Abstract

Background: Antenatal care (ANC) is a core strategy for reducing maternal and neonatal mortality through early detection and management of pregnancy complications. However, ANC coverage in Kualuh Hulu District, North Labuhanbatu Regency, remains suboptimal, with fluctuating maternal and infant death rates signaling persistent service gaps. Identifying the local determinants of ANC compliance is essential for designing targeted interventions. Methods: A cross-sectional study was conducted among 153 second- and third-trimester pregnant women recruited through total sampling from four public health centers in Kualuh Hulu District. Data were collected using a structured, validated questionnaire. Analysis included univariate description, bivariate analysis using Chi-square tests, and multivariate binary logistic regression to identify independent determinants. Results: High ANC compliance was observed in 65.4% of participants. Bivariate analysis showed significant associations with six factors: knowledge (p<0.001; OR=4.97), attitude (p<0.001; OR=7.52), decision-making autonomy (p<0.001; OR=6.01), perceived service quality (p=0.002; OR=3.89), availability of staff and facilities (p=0.014; OR=2.90), and transportation and examination costs (p=0.006; OR=2.36). After multivariate adjustment, only knowledge (AOR=5.25; 95% CI: 2.17–12.68), attitude (AOR=4.38; 95% CI: 1.72–11.12), and decision-making autonomy (AOR=4.57; 95% CI: 1.87–11.16) remained significant independent predictors. Conclusions: Knowledge, attitude, and decision-making autonomy are the dominant determinants of ANC compliance in this rural setting. Interventions should prioritize continuous health education, awareness-building counseling, and empowerment of women in health-related decisions, while service quality improvements and cost support serve as essential enabling factors.

Abstract

ABSTRAKRumah sakit memerlukan informasi biaya yang akurat untuk menetapkan tarif layanan, menyusun anggaran, dan mengendalikan biaya operasional. Di RS Pratama Krayan, penetapan tarif rawat inap masih berpotensi menimbulkan distorsi biaya karena belum sepenuhnya menggunakan pendekatan berbasis aktivitas. Penelitian ini menganalisis penerapan Activity Based Costing (ABC) dalam pengendalian biaya operasional rawat inap di RS Pratama Krayan, Kalimantan Utara, tahun 2025. Penelitian menggunakan metode deskriptif kuantitatif dengan pendekatan studi kasus. Data dikumpulkan melalui dokumentasi dan wawancara, lalu dianalisis melalui tahapan ABC, yaitu identifikasi aktivitas, pengelompokan biaya ke dalam cost pool, penetapan cost driver, perhitungan tarif aktivitas, dan pembebanan biaya pada masing-masing kelas rawat inap untuk memperoleh unit cost. Hasil penelitian menunjukkan bahwa penerapan ABC menghasilkan unit cost per hari sebesar Rp 179.494 untuk VIP, Rp 107.782 untuk Kelas I, Rp 88.608 untuk Kelas II, dan Rp 86.881 untuk Kelas III, yang lebih rendah dibandingkan tarif yang berlaku. Selain itu, ditemukan sembilan aktivitas biaya utama dengan total biaya operasional rawat inap sebesar Rp 2.744.000.000, dengan cost driver berupa hari rawat inap, luas ruangan, dan konsumsi listrik. Temuan ini menunjukkan bahwa ABC mampu menghasilkan informasi biaya yang lebih rinci, proporsional, dan transparan. Dengan demikian, ABC dapat menjadi dasar yang lebih akurat dalam evaluasi tarif serta pengendalian biaya operasional rumah sakit.ABSTRACTHospitals need accurate cost information to determine service tariffs, prepare budgets, and control operational costs. At RS Pratama Krayan, inpatient tariff setting may still cause cost distortion because it has not fully applied an activity-based approach. This study analyzed the implementation of Activity Based Costing (ABC) in controlling inpatient operational costs at RS Pratama Krayan, North Kalimantan, in 2025. A quantitative descriptive case study was conducted. Data were collected through documentation and interviews, then analyzed using ABC procedures: activity identification, cost pooling, cost driver determination, activity rate calculation, and cost allocation to each inpatient class to obtain unit cost. The results show that ABC produces unit costs per day of Rp 179,494 for VIP, Rp 107,782 for Class I, Rp 88,608 for Class II, and Rp 86,881 for Class III, all of which are lower than the existing tariffs. In addition, nine major cost activities were identified with total inpatient operational costs of Rp 2,744,000,000, using cost drivers such as inpatient days, room area, and electricity consumption. These findings indicate that ABC provides more detailed, proportional, and transparent cost information. Therefore, ABC can serve as a more accurate basis for tariff evaluation and operational cost control in hospitals.

