Abstract

AbstrakBerdasarkan data jumlah kasus HIV/ AIDS pada ibu hamil dari Dinas Kesehatan Kabupaten Demak pada tahun 2020 yaitu dari 21.980 ibu hamil yang dilakukan pemeriksaan sebanyak 17.045 (77%). Didapatkan hasil bahwa Jumlah kasus HIV/ AIDS pada ibu hamil di Kabupaten Demak tahun 2020 adalah sebanyak 13 orang (16,3%). Sejak masa pandemi COVID-19 tahun 2019 kasus HIVAIDS pada ibu hamil sebanyak 5 orang (9,8%) dan tahun 2020 lebih banyak menjadi 13 orang (16,3%) yang tersebar di 9 Puskesmas. Sedangkan pada tahun 2021 dalam kurun waktu 5 bulan pertama ditemukan kasus baru HIV pada ibu hamil sebanyak 5 orang yang tersebar di 4 puskesmas. Berdasarkan data penularan HIV pada ibu kepada bayi diperoleh bahwa terdapat penularan HIV pada ibu kepada bayi pada tahun 2020 terdapat 2 bayi. program PMTCT di Kabupaten Demak dilakukan melalui 4 prong. Selama masa pandemi Covid-19 ini, dalam kurun waktu tahun 2019 hingga 2021 kurang maksimal, sehingga diperlukan penguatan program baik dari segi health promotion, konteks, input, proses, dan output. Tujuan penelitian ini menjelaskan untuk mengevaluasi Pelaksanaan Pelayanan Kegiatan PMTCT Pada Ibu Hamil dengan HIV di wilayah kerja Dinas Kesehatan Kabupaten Demak. Metode dalam penelitian ini adalah studi kualitatif dengan rancangan studi kasus. Subjek dalam penelitian ini merupakan petugas yang terlibat dalam pelayanan PMTCT pada ibu hamil dengan HIV dan sudah mendapatkan pelatihan PDP meliputi 6 Puskesmas yang terdapat kasus ibu hamil HIV positif dalam kurun waktu 2020 hingga 2021. Pengumpulan data menggunakan wawancara mendalam pada informan utama pemegang program HIV dari 6 puskesmas, dan informan triangulasi Kepala puskesmas, Pemegang program HIV Dinas Kesehatan Kabupaten Demak, pemegang program Kesehatan Ibu Dinas Kesehatan, Bidan Koordinator Rumah Sakit, dan ibu hamil HIV. Hasil penelitian yang dilakukan menunjukkan sebagian besar pelayanan PMTCT masih berjalan meskipun ditemukan beberapa kendala, hal ini dapat dilihat berdasarkan variabel health promotion, media promosi yang kurang menarik dan belum maksimalnya kunjungan rumah oleh petugas di masa pandemi dalam melakukan promosi kesehatan sehingga ibu hamil kurang mengerti. Pada variabel konteks masih adanya stigma negative terhadap ibu hamil HIV. Pada variabel input yang terdiri dari sumber daya manusia masih ditemukannya petugas yang berpindah tempat kerja sehingga petugas yang baru belum mendapatkan pelatihan, pada aspek money berasal dari dana BOK yang penganggarannya tergabung dengan program TB dan penyerapannya belum maksimal, pada aspek sarana dan prasarana mengenai ketersediaan reagen mengalami keterlambatan dalam pendistribusiannya, pada aspek method masih ditemukan pada beberapa puskesmas yang belum memiliki SOP. Pada aspek pencatatan dan pelaporan sering terjadi keterlambatan dalam mengirimkan laporan selama masa pandemi Covid – 19. Pada variabel produk masih ditemukan adanya penularan HIV dari ibu ke anak sebanyak 1 orang. Dengan demikian, perlu adanya perencanaan pelayanan PMTCT baik dari tingkat puskesmas maupun Dinas Kesehatan kabupaten Demak sehingga tidak ditemukan adanya penularan HIV dari ibu ke anak.Kata Kunci: Evaluasi, PMTCT, ibu hamil, Pandemi COVID – 19 AbstractBased on data on the number of HIV/AIDS cases in pregnant women from the Demak District Health Office in 2020, from 21,980 pregnant women, 17,045 (77%). PMTCT program in Demak Regency is carried out through 4 prongs. During the Covid-19 pandemic, the period from 2019 to 2021 was not optimal, so program strengthening is needed both in terms of health promotion, context, input, process, and output. The purpose of this study is to evaluate the implementation of PMTCT activities for pregnant women with HIV in the working area of the Demak District Health Office. The method in this research is a qualitative study with a case study design. The subjects in this study were officers who were involved in PMTCT services for pregnant women with HIV and had received PDP training covering 6 public health centers with cases of HIV positive pregnant women from 2020 to 2021. Data collection used in-depth interviews with key informants holding HIV programs from 6 public health centers, and triangulation informants from the Head of public health centers, HIV program holders from the Demak District Health Office, holders of the Maternal Health program at the Health Office, Hospital Coordinator Midwives, and HIV pregnant women. The results of the research conducted showed that most of the PMTCT services were still running even though several obstacles were found, this could be seen based on the health promotion variable, promotional media that were less attractive and home visits by officers during the pandemic were not maximal in conducting health promotion so that pregnant women did not understand. In the context variable, there is still a negative stigma against HIV pregnant women. In the input variable consisting of human resources, there are still officers who change places of work so that new officers have not received training, in the money aspect it comes from BOK funds whose budgeting is combined with the TB program and the absorption has not been maximized, in the aspect of facilities and infrastructure regarding the availability of reagents experienced delays in its distribution, in the method aspect it was still found in several health centers that did not yet have SOPs. In the aspect of recording and reporting, there are often delays in submitting reports during the Covid-19 pandemic. In the product variable, it is still found that there is 1 person transmitting HIV from mother to child. Thus, there is a need for planning for PMTCT services from both the public health centers level and the Demak district health office so that there is no mother-to-child transmission of HIV. Keywords: Evaluation, PMTCT, pregnant women, Covid-19 pandemic

