Abstract

Vaksin COVID-19 terus dikembangkan, untuk mempersiapkan ketersediaan publik, penerimaan vaksin COVID-19 terus menurun, khususnya vaksin booster. Keraguan melakukan vaksin merupakan masalah kesehatan yang kompleks. Efektifitas, keamanan dan efek samping menjadi salah satu penyebab perilaku atau niat masyarakat menurun dan semakin ragu dalam melakukan vaksinasi booster COVID-19. Tujuan penulisan tinjauan sistematis ini untuk memprediksi model keyakinan masyarakat untuk menilai niat dan penerimaan dalam melakukan vaksinasi booster COVID-19. Pencarian literature dilakukan pada 5 database yaitu Scopus, PubMed, Web Of Science, Sage dan Pro Quest. Pencarian dilakukan pada Oktober 2022. Kata Kunci yang dilakukan dengan menggunakan “COVID-19 OR Coronavirus Disease AND Vaccine AND Accept AND Booster AND Dose AND Health Belief Model” dengan melakukan restraksi pada tahun 2019-2022 dalam bahasa Inggris dan indonesia artikel full text, sehingga mendapatkan artikel yang relevan. Jumlah artikel yang diidentifikasi adalah Scopus 6 artikel, PubMed 8 artikel , Web Of Science 6 artikel, Sage 232 artikel dan Pro Quest 179 artikel dengan total 431 artikel. Dari 431 artikel yang didapatkan, ada 11 artikel yang sesuai dengan kriteria penulisan tinjauan sistematis ini. Prediktor penting dari niat pasti untuk mengambil vaksinasi booster COVID-19 termasuk manfaat yang dirasakan tinggi dan hambatan yang dirasakan lebih rendah untuk menerima vaksin, dan kerentanan yang dirasakan lebih tinggi terhadap infeksi.

Abstract

This study aims to analyze the effect of spiritual mindfulness based on adaptation theory on anxiety and vital signs in pneumonia patients. The research method uses a quasi-experiment in pneumonia patients at RSUD Dr. Soetomo, Surabaya. Forty-eight respondents were divided into 24 intervention groups and 24 control groups recruited using purposive sampling. The independent variable is spiritual mindfulness based on adaptation theory. The dependent variable is anxiety, measured using the Zung SRAS questionnaire, and vital signs are measured using observational data. Data analysis using Paired t-test and independent t-test. The study results in the intervention group showed a significant effect of the intervention on anxiety (p=0.000; d=1.436) and vital signs (systolic blood pressure p=0.000, diastolic blood pressure p=0.001, pulse p=0.000, respiratory rate p=0.000 and SPO2 p = 0.001. In conclusion, spiritual mindfulness techniques based on adaptation theory can reduce anxiety and vital signs of pneumonia patients at Dr. Soetomo Hospital, Surabaya. Abstract

Introduction: Pressure ulcers in high-risk patients are contributes to patient burden and cost financially. Identifying strategies on the prevention of high ulcers among those with high risk is necessity. Nevertheless, an intense study exploring those issues is scarce. The study aimed to analyze the prevention method and their effectiveness in pressure ulcer prevention. Method: This study used PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis) to guide the systematic review process. The article searches were conducted on Web of Science (WOS), ScienceDirect, Wiley Online Library, and Scopus using the keywords used Decubitus Ulcer, Pressure Ulcer, Management, and Prevention, and any possible combinations. Results: A total of 18 studies met the inclusion criteria for this review and show some different prevention for a pressure ulcer. The findings of the present study are foam dressings, support surfaces, repositioning, topical-based interventions, wearable patient sensors, and two distinct endotracheal tube fixations affect the incidence of pressure ulcers. Conclusions: This current review emphasized a bundle care consisted of two methods or more are effective to reduce the incidence of pressure ulcers rather than a single treatment. Nurses should consider intervention to prevent ulcers among high-risk patients by way analyses of each single treatment prior bundling. Further study investigating factors associated wound’s ulcer healing is necessary.

