Abstract

Highlights: 1. As there is a scarcity of publications on the use of metformin for COVID-19 in Indonesia, the findings of this present study may contribute more insight to the existing body of research and provide data specific to the Southeast Asian population. 2. This study revealed a decreased mortality rate associated with metformin use in diabetic patients with mild to moderate COVID-19 infection. 3. This study suggests that diabetic patients may continue metformin treatment during a COVID-19 infection as the medication has sustained therapeutic effects. Abstract COVID-19 patients with comorbidities, such as type 2 diabetes (T2DM), have a higher mortality rate compared to those without any comorbidities. T2DM patients usually receive metformin as their first-line treatment. However, the effectiveness of metformin in reducing mortality rates still requires further analysis. The objective of this study was to analyze the effectiveness of metformin in reducing mortality rates among COVID-19 patients with T2DM. An analytic observational design with a retrospective cohort approach was used in this study. Samples were acquired from hospitalized COVID-19 patients with T2DM medical records at Fatmawati Central General Hospital, Jakarta, Indonesia, throughout 2020–2021. The samples were collected using a purposive sampling technique and analyzed using Chi-square test (p<0.05; RR<1). This study comprised 137 samples, with 56 samples receiving metformin and 81 not receiving metformin. The mortality rate in the sample group that received metformin was lower (19.6%) compared to the group that was not given the medication (38.3%). The Chi-square test results indicated a statistically significant relationship between metformin treatment and a lower mortality rate among COVID-19-contracted individuals with T2DM (p=0.020; RR=0.513). Therefore, this study concludes that the administration of metformin treatment reduces mortality among COVID-19 patients with T2DM.

Abstract

Acne is a disease that has affected many populations globally. One of the causes of acne is the presence of acne-causing bacteria, namely Cutibacterium acnes, which is reportedly resistant to several antibiotics. Andaliman fruit (Zanthoxylum acanthopodium DC.) is traditionally used by Indonesian people as a spice which has antibacterial potential. This study aims to evaluate the antibacterial activity of the ethanolic extract of Andaliman fruit against C. acnes using the microdilution method. The extraction method for andaliman fruit was the Ultrasonic-Assisted Extraction with variations in frequency (30, 40, and 50 kHz) and duration (10, 15, and 20 minutes). Phytochemical screening showed the presence of flavonoids, alkaloids, tannins, steroids, and saponins in the extract. The results showed the antibacterial activity of Andaliman fruit extract against C. acnes, with the Minimum Inhibitory Concentration (MIC) observed in the extract with a frequency of 40 kHz and an extraction duration of 10 and 15 minutes at a concentration of 5000 ppm. However, the Minimum Bactericidal Concentration (MBC) test showed negative results, indicating that Andaliman extract did not have a bactericidal effect on these bacteria. Statistical analysis using the Kruskal–Wallis and Dunn tests revealed significant differences (p < 0.05) between extraction treatments and the positive control in MIC and MBC values. These findings indicate that the ethanolic extract of andaliman fruit possesses inhibitory but not bactericidal activity against C. acnes, suggesting its potential as a natural anti-acne agent for further formulation and mechanistic studies.

Abstract

Background: More than 90% of adults with type 2 diabetes are overweight or obese. This condition is known to increase oxidative stress, which contributes to cellular damage and disease complications. As a functional food, kefir has long been recognized for its various health benefits, including anti-inflammatory, antioxidant, and antidiabetic properties. Objectives: This study aimed to analyze the potential of randu honey kefir in preventing oxidative stress in diabetic rats. Methods: This study employed a randomized block design with a 21-day intervention period. A total of 42 male Sprague Dawley rats aged 6–8 weeks were divided into six groups: KS (standard), KN (negative control), K1 (quercetin), K2 (metformin), P1 (randu honey kefir 1.8 mL/200g BW), and P2 (preventive group). Diabetes was induced using streptozotocin at 40 mg/kgBW combined with a high-fat diet. Malondialdehyde (MDA) levels were analyzed using spectrophotometry, while interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) levels were measured using ELISA kits. Results: Significant differences (p-value<0.05) were observed in MDA, TNF-α, and IL-6 levels between the P1 and P2 groups compared with the KN group. Randu honey kefir groups showed mean oxidative stress marker levels comparable to those of quercetin and metformin treatments, including MDA (P1: 32.9 ± 29.35 nmol/mL; P2: 23.77 ± 11.63 nmol/mL), TNF-α (P1: 45.37 ± 31.25 pg/mL; P2: 37.81 ± 27.00 pg/mL), and IL-6 (P1: 64.81 ± 21.35 pg/mL; P2: 59.23 ± 14.95 pg/mL). Conclusions: Randu honey kefir intervention demonstrates potential in suppressing oxidative stress in diabetic rats.
