Abstract

Abstract

Electronic medical records (EMRs) are essential to modern healthcare systems, yet their implementation continues to face challenges such as disruption of doctor–patient communication, administrative burden, and documentation structures misaligned with clinicians’ clinical reasoning processes. This narrative review aims to assess the limitations of conventional EMRs and examine the potential of artificial intelligence (AI) to improve documentation efficiency, accuracy, and overall clinical performance. The literature indicates that AI technologies, including machine learning, deep learning, and natural language processing, can automate documentation, streamline workflows, enhance decision support, and reduce physician burnout, thereby allowing clinicians to devote more attention to patient care. Nevertheless, the integration of AI into EMRs must address ethical concerns such as algorithmic bias, data privacy, and accountability for AI-assisted decisions. In conclusion, AI-driven EMR optimization has the potential to create a more human-centered, efficient, and data-driven documentation ecosystem when supported by strong regulatory and ethical frameworks.

Abstract

Optimal cancer management requires a multidisciplinary approach to improve the quality of patient care. Multidisciplinary teams (MDTs) enable interprofessional collaboration in designing comprehensive, patient-centered care. However, the implementation of MDTs in Indonesia still faces various challenges, necessitating a systematic review to understand their effectiveness in improving cancer management outcomes. This systematic review employed the PRISMA method, searching articles from the ScienceDirect, Semantic Scholar, and PubMed databases, published between 2014 and 2024. Ten scientific articles were analyzed from 745 articles retrieved to understand the effectiveness of MDTs in improving cancer management outcomes. The results indicate that MDTs improve the quality of clinical decisions, expedite diagnosis, and increase adherence to treatment protocols. MDTs also reduce postoperative complications and improve patients' quality of life through a more holistic approach. However, limitations such as resource constraints, decision fatigue, and communication challenges can hinder their effectiveness. MDTs improve the quality of clinical decisions, expedite diagnosis, and optimize patient care plans by holistically considering medical, psychosocial, and nutritional needs. Therefore, the development and implementation of more effective MDTs are crucial to ensure equitable access and quality of cancer care.

Abstract

Strengthening primary care services in Indonesia requires that community health centers, namely Pusat Kesehatan Masyarakat (Puskesmas), involve medical personnel with family medicine competence. However, the fulfillment of physicians with this competence, especially Primary Care Family Medicine Specialists or Spesialis Kedokteran Keluarga Layanan Primer (Sp.KKLP), still face significant obstacles. This qualitative research uses the integrated Easton's System Model and Buse's Policy Framework to analyze the dynamics of this policy. The findings reveal a gap between the strong legal foundation and weak implementation mechanisms on the ground. First, there is ambiguity in terminology and lack of harmonization among regulations, especially concerning the Sp.KKLP nomenclature, which creates confusion in terms of legal status and professional recognition. Second, an implementation gap (policy–practice gap) exists because the central government has not yet issued technical derivative regulations (technical guidelines, compensation mechanisms). Furthermore, the decentralization of authority and limited regional budgets impede the equitable placement of the Sp.KKLP. The success of the policy relies heavily on the synergy among the Ministry of Health, Health Council, Family Medicine Collegium, educational institutions such as Program Pendidikan Dokter Spesialis (PPDS) and Recognition of Prior Learning (RPL), and professional organizations such as Pengurus Besar Ikatan Dokter Indonesia (PB IDI). Recommendations include the development of technical implementation regulations, clarification of the Sp.KKLP definition, and establishment of inclusive pathways for competence enhancement such as PPDS and RPL.

Abstract

RSUD Anugerah Sehat Afiat (ASA) Depok faces challenges in enhancing service utilization and competitiveness as a public hospital in the increasingly competitive healthcare environment of eastern Depok City. This study aimed to identify factors influencing community preferences in hospital selection and to determine appropriate positioning strategies for RSUD ASA. A mixed-methods case study design was employed. Quantitative data were collected from 132 respondents aged over 25 years who had visited four hospitals included in the study, using structured questionnaires. Qualitative data were obtained through in-depth interviews with eight key informants representing RSUD ASA management. Data analysis was based on five dimensions: availability, affordability, accessibility, accommodation, and acceptability. The results showed that acceptability was the most dominant factor influencing hospital preference (p-value < 0.001), followed by accessibility (p-value = 0.007) and availability (p-value = 0.026). In contrast, affordability and accommodation were not significantly associated with community preference. Qualitative findings supported these results, indicating that patient trust, hospital reputation, service readiness, and ease of access were perceived as the main determinants of hospital choice. These findings suggest that community preference is driven primarily by perceived service value rather than cost or physical facilities. Therefore, positioning RSUD ASA as the community’s primary hospital should focus on strengthening service quality, trust, accessibility, and hospital image through a value-based strategic approach.