Abstract

Community Health Centers are required to implement a Community Health Center Information System (SIP) in accordance with Minister of Health Regulation No. 31 of 2019. Medan Teladan Community Health Center has implemented various information systems, one of which is e-Puskesmas, but still found obstacles. This study aims to evaluate the implementation of the Management Information System (SIMPUS) at Medan Teladan Community Health Center using the Human Organization Technology (HOT)-Fit method. This type of research is descriptive qualitative research. The study was conducted at Medan Teladan Community Health Center starting from December 2024 - July 2025. Informants in this study were 5 people consisting of the head of the community health center and medical record officers as key informants, registration officers, SP2TP officers and pharmacy service officers as supporting informants. Data analysis was carried out by data reduction, data presentation and drawing conclusions to identify relevant finding patterns in each aspect of HOT-Fit. The results of the study obtained an evaluation of the implementation using the HOT-Fit method on the human aspect, the user side has problems with input errors, where around 60% of informants reported data input. Organizational aspects, the organizational structure is in place, monitoring and evaluation have been carried out and guidelines for using the information system are in place even though they only adopt those from the Ministry of Health. Technology aspects, the quality of the system is still not optimal because the system has not been integrated with all programs and frequent server disruptions or data bridging failures, differences were also found between drug data in the system and actual stock in the pharmacy warehouse. Net benefit aspects, this system has succeeded in reducing dependence on manual processes and increasing data accuracy, but data integration across all programs is still an obstacle. It is recommended to the Medan City Health Office to encourage the integration of information systems between programs into one platform and ensure server stability. Abstract

Abstrak Keselamatan pasien merupakan indikator utama dalam mutu pelayanan rumah sakit, khususnya di ruang rawat inap yang memiliki risiko tinggi terjadinya kejadian tidak diinginkan. Pelaksanaan keselamatan pasien dipengaruhi oleh berbagai faktor, termasuk komunikasi interpersonal, motivasi, sikap, pengetahuan, dan budaya keselamatan. Penelitian ini bertujuan untuk menganalisis pengaruh faktor-faktor tersebut terhadap pelaksanaan keselamatan pasien serta mengidentifikasi variabel yang paling dominan. Penelitian ini menggunakan metode kuantitatif dengan desain potong lintang. Populasi penelitian adalah seluruh perawat ruang rawat inap di Rumah Sakit Umum Royal Prima Medan, dengan jumlah sampel sebanyak 246 responden yang diambil menggunakan teknik total sampling. Data dikumpulkan melalui kuesioner dan dianalisis secara univariat, bivariat menggunakan uji Chi-Square, serta multivariat. Hasil penelitian menunjukkan bahwa komunikasi interpersonal, motivasi, sikap, pengetahuan, dan budaya keselamatan pasien berpengaruh signifikan terhadap pelaksanaan keselamatan pasien (p < 0,05). Analisis multivariat menunjukkan bahwa sikap merupakan faktor yang paling dominan, dengan pengaruh yang lebih kuat dibandingkan variabel lainnya dalam menjelaskan pelaksanaan keselamatan pasien. Penelitian ini menyimpulkan bahwa seluruh variabel independen berpengaruh signifikan, dengan sikap sebagai faktor utama yang perlu mendapat perhatian dalam upaya peningkatan keselamatan pasien. Kata Kunci: Keselamatan Pasien; Penerapan Keselamatan Pasien; Komunikasi Antarpersonal; Motivasi; Budaya keselamatan Abstract Patient safety is a key indicator of healthcare quality, particularly in inpatient units where the risk of adverse events is relatively high. The implementation of patient safety is influenced by several factors, including interpersonal communication, motivation, attitude, knowledge, and safety culture. This study aimed to analyze the influence of these factors on patient safety implementation and to identify the most dominant variable. A quantitative method with a cross-sectional design was employed. The study population consisted of all inpatient nurses at Royal Prima General Hospital Medan, with a total sample of 246 respondents selected using total sampling. Data were collected through questionnaires and analyzed using univariate analysis, bivariate analysis with the Chi-Square test, and multivariate analysis. The results showed that interpersonal communication, motivation, attitude, knowledge, and safety culture had a significant effect on patient safety implementation (p < 0.05). Multivariate analysis revealed that attitude was the most dominant factor, demonstrating a stronger influence compared to other variables in explaining patient safety implementation. In conclusion, all independent variables significantly affect patient safety implementation, with attitude as the key factor that should be prioritized to improve patient safety. keywords: Patient Safety; Patient Safety Implementation; Interpersonal Communication; Motivation; Safety Culture

