Abstract

Background: Hepatitis B virus (HBV) infection is a worldwide problem associated with morbidity and mortality. Hepatitis B virus (HBV) was identified as a major risk factor for carcinoma, one of them is associated with hepatocellular carcinoma. Aim: This study aims to determine the profile of hepatitis B patients with malignancy at the Gastroenterohepatology Outpatient Polyclinic at Saiful Anwar Regional General Hospital. Methods: This research is an observational study with a cross-sectional design. Data was collected from 2013 to 2018. The chronic hepatitis B patients were found in the Gastroenterohepatology polyclinic. Data was collected in the form of secondary data, including patient demographic data including age, sex, and type of drug. The results are described descriptively and presented in tables/percentages. Results: From the results of the study, the total number of chronic hepatitis B patients with malignancy in was 218 people. Most of the patients were aged 20-60 years with an average age of 49.7 ± 12.6 years. Most of the subjects were male, 78.0%. Most of the therapy given to patients was tenofovir 300 mg (93.2%). Cases were dominated by liver malignancy (52.8%). Most of the patients who had non-liver malignancy were diagnosed with ca mammae (37.0%). Conclusion: Chronic hepatitis B virus infection may have the potential to damage liver tissue which can lead to malignancy. The prevalence of liver and non-liver malignancies in chronic hepatitis B patients in this study were 52.8% and 47.3%, respectively.

Abstract

Introduction. Hypoalbuminemia is a strong predictor of mortality in nonoperative and operative patients. Albumin plays a role in maintaining oncotic pressure in critical conditions such as sepsis. The administration of exogenous Human Serum Albumin (HSA) in sepsis with hypoalbuminemia conditions is still controversial with varying outcomes.This study aimed to know the mortality ratio between intravenous albumin administration and no intravenous albumin administration in sepsis patients with hypoalbuminemia conditions and also to know the factors that influence the mortality of sepsis patients. Methods. An observational study with a prospective approach involving 75 research subjects aged >18 years with sepsis accompanied by hypoalbuminemia (<2.5 g/dL) treated in the high care unit of RSUD dr. Saiful Anwar Malang during the period 1 September 2018-31 August 2019. Subjects were divided into two groups namely albumin and nonalbumin groups. Both groups were followed during hospitalization until they discharged or died. Differences in mortality between the two groups were analyzed by chi square bivariate test. The most influential factors on mortality were analyzed by multivariate binary logistic regression tests. Results. There were 39 people (52%) from the albumin group and 36 people (48%) from the non-albumin group. Difference in mortality between albumin and nonalbumin groups {25 (64.1%) vs 16 (44.4%) with OR 2, p=0.138} means there was no statistically significant difference. Factors that influence mortality included: SOFA score (OR 34.27, p <0.001), MAP value (OR 8, p<0.001), septic shock (OR 4.31, p=0.03), diabetes mellitus (OR 0.28, p=0.009), respiratory failure (OR 8.02, p <0.001), decreased of consciousness (OR 64.75, p <0.001), cardiovascular failure (OR 6, p <0.001), hematological failure (OR 3.05, p=0.027). The most dominant factor affecting mortality in sepsis patients is decreased of consciousness (OR 2.67, p=0.001). Conclusion. The administration of albumin transfusion did not make a significant difference in the incidence of mortality in sepsis patients with hypoalbuminemia. The most influential factor on mortality of sepsis patients is decreased of consciousness.

Abstract

Introduction. Resident physicians have a high risk of GERD due to excessive fatigue especially during a COVID-19 pandemic. However, research on resident physicians risk factors for the incidence of GERD in the COVID-19 pandemic era has never been conducted. This study aimed to analyze the risk factors for GERD in the era of the COVID-19 pandemic on resident physicians. Methods. This study used an analytic observational design with a cross sectional approach. Subjects were resident physicians in clinical and pre-clinical stage at the Faculty of Medicine Brawijaya University which were selected using random sampling method. Questionnaires were given to subjects using google forms, consisted of education burden, diet, psychosocial-economic, and GERDQ quiestionnaire. Risk factor analysis was performed using chi-square and logistic regression with a significance level of p Results. Out of 230 subjects, most of them were female (54.5%) and mean of age was 30.32 (SD 2.60) years. The risk factors for GERD in resident physicians at Brawijaya University are hypertension (p=0.035), smoking (p=0.019), educational burden [total working hours ≥40 hours/week (p Conclusion. Education burden, diet, and psychosocial-economic factors are associated with the incidence of GERD in resident physicians in the pandemic era.