Abstract

Latar Belakang: Anak usia di bawah lima tahun memerlukan gizi optimal, dan praktik pemberian MP-ASI dipengaruhi faktor sosial ekonomi. Penelitian ini bertujuan menganalisis hubungan faktor sosial ekonomi dengan praktik pemberian MP-ASI melalui systematic review.Metode: Penelitian ini merupakan systematic review berdasarkan pedoman PRISMA. Pencarian artikel dilakukan melalui Scopus, ScienceDirect, ProQuest, dan PubMed menggunakan kata kunci “socioeconomic factors” dan “complementary feeding”. Dari 1.912 artikel, 13 memenuhi kriteria inklusi (2020–2024) dan dianalisis secara tematik.Hasil: Pendidikan ibu yang lebih tinggi berhubungan dengan praktik Pemberian Makanan Pendamping ASI (MP-ASI) yang lebih baik. Pekerjaan ibu dapat meningkatkan kapasitas finansial, namun keterbatasan waktu menjadi kendala. Pendapatan rendah membatasi akses terhadap Pemberian Makanan Pendamping ASI (MP-ASI) berkualitas. Faktor sosial dan budaya, termasuk keterlibatan ayah, berpengaruh terhadap variasi dan frekuensi pemberian makan.Kesimpulan: Faktor sosial ekonomi berperan signifikan terhadap praktik pemberian Makanan Pendamping ASI (MP-ASI). Intervensi gizi anak perlu menggabungkan edukasi, dukungan ekonomi, dan pendekatan berbasis budaya untuk meningkatkan status gizi balita.

Abstract

Length of Stay (LOS) menjadi satu jenis faktor sehingga mengakibatkan tumpuan harga rawat inap (ranap) dan sebagai alat ukur mutu pelayanan rumah sakit. Lama rawat inap dipengaruhi oleh berbagai faktor, tujuan dari penelitian ini adalah untuk mengetahui prediktor lama rawat inap pasien stroke. Metode: Penelitian ini dilakukan dengan metode scoping review dengan menggunakan database elsivier scopus, google scholar, academia.edu dan pub med. Langkah – langkah penelitian ini terdiri dari: 1) Identifikasi pertanyaan, 2) Identifikasi artikel, 3) Penyaringan artikel, 4) Pemetaan data, 5) Penyusunan hasil, dan 6) Konsultasi. 881 artikel dipilih berdasarkan judul, abstrak, isi dan hasil sesuai dengan kriteria inklusi dan tujuan studi literatur ini. Hasil skrining diterjemahkan ke dalam literatur review chart sehingga diperoleh 17 artikel dan kemudian dilakukan analisis. Kriteria inklusi yang ditetapkan adalah : artikel teks lengkap, fokus pembahasan tentang prediktor lama rawat inap pasien stroke, serta populasi dan sampel pasien stroke. Hasil: Scoping review ini menunjukkan prediktor lama rawat inap pasien stroke, yaitu karakteristik responden, komorbiditas, jenis stroke, penatalaksanaan, komplikasi, gejala stroke, riwayat merokok, ekonomi tempat tinggal, dan waktu rujukan dapat meningkatkan lama rawat inap pasien stroke. Meskipun riwayat merokok dalam scoping review mempunyai hasil yang berbanding terbalik yaitu dapat menurunkan lama rawat inap pasien stroke. Kesimpulan: Lama rawat inap pasien stroke bisa meningkat atau menurun dipengaruhi oleh prediktor lama rawat inap pasien stroke, sehingga penelitian lebih lanjut diperlukan untuk meneliti secara menyeluruh terkait prediktor lama rawat inap pasien stroke. Abstract

