Abstract

Telekonsultasi telah berkembang di Indonesia terlebih di masa pandemi Coronavirus Disease -19 (COVID-19). Teknologi ini merupakan layanan konsultasi kesehatan jarak jauh yang merupakan bagian dari telemedisin. Indonesia telah mengatur pelaksanaan telemedisin melalui Permenkes Nomor 20 tahun 2019 mengenai Penyelenggaraan Telemedisin. Meskipun demikian,  regulasi tersebut belum spesifik mengatur tentang telekonsultasi, terutama mengenai startup telekonsultasi yang saat ini banyak bermunculan. Penelitian ini bertujuan untuk mengetahui perbandingan regulasi mengenai telekonsultasi yang ada di Indonesia dengan negara lain yaitu Singapura. Penelitian ini merupakan penelitian normatif dengan pendekatan perundang-undangan (statue approach) dan pendekatan komparatif (comparative approach). Bahan hukum primer diperoleh dari peraturan perundang-undangan maupun yurisprudensi. Sedangkan bahan hukum sekunder diperoleh dari jurnal, buku, maupun prosiding mengenai regulasi terkait konsultasi. Berdasarkan penelitian, telekonsultasi yang dilakukan di Indonesia hanya diperbolehkan jika terintegrasi dengan fasilitas pelayanan kesehatan tertentu. Selain itu, regulasi mengenai monitoring, pertanggung jawaban dan keamanan data masih belum detail dan spesifik. Sanksi bagi startup yang menyelenggarakan layanan konsultasi kesehatan mandiri pun belum jelas dan tegas. Singapura memiliki regulasi telekonsultasi yang tercantum dalam regulatory sandbox sehingga dapat menjembatani layanan konsultasi kesehatan online yang diselenggarakan oleh startup yang ada dengan syarat tertentu. Regulasi mengenai telekonsultasi di Indonesia perlu disusun lebih detail, spesifik, dan jelas sehingga masyarakat dapat memperoleh layanan konsultasi kesehatan yang aman dan dapat dipertanggungjawabkan.

Abstract

Background: Performance-based capitation payments are an innovative approach in the health financing system implemented by the health social security administering body. Purpose: To analyze the impact and effectiveness of performance-based capitation payments in improving the quality of health services and resource management in the national health insurance system. Method: The literature review was conducted systematically through databases such as Google Scholar, PubMed Central, Researchgate, and other sources. The keywords used in the literature search were capitation, based, performance, commitment, service, and capitation-based, performance, and commitment. Results: Performance-based capitation payments have varying impacts on the quality of social security agency for health patient services at primary health facilities (PHF). Conclusion: Achievement of performance-based capitation indicators is influenced by several factors, including the availability of human resources (medical and non-medical personnel), infrastructure, information systems, governance and organization, and financing. These factors align with the primary health care capacity domain components in the WHO primary health care monitoring framework.   Keywords: Performance-Based Capitation; Quality of Health Services; Social Health Security Administration.   Pendahuluan: Pembayaran kapitasi berbasis kinerja merupakan suatu pendekatan inovatif dalam sistem pembiayaan kesehatan yang diimplementasikan oleh Badan Penyelenggara Jaminan Sosial Kesehatan (BPJS Kesehatan). Tujuan: Untuk menganalisis dampak dan efektivitas pembayaran kapitasi berbasis kinerja dalam meningkatkan kualitas pelayanan kesehatan dan pengelolaan sumber daya di dalam sistem jaminan kesehatan nasional. Metode: Penelitian literature review dilakukan secara sistematis melalui database seperti Google Scholar, PubMed Central, Researchgate, dan sumber lainnya. Kata kunci yang digunakan dalam penelusuran literatur adalah kapitasi, berbasis, kinerja, komitmen, pelayanan dan kapitasi berbasis, kinerja, dan komitmen. Hasil: Pembayaran kapitasi berbasis kinerja (P4K) memberikan pengaruh yang bervariasi terhadap kualitas pelayanan pasien BPJS kesehatan di fasilitas kesehatan tingkat pertama (FKTP). Simpulan: Tercapainya indikator KBK dipengaruhi oleh beberapa factor, meliputi ketersediaan sumber daya manusia (tenaga medis dan non medis), ketersediaan sarana prasarana, ketersediaan sistem informasi, tata kelola dan organisasi, serta pembiayaan. Faktor-faktor ini selaras dengan komponen domain kapasitas FKTP dalam kerangka pemantauan layanan primer WHO.   Kata Kunci: BPJS Kesehatan; Kapitasi Berbasis Kinerja; Kualitas Pelayanan Kesehatan.