Abstract

ABSTRAK Malnutrisi adalah masalah yang umum terjadi pada pasien sirosis hati. Kurangnya pengetahuan tentang gizi sehingga menyebabkan adanya pembatasan diet yang tidak perlu dan kemudian berkontribusi pada kejadian malnutrisi. Penelitian ini bertujuan untuk mengetahui adanya pengaruh pemberian edukasi gizi terhadap status gizi dan asupan pasien sirosis hati di Poli Gastroenterohepatologi Rumah Sakit dr. Saiful Anwar Malang. Jenis penelitian yang dilakukan termasuk dalam penelitian experimental dengan desain penelitian pre-post test control group design. Kelompok intervensi adalah kelompok yang diberi edukasi gizi pada awal pengambilan data, sedangkan kelompok kontrol adalah kelompok yang tidak diberi edukasi gizi. Data yang diambil pada penelitian ini adalah data antropometri, data asupan saat pengambilan data awal dan akhir serta data asupan makan partisipan yang dimonitoring selama 1 bulan menggunakan food diary. Hasil penelitian ini menunjukkan tidak terdapatnya penurunan yang signifikan (p=0.48) pada Indeks Massa Tubuh partisipan di kelompok intervensi dan kontrol. Sedangkan asupan energi dan karbohidrat pada kelompok intervensi mengalami peningkatan yang signifikan (p<0.05). Pemberian edukasi akan memberikan efek positif dan efektif jika dilakukan bersama dengan peningkatan aktivitas fisik. Efek dari pemberian edukasi gizi akan nampak bila durasi monitoring dan evaluasi diperpanjang. Kata kunci: edukasi gizi, sirosis hati, status gizi, asupan zat gizi, antropometri giziABSTRACT Malnutrition is a common problem among liver cirrhosis patients. Lack of knowledge about nutrition causes unnecessary dietary restrictions and leads to the incidence of malnutrition. This study aimed to determine the effect of nutrition education on intake and nutritional status of patients with liver cirrhosis at the Gastroenterohepatology Department, dr. Saiful Anwar General Hospital, Malang. The design used was an experimental study with a pre-post test control group. The intervention group is the group given nutrition education, while the control group is the group without nutrition education. The data taken in this study were anthropometric, intake data when taking initial and final data, and participants' daily food intake monitored for a month using a food diary. The study results showed no significant decrease (p = 0.48) in the body mass index of participants in the intervention and control groups. However, the energy and carbohydrate intake in the intervention group experienced a significant increase (p <0.05). Providing nutrition education will have a positive and effective effect if done together with increased physical activity. Abstract

Introduction: Colorectal cancer (CRC) occurring in younger women can lead to an increased incidence of colorectal cancer during pregnancy. The estimated frequency of malignancy during pregnancy with colorectal cancer specifically estimated at 1 in 13,000 pregnancies. However, the exact causes of colorectal cancer during pregnancy are not yet fully understood. Diagnosing and managing colorectal cancer during pregnancy pose significant challenges because the symptoms can be masked by the normal signs and symptoms of pregnancy. Case Pesentation: A 42-year-old pregnant patient, Mrs. L, presented with abdominal pain, a history of dark-colored bowel movements, and a lump on the left side of her neck that had been present for a year. She had also experienced a weight loss of 5 kg both before and during pregnancy, and there was no previous history of malignancy. The patient underwent various diagnostic procedures, including pregnancy ultrasound, abdominal ultrasound, and colonoscopy biopsy. Based on the comprehensive evaluation of the patient's medical history, physical examination, and supporting tests, she was diagnosed with stage IV rectal adenocarcinoma with metastases to the liver, kidney, and lymph nodes. The patient was in her second pregnancy (G2P1Ab0) at 27-29 weeks of gestation. Conclusion: Colorectal cancer during pregnancy poses diagnostic and therapeutic challenges, as the signs and symptoms are often attributed to the normal progression of pregnancy, resulting in delayed diagnosis at an advanced stage. Early detection, evaluation, and monitoring of nonspecific symptoms are crucial, and a multidisciplinary approach is necessary for the management of colorectal cancer during pregnancy.

