Abstract

Good control of the supply of drugs and consumables (BMHP) by Hospital Pharmaceutical Installations (IFRS) can avoid shortages or overstocks in pharmaceutical warehouses. The Type C "X" Special Hospital in Semarang City experienced a stockout of IVF drugs and embryo laboratory BMHP which resulted in delays in medical procedures and a decrease in the quality of service. The purpose of this study is to determine the factors that affect the efficiency and effectiveness of pharmaceutical preparations using the MMSL method. This study is a quasi-experimental non-equivalent without control group, samples were taken purposively using retrospective data from October – December 2023 with a stable supply and the research was applied prospectively from January – March 2024. Twelve types of IVF drugs and 56 types of BMHP embryo laboratories were used as research samples. The statistical test used cross-tabulation between efficiency variables (inventory value, inventory turnover ratio [ITOR], stockout value) and effectiveness (number of stockout events). The test results showed that the lower the price of IVF drugs and BMHP in embryo laboratories led to lower inventory values. The more expensive the BMHP price of embryo laboratories results in higher ITOR. The faster the lead time supplier BMHP embryo laboratory, the smaller the stockout will occur. The larger the Smin, Smax, and BMHP safety stock of the embryo laboratory, the lower the incidence of stockout. This study provides an insight into the factors that support the application of MMSL to inventory control in IFRS "X".

Abstract

This study examines the implementation and challenges of performance-based capitation (KBK) at Community Health Centers (Puskesmas) in Indonesia, as a part of the National Health Insurance Program, aiming to improve the efficiency and quality of primary health services. Despite its potential, the implementation of KBK faces various challenges including inadequate human resources, limited infrastructure, and insufficient monitoring systems. The study uses a scoping review framework based on the Public Health Care Monitoring Conceptual Framework by WHO, analyzing 11 selected studies from 2021 to 2024. Findings indicate that internal factors such as management capacity, health worker competence, and organizational culture significantly impact the effectiveness of KBK. External factors like local government support, funding policies, and infrastructure availability also play crucial roles. Technological barriers, particularly in remote areas, hinder optimal policy implementation. The study emphasizes the need for clear regulations, consistent policy dissemination, and effective monitoring to ensure successful KBK implementation. Recommendations include strengthening leadership, training health workers, improving infrastructure, and enhancing digital health technology utilization. These strategies aim to optimize resource allocation, enhance service access, and improve patient satisfaction in Indonesia’s primary health care system.

Abstract

In terms of their impact on the workforce, mental disorders in the workplace are a major concern. Poor mental health and stress can have a negative impact on worker performance and productivity, job attachment, collegial communication, physical performance, and daily functioning. The objective of this study is to investigate the relationship between mental health and job performance in medical and non-medical workers at general region hospital X. A quantitative cross-sectional study was conducted on 222 respondents, two of whom were medical and two of whom were non-medical workers, using a DASS-21 questionnaire and a job performance questionnaire. The findings show a relationship between age and job performance among medical and non-medical employees at General Region Hospital X. There is no significant relationship between mental health and job performance among medical workers. Gender, age, employment status, and work period all had a significant relationship with non-medical workers' job performance. Anxiety, gender, and age all have a significant relationship in non-medical workers. The conclusions is 44.1% reported poor job performance, 14.5% reported depression, 30.7% reported anxiety, and 11.3% reported stress. In both medical and non-medical workers, there was a significant interaction between anxiety and gender on job performance. Anxiety, gender, and age all had an impact on non-medical workers' job performance. It is critical to develop new health policies to prevent and treat mental health issues while also improving job performance. intervene and assist workers suffering from mental illnesses. Training and management on mental health, a conducive workplace, and social support can enhance productivity and decrease mental problems.

Abstract

Minimum Service Standard (MSS) in health sector is a mandatory that must be implemented by every regional government in Indonesia, so the MSS target must be 100% annually. However, the report shows that the MSS in health sector has not reached the target. National achievements in 2021 for trimesters I, II, III and IV respectively is 2.38%, 7.55%, 9.95% and 52.85%. In 2022, the achievements increased and the percentages for each trimester are 26.53%, 31.05%, 39.25% and 77.49%, but still not meet the target. This condition shows the unoptimized implementation of Minimum Service Standard (MSS) in health sector that could affected to health service delivery and society welfare. This study aims to identify the implementation and obstacles of Minimum Service Standard (MSS) in health sector at district level. This study is a systematic literature review. 9 literatures on Google Scholar database and Garuda Portal selected according PRISMA guidelines (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). The result shows some obstruction in the MSS in health sector’s implementation including human resources quantity and quality, inadequate infrastructure, lack of cross-sectoral and community support, and unoptimal planning and budgeting system. Based on these results it can be concluded that regional government should focused on recource factors including human recources and infrastructure, organizational factor to optimilize internal and external commitment and support including community support and budgeting support.