Abstract

Purpose: This study aims to analyze the implementation of tuberculosis preventive therapy (TPT) in Majalengka District using the Van Meter and Van Horn policy implementation model, focusing on inter-organizational communication, resources, implementer disposition, implementing agency characteristics, and economic-socio-political conditions. Methods: This qualitative study employed a case study approach involving six selected community health centers with the highest and lowest TPT coverage. Data were collected through in-depth interviews with six tuberculosis (TB) program managers, five triangulation informants, direct observations, and document reviews. Data were analyzed thematically, involving data reduction, presentation, and inductive conclusion drawing. Results: The study found that communication inconsistencies across national, provincial, and district levels led to information gaps at the community health center level. Implementation was hindered by limited human resources, diagnostic tools, and logistics, as well as the absence of clear internal regulations. However, proactive roles by TB program managers and cadres, supported by community-based outreach strategies, helped mitigate barriers. Social stigma, indirect costs, and low intersectoral coordination remained significant challenges. Conclusion: TPT implementation faced barriers such as inconsistent communication, limited resources, and unclear regulations. Effective TPT implementation requires coordination by the Ministry of Health, improved diagnostics by health facilities, empowerment of health workers and cadres, and strong support from local governments.

Abstract

Background: Standard precautions practice is the main action in preventing and controlling infections in hospitals. Nurse compliance with standard precautions is influenced by various factors, including knowledge, perception, and motivation. Purpose: To analyze the relationship between the level of knowledge, perception, and motivation of nurses with the practice of standard precautions. Method: A quantitative approach, specially focusing on analytical survey and cross-sectional method. The study took place from December 2024-Febuary 2025 in Islamic Hospital of Muhammadiyah Medical Care Center, Pekajangan. The independent variable was knowledge, risk perception, and motivation of nurses, dependent variable is standard precautionary practices in hospitals. Using systematic random sampling and slovin formula, 173 respondents were selected, who met specific inclusion and exclusion criteria. The analysis proceeded with a univariate test to determine the frequency distribution of respondent’s characteristics, followed by bivariate analysis using spearman rank test Results: The study revealed significant factors influencing the practice of standard precautions. It showed a gender-based difference in adherence (p = 0.008), a positive relationship between the level of perception and practice (p = 0.033), and a significant association with motivation level (p = 0.01). Conclusion: Nurses generally had good to fair levels of knowledge, perception and motivation regarding standard precautions, indicating effective implementation of patient safety. Perception and motivation significantly influenced compliance, while knowledge without significant influence.