Abstract

This study aims to analyze the effect of the Government’s Free Nutritious Food Program (MBG) on changes in learning concentration and nutritional status among students at SDN No. 075062 Doli-Doli, Mandrehe District, West Nias Regency. This research used a quantitative, quasi-experimental design with a pretest and posttest (no control group). The population consisted of 168 students, with a sample of 128 students selected using the Slovin formula and purposive sampling. Learning concentration was measured using a structured test, while nutritional status was assessed using Body Mass Index (BMI) based on anthropometric measurements. Data were analyzed using univariate methods and the Wilcoxon signed-rank test due to non-normal distributions. The results showed a significant increase in learning concentration (from pretest 30.19 to posttest 34.91; p < 0.001) and nutritional status (from pretest 15.33 to posttest 16.17; p < 0.001). These findings indicate that the MBG program significantly improves students' cognitive performance and nutritional status. The study concludes that school-based nutrition interventions are effective in supporting both health and educational outcomes, particularly in disadvantaged areas.

Abstract

This study evaluates the effectiveness of internal verifiers at Muhammadiyah Hospital North Sumatra in minimizing delayed inpatient claims submitted to BPJS Kesehatan, Indonesia's national health insurance agency. A mixed-methods sequential explanatory design was employed, combining quantitative data from 2020 (prior to verifier recruitment) and 2022 (post-recruitment) with qualitative insights from interviews with internal verifiers, coders, and other stakeholders, as well as a review of claim files processed by the hospital's BPJS Unit. Interview protocols were adapted from a 2018 study at Koja Hospital, North Jakarta, and data were analyzed using the Miles and Huberman approach, with findings integrated across methods. Source triangulation ensured validity and reliability. Results showed that internal verifiers significantly reduced the proportion of delayed claims despite an overall increase in submissions, with the highest rate of pending claims dropping from 25.6% in 2020 to 17.9% in 2022. Of the 2022 delays, 31.2% were due to BPJS Kesehatan system errors or confirmation processes, indicating that only two-thirds stemmed from hospital-related issues. Qualitative analysis identified key contributors to delays, including diagnostic coding errors, inconsistent understanding among BPJS verifiers, incomplete documentation, limited staff knowledge, and human resource inaccuracies. The presence of internal verifiers improved claim management and hospital revenue by increasing eligibility and accelerating approvals. To further optimize claim processing, strategies such as enhancing coding accuracy, improving staff competency, ensuring documentation completeness, and adopting electronic systems with supportive incentives are recommended. Abstract

Pendahuluan: Secara global, data WHO tahun 2022 menunjukkan bahwa kanker payudara merupakan tantangan kesehatan yang serius dengan 2,3 juta kasus baru dan 685.000 kematian. Di Indonesia, angka ini juga sangat mengkhawatirkan dengan catatan 68.858 kasus baru atau sekitar 16,6% dari total seluruh jenis kanker di tanah air. Tujuan: Untuk mengetahui pengaruh intervensi promosi kesehatan tentang SADARI sebagai deteksi dini kanker payudara terhadap pengetahuan sikap dan perilaku pada wanita usia subur di wilayah kerja Puskesmas Matsum Kota Medan. Hasil: Hasil studi menunjukkan adanya perbedaan rerata yang nyata antara kelompok eksperimen dan kelompok kontrol dalam hal tingkat pengetahuan, sikap, serta perilaku terkait metode SADARI. Secara spesifik, intervensi promosi kesehatan terbukti efektif dalam meningkatkan pemahaman responden, membentuk cara pandang yang lebih positif, dan mendorong perubahan perilaku konkret dalam melakukan pemeriksaan payudara sendiri secara mandiri setelah diberikan edukasi. Kesimpulan: Penelitian di wilayah kerja Puskesmas Matsum Kota Medan menyimpulkan bahwa pemberian promosi kesehatan memberikan dampak yang signifikan terhadap upaya deteksi dini kanker payudara pada wanita usia subur.

