Abstract

Introduction: Schizophrenia patients require lifelong care, which necessitates the family's ability to provide that care. Efforts to enhance the family's ability are carried out by giving psychoeducation to improve the skills of caring for schizophrenia patients. The problem with providing psychoeducation directly is that distance and time become obstacles; an effective, efficient, and easily accessible method can be achieved by utilizing information technology through online media. This scoping review aimed to identify the effectiveness of online psychoeducation in enhancing the ability of families to care for patients with schizophrenia. Methods:  This scoping review used the PICO framework to develop article questions. (P: family of schizophrenia patients, I: psychoeducation, C: online and offline, and O: family's ability to care for schizophrenia patients). The databases were PubMed, ScienceDirect, and Google Scholar. The selected articles were published in the last 5 years, from 2019 to 2024. The keywords were used "psychoeducation" AND "online" AND "family capacity" AND "schizophrenia". Results: The search results found 434 articles, only 14 of which met the criteria (one article from PubMed, one article from Science Direct, and 12 articles from Google Scholar). There were four online psychoeducation articles and ten direct psychoeducation articles. All articles were experimental studies. The search results found that online psychoeducation was more effective in improving the ability of families to care for schizophrenia patients. Offline psychoeducation has the disadvantage of making it more difficult to get families to attend all therapy sessions, so in general, online psychoeducation is better because it makes it easier for families to engage in the therapy process, allowing them to complete all sessions. Conclusions: Offline psychoeducation has the disadvantage of requiring family members to attend all therapy sessions, so in general, online psychoeducation is better.

Abstract

Introduction: Palliative end-of-life care (PEOLC) is heavily impacted by culture and always discusses death, bereavement, uncertainty, loss, grief, and incurable disease. Long-term care and more culturally sensitive nursing care are essential to address the diverse needs of patients. This study aimed to develop a PEOLC model grounded in transcultural nursing principles, with a focus on enhancing the quality of care for patients requiring long-term support. Methods: This study employed a cross-sectional explanatory design involving 180 patients with chronic illnesses as participants selected using simple random sampling. The proposed model involved eight variables: patient factors, nurse factors, service factors, family factors, technology factors, the PEOLC model, quality of life, and quality of death. A valid questionnaire was used to collect data, and we employed SEM-PLS to analyse the results. Results: The PEOLC model had an R2 value of 0.61. These findings showed that 60.1% of the variability of PEOLC variables was explained by the patient, family, nurse, service, and technology aspects. For each construct, a value of Q2 > 0 (0.083) indicated that the model was appropriate. The path coefficient value of 0.343 indicates that the service factors make the most significant contribution to the PEOLC model. Conclusions: This research led to the development of the PEOLC model, which comprises eight variables. It provides a comprehensive framework for integrating transcultural nursing principles into long-term care and emphasises the importance of cultural sensitivity in delivering personalised and effective end-of-life care.

Abstract

Background: Cataracts remain the leading cause of blindness globally, and surgery is the primary treatment. Inadequate postoperative care can lead to serious complications, including endophthalmitis. Nurse-led education is critical for prevention, and its impact can be strengthened through a management function approach. Purpose: To review nursing education strategies for preventing postoperative cataract infection from a management function perspective. Method: A scoping review was conducted following PRISMA guidelines using the PCC framework (P: nurse, C: educational management, C: postoperative cataract infection). Searches were performed in PubMed (MeSH terms) and Science Direct (standard keywords) for English-language, full-text articles published between 2014 and 2024. Narrative, integrative, scoping, and systematic reviews, as well as meta-analyses, were excluded. Results: The search retrieved 166 articles (75 PubMed, 91 Science Direct). After screening and eligibility assessment, 12 articles were included. Although none explicitly examined management functions, analysis revealed their implicit application. Educational interventions, when systematically planned, organized, implemented, and evaluated, enhanced effectiveness in preventing postoperative cataract infections. Conclusion: Incorporating a structured management function approach can improve the effectiveness of nursing education in preventing postoperative cataract infections. No studies were found that directly applied this framework in cataract postoperative care, indicating a gap for future research.

Abstract

Health facilities are experiencing overcapacity, oxygen scarcity, and a limited number of healthcare providers due to the coronavirus disease 2019 (COVID-19), thus impacted on anxiety. This study aimed to determine predictors of anxiety among healthcare providers toward the Delta variant of COVID-19 in Indonesia. A cross-sectional study was conducted with 371 healthcare providers in Java and Bali Islands, and the snowball sampling technique was used. Data were collected using a questionnaire and distributed through social media (WhatsApp), then analyzed using univariate analysis, bivariate analysis (Chi-square test), and multivariate analysis (multiple logistic regression). The results showed that 81 (21.8%) respondents experienced anxiety. The workplace (AOR: = 0.617; p-value = 0.011), a history of confirmed positive for COVID-19 (AOR = 2.965; p-value<0.001), and the respondent's comorbidities (AOR = 8.753; p-value<0.001) were significantly associated with anxiety toward the Delta variant. Healthcare managers must regularly evaluate the psychological condition of their subordinates during the COVID-19 pandemic so that anxiety can be detected and overcome early through constructive self-adaptation and positive coping mechanisms.

