Abstract

Multidrug-resistant Pseudomonas aeruginosa poses a significant challenge in chronic wound management due to its robust biofilm formation and antibiotic resistance. Green-synthesized silver nanoparticles (AgNPs) using Tamarindus indica (tamarind) leaf extract offer an eco-friendly and effective approach to combat these infections. The bioactive phytochemicals in tamarind leaves function as both reducing and stabilizing agents, producing biocompatible AgNPs with potent antimicrobial and antibiofilm activities. These nanoparticles disrupt bacterial membranes, generate reactive oxygen species, inhibit virulence factors such as pyocyanin, and compromise biofilm integrity. Incorporation of tamarind-derived AgNPs into hydrogels or composite wound dressings further enhances wound healing by maintaining a moist environment and providing sustained antimicrobial release. Although in vivo studies specifically using tamarind AgNPs against P. aeruginosa are limited, evidence from other plant-derived AgNPs supports their promising potential for treating biofilm-associated wound infections. Green synthesis using tamarind thus represents a sustainable strategy for developing next-generation antimicrobial wound therapies.

Abstract

Curcumin possesses strong antioxidant and photoprotective activities, but its poor aqueous solubility, limited skin penetration, and high photoinstability restrict topical efficacy. This study developed curcumin-loaded microsponges using Eudragit RS100 by the quasi-emulsion solvent diffusion method. The optimized formulation (MS-F5) showed high encapsulation efficiency (91.7 ± 1.6%), porosity (61.3 ± 2.8%), positive zeta potential (+28.3 ± 2.1 mV), and sustained 24-h drug release following Higuchi kinetics (R² = 0.9952). Incorporated into a 2% Carbopol gel, MS-F5 significantly enhanced skin permeation (2.89-fold), epidermal retention (3.52-fold), antioxidant activity (IC₅₀ 12.4 ± 0.8 vs. 18.7 ± 1.2 µg/mL), and photostability (72.4% vs. 18.3%) compared with plain curcumin gel (p < 0.05). Stability studies following ICH Q1A(R2) and Q1B guidelines confirmed acceptable physicochemical and microbiological stability over six months. These findings demonstrate that microsponge technology improves curcumin stability, controlled release, skin delivery, and antioxidant performance, highlighting its potential for dermatological and cosmeceutical applications.

Abstract

Losartan potassium, a Biopharmaceutics Classification System (BCS) Class II drug, exhibits poor aqueous solubility and limited oral bioavailability. This study aimed to develop and optimize a Self-Nanoemulsifying Drug Delivery System (SNEDDS) to enhance its dissolution and absorption. SNEDDS formulations were prepared using Capmul MCM C8, Tween 80, and PEG 400 based on solubility screening and ternary phase diagram analysis. The optimized formulation (F3) was evaluated for droplet size, polydispersity index (PDI), in vitro drug release, pharmacokinetics, and stability. Pharmacokinetic studies were conducted in experimental rats Pharmacokinetic studies were conducted in experimental rats (n = 3 per group) to assess bioavailability. The optimized SNEDDS produced nanoemulsions with a droplet size of 68.3 nm, low PDI, and good dilution stability. In vitro studies demonstrated rapid drug release (>96% within 30 minutes), significantly higher than the plain suspension. Pharmacokinetic results showed a 3.09-fold increase in bioavailability and a 2.79-fold increase in Cmax without altering Tmax. Stability studies confirmed good physicochemical stability over six months under ICH conditions. In conclusion, SNEDDS is a promising strategy to improve the oral bioavailability of poorly water-soluble drugs such as losartan potassium.

Abstract

Type 2 diabetes mellitus (T2DM) is a chronic disease with a high prevalence in primary healthcare settings; however, real-world evidence regarding the effectiveness of oral antidiabetic therapies remains limited. This study aimed to compare the effectiveness of oral antidiabetic monotherapy and combination therapy in patients with T2DM at the Teladan Community Health Center (Puskesmas) in Medan. A non-experimental observational study with a cross-sectional design was conducted on 78 patients selected via systematic random sampling, utilizing medical records and data from the "referral-back" program covering the period from September to November 2025. Effectiveness was assessed using random blood glucose (RBG) levels and analyzed with the Kruskal–Wallis test. The results indicated that the combination of glimepiride and metformin was the most frequently used regimen (64.10%), followed by metformin monotherapy (26.92%) and glimepiride monotherapy (8.97%). The lowest mean RBG level was observed in the metformin monotherapy group (147.80±47.27 mg/dl), followed by the metformin-glimepiride combination group (168.72 ± 60.56 mg/dl) and the glimepiride monotherapy group (178±61.31 mg/dL); however, no statistically significant difference was found among the treatment groups (p=0.322). The study concludes that there is no significant difference in effectiveness between monotherapy and combination therapy regarding glycemic control, although metformin showed a trend toward better outcomes. These findings underscore the importance of an individualized approach to therapy for T2DM patients in primary healthcare facilities.