Abstract

Penyakit gagal jantung merupakan masalah kesehatan yang progresif di negara maju maupun negara berkembang seperti Indonesia. Cara terbaik dalam mengontrol kekambuhan dari gejala penyakit gagal jantung yaitu dengan manajemen keseimbangan cairan tubuh. Aplikasi edukasi presisi manajemen cairan dapat digunakan sebagai upaya dalam mengatasi masalah manajemen cairan yang dialami pasien CHF dengan meningkatkan self care pasien sehingga tujuan mengembangkan aplikasi edukasi presisi berbasis android dapat tercapai. Desain penelitian menggunakan Research and Development (R&D) yang terdiri dari dua tahap penelitian. Sampel diambil menggunakan teknik consequtive sampling yakni 30 pasien CHF dan 4 perawat penanggung jawab ahli jantung. Instrument yang digunakan adalah rekam medis, wawancara, dan kuesioner. Uji kelayakan kuesioner SUS pada aplikasi edukasi presisi didapatkan hasil 84 artinya acceptable. Uji validitas aplikasi edukasi presisi hasilnya 0,868 artinya sangat relevan sehingga aplikasi edukasi presisi manajemen cairan dapat digunakan untuk membantu pasien CHF melakukan manajemen cairan. Pada tahap 1 menggunakan analisa univariat dan bivariat, sedangkan pada tahap 2 menggunakan analisa univariat untuk menunjukkan presentasi hasil kelayakan uji aplikasi. Aplikasi edukasi presisi manajemen cairan layak untuk digunakan sebagai media edukasi dengan kondisi Acceptability Ranges yakni acceptable. Grade Scale termasuk B. Adjective Rating: Worst Imaginable hasilnya Excellent dan secara Percentiles hasilnya Baik.

Abstract

Latar Belakang: Perawatan paliatif merupakan layanan keperawatan yang seharusnya diberikan sejak pasien didiagnosis sakit kronis, masa perawatan hingga meninggal dunia dan berduka. Namun kondisi ini dimaknai berbeda oleh Indonesia dimana perawatan paliatif didefinisikan sebagai perawatan menjelang ajal kematian sehingga dalam perawatan pasien paska bedah jantung tim medis tidak berkenan melakukan kolaborasi dalam tindakan paliatif sedangkan kebutuhan dasar perawatan harus tetap dijalankan untuk memaksimalkan kualitas hidup pasien. Tujuan: Studi kasus ini bertujuan untuk membahas peran tindakan perawatan paliatif dalam perawatan pasien paska bedah jantung pada pasien paska CABG di Ruang ICCU RSUP Dr. Kariadi Semarang pada bulan Juli 2023. Metode: Penelitian dilakukan dengan pendekatan deskriptif kualitatif studi kasus melalui wawancara pasien, keluarga dan analisa kasus hasil pemeriksaan medis pasien. Hasil: Program perawatan paliatif pasien paska tindakan CABG belum terintegrasi dengan tim perawatan paliatif. Hasil pengkajian kebutuhan dasar pasien paliatif di dapatkan data bahwa pasien dan keluarga membutuhkan perawatan paliatif untuk mengurangi gejala nyeri, kelelahan, insomnia, kehilangan nafsu makan dan dampak finansial akibat kondisi sakit pasien. Kesimpulan: Hasil penelitian ini dapat digunakan sebagai pengembangan ilmu pengetahuan terkait pentingnya pengkajian kebutuhan dasar untuk memaksimalkan kualitas hidup pasien paska CABG. Diperlukan alat ukur pengkajian kebutuhan dasar perawatan paliatif guna mengukur kebutuhan dasar perawatan paliatif.

Abstract

Documentation instrument for quality nursing care according to standards. However, there is no instrument for nursing care for patients installing ventilators according to Indonesian nursing standards. This research aims to develop a 3S-based nursing care instrument (SDKI, SLKI, SIKI) for ventilator patients. The research design is Research and Development carried out in 2 stages. The first phase population is 100 nursing care. The first stage was FGD 1, divided into 3 groups, namely 10 executive nurses, 6 nursing management staff, and 1 expert. The second stage population was 5 nurses. The second stage was carried out FGD 2 with participants from 6 levels of nursing management. The sample used in this research was purposive sampling. The design developed is an instrument for documenting nursing care for ventilator patients. The analysis uses the CVI and KR20 tests. Results: Evaluation of nursing care instruments shows that aspects of nursing outcomes, implementation, and evaluation are not by standards. Instrument development consists of 5 main problems that often arise in ventilator patients. The I-CVI result is ≥ 0.78, which is interpreted as valid. The KR result is > 0.6, which means it is reliable. The recommendation from the research results is that the development of nursing care instruments can be applied in treating patients with ventilators. Conclusion: The development of a nursing care instrument was declared to be valid and reliable in categories in terms of functionality, efficiency, and usability, so it is recommended that nurses use it to improve the quality of hospital services.