Abstract

Introduction: The success of pulmonary TB treatment remains low due to patients' lack of compliance with the treatment regimen. This study aims to determine factors related to treatment compliance among TB patients at the Internal Medicine Outpatient Clinic of the Raja Ampat Regional General Hospital. Methods: An observational research design with a cross-sectional study approach was employed, involving 125 tuberculosis patients receiving treatment at the Internal Medicine Outpatient Clinic of the Raja Ampat Regional General Hospital, selected through total sampling technique. Data collection utilized a structured questionnaire. Data were analyzed using univariate, bivariate (Chi-Square test), and multivariate (Binary Logistic Regression) analyses. Results: The majority of treatment-compliant TB patients among the 125 respondents were 74 (59.2%) in the compliant category. There was a significant relationship between age, education, knowledge, attitude, perception, motivation, and staff support with the level of treatment compliance. Good knowledge (AOR: 2.18; CI: 1.05-1.56; p 0.003) and high staff support (AOR: 1.27; CI: 1.11-1.68; p 0.006) increased TB patient treatment compliance. Conclusion: TB patient treatment compliance is still below the established standards. Knowledge and staff support enhance better treatment compliance, emphasizing the importance of education and improving service access, such as health worker visits to TB patients' homes.

Abstract

Background: Nursing care provided to patients, especially in hospitals, is expected to be of high quality, and its implementation is in accordance with the nurse's clinical competence. Objectives: This scoping review aimed to assess the implementation of the clinical competence of nurses in hospitals so that a clear picture of the implementation of these competencies is obtained. In addition, this study can be evidence-based and related to implementing these clinical competencies. Methods: This literature review used PICO framework to develop articles questions (P: nurses, I: not applicable, C: not applicable, and O: application of clinical nursing competency). Articles search for literature review was conducted during April and May 2023. This study used Pubmed and ScienceDirect Databases and other sources. The keywords that used in this scoping review was “nursing competency” OR “clinical nursing competency” AND “evaluation” OR “implementation” AND “hospital”.Results: There were 1,150 articles from the search results, but only seven were included for review. One article is a descriptive comparative study using a qualitative approach, and six articles are cross-sectional studies. The articles are publications within five years (2019-2023). In its implementation, the average clinical competency of nurses is at a low to moderate level, and only one study found was at high nurse competency. The results of the supervisor's clinical competency assessment were significantly lower than those assessed by oneself and the group. In addition, the nurse competency level was moderate, and there was no significant difference in the clinical competency of nurses before and during the COVID-19 pandemic.Conclusions: The implementation of nurses' clinical competence has yet to be optimal, so hospital management needs to improve nurses' clinical competence so that nurses can provide higher quality and professional nursing care in the hospital.

Abstract

Pandemi coronavirus disease 2019 (COVID-19) tidak hanya berdampak pada ekonomi dan kesehatan, tetapi juga aspek pendidikan termasuk praktek klinik mahasiswa profesi kesehatan di rumah sakit. Berbagai perubahan dan penyesuaian jumlah mahasiswa yang melaksanakan praktik klinik, peralatan praktik dan juga metode bimbingan dapat berpengaruh terhadap kepuasan mahasiswa. Penelitian ini bertujuan untuk mengidentifikasi kepuasan mahasiswa profesi kesehatan dan prediktornya dalam melaksanakan praktik klinik selama pandemi COVID-19 di Rumah Sakit Daerah (RSD) Mangusada, Kabupaten Badung. Penelitian ini adalah sebuah studi potong lintang yang melibatkan 341 mahasiswa profesi kesehatan (Mahasiswa Koas, Profesi Ners, Profesi Bidan dan Profesi Fisioterapis) yang melaksanakan praktik klinik di RSD Mangusada. Pemilihan sampel dalam penelitian ini menggunakan teknik consecutive sampling. Pengumpulan data berlangsung dari bulan Januari hingga Maret 2022 dan alat pengumpulan data yang digunakan adalah kuisioner yang diisi sendiri oleh responden. Data selanjutnya dianalisis menggunakan analisis univariate (fekuensi dan proporsi), bivariate (Chi square test) dan multivariate (Binary logistic regression). Hasil penelitian menujukkan bahwa sebagian besar responden yaitu : 260 (76,2%) mahasiswa puas dalam melaksanakan praktik klinik di rumah sakit. Ada dua prediktor yang secara signifikan berpengaruh terhadap kepuasan mahasiswa profesi kesehatan yaitu persepsi terhadap pola bimbingan dari klinik [Adjusted Odds Ratio (AOR)=6,177; p-value <0,001] dan persepsi terhadap tempat praktik klinik (AOR=3,102; 95% CI: 1,71-5,63). Pembelajaran praktik klinik perlu ditingkatkan melalui praktik klinik yang efektif dengan memperbaiki pola bimbingan mahasiswa dan meningkatkan kelengkapan tempat praktik klinik sehingga kepuasan mahasiswa dapat ditingkatkan.