HIV/AIDS remains a global health problem with an increasing number of cases, including in Indonesia. One major challenge in HIV management is Loss to Follow-Up (LtFU), defined as patients missing clinical visits for more than 90 days, which threatens antiretroviral (ARV) treatment success by increasing the risk of drug resistance, treatment failure, morbidity, and mortality. This scoping review aimed to identify determinants associated with LtFU among people living with HIV/AIDS. Literature searches were conducted in national and international databases for studies published between 2021-2025, yielding 38 eligible studies. Result of thematic analysis finding indicate that LTFU is influenced by multidimensional factors, including demographic (young age, male, unmarried/divorced), socio-economic and geographic (rural residence, long distance to healthcare facilities, low education, loss of income), clinical and biomedical (low CD4 count, high viral load, advanced HIV stage, malnutrition, TB coinfection), treatment and adherence (low ART adherence, no regimen change, early ART initiation, absence of prophylactic therapy), and psychosocial and health system factors (stigma, non-disclosure, perceived wellness, side effects, limited provider support, weak tracing systems). In conclusion, addressing LTFU requires multidimensional interventions integrating clinical, educational, social, and system-level strategies to enhance patient retention in HIV care.

Abstract

Delays in submitting reimbursement claims for chronic drug result in deferred payments, adversely affecting hospital cash flow. This financial strain leads to delays in employee compensation and challenges in ensuring adequate chronic drug availability for patients. This study aims to analyze how chronic drug claim processing delays impact the quality of healthcare services delivered by hospitals. This study utilizes a systematic literature review methodology, searching the PubMed, Scopus, and Google Scholar databases for publications from 2015-2025. The search strategy employs the following keywords: "chronic drug claims delay," "healthcare service quality," "chronic medication reimbursement," "hospital cash flow," and "pharmaceutical claims processing." The literature selection process follows the PRISMA guidelines. Based on the analysis of six selected articles, this study identifies the multidimensional impact of delayed chronic drug claims on hospital healthcare service quality across four key dimensions: Competency Dimension:delayed claims create hospital revenue instability, which adversely affects funding for human resource development and staff competency training programs. Service Effectiveness Dimension: claim processing delays necessitate changes in hospital budget allocation and strategic planning policies, resulting in suboptimal service delivery. Safety Dimension:delayed reimbursements lead to medication shortages, causing delays in providing essential chronic medications to patients and potentially compromising patient safety. Service Comfort Dimension:claims processing delays negatively impact pharmaceutical inventory management, affecting medication availability and overall patient comfort during treatment. Conclusion: Based on the analysis of six research articles published between 2015-2025, delayed chronic drug claims significantly impact hospital healthcare service quality across multiple dimensions, including competency, service effectiveness, safety, and service comfort.

Abstract

Background: Smoking is a leading cause of non-communicable diseases and remains a significant public health challenge, particularly among children and adolescents in Asia, where prevalence rates are high. This behavior is influenced by a complex interplay of individual traits, interpersonal relationships, and broader social-environmental factors. Understanding these influences is crucial for developing effective interventions to reduce smoking among youth. Objectives: This study aimed to identify and map the key factors influencing smoking behavior among Asian children and adolescents, providing insights into potential strategies for targeted interventions.  Methods: A scoping review was conducted through systematic searches of the Scopus and ScienceDirect databases using keywords related to smoking among Asian youth. From an initial pool of 312 articles, four met the inclusion criteria for in-depth analysis. Thematic analysis categorized influencing factors into individual, interpersonal, and socio-environmental domains. Results: The review identified three main categories of factors. Individual factors included age, gender, and curiosity, which often trigger initial smoking. Interpersonal influences such as peer pressure and family members who smoke played a significant role. Social and environmental factors, including exposure to tobacco advertising, easy access to cigarettes, and weak regulatory frameworks, further increased smoking risks. These factors frequently interacted, compounding their impact on youth smoking behaviors. Conclusion: Addressing smoking among Asian youth requires comprehensive strategies that encompass education, regulation, and family involvement. School-based programs, social media campaigns, stricter laws, and family-focused interventions are essential to reduce smoking prevalence and foster healthier generations.

