: Persistent hyperglycemia resulted from several interacting factors: the impact of glucocorticoids on glycemic control, effects of severe pulmonary infection on insulin resistance, the role of electrolyte disturbances in exacerbating hyperglycemia, the relationship between heart failure and diuretic use, the impact of malnutrition and sarcopenia on insulin resistance, and the need for an integrated therapeutic approach. Each factor contributed to increased insulin resistance and complicated glycemic control. Conclusion: Optimal management of refractory hyperglycemia in elderly DKA patients requires early identification and simultaneous correction of underlying factors, supported by intensive critical care nursing and multidisciplinary collaboration.

Abstract

Postoperative pneumonia (POP) is the most common complication in patients after CABG surgery. It increases morbidity, mortality, length of stay, and hospitalization costs. This review aims to identify the factors that influence pneumonia and the microorganisms that cause it in CABG postoperative patients. This scoping review used the framework developed by Arksey and O'Malley (2005). Primary sources of information were searched through five databases: PubMed, Scopus, Science Direct, Oxford, and EBSCO, and 2 search engines: Sage and Google Scholar. Article quality was assessed using the Joanna Briggs Institute critical appraisal checklist. A total of 8 articles with a total of 35235 participants were included in this review. The prevalence of post-CABG POP ranged from 2.4% to 18%. Dominant factors influencing postoperative pneumonia included duration of surgery, CPB time, blood transfusion, mechanical ventilation, LVAD/Tx, smoking history, chronic lung disease, and preoperative malnutrition. Meanwhile, the microorganisms that cause postoperative pneumonia are gram-positive and negative bacteria, fungi, and other bacteria. Infection prevention can be an important point in the CABG postoperative care continuum. By understanding the factors that influence POP, healthcare professionals can optimize patient outcomes, reduce complications, and improve the efficiency of postoperative care.

Abstract

Cardiovascular disorders have been associated with coronavirus disease 2019 (COVID-19). One of the emerging complications is acute pericarditis, characterized by inflammation of the pericardium. Pericarditis is a complication that arises after COVID-19 infection because the virus can cause widespread inflammation, including in the pericardium, the outer layer of the heart. Risk factors that may exacerbate pericarditis in COVID-19 patients include the severity of COVID-19 disease, an immunocompromised state, or an excessive immune system response to infection. The aim of the study is to describe a case of acute pericarditis that occurred after Severe Acute Respiratory Syndrom - Coronavirus 2 (SARS-CoV-2) infection. This study used a case study design. This study was conducted on one of the patients who experienced pericarditis after Covid-19. Researchers conducted nursing assessments and analyzed nursing data. Data analysis was carried out descriptively to describe the results of nursing assessment. A few days after experiencing SARS-CoV-2 pneumonia, 51-year-old Mr. S. developed chest pain that became more painful during exertion, and received a diagnosis of acute pericarditis. As a result, medical treatment was escalated with clinical improvement continuing to improve. Electrocardiography showed elevation in ST segment and depression in PR interval. Echocardiography showed grade I diastolic phase disruption. Acute pericarditis is a rare complication following viral infection. This report shows the assessment and management of acute pericarditis so as to prevent complications that can occur.

Abstract

The management of hypoglycemia emergencies is a unique challenge for emergency medical services (EMS) providers. Hypoglycemia can occur in many different conditions. If left untreated, it can lead to serious complications, including seizures. The correct management of hypoglycemia in the pre-hospital setting can be a preventative measure against a range of complications that can be associated with hypoglycemia. This study aims to describe cases of prehospital management of hypoglycemic patients with seizures. This study uses a case report approach with an emergency care approach consisting of airway, breathing, circulation, disability and exposure. The case used in this case report is a woman with a history of diabetes mellitus, hypertension and Guillain-Barre Syndrome who experienced emergency hypoglycemia with seizures and decreased consciousness. The case study discusses how pre-hospital care is provided and highlights the pre-hospital management of patients with hypoglycemia associated with seizures. Data were collected by observing the clinical signs and symptoms that appeared, then analyzed to determine the appropriate treatment of hypoglycemia emergencies according to evidence. The results showed that pre-hospital interventions can be pharmacological and non-pharmacological. Recommendations from this case study can be used to consider interventions to be used in prehospital management of patients with hypoglycemia with seizures.
