Abstract

Background: The process of grieving and depression can affect the treatment process and the condition of patients with coronary heart disease. The process experienced by the patient can be uncomplicated grief, complicated grief and depression. It is important to distinguish between uncomplicated grief, complicated grief and depression in response to normal and healthy loss and with response to the stresses of personal disorders that require more intensive intervention. The nurse is responsible for helping the patient go through the grieving process so as to prevent the adverse effects on the patient, so that the grieving and depression process are needed to determine the problems and to plan the nursing treatment. Objective: This study aimed to identify the process of grieving and depression in patients with coronary heart disease in the Intensive Room of Hospital Dr. Hasan Sadikin Bandung. Methods: This research method was quantitative descriptive. The study population of patients with coronary heart disease was an average of 37 people per month. Consecutive Sampling technique was carried out for one month with 34 respondents. Data collection technique used in this sresearch was the Grief / Depression Assessment Inventory questionnaire that had been tested for validity with the face validity and content validity. The collected data were analyzed by analytic descriptive method using frequency distribution.Result: The results showed that from 34 respondents, most of them (94.1%) had complicated grief while few of them (5.9%) had uncomplicated grief. The highest response to grieving and depression was in the domain of attitude, thought, feeling, and loss. Conclusion: Based on the results of the study, most patients with coronary heart experienced complicated grief. Therefore it was important for nurses to facilitate patients to be able to consult with treating doctors, psychologists, religious leaders or psychiatrists. Keywords: Coronary Heart Disease, Depression, Grieving

Abstract

The weaning of mechanical ventilation is a complex process and depends on many factors. The failure to wean mechanical ventilation may lead to prolonged duration of mechanical ventilation, which may increase the risk of ventilator associated pneumonia (VAP), morbidity, mortality, increased hospital costs and lower quality of hospital services. In the year 2016, in ICU RSUD Prof.Dr.Margono Soekarjo Purwokerto, in average there is a 20% failure of mechanical ventilation, if not followed up will adversely affect the patient. Factors investigated in this study included patient, nurses, collaboration and organization. The purpose of this study is to analyze the factors that affect implementation of the weaning of mechanical ventilation in ICU Room Prof.Dr.Margono Soekarjo General Hospital Purwokerto. The method used in this research was descriptive quantitative analytic design with cross sectional approach on 47 mechanical ventilation weaning activities. The sample selection was conducted by accidental sampling technique. Measurements for nurses were carried out using validated values including knowledge, experience and collaboration. While the observation sheet included the implementation of standard operating procedures for mechanical ventilation weaning, collaboration, patients APACHE II score and weaning results. The result of bivariate test showed that there were influence of patient (0.000), nurse (0,021), collaboration (0,024) and organization (0,011) to mechanical ventilation weaning activity. The result of logistic regression test showed that the patient was the most influential factor on mechanical ventilation weaning activity with 87% probability.Nurses should pay more attention to patients especially with high APACHE II scores (≥20) because of the risk of mechanical ventilation weaning failure. Assessment of weaning readiness must be carried out daily with appropriate assessment. While hospitals need to revise policies on mechanical ventilation weaning procedure and continue education and training programme related to patients criticality and collaboration.

Abstract

Respiratory failure is the failure to provide oxygen to the entire body and the exchange of gases from the atmosphere to the blood. One of the treatments carried out in patients with respiratory failure is the installation of invasive or non-invasive mechanical ventilation. Weaning is a gradual process of reducing ventilator support carried out at the end of ventilator use. The weaning process can be successful or unsuccessful. This case report aims to discuss the inhibiting factors for weaning preparation in patients with mechanical ventilation with respiratory failure in the Intensive Care Unit (ICU). The design of this study uses the case report method to report the inhibiting factors for weaning preparation in patients with mechanical ventilation with respiratory failure in the Intensive Care Unit (ICU). A patient, a 25-year-old man came to the hospital, was transferred to the ICU due to respiratory failure after urethroplasty surgery with a condition that was already intubated with an endotracheal tube and under sedation. On the 9th day of treatment in the weaning readiness monitor. Based on the results of the monitor, it was found that the patient had a history of asthma and infection so that the nurse carried out collaborative interventions so that the asthma he had was controlled and also prevented infection. In patients, it was found that there were inhibiting factors for weaning, namely comorbid diseases and infections. For the prevention of primary infection that can be done is to maintain hand and environmental hygiene.