Abstract

The Emergency Unit is the first department to provide action to patients in emergencies to save lives, however patients with psychiatric anxiety disorder still receive general treatment. Therefore, this research aims to develop SOPs for handling emergency patients with anxiety disorders. Method: The research design was Research and Development carried out in 2 stages. The first phase population was 5 SOPs for emergency management. The first stage was FGD 1, divided into 3 groups, namely 17 executive nurses, 6 nursing management staff, and 2 experts (academics and practitioners). The population for the second stage of SPO development was filled by 5 nurses. The second stage was carried out FGD 2 with participants from 6 levels of nursing management. The sample used in this research was purposive sampling. The design developed is an SOP for the management of emergency patients with anxiety disorder. The analysis uses the CVI and KR20 tests. Results: There ware no SOPs for handling emergency patients with anxiety disorders. The development of SPO combines psychiatric emergency management in the form of environmental manipulation, verbal de-escalation, mechanical fixation, and pharmacological intervention with medical emergency management procedures including primary survey (airway, breathing, circulation, disability). The validity results show the CVI test is 0.8-1 and the KR20 is 0.83. Research recommendations that the development of SPO can be applied in the treatment of emergency patients with anxiety disorder. Conclusion: The development of SPO is declared to be valid and reliable in categories both in terms of functionality, efficiency, and usability, so it is recommended that nurses use it to improve the quality of hospital services.

Abstract

Palliative care is start from diagnosis of serious illness until death and bereavement care. But, the term “Palliative Care” often mistakenly regarded identical to “End of Life Care” without any treatments. It’s caused dilemma among nurses to give maximum care of palliative care needs or let the patients get minimum of care. Methods: The aim of this article is to evaluate the dilemma palliative care needs in Intensive Care Unit. This is a case study report used a qualitative approach of 3 palliative patients in ICU used descriptive case study as well as in depth-interviews with patient’s family, nurses and internist. Results and Discussion: We present a case report of 3 patient who had been admitted to hospital with cancer and need palliative care in ICU. Patients subsequently suffered a pulmonary metastase with multiple organ failure. However the goal of end of life care for dying patients is to prevent or relieve suffering as much as possible while respecting the patient’s desires. But it is still a dilemma in Indonesia, especially palliative patients in ICU. According to medical decision, patients in ICU should receive maximum treatment, but if patient is facing multiple organ failure nurse should consider to providing palliative care. Conclusions: Palliative care approaches and provision within intensive care units can significantly impact care outcomes and increase the quality-of life people with end-of life period. Palliative care not to reduce the dose of drugs and patient care.

Abstract

Introduction: Circumcision is a minor surgical operation that cuts the foreskin at the tip of the penis. Pain and anxiety are problems that all children who undergo surgery experience. This study aimed to review virtual reality game interventions to reduce pain and anxiety from pre, intra, and post-circumcision in children.Methods: Literature study using four databases (Scopus, Science Direct, PubMed, and Google Scholar) using quantitative research with RCT and quasi-experimental designs, using virtual reality, audio-visual and game interventions which were limited from 2018 to 2022 according to combination of keywords and using English. The framework investigation using PICOT, guidance protocol with PRISMA, and accessing the eligibility of articles systematically by checking for duplicates, titles, and abstracts and determining the eligibility of the full text according to the study of the goals.Results: Thirteen articles met the inclusion criteria. Of the thirteen articles, 1074 children were circumcised. Circumcision causes pain and anxiety in children, so non-pharmacological interventions are needed to overcome it. Virtual reality games are a distraction technique to reduce circumcision pain by diverting the child's attention and reducing anxiety by providing interventions that children like using audio-visual media and games, with virtual reality or not.Conclusions: Virtual reality games intervention is a distraction technique to reduce pain and anxiety in circumcised children. The combination of interventions using traditional games or the latest technology must be considered to improve service quality. Keywords: anxiety; children; circumcision; distraction technique; pain