Abstract

Hemodialysis (HD) patients with chronic kidney disease (CKD) lose muscle mass and strength due to protein energy wasting (PEW). The purpose of this study was to test the effectiveness of intradialytic resistance training (IRT) in increasing muscle mass and strength. It used a pre-experimental, one-group pretest-posttest design. Based on a sample size estimate using G*power software, 33 CKD patients undergoing HD at a private hospital in Bali were included. The patients were selected using purposive sampling. Muscle mass was measured using Bioelectrical Impedance Analysis (BIA), while muscle strength was measured using the Medical Research Council Muscle Scale. The Wilcoxon test evaluated IRT's efficacy on muscle strength, while the dependent samples t-test evaluated its impact on muscle mass. The results of this study found that IRT had no effect on muscle mass, with a mean difference of -0.330 (p = 0.274). On the contrary, IRT significantly improved muscle strength (p = 0.018). In conclusion, IRT proved beneficial for strengthening muscle but not for gaining muscle mass. Therefore, to improve muscle strength, patients with CKD undergoing HD are advised to actively perform IRT under supervision for a minimum of 30 minutes per day. Pasien hemodialisis (HD) dengan Penyakit Ginjal Kronis (PGK) kehilangan massa dan kekuatan otot yang disebabkan oleh Protein Energy Wasting (PEW). Tujuan penelitian ini adalah untuk menguji efektifitas Intradialytic Resistance Training (IRT) terhadap peningkatan massa dan kekuatan otot. Penelitian ini menggunakan pre-experimental one-group pretest, post-test design. Berdasarkan perhitungan besar sampel menggunakan G*Power Software, 33 pasien PGK yang menjalani HD di sebuah rumah sakit swasta di Bali terlibat dalam penelitian ini. Pasien dipilih menggunakan teknik purposive sampling. Massa otot diukur menggunakan Bioelectrical Impedance Analysis (BIA), sedangkan kekuatan otot menggunakan Medical Research Council Muscle Scale. Uji efektifitas kekuatan otot menggunakan Wilcoxon test, sedangkan t-test digunakan untuk menguji efektifitas massa otot. Hasil penelitian menunjukan IRT tidak efektif dalam meningkatkan massa otot, dengan mean -0,330 (p = 0,274). Namun, IRT terbukti efektif dalam mening-katkan kekuatan otot, (p = 0,018). Kesimpulannya, IRT bermanfaat untuk meningkatkan kekuatan otot, tetapi tidak untuk meningkatkan massa otot. Oleh karena itu, untuk meningkatkan kekuatan otot, pasien PGK yang menjalani HD disarankan aktif melakukan IRT dengan pengawasan minimal 30 menit dalam sehari.  Kata Kunci: intradialytic resistance training, kekuatan otot, massa otot, protein energy wasting

Abstract

Cataract is the leading cause of blindness worldwide, requiring surgery and appropriate patient and family education to ensure proper operative care. This literature review aimed to identify health education on operative cataract care. The type of this study is a scoping review using the PubMed database, ScienceDirect, and other sources that were published in the last 10 years (2015-2024). The PICO model (P: patients and families; I: audiovisual, booklets, leaflets, and others; C: standard care or education; O: knowledge and skills in cataract operative care) was used. The keywords used to search for articles are cataract surgery AND video OR audiovisual OR booklets OR leaflets. The initial search results for articles, until included, are described in the PRISMA diagram. The results of the article search from PubMed obtained 406 articles, and from ScienceDirect 258 articles; other sources 15 articles, so the total number of articles obtained was 679 articles. Records screened 664 articles, reports sought for retrieval 247 articles, and reports assessed for eligibility 60 articles. For articles from sources, the report assessed for eligibility was three articles. Furthermore, the total studies included in the review were 15 articles. The educational interventions (video-based, telephone, social media, and web) were effective in increasing patient knowledge. However, their comparative effectiveness is unknown. Abstract

Background: Efforts to control the transmission of coronavirus disease 2019 (COVID-19) and associated deaths continue, one of which is providing COVID-19 vaccination not only to adolescents, adults, and older adults but also to elementary school children. However, parental approval for the vaccination of the children is needed for the government’s vaccination program to succeed. Objectives: This study sought to determine parental acceptability of the COVID-19 vaccine and related factors for primary school-aged children in Bali, Indonesia. Methods: This cross-sectional study involved 420 respondents recruited through a multistage random sampling process. It collected data using a questionnaire shared through Google Forms. It analyzed the data using univariate, bivariate, and multivariate regression analyses. Results: Of the 420 respondents, 249 (59.3%) stated they accepted COVID-19 vaccination for elementary school students. Vaccine safety factors (adjusted odds ratio [AOR]: 14.376, 95% confidence interval [CI]: 7.682–26.903, p < 0.001), place of residence (AOR: 0.430; 95% CI: 0.271–0.683, p < 0.001), and history of comorbidities (AOR: 0.402, 95% CI: 0.222–0.726, p = 0.003) were significantly associated with parents’ approval of COVID-19 vaccination for elementary school children. Conclusion: Parents must be educated about vaccine safety, especially those living in cities and whose children have a history of comorbidities, to improve parental approval of COVID-19 vaccination according to the government’s target.