Abstract

This research aims to analyze the implementation of the Youth Posyandu/Integrated Health Post program in Pekalongan city. The research design used qualitative with in-depth interview techniques and data triangulation. The informants consisted of relevant officials, Posyandu cadres, and stakeholders. The results indicate that strategies for increasing youth participation include intensifying education, involving community leaders, creating a youth-friendly atmosphere, and strengthening the role of cadres. The barriers to participation can be resolved through a creative and collaborative approach. Adolescents' health awareness is increased through understanding their needs. Health information was delivered using an attractive digital platform according to teenagers' interests. Overcoming access barriers required stigma-free educational campaigns, supportive environments, and active youth involvement. The positive role of parents and the social environment creates norms that support adolescent participation. A youth-friendly environment was created through participatory design, adequate facilities, and organizing activities by teenagers. Moreover, evaluation of program effectiveness involved measurable performance indicators, comprehensive data analysis, and ongoing feedback to optimize impact on youth knowledge, behavior, and well-being.

Abstract

Length of Stay (LOS) menjadi satu jenis faktor sehingga mengakibatkan tumpuan harga rawat inap (ranap) dan sebagai alat ukur mutu pelayanan rumah sakit. Lama rawat inap dipengaruhi oleh berbagai faktor, tujuan dari penelitian ini adalah untuk mengetahui prediktor lama rawat inap pasien stroke. Metode: Penelitian ini dilakukan dengan metode scoping review dengan menggunakan database elsivier scopus, google scholar, academia.edu dan pub med. Langkah – langkah penelitian ini terdiri dari: 1) Identifikasi pertanyaan, 2) Identifikasi artikel, 3) Penyaringan artikel, 4) Pemetaan data, 5) Penyusunan hasil, dan 6) Konsultasi. 881 artikel dipilih berdasarkan judul, abstrak, isi dan hasil sesuai dengan kriteria inklusi dan tujuan studi literatur ini. Hasil skrining diterjemahkan ke dalam literatur review chart sehingga diperoleh 17 artikel dan kemudian dilakukan analisis. Kriteria inklusi yang ditetapkan adalah : artikel teks lengkap, fokus pembahasan tentang prediktor lama rawat inap pasien stroke, serta populasi dan sampel pasien stroke. Hasil: Scoping review ini menunjukkan prediktor lama rawat inap pasien stroke, yaitu karakteristik responden, komorbiditas, jenis stroke, penatalaksanaan, komplikasi, gejala stroke, riwayat merokok, ekonomi tempat tinggal, dan waktu rujukan dapat meningkatkan lama rawat inap pasien stroke. Meskipun riwayat merokok dalam scoping review mempunyai hasil yang berbanding terbalik yaitu dapat menurunkan lama rawat inap pasien stroke. Kesimpulan: Lama rawat inap pasien stroke bisa meningkat atau menurun dipengaruhi oleh prediktor lama rawat inap pasien stroke, sehingga penelitian lebih lanjut diperlukan untuk meneliti secara menyeluruh terkait prediktor lama rawat inap pasien stroke. Abstract

Background: Performance-based capitation payments are an innovative approach in the health financing system implemented by Social Security Agency on Health (BPJS Health). Purpose: To analyze of the implementation of capitation policy based on service commitment. Method: A descriptive qualitative approach with a case study approach, which aims to analyzing the implementation of the Performance-Based Capitation policy, by taking four variables that are considered most crucial in determining the success of the implementation of the Performance-Based Capitation policy in the city of Semarang, with the policy implementation model approach from Donald Van Meter and Carl Van Horn, namely communication factors, resources, implementer disposition, and implementer characteristics. Results: This approach aims to stimulate the provision of higher quality and more efficient health services, by providing incentives to health service providers based on optimal performance and service outcomes. The implementation of performance-based capitation payments provides several benefits, including increased accountability of health service providers, more effective cost management, and stimulation of innovation in service provision. Conclusion: Performance-based capitation payments by BPJS Health are a step forward in creating a health insurance system that focuses on service quality.
