Abstract

Dyspnea is a subjective sensation of shortness of breath. The emergency nursing problem that arises in dyspnea is ineffective breathing patterns. Ineffective breathing patterns are inspirations and/or expirations that do not provide adequate ventilation. Patients with ineffective breathing patterns are given airway management interventions, in addition to providing oxygen therapy to maximize oxygen entering the body, by providing the Respiratory Muscle Stretch Gymnastics (RMSG) breathing movement technique to help loosen the respiratory muscles to normalize the patient's breathing pattern. At the assessment stage, data was obtained on a case of dyspnea, namely COPD, with complaints of shortness of breath for three days but worsened on the day he was taken to the emergency room with the patient having an irregular breathing pattern, rapid pulse and a history of hypertension. Based on this assessment, a nursing diagnosis of ineffective breathing pattern was obtained with a nursing plan for 1x3 hours. The nursing plan that will be given is airway management with Respiratory Muscle Stretch Gymnastics (RMSG) therapeutic measures. The results of the analysis show that Respiratory Muscle Stretch Gymnastics (RMSG) is not appropriate to carry out in the emergency room where this action is less able to overcome ineffective breathing patterns in dyspnea patients in the emergency room.

Abstract

Following the enactment of Law No. 17 of 2023 on Health, which stipulates that nominal amounts do not determine the allocation of health financing, provincial and district governments are have been striving to implement teh law based on regional priority scales. The World Health Organization (WHO) recommends six patterns and seven health financing mechanisms, which have been variably adopted across regions, including in Jambi Province. The study aims to examine the influence of family socioeconomic status on the selection of health financing models among vulnerable population group. A quantitative approach with a cross-sectional design was employed, involving 271 households as the sample. Data were analyzed using multiple logistic regression. The finding indicate that socioeconomic status has a positive influence (0.945) on the ability to afford healthcare services. This means that for every one-unit increase in socioeconomic status, the likelihood of being able to purchase healthcare increases by 0.945 times. The predictive model for the choice of health financing mechanisms reveals a preference for per package payments, Diagnosis Related Groups (DRGs), monthly salaries, and fee-for-service models. Conversely, global budgets, capitation, and pre-payment reimbursement schemes were the least preferred. These finding inderscore the importance of socioeconomics factors in determining access to and preferences for health financing mechanisms, particularly among vulnerable groups.

Abstract

Open pneumothorax occurs due to a stab wound to the chest wall and penetrates the chest cavity. The wound remains open (sucking chest wound). Open pneumothorax cases require immediate wound closure with an occlusive bandage as a temporary measure. The purpose of this study was to discuss emergency management by observing changes in hemodynamic status in patients with open pneumothorax after the implementation of an occlusive bandage. This study was a case report on a single patient. The case was selected due to a rare emergency situation. The patient was given an occlusive bandage. The results showed that the patient was a 24-year-old man who presented with a stab wound to the left chest due to being stabbed with a knife. Signs of respiratory distress were found. Left chest wall movement was delayed, there was a sucking chest wound on the left side, decreased breath sounds on the left side, and a hyperresonant left chest. Physical examination findings included blood pressure 80/43 mmHg, heart rate 106 beats/min, respiratory rate 25 beats/min, SpO2 92%, temperature 36.1°C, Glasgow coma scale 15. A chest X-ray revealed a left pneumothorax, while laboratory results were within normal limits. In the emergency department, a modified occlusive bandage made of plastic, cut to size, was applied and taped on three sides. Following the occlusive bandage, hemodynamic status improved, allowing for definitive intervention and further management. It was concluded that the application of an occlusive bandage can improve the patient's hemodynamic status, making it important to implement this procedure before definitive intervention. Keywords: open pneumothorax; sucking chest wound; occlusive bandage ABSTRAK Open pneumothorax terjadi akibat luka tusuk pada dinding dada dan menembus rongga dada. Abstract

Background: Drowning is defined as death caused by asphyxia due to immersion in water that occurs within 24 hours of the incident. Immediate and appropriate first aid is essential to increase the chances of survival. Lifeguards, as the front line in aquatic emergencies, must have adequate knowledge and skills in performing first aid for drowning victims. Health education is one of the efforts to improve lifeguards' understanding and ability to act promptly and correctly in such critical situations. Method: This study employed a pre-experimental research design using a one-group pretest-posttest approach. A total of 33 lifeguards from three different swimming pools in Jambi City were selected using a total sampling technique. Data were collected through a structured questionnaire measuring the level of knowledge regarding first aid for drowning victims before and after the health education intervention. To determine the significance of the changes in knowledge, data were analyzed using the Wilcoxon signed-rank test. Results: The results of the bivariate analysis using the Wilcoxon test showed a Z value of -5.205 with a significance level of 0.001 (p < 0.05). These findings indicate a statistically significant improvement in lifeguards’ knowledge after receiving health education. Therefore, the null hypothesis (H0) is rejected, and the alternative hypothesis (H1) is accepted. Conclusion: Health education has a significant positive effect on increasing lifeguard knowledge regarding first aid for drowning victims in Jambi City swimming pools. These results emphasize the importance of continuous education and training programs to enhance the readiness and effectiveness of lifeguards in responding to drowning emergencies.

Abstract

Background: First aid is a crucial initial step in the event of an accident, serving to minimize the severity of trauma and facilitate subsequent medical treatment. A prompt and appropriate response can significantly influence the survival and recovery outcomes of accident victims. To foster such responsiveness, community involvement and awareness are essential. Community members, as first responders at the scene, play a critical role in administering immediate aid. However, their effectiveness is heavily influenced by their knowledge and attitudes toward first aid. This study aims to assess the level of knowledge and attitudes of the community regarding first aid for traffic accident victims along the Palembang–Jambi route, specifically in the Bayung Lencir area. Method: This research employed a quantitative approach with a descriptive research design using a cross-sectional method. The study was conducted in Bayung Lencir District, Musi Banyuasin Regency from June 19, 2024, to July 16, 2024. The sample consisted of 408 respondents, selected using a non-probability purposive sampling technique. Data were collected using a structured questionnaire assessing the respondents' knowledge and attitudes toward first aid in traffic accidents. Results: Out of 408 respondents, the majority had a sufficient level of knowledge, with 247 respondents (60.5%). Those categorized as having good knowledge were 88 respondents (21.6%), while 73 respondents (17.9%) had poor knowledge. Regarding attitude, most respondents demonstrated a positive attitude, totaling 332 individuals (81.4%), whereas 76 respondents (18.6%) exhibited a negative attitude toward providing first aid in traffic accident situations. Conclusion: This study reveals that the overall community knowledge and attitude toward first aid in traffic accidents along the Palembang–Jambi Bayung Lencir road fall within the sufficient category. Nevertheless, a notable portion of the population still lacks adequate understanding and confidence in properly administering first aid. These findings highlight the need for targeted community education programs and practical first aid training to ensure a more prepared and responsive public in emergency traffic accident situations.