Abstract

People Living with HIV/AIDS in Demak, Indonesia were still facing social stigma. This research aims to analyze the influence of film and focus group discussions on stigma. A quasi-experimental study with a non-equivalent control group pre-test and post-test design was applied. This research was conducted in two villages, Ngemplik Wetan as the intervention group and Tembok as the control group. The film with focus group discussion was given four times for one month, only to the intervention group. To measure stigma, the Wilcoxon test was used to compare between pre-test and post-test in a group meanwhile the Mann-Whitney test was used to compare between groups. The sample was determined by purposive sampling and used the comparison formula of two sample proportions. The sample of each group is 45 respondents. The results showed that the intervention group have a significant difference in stigma (p is 0.000; Z is -4.716; CI is 95 percent) and the average stigma decreased by 3.65. Whereas the control group did not have a significant difference in stigma (p is 0.200; Z is -0.924; CI is 95 percent) and the average stigma decreased by 0.78. Film and focus group discussions can be used as a health promotion strategy to reduce the social stigma toward ODHA.

Abstract

Minimum Service Standard (MSS) in health sector is a mandatory that must be implemented by every regional government in Indonesia, so the MSS target must be 100% annually. However, the report shows that the MSS in health sector has not reached the target. National achievements in 2021 for trimesters I, II, III and IV respectively is 2.38%, 7.55%, 9.95% and 52.85%. In 2022, the achievements increased and the percentages for each trimester are 26.53%, 31.05%, 39.25% and 77.49%, but still not meet the target. This condition shows the unoptimized implementation of Minimum Service Standard (MSS) in health sector that could affected to health service delivery and society welfare. This study aims to identify the implementation and obstacles of Minimum Service Standard (MSS) in health sector at district level. This study is a systematic literature review. 9 literatures on Google Scholar database and Garuda Portal selected according PRISMA guidelines (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). The result shows some obstruction in the MSS in health sector’s implementation including human resources quantity and quality, inadequate infrastructure, lack of cross-sectoral and community support, and unoptimal planning and budgeting system. Based on these results it can be concluded that regional government should focused on recource factors including human recources and infrastructure, organizational factor to optimilize internal and external commitment and support including community support and budgeting support.

Abstract

This research aims to analyze the implementation of the Youth Posyandu/Integrated Health Post program in Pekalongan city. The research design used qualitative with in-depth interview techniques and data triangulation. The informants consisted of relevant officials, Posyandu cadres, and stakeholders. The results indicate that strategies for increasing youth participation include intensifying education, involving community leaders, creating a youth-friendly atmosphere, and strengthening the role of cadres. The barriers to participation can be resolved through a creative and collaborative approach. Adolescents' health awareness is increased through understanding their needs. Health information was delivered using an attractive digital platform according to teenagers' interests. Overcoming access barriers required stigma-free educational campaigns, supportive environments, and active youth involvement. The positive role of parents and the social environment creates norms that support adolescent participation. A youth-friendly environment was created through participatory design, adequate facilities, and organizing activities by teenagers. Moreover, evaluation of program effectiveness involved measurable performance indicators, comprehensive data analysis, and ongoing feedback to optimize impact on youth knowledge, behavior, and well-being.

Abstract

To prevent and control the COVID-19 pandemic, the government establishes a policy of new normal adaptation by implementing health protocols in daily life. However, the community should be active in implementing new normal adaptation. Forum Kesehatan Kelurahan (FKK) as a forum for community participation in the health sector has an important role in the success of supporting health programs at the village level. Based on research studies, the application of new normal adaptation in Tembalang District by the community hasn’t been optimal. Therefore, the role of FKK in Tembalang District has to adapt new normal from the family. The ability and knowledge of FKK cadres regarding adaptation of new normal must be good in order to have sufficient capacity to support new normal families in Tembalang District. In this case, the capacity building of the FKK carried out through activities to 38 FKK cadres representing each sub-district in Tembalang District. Counseling done by doing pre-test, presenting materials, discussions with participants, and ended by post-test. The results of discussion, the community had made efforts to support the implementation of the new normal, there are education, field operations, reporting of COVID-19 cases, and jogo tonggo. Challenges for FKK are community’s lack awareness of COVID-19 pandemic and health protocols impportance. Post-test result showed increasing number of cadre with good knowledge about New Normal. In this activity, participants were also given a booklet. The capacity of FKK cadres to mobilize community will be stronger that hopefully can contribute more efforts to support the adaptation of new normal in the family and be able to solve challenges well.
