Abstract

Sleep disturbances are highly prevalent among patients with Chronic Kidney Disease (CKD), occurring both in the pre-dialysis stage and among individuals undergoing hemodialysis or peritoneal dialysis. These disturbances can significantly impair overall quality of life. Various factors contribute to sleep disorders in CKD patients, including the accumulation of uremic toxins, pain, pruritus, dyspnea, anxiety, and the side effects associated with dialysis therapy itself. Physical activity-based interventions, such as aerobic exercise and yoga, have shown substantial potential in improving sleep quality. The objective of this systematic review is to evaluate the effects of yoga and aerobic exercise interventions on sleep quality in patients with CKD. A comprehensive literature search was conducted using databases including ProQuest, PubMed, Scopus, the International Journal of Health Sciences and Research, Springer, and the Iranian Journal of Nursing and Midwifery Research. Only full-text articles published in English between 2019 and 2024 were included. The review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. Thirteen studies were identified: 11 Randomized Controlled Trials (RCTs), 1 quasi-experimental study, and 1 qualitative study. Analysis of the 11 RCTs revealed that various interventions—particularly yoga, aerobic exercise, and cognitive behavioral therapy (CBT) were employed to improve sleep quality. Among these, yoga and aerobic exercise were found to significantly enhance sleep quality in CKD patients, especially those undergoing hemodialysis, with aerobic exercise demonstrating more consistent benefits. Conversely, CBT did not produce significant improvements in sleep quality compared to the control group, though it did show limited effects on reducing depressive symptoms and dysfunctional beliefs. The quasi-experimental study examined the impact of yoga exercise and found it to be a contributing factor to improved quality of life, including better sleep. The qualitative study employed a phenomenological approach to explore the subjective experiences of CKD patients with fatigue caused by poor sleep, aligning with the study's aim of understanding this phenomenon from the patient’s perspective.

Abstract

Sleep disturbances are common in patients with stroke and chronic kidney disease, potentially worsening health conditions and reducing quality of life. Non-pharmacological interventions have received less research attention, despite the potential for non-pharmacological approaches to improve sleep quality without causing side effects. This systematic review aimed to evaluate the effectiveness of non-pharmacological interventions in this population. Randomized controlled trials involving patients with stroke and chronic kidney disease aged >18 years were searched in seven English-language databases. Methodological quality was assessed using the JBI critical appraisal tool, and data were synthesized. 9 RCTs met inclusion criteria and demonstrated significant benefits. Cognitive Behavioral Therapy (4 studies) consistently improved sleep quality and reduced fatigue, anxiety, and depression (p < 0.001). Reflexology (2 studies) decreased anxiety and improved sleep quality (p < 0.001). Acupuncture significantly improved ISI, PSQI, and HADS scores (p < 0.001–0.003). Lavender aromatherapy reduced depression and improved sleep quality (p < 0.001), while spiritual care improved sleep quality and spiritual well-being (p < 0.001). These findings highlight the role of nurses in implementing non-pharmacological interventions to address sleep disturbances and related psychological symptoms in patients with stroke and chronic kidney disease. Cognitive Behavioral Therapy is recommended for patients with insomnia and psychological comorbidities, reflexology and massage may be beneficial for those undergoing hemodialysis, while acupuncture, aromatherapy, and spiritual care can be tailored to individual needs. Further research is needed to assess their long-term effectiveness and their integration into broader clinical practice.

Abstract

The effective management of postoperative pain and anxiety remains a challenge in patient care. Immersive virtual reality (IVR)-based distraction has been proposed as a non-pharmacological intervention to help reduce these symptoms. This study aimed to evaluate the effectiveness of immersive VR-based distraction in reducing postoperative pain and anxiety. This systematic review followed the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines by identifying studies that assessed immersive VR-based distraction in postoperative patients. Studies published from 2015 to 2025 were searched in ClinicalKey Nursing, EbscoHost, ProQuest, ScienceDirect, Scopus, and Taylor and Francis databases. Fourteen randomized controlled trials (RCTs) were included, covering various surgical contexts such as orthopedic, abdominal, cardiac, and oral surgery. Most studies showed that immersive VR-based distraction effectively reduced postoperative pain and anxiety, although some reported no significant effect. Variations in outcomes were influenced by patient characteristics, duration and frequency of VR use, and the type of VR content. Studies were excluded if they involved non-postoperative populations, pediatric samples, non-immersive VR, or non-interventional designs. Overall, immersive VR-based distraction shows promise in reducing postoperative pain and anxiety, although variability in effectiveness highlights the need for further research on technical parameters and clinical implementation

Abstract

Sifon is a post-circumcision ritual of sexual intercourse in the Atoin Meto tribe with women who are not their partners. Sifon women have various reasons for doing this practice such as fulfilling their economic and daily needs. Most of Sifon women are the heads of families. In addition to having an impact on the risk of contracting HIV, AIDS and STD Infection, Sifon also has other impacts on women such as social stigma and shameful accusation. Objective:The purpose of this study is to explore the psychological experiences of Sifon women in Timor Island.Method:This study used a phenomenological approach and purposive sampling technique. Participants were with inclusion criteria. They have to be more than 18 years old and have practiced Sifon in the last 6 (six) months. Data collection was carried out from March to July 2020 in Kupang Regency and South Central Timor Regency, East Nusa Tenggara Province. The data was processed using Collaizi method. Results: There were 13 women who were successfully interviewed and 7 of them shared their psychological experiences. Most of them are farmers, have elementary school education and become the head of their families. Based on the results of data analysis, the researcher found 3 themes: (1) feeling shameful, (2) feeling humiliated and not worthed, (3) having trauma. Conclusions:The decision to become a Sifon woman was made with various considerations, including meeting economic needs and earming money for all family members. It is necessary to take a culturally sensitive approach and deeper psychological assistance with Sifon women to help improve their wellbeing. The findings of this study show that most of these Sifon women are heads of families.

Abstract

Hemodialysis (HD) is the main therapy for patients with end-stage renal disease (ESRD) in Indonesia, but the patients' quality of life is lower than other methods due to the physical and psychological effects of HD. One of the therapies that can be used to reduce the effects of HD is relaxation therapy. Objective: This review aims to analyse the effectiveness of relaxation therapy in managing physical and psychological symptoms to improve the quality of life of HD patients. Method: A literature review using databases such as Pubmed, Science Direct, Scopus, Sage Journal and Proquest to search for relevant articles within the period 2020-2024. Results: A review of twelve articles shows that relaxation therapy is effective for pain, fatigue, stress, depression, anxiety, sleep quality and patient quality of life. Conclusions: Relaxation therapy can be integrated into routine care and is feasible for nurses to provide because of its simplicity so that patients can do it themselves at home. Nurses also need to conduct further research into other non-pharmacological therapies that are widely effective for patients with ESRD.