Abstract

Khitan merupakan prosedur bedah minor yang umum dilakukan sebagai bagian dari tradisi agama dan budaya di Indonesia. Namun, praktik ini tetap memiliki risiko komplikasi, terutama apabila perawatan luka pasca-prosedur tidak dilakukan dengan benar. Kegiatan pengabdian ini bertujuan untuk meningkatkan pengetahuan dan keterampilan orang tua atau wali dalam penggunaan obat dan perawatan luka pasca-khitan pada kegiatan khitanan massal di Desa Amplas, Kecamatan Percut Sei Tuan, Kabupaten Deli Serdang. Metode pelaksanaan meliputi pemberian paket obat-obatan, penyuluhan, serta demonstrasi praktik perawatan luka kepada 30 peserta khitan dan orang tua pendamping. Hasil kegiatan menunjukkan bahwa mayoritas peserta berusia 8–10 tahun, sehingga peran orang tua menjadi krusial dalam mendukung penyembuhan luka. Edukasi yang diberikan berhasil meningkatkan pemahaman orang tua mengenai dosis dan cara penggunaan obat, teknik pembersihan luka, prinsip asepsis, serta tanda-tanda infeksi yang harus diwaspadai. Evaluasi lisan menunjukkan adanya peningkatan rasa percaya diri orang tua dalam melakukan perawatan luka di rumah. Kegiatan ini diharapkan dapat menjadi model edukasi yang efektif untuk diterapkan pada kegiatan khitanan massal di komunitas lainnya guna meminimalkan risiko komplikasi pasca-khitan.

Abstract

Dispepsia merupakan salah satu gangguan saluran pencernaan yang umum terjadi di masyarakat dan berkaitan erat dengan pola hidup tidak sehat serta penggunaan obat yang tidak rasional. Rendahnya literasi kesehatan masyarakat mengenai faktor risiko dispepsia dan penggunaan obat lambung yang aman menjadi permasalahan yang perlu mendapat perhatian. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan pengetahuan masyarakat mengenai dispepsia, mendorong penerapan gaya hidup sehat, serta meningkatkan penggunaan obat lambung yang rasional. Kegiatan dilaksanakan di Puskesmas Teladan Kota Medan dengan melibatkan 87 peserta yang terdiri dari pasien rawat jalan, keluarga pasien, dan kader kesehatan. Metode yang digunakan meliputi sosialisasi, edukasi interaktif, distribusi media edukasi (leaflet, booklet, dan video), pendampingan, serta evaluasi menggunakan pre-test dan post-test. Analisis data dilakukan secara deskriptif. Hasil kegiatan menunjukkan adanya peningkatan rata-rata skor pengetahuan peserta dari 55,6 menjadi 83,6 dengan selisih sebesar 28,0 poin. Selain itu, terjadi perubahan perilaku kesehatan, dimana 71% peserta mulai menerapkan pola makan teratur, 64% mengurangi konsumsi makanan pemicu, dan 58% mulai berkonsultasi dengan tenaga kesehatan sebelum menggunakan obat lambung. Kegiatan ini terbukti efektif dalam meningkatkan literasi kesehatan masyarakat serta mendorong perubahan perilaku terkait pencegahan dan pengelolaan dispepsia. Program ini berpotensi untuk dikembangkan secara berkelanjutan dan direplikasi di fasilitas kesehatan lainnya.

