Abstract

Sperm selection technique and Assisted Reproductive Technology (ART) often overlook patients with teratozoospermia, who are more likely to have Deoxyribonucleic Acid (DNA) fragmentation in their sperm, leading to reduced fertilization rates, poor embryo quality, and lower implantation rates during In Vitro Fertilization (IVF). Hyaluronic Acid (HA) binding sperm offers enhanced outcome in ART. This study aimed to obtain information from global scientific research related to HA and sperm selection during In Vitro Fertlization (IVF) procedure by identifying key research trends, influential studies, and emerging themes through bibliometric analysis. The Scopus database was employed to search articles published with keywords “HA,” “teratozoospermia,” and “IVF.” VOSviewer was used to visually represent the bibliometric data, revealing trends in research related to HA for IVF procedure in patients with teratozoospermia. There were 150 articles identified, of which terms like “fragmentation”, “teratozoospermia”, and “sperm abnormalities” are the highlight emerging topics to understand male infertility. Through the years, newer methodologies and techniques in sperm analysis and fertility treatments research had evolved, while research in HA and IVF had increased significantly in many publications and institutions. In conclusion, this bibliometric mapping study has identified key themes and emerging trends, including the critical roles of sperm morphology, motility, and selection in IVF. The growing interest in the relationship between HA and sperm selection techniques highlights the need for ongoing research into HA's application in sperm selection, particularly regarding DNA fragmentation and its effects on fertilization and embryo quality to improve reproductive outcomes.

Abstract

Nutrition is one of the factors that contribute to male infertility. Vitamin D3 are known to affect spermatozoa and usually served in fertility supplementation in doses of 400 and 1000 IU. Low levels of vitamin D3 have been linked to decreased sperm quality and quantity. This research would like to know the effect of Vitamin D3 to the DNA Fragmentation Index (DFI) and Testosterone blood level in men undergoing intrauterine insemination (IUI). This was an analytical observational study with two groups and a pretest-posttest design. Seventeen men aged 20 to 45 who were undergoing the IUI program and did not have diabetes or hypertension are participate in this study. Sperm DFI was determined by using an ELISA method with a DNA Kit. Blood testosterone levels in ng/mL units was measure using ELISA. IBM-SPSS version 22 was used to perform univariate and bivariate analysis on the collected data. The blood level of Vitamin D3 was significantly increased after the intervention of six week Vitamin D3. Meanwhile, this study shows there are no differences in the level of blood testosterone before and after treatment. The DFI was significantly increased in the 400 IU groups.

Abstract

Objective: To report a case of breast growth disorder in a mosaic form of Turner Syndrome. Turner syndrome is a chromosomal condition characterized by small height and primary ovarian insufficiency that affects one in every 2500 female births. Mosaicism is likely to occur when monosomy X develops in only a few cells during development. The clinical presentation of Turner syndrome mosaicism is atypical, with symptom severity varying based on the number of affected cells. This case discusses issues with secondary sex development, including mild hyperandrogenism, and explores how combination hormonal treatment can aid in enhancing secondary sex development. Method: Case Report. Case: A 21-year-old woman presented with chief complaint of the lack of breast enlargement. She exhibited normal genitalia internally and externally and had a regular menstrual cycle. Karyotyping revealed a mosaic pattern of 45, X/46, XX (1 percent/99%) with normal estradiol levels and elevated testosterone levels (indicating mild hyperandrogenism). The patient underwent two cycles of hormone therapy using Ethinyl Estradiol and Drospirenone, resulting in breast growth progression from Tanner stage 1 to Tanner stage 2. Conclusion: Mosaicism in Turner syndrome is plausible, and the severity of clinical symptoms correlates with the number of defective chromosomes. The presentation of Turner syndrome mosaicism varies, and therapy should be tailored to address specific symptoms. While breast development is observed in some girls with Turner Syndrome, instances of breast growth disorder may occur, involving estrogen activity and estrogen receptor sensitivity. Although the exact cause of impaired breast growth remains unknown, administering estrogen in such cases can improve secondary sexual characteristics. Keywords: mild hyperandrogenism, mosaicism, turner syndrome

Abstract

Endometriosis significantly affects women’s sexual health, manifesting as pain and dyspareunia, which reduces quality of life. In Indonesia, hormonal and surgical interventions are both used to manage endometriosis. However, the study of their effect on sexual function among Indonesia’s population is still unclear. This quasi-experimental study included 60 women, divided into two groups based on the type of therapy. This study was conducted from June to September 2024 at the Department of Obstetrics and Gynecology, Dr. Moewardi General Hospital. Outcomes were assessed with the Female Sexual Function Index (FSFI) questionnaire, before and after treatment. The analysis showed the mean rank of the surgical intervention (44.97) is higher than the hormonal intervention (16.03). The Mann-Whitney U test revealed a significant difference between the two groups of intervention (p=0.000). These findings indicate that surgical intervention is superior to hormonal therapy in improving the FSFI score that reflects sexual function among women.

Abstract

Background: Assisted reproductive technology (ART) programs have proven to be a solution to help infertility couples to conceive. However, most couples face a financial crisis related to the relatively expensive cost of ART treatment. This service aims to educate about ART's meaning, direction, and procedures. Thus, couples want children to be able to understand the ART program at an affordable cost (low-cost) and undergo the ART program without any concerns about costs.Method: Seminar participants be given socialization in person and online through zoom software by the service provider. It is in two stages, in-person and online, so participants who cannot attend in person can follow this service program. In addition, participants are given a pre-test and post-test questions to assess their understanding of the ART program at an affordable cost.Result: The pre-test and post-test results were analyzed using Wilcoxon, which showed that after providing education about the ART program, the post-test results were significantly better than the pre-test results. Down value: 5.5%, fixed value: 15.2%, value rise: 79.3% with significance p<0.001.Conclusion: There are significant differences in the pre-test and post-test scores, which means that the participants understood the explanation the servicer gave.