Abstract

ABSTRACT Delayed arrival of stroke patients at the hospital is a major factor causing delays in stroke management, every 60-minute delay in the arrival of stroke patients at the hospital has the potential to increase short-term mortality significantly. This study aims to identify factors associated with delayed arrival of patients at the hospital. This research used the systematic review method with keywords “related factors OR related factor OR associated factor” AND “late arrival OR delayed arrival” AND “stroke or acute stroke or ischemic stroke” by including all full-text primary studies written in English and published between 2013 to 2024 from four databases, EBSCO-host, PubMed, and Scopus, EBSCO (Cinahl), Science Direct, and two e-resources, Google Scholar and Sage Journals. Twenty-nine articles were obtained from 7280 articles that discussed the causes of stroke patients' delayed arrival. There were six causes of delayed arrival: Socio-Demographic Factors, Knowledge and Awareness Factors, Family and Social Support Factors, Transportation and Distance Factors, Health System Factors, and Patient Perception of Stroke Condition Factors.Overall, this systematic review identifies factors contributing to delays, including socio-demographic aspects, knowledge, family support, transportation, patient perception, and health system readiness. Addressing these factors through education, infrastructure improvements, and enhancing healthcare service readiness can help accelerate patient arrival at the hospital and improve stroke treatment outcomes. Keywords: Delayed arrival, Pre- Hospital, Stroke

Abstract

Many emergency situations require immediate first aid. The community plays a crucial role in saving victims of cardiac arrest or traffic accidents, which are among the leading causes of premature death in Indonesia. Unfortunately, the level of public knowledge and preparedness in providing first aid remains low, highlighting the need for improvement through education and training. Community health cadres were chosen as the target group because they are closely connected to the community, easily accessible, and can act as first responders before professional medical personnel arrive. This study aimed to enhance the knowledge and readiness of community cadres in recognizing and providing effective first aid through CPR and first aid training in Pangandaran Village. A pre-experimental one-group pre-test and post-test design was used, involving 33 participants selected through accidental sampling. Data analysis was conducted using a paired sample t-test. The results showed a significant improvement (p<0.05) in the cadres’ knowledge and preparedness after the training. Based on these findings, it is recommended that similar training programs be conducted regularly by health institutions and community organizations to strengthen the capacity of cadres in providing first aid, thereby increasing the survival rate of victims of accidents and cardiac arrest within the community. Keywords: Accident, cardiac aArrest, knowledge, preparedness.

Abstract

Recurrent ischemic stroke (RIS) is a major global health issue due to its high morbidity, mortality, and economic burden. While numerous studies have explored the timing and pattern of RIS globally, there is a limited understanding of this issue within the Indonesian context. In the absence of sufficient local data, synthesizing global evidence becomes crucial to inform clinical practice and policy development in regions like Indonesia. This review aimed to explore timing patterns of RIS especially early and late phases and estimate risks based on follow-up, to inform evidence-based interventions. A scoping review based on Arksey & O’Malley framework and PRISMA-ScR guidelines. Literature search (2014–2024) in PubMed, Scopus, ScienceDirect, and CINAHL. Nine studies were included and analyzed thematically. From 2,987 articles, 9 met inclusion criteria. RIS risk ranged 4.3–23.4%, with median recurrence time 21–25 days. Early RIS (<90 days) had distinct risk factors (e.g. hypertension, prior stroke, heart disease) compared to late RIS. Modified Rankin Scale (mRS) and National Institutes of Health Stroke Scale (NIHSS) were key instruments; follow-ups done via clinics, registries, or surveys. Early (<90 days) and late recurrent ischemic strokes differ in both risk factors and clinical implications. Early post-stroke monitoring is critical to reducing recurrence. To address long-term prevention and support adherence to secondary prevention strategies, technology-assisted monitoring or broader digital health solutions should be considered as potential interventions, particularly in overcoming challenges related to long-term follow-up and patient engagement.