Abstract

Akumulasi kasus HIV/AIDS di Provinsi Jawa Tengah, tahun 1993 _ 2008 adalah pada usia 20 _ 24 tahun sekitar 12,54% dan usia 25 _ 29 tahun sekitar 37,31% merupakan kategori remaja dan dewasa muda. Di kabupaten Magelang kasus HIV positif pada remaja mulai muncul tahun 2008 dan selalu muncul pada tahun berikutnya. Remaja sangat membutuhkan informasi tentang seksualitas dan peran ibu sangat penting. Tujuan penelitian ini adalah untuk mengetahui determinan perilaku ibu yang meliputi umur, pekerjaan, pendidikan, pengetahuan kesehatan reproduksi remaja, persepsi kemampuan diri dan sikap dalam pendidikan seksualitas. Jenis penelitian adalah survei dengan pendekatan potong lintang. Populasi terjangkau adalah ibu yang mempunyai anak remaja berusia 10 _ 14 tahun dan mengikuti program Bina Keluarga Remaja percontohan di Kabupaten Magelang. Pemilihan sampel menggunakan klaster sampling dan berjumlah 92 orang. Pengumpulan data menggunakan kuesioner. Analisis bivariat menggunakan uji kai kuadrat dan analisis mulitivariat menggunakan regresi logistik. Hasil penelitian menunjukkan mayoritas ibu belum memberikan pendidikan seksualitas dengan baik. Variabel yang berhubungan adalah pendidikan ibu, pengetahuan ibu tentang kesehatan reproduksi remaja, persepsi kemampuan diri ibu dan sikap ibu. Persepsi kemampuan diri ibu merupakan variabel yang paling berpengaruh terhadap perilaku ibu dalam memberikan pendidikan seksualitas. The selection of samples using cluster sampling and numbered 92 people. Collecting data using a questionnaire. Bivariate analysis used chi squared test and multivariate used logistic regression. The results showed the majority of mother have not provided good sexuality education. There were relations between mother’s education level, mother’s level of knowledge about adolescent reproductive health, mother’s perception and mother’s attitudes with mother’s behavior in providing sexuality education. Mother’s perception is the most influential variable on mother’s behavior in providing sexuality education.

Abstract

Purpose: Electronic medical records (EMR) are information technology-based systems that store and manage patient medical data. EMR are beneficial for improving healthcare services' efficiency, effectiveness, and security. The COVID-19 pandemic has driven the adoption of EMR, but there are still challenges, such as the surge in patient numbers and problems with EMR filling. This study aimed to investigate the use of EMR in hospitals during the COVID-19 pandemic in Indonesia. Method: This study was a systematic literature review using the PRISMA diagram. Article search was conducted based on the PICO model eligibility criteria. The articles used were retrieved from 3 databases: Google Scholar, ResearchGate, and Cerdika. The researcher limited the publication year of the journals to 2020-2021, when the COVID-19 pandemic was ongoing, and added the keyword "COVID-19" to the journals published in 2022-2023. Results: The results of the study showed that the COVID-19 pandemic has had a significant impact and the need for the implementation of EMR due to its various benefits, such as improving the efficiency and effectiveness of healthcare services, facilitating access to patient health information, increasing collaboration among healthcare workers, accelerating the process of clinical decision-making, and improving the security and accuracy of health data. However, implementing EMR during the COVID-19 pandemic has only sometimes been smooth. Some challenges include suboptimal infrastructure, untrained human resources, and inadequate policies. Conclusion: To overcome these challenges, efforts from various parties, including the government, hospitals, and healthcare workers, are needed.

Abstract

Purpose: This study aims to analyze the roles and positions of stakeholders in health-sector minimum service standards policy implementation (HS-MSSPI) in Wonosobo based on stakeholders' interests, authority, attitudes, influence, and involvement. Methods: This research is a descriptive qualitative study. The stages in this research are stakeholder identification, data collection through in-depth interviews, data analysis based on the stakeholder classifi- cation approach and stakeholder mapping theory. Results: The results of stakeholder classification show that decision-makers consist of the Regency Legislative Council, Regency Secretariat, Agency for Regional Development, and Health Office. Providers comprise the Health Office, Public Health Center, and Health Cadres, while Representatives comprise the Sub-District Govern- ment, Village Government, and Family Welfare Guidance (PKK) Cadres. Stakeholder mapping results show that there are two types of positions in the decision-maker category: savior and time bomb. In the provider category, there are three positions - savior, friend, and sleeping giant. At the same time, all representatives are in the friend position. Conclusion: It appears that stakeholders involved in the implementation of the HS-MSSPI in Wonosobo have not utilized their full authority to provide support and involvement. Perhaps there is room for improvement in terms of stakeholder engagement and collaboration to ensure the program's success.