Abstract

Tablet hisap ialah sediaan yang dapat memuat satu atau lebih zat aktif dengan pemberian bahan tambahan dan pemanis serta dapat larut di dalam mulut. Tablet hisap memiliki berbagai keuntungan yaitu rasanya manis dan penggunaanya mudah. Penelitian ini mengunakan metode heat and congeling yang memenuhi syarat karakteristik tablet hisap yang baik. Pengujian dilakukan menggunakan serbuk sari buah pepaya (SSP) yang diformulasikan mengunakan pengikat gom arab dan putih telur dengan formula F1 (SSP 3 gram dan gom arab 7,5%), F2 (SSP 3 gram dan gom arab 10%), F3 (SSP 3 gram dan putih telur 5%), dan F4 (SSP 3 gram dan putih telur 7%). Selanjutnya dilakukan penilaian fisik untuk tablet hisap yang mencakup sifat-sifat, konsistensi berat, konsistensi ukuran, tingkat kekerasan, tingkat kerapuhan, dan waktu pembubaran. Berdasarkan formula F1, F2, F3, dan F4 yang dibuat dalam penelitian ini, tablet hisap yang dihasilkan yaitu dari formula F1 dan F2. Evaluasi fisik tablet hisap dari formula F1 dan F2 menghasilkan sifat fisik tablet hisap yang baik serta menghasilkan waktu hancur yang cepat yaitu selama 12 menit dan 10 menit. Menggunakan metode heat and congeling memiliki karakteristik tablet hisap yang baik serta dapat hancur di dalam mulut dalam waktu yang singkat. Hal ini menunjukan bahwa waktu hancur tablet hisap serbuk sari buah pepaya telah memenuhi persyaratan Farmakope Indonesia.

Abstract

Irrational medication use remains a significant public health concern, often stemming from inadequate knowledge of symptom-based drug selection and self-medication practices. This community service activity aimed to enhance public knowledge on rational medication use, specifically focusing on drug classification, reading medication labels, and appropriate dosage instructions. Employing a community-based participatory approach, the activity was conducted at the UPT Puskesmas Teladan and involved 17 purposively selected visitors. The intervention, delivered through interactive lectures, two-way discussions, and medication label simulations supported by educational leaflets, was evaluated using pre-test and post-test instruments. The results demonstrated a notable improvement in participants' understanding, with average scores increasing from 4.94 (low category) to 7.94 (good category) on a 10-point scale. This improvement suggests that a targeted, interactive educational approach is feasible and holds promise for strengthening public knowledge on prudent medication use. This initiative highlights the potential for integrating similar community-based education programs into primary health services to promote safer self-medication behaviors.

Abstract

Consumption of sugar-sweetened packaged beverages among elementary school children continues to increase and may contribute to various health problems, including kidney damage. Limited knowledge regarding the health risks of sugary beverages is one of the factors contributing to high consumption among children. This community service program aimed to improve students’ knowledge about the dangers of packaged sugary drinks on kidney health through health education activities conducted at SD Ar-Ridha, Biru-Biru District, Deli Serdang Regency. The activity was carried out on November 23, 2024, involving 30 elementary school students. The methods used included interactive lectures, discussions, question-and-answer sessions, and evaluation through pre-test and post-test questionnaires. Educational materials covered types of sugary packaged beverages, added sugar content, kidney function, the effects of excessive sugar intake, and kidney disease prevention efforts. The results demonstrated an increase in participants’ knowledge levels after the educational intervention. The percentage of students categorized as having good knowledge increased from 20.0% during the pre-test to 80.0% during the post-test, while the poor knowledge category decreased from 46.7% to 3.3%. These findings indicate that interactive health education methods are effective in improving students’ understanding of the importance of limiting sugary packaged beverage consumption to maintain kidney health from an early age.

Abstract

Amoxicillin is widely used to treat gastric infections; however, conventional oral dosage forms often exhibit rapid gastric emptying and short gastric residence time, which may reduce therapeutic effectiveness. Gastroretentive floating drug delivery systems can prolong gastric retention and provide sustained drug release. This study aimed to develop gastroretentive floating beads of amoxicillin using pectin extracted from breadfruit peel (Artocarpus altilis) and compare their characteristics with those prepared using commercial pectin. Floating beads were prepared by ionotropic gelation using calcium chloride as a crosslinking agent and sodium bicarbonate as a gas-forming agent. The beads were characterized for morphology, floating behavior, entrapment efficiency, and drug release. All evaluations were performed in triplicate (n = 3). The prepared beads exhibited spherical morphology with diameters of 0.24–0.42 mm and weights of 9.26–11.38 mg. Floating lag time ranged from 10–22 seconds, and all formulations remained buoyant for up to 8 hours. Entrapment efficiency ranged from 31–47%, with formulations prepared using commercial pectin showing values of 34–47% and breadfruit peel-derived pectin showing values of 31–43%, with formulation F3 containing 3.5% commercial pectin showing the highest value. In vitro drug release studies demonstrated sustained release of amoxicillin for up to 8 hours, whereas conventional tablets released 64.77% of the drug within 2 hours. Drug release followed the Higuchi model (R² = 0.989–0.996), indicating diffusion-controlled release. These findings demonstrate that breadfruit peel-derived pectin is a promising natural polymer for gastroretentive floating bead formulations and exhibits performance comparable to commercial pectin.