Abstract

Metode pendekatan yang digunakan dalam penelitian ini adalah pendekatan yuridis sosiologis atau socio legal research, yaitu cara atau prosedur yang digunakan untuk memecahkan masalah penelitian dengan meneliti data sekunder yang berupa bahan-bahan hukum atau peraturan peraturan hukum yang berlaku kemudian dilanjutkan dengan mengadakan penelitian terhadap data primer di lapangan. Dalam Putusan Nomor : 236/Pdt.G/2022/PN TJK Penggugat dan Tergugat  saling menggugat dengan gugatan perbuatan melawan hukum berdasarkan Pasal 1365 KUH Perdata dan gugatan wanprestasi berdasarkan Pasal 1320 KUH Perdata.

Abstract

AbstractHepatitis B is an illness resulting from the hepatitis B virus, which can spread through both horizontal and vertical transmission. According to the World Health Organization (WHO) in 2019, approximately 296 million individuals across the globe were afflicted with hepatitis B, with an anticipated annual increase of 1.5 million cases. Various approaches and solutions have been implemented to prevent and address the impact of this disease, including hepatitis B immunization policies from birth. According to the Regulation of the Minister of Health No. 25 of 2015, other approaches are also being pursued through health promotion using various media. This study aims to determine the effectiveness and factors related to public knowledge about hepatitis B through interactive educational media for the community in Gondanglegi Village, Malang Regency. A total of 176 respondents completed the questionnaire, with 75% being female and 24% male. The majority of respondents fell within the age range of 51-60 years (34.7%). The Wilcoxon test results show a significant difference between pre-test and post-test scores (p=0.000). The Spearman test, which relates age and the last level of education to pre-test and post-test scores, reveals a significant relationship between pre-test scores and age (p=0.000, r=-0.458) as well as the last level of education (p=0.000, r=0.472). The correlation analysis results indicate that older age is associated with lower pre-test scores, while higher levels of education are associated with higher pre-test scores. A similar pattern is observed in the correlation test between post-test scores and age (p=0.082, r=-0.204) and the last level of education (p=0.013, r=0.013). These results indicate that the community engagement program to increase awareness of hepatitis B has successfully improved participants' knowledge, as reflected in the difference between pre-test and post-test scores."

Abstract

Dysbiosis, marked by a decline in Firmicutes (such as Clostridium spp.) and a rise in Proteobacteria (such as Escherichia coli), is recognized as a pathway linked to the progression of non-alcoholic fatty liver disease (NAFLD). High-fat food may influence the development of NAFLD. This study investigates the effect of different high-fat diets on gut microbiota composition and butyrate levels in a rat model. Four groups of Rattus norvegicus strain Wistar (n=27) were fed specific diets for 12 weeks: normal diet (ND, n=7), high-fat diet (HFD, n=6), western diet (WD, n=7), and high-fat-high-fructose diet (HFHFD, n=7). Stools were cultured on Eosin Methylene Blue (EMB) for Escherichia coli and Forget Fredette Agar (FFA) for Clostridium spp. Colonies were counted using Total Plate Count (TPC), and butyrate levels were measured with gas chromatography. Liver histology was assessed with the NAFLD activity score (NAS). Gut microbiota were analyzed using Crosstab and Chi-Square tests, and butyrate levels with the Kruskal-Wallis test. Butyrate levels decreased in NAFLD-inducing diet groups (HFD, WD, HFHFD) compared to ND (p=0.021). Escherichia coli colony counts were significantly higher in NAFLD-inducing diet groups (p=0.048), while Clostridium spp. counts were higher in ND (p=0.001). Liver histological changes, particularly in the WD group, resembled NAFLD. These findings imply that different high-fat diets can alter the gut microbiome, potentially influencing NAFLD development.