Abstract

Purpose: Postpartum depression (PPD) affects about 17.22% of women globally, with higher rates in regions like Southern Africa and Southeast Asia, and is associated with negative impacts on breastfeeding, self-care, infant care, and maternal mental health. Support from family members plays a crucial role in protecting mothers from PPD by providing emotional and practical assistance. However, in some family arrangements, while in-laws can be supportive, conflicts or unmet expectations may also increase the risk of emotional distress and PPD. This study seeks to identify existing literature on the roles of family support in postpartum care. Methods: A scoping review was conducted using PubMed and ScienceDirect as databases, applying search terms such as “Mothers”, “Support”, and “Postpartum Depression”. Results: A total of 11 published articles between 2020 and 2025 were selected after meeting the inclusion criteria. This review emphasizes the essential role of familial relationships in shaping postpartum mental health within traditional household settings. Supportive behaviors from husbands consistently emerged as protective factors against PPD by reducing psychological stress, which can contribute to depressive symptoms. Likewise, the role of in-laws can either alleviate or intensify maternal stress. Conclusion: Husbands are encouraged to engage in open and empathetic communication with their partners, providing consistent emotional support and reassurance. Furthermore, husbands can serve a pivotal role in managing dynamics within the extended family by establishing appropriate boundaries and fostering a supportive and respectful environment that prioritizes the mother’s psychological well-being.

Abstract

The percentage of pregnant women with Chronic Energy Deficiency (CED) in Indonesia is still quite high. This will give a negative impact on the mother’s and baby’s health. Indonesian government overcomes the problem of CED pregnant women through the provision of supplementary food. The identified obstacles in implementing supplementary food program for pregnant women can be used as consideration in planning, implementation, monitoring and reporting at the Community Health Center. The method used in this study is scoping review. Article identification uses two databases, namely Google Scholar and Garuda. The article searched in the database was 919 articles. After screening with inclusion and exclusion criteria, there were 7 eligible articles. In the input aspect, deficiencies were found in the quantity and quality of health human resources, funds and facilities. In the process aspect, the distribution of PMT from the center was uncertain and there was no monitoring of the use of PMT by the target. Some CED pregnant women were also less compliant in taking PMT and consuming PMT. In the output aspect, the system of program’s monitoring and evaluation at the Community Health Center was not carried out well.

Abstract

Tujuan utama dari penelitian ini adalah untuk mengidentifikasi dan menganalisis hambatan-hambatan yang ada dalam pelaksanaan Posyandu Remaja di Indonesia. Penelitian ini menggunakan tinjauan literatur sistematis (SLR) untuk mensintesis temuan penelitian mengenai hambatan-hambatan pada Posyandu Remaja. Tinjauan ini mengikuti proses lima langkah yang terstruktur, dimulai dengan merumuskan pertanyaan penelitian dan fokus pada studi-studi relevan dari tahun 2019 hingga 2023. Proses ini melibatkan identifikasi, pemilihan, dan evaluasi artikel menggunakan basis data Google dan Google Scholar, serta perangkat lunak Publish or Perish untuk strategi pencarian. Kriteria utama mencakup publikasi dalam bahasa Indonesia, aksesibilitas teks lengkap, dan relevansi dengan tema hambatan pada Posyandu Remaja. Seleksi akhir mencakup 10 artikel yang memenuhi kriteria ini, memastikan analisis yang komprehensif tentang topik tersebut. Hasil penelitian menunjukkan bahwa tinjauan literatur terbaru mengenai program Posyandu Remaja mengungkapkan beberapa tantangan signifikan yang menghambat efektivitasnya. Masalah utama meliputi keterbatasan sumber daya, kurangnya keterlibatan pemangku kepentingan, dan dukungan serta infrastruktur yang tidak memadai. Dampak anemia pada remaja, terutama selama pandemi Covid-19, menyoroti perlunya intervensi kesehatan yang terarah. Untuk meningkatkan program Posyandu Remaja, pendekatan komprehensif diperlukan, dengan fokus pada peningkatan alokasi sumber daya, peningkatan keterlibatan pemangku kepentingan, dan penanganan masalah kesehatan spesifik seperti anemia. Penelitian dan evaluasi program yang berkelanjutan sangat penting untuk mengatasi hambatan-hambatan ini dan memastikan keberhasilan inisiatif kesehatan ini.
