Abstract

Covid-19 is a respiratory infection caused by the newly emerging coronavirus. Covid-19 can also cause skin abnormalities, although the pathophysiology of manifestations of skin disorders associated with Covid-19 has not been understood with certainty. This study aims to discuss the pathophysiology of cutaneous manifestation related to Covid-19. The study was conducted based on literature and data mining related to Covid-19 using PubMed and Google Scholar. Eligible papers provided pathophysiology of cutaneous manifestations related to Covid-19. Papers not available in English were excluded. As a result of the presence of edema, vasodilation, and cellular infiltration, skin lesions such as rash, urticarial, or extensive purpura, or even thrombosis and haemorrhage, arise if damage to the vessels became severe. The pathophysiology of Covid-19-associated skin manifestations is estimated through some different mechanisms, such as lesions found in various Covid-19 patients, namely, the presence of varicella such as eruption, non-characteristic rash, and urticarial. Further studies are required to establish a Covid-19 diagnosis based on skin manifestations.

Abstract

   Pendahuluan: Herpes genitalis pada pasien geriatri imunokompromi kerap bermanifestasi atipikal, meningkatkan risiko misdiagnosis. Tujuan laporan kasus ini adalah untuk mengetahui dan memahami manifestasi klinis, alur diagnosis, serta menekankan pentingnya terapi antivirus sistemik yang adekuat dan edukasi komprehensif sebagai pilar utama dalam penatalaksanaan herpes genitalis terutama pada pasien geriatri dan keganasan. Kasus: Tn. P (60 tahun) dengan tumor paru kanan stadium lanjut (manifestasi sesak, massa paru, dan metastasis hepar), dikonsulkan akibat ulkus genital nyeri. Pemeriksaan fisik menunjukkan ulkus multipel berbatas tegas pada penis. Meski apus Tzanck negatif, diagnosis herpes genitalis ditegakkan berdasarkan gambaran klinis khas pada imunokompromi dan dikonfirmasi melalui respons penyembuhan signifikan pasca-terapi asiklovir, menyingkirkan diagnosis banding ulkus mole. Diskusi: Penuaan menyebabkan immunosenescence yang melemahkan respons seluler terhadap virus. Kondisi ini diperberat oleh supresi imun sistemik akibat keganasan paru, memicu reaktivasi HSV dengan presentasi klinis atipikal (seperti ulkus tanpa vesikel) dan menurunkan sensitivitas pemeriksaan penunjang standar. Simpulan: Terdapat hubungan erat antara penurunan imunitas pada usia tua dan keganasan paru dengan kejadian, atipikalitas gejala, dan tantangan diagnostik herpes genitalis.

Abstract

Introduction Henoch–Schönlein purpura (HSP), also known as IgA vasculitis, is a systemic small-vessel vasculitis characterized by IgA immune complex deposition. Although predominantly a childhood disease with a generally favorable prognosis, adult cases often exhibit more severe systemic involvement, particularly affecting the gastrointestinal tract and kidneys. Case A 22-year-old female presented with palpable purpura on the extremities, arthralgia, and melena. Physical examination revealed confluent purpuric lesions over the arms and legs. Endoscopy revealed esophagitis and antral gastritis. Skin biopsy demonstrated flattening of rete ridges and perivascular infiltrates, consistent with leukocytoclastic vasculitis. The patient was treated with intravenous methylprednisolone and supportive therapy, resulting in significant improvement of skin lesions, joint pain, and resolution of gastrointestinal bleeding. Discussion Adult HSP is less common but typically has a more severe course compared to pediatric cases. Gastrointestinal involvement, as seen in this case, may present as bleeding, ischemia, or ulceration, and can be life-threatening. Corticosteroids are effective in controlling inflammation and alleviating symptoms but do not prevent renal complications. Continuous renal monitoring is crucial, as kidney involvement remains the primary determinant of long-term prognosis. Conclusion This case highlights the clinical relevance of HSP in adults, where gastrointestinal manifestations may predominate and require aggressive management. Long-term follow-up, especially for renal involvement, is essential to improve outcomes.

Abstract

Latar belakang: Remaja merupakan kelompok usia yang rentan terhadap infeksi menular seksual (IMS) dan human immunodeficiency virus/acquired immuno deficiency syndrome (HIV/AIDS) akibat kurangnya akses terhadap informasi kesehatan yang akurat dan edukatif. Kondisi ini menunjukkan perlunya intervensi edukatif yang sistematis dan berbasis sekolah guna meningkatkan pemahaman remaja mengenai kesehatan reproduksi serta pencegahan IMS dan HIV/AIDS. Tujuan: Kegiatan ini bertujuan untuk meningkatkan pengetahuan siswa terkait kesehatan reproduksi dan pencegahan IMS serta HIV/AIDS melalui penyuluhan interaktif berbasis sekolah. Metode: Metode pelaksanaan menggunakan desain pre-test dan post-test. Peserta kegiatan sebanyak 50 siswa kelas XI dan XII SMA Negeri 1 Surakarta. Evaluasi dilakukan menggunakan instrumen berupa 10 soal pilihan ganda yang diberikan sebelum dan sesudah penyuluhan. Data dianalisis menggunakan uji paired t-test dengan tingkat signifikansi p < 0,05. Hasil: Hasil menunjukkan peningkatan rerata skor dari 5,52 ± 1,11 (pre-test) menjadi 9,18 ± 0,75 (post-test). Uji paired t-test menunjukkan perbedaan signifikan (p< 0,000). Sesi diskusi juga menunjukkan antusiasme tinggi dari peserta dan mengungkap adanya miskonsepsi yang umum mengenai HIV/IMS. Kesimpulan: Penyuluhan terbukti efektif dalam meningkatkan pemahaman peserta tentang kesehatan reproduksi dan IMS, serta memperkuat sikap positif terhadap pencegahan penyakit menular seksual. Diperlukan perluasan program serupa secara berkelanjutan untuk menjangkau lebih banyak remaja dan mengoptimalkan dampak edukatif dalam jangka panjang. Kata kunci: edukasi berbasis sekolah, HIV/AIDS, infeksi menular seksual, penyuluhan kesehatan, remaja _________________________________________________________________________________ Abstract Background: Adolescents are a vulnerable population with respect to sexually transmitted infections (STIs) and human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS), primarily due to limited access to accurate and comprehensive health information. This condition underscores the need for systematic, school-based educational interventions to enhance adolescents’ understanding of reproductive health and the prevention of STIs and HIV/AIDS. Objective: This program aimed to improve students’ knowledge of reproductive health and the prevention of STIs and HIV/AIDS through interactive, school-based health education. Method: The program employed a pre-test and post-test design without a control group. A total of 50 students in grades XI and XII at SMA Negeri 1 Surakarta participated in the activity. Evaluation was conducted using a 10-item multiple-choice questionnaire administered before and after the educational session. Data were analyzed using a paired t-test, with a significance level set at p < 0.05. Result: The results demonstrated an increase in the mean score from 5.52 ± 1.11 (pre-test) to 9.18 ± 0.75 (post-test). The paired t-test indicated a statistically significant difference (p< 0.001). The discussion session also revealed high participant enthusiasm and identified common misconceptions regarding HIV and STIs. Conclusion: The educational intervention was effective in improving participants’ understanding of reproductive health and STIs, as well as in strengthening positive attitudes toward the prevention of sexually transmitted infections. The expansion of similar programs on a sustained basis is recommended to reach a broader adolescent population and to optimize long-term educational outcomes.