Pada RS Pratama Krayan, penetapan tarif rawat inap masih berisiko menimbulkan distorsi biaya karena belum sepenuhnya menggunakan pendekatan berbasis aktivitas.Penelitian ini bertujuan menganalisis penerapan Activity Based Costing (ABC) dalam pengendalian biaya operasional rawat inap di RS Pratama Krayan, Kalimantan Utara, Tahun 2025.Penelitian menggunakan metode deskriptif kuantitatif dengan pendekatan studi kasus. Data dikumpulkan melalui dokumentasi dan wawancara, kemudian dianalisis dengan tahapan ABC, yaitu identifikasi aktivitas, pengelompokan biaya ke dalam cost pool, penetapan cost driver, perhitungan tarif aktivitas, dan pembebanan biaya ke masing-masing kelas rawat inap untuk memperoleh unit cost. Hasil penelitian menunjukkan terdapat sembilan aktivitas biaya utama dengan total biaya operasional rawat inap sebesar Rp 2.744.000.000. Tarif aktivitas dihitung menggunakan cost driver berupa hari rawat inap, luas ruangan, dan konsumsi listrik. Perhitungan ABC menghasilkan unit cost rawat inap per hari sebesar Rp 179.494 pada kelas VIP, Rp 107.782 pada Kelas I, Rp 88.608 pada Kelas II, dan Rp 86.881 pada Kelas III. Seluruh hasil tersebut lebih rendah dibandingkan tarif yang berlaku, dengan penurunan sebesar 48,7% pada VIP, 46,1% pada Kelas I, 50,8% pada Kelas II, dan 42,1% pada Kelas III. Penerapan ABC mampu menghasilkan informasi biaya rawat inap yang lebih rinci, proporsional, dan transparan, sehingga dapat digunakan sebagai dasar evaluasi tarif dan pengendalian biaya operasional rumah sakit.

Abstract

The impact of errors in storing medical records causes delays for officers in searching for medical record documents so that services at the polyclinic are hampered. It is necessary to control the incidence of incorrect insertion of medical record documents so that the incidence of incorrect insertion of medical record documents can be reduced. Electronic Medical Record (RME) is a form of health information service that is computerized and recorded. Health service facilities that implement RME in an effort to improve the quality and quality of services. This research is a qualitative research, case study design which aims to evaluate the timing of providing electronic medical record documents for outpatients at Murni Teguh Hospital. Determination of research subjects using purposive sampling technique. The research subjects consisted of the Director of Murni Teguh Hospital, the head of the Medical Record Installation as the main informant, the Patient Registration Officer, and 3 medical record staff as triangulation informants. In collecting data, researchers conducted in-depth interviews, direct observation and document studies. Data analysis was carried out using interactive models, data reduction processes, verification and drawing conclusions. In this study, it was found that the time required to provide electronic medical record documents for outpatients at Murni Teguh Hospital was a minimum of 4 minutes, a maximum time of 9 minutes with an average time needed to provide electronic medical record documents was 6.6 minutes. Obstacles encountered are errors during use, as well as HR who still do not understand the use of electronic medical record systems. By providing electronic medical record documents, the work of the patient medical record department becomes more efficient, makes it easier for officers to search for patient medical records and the benefits that patients get are shorter waiting times.

Abstract

Health service is a concrete form of public service. Mobile JKN is an application made by BPJS Health to make it easier for participants to obtain services. The aim of the research is to evaluate health insurance services through mobile JKN at the Dental Clinic of Haj Hospital Medan in 2022. The research method is qualitative and quantitative design (Mix Methode). The research was conducted at the Dental Polyclinic at Haji Hospital Medan in May 2023. A qualitative research sample of 10 participants came to dental polo, a quantitative sample of 100 respondents came to dental polo. Collecting data with questionnaires and in-depth interviews. The results of the study, the majority of respondents who came to the dental polyclinic were aged 36-45 years (late adulthood) totaling 26 respondents (26%), health insurance services through mobile JKN are not good enough (20-55) as many as 55 respondents (55%). Participants who haven't used mobile JKN don't even know about mobile, don't know the convenience with mobile JKN, don't know the benefits of mobile JKN, participants are not interested in using mobile JKN because they haven't received education about mobile JKN. Participants who use mobile JKN assume that the JKN car program is an application that is used to register for health services without having to come to the hospital, the JKN mobile application simplifies and reduces queues at the hospital so that participants also assume that mobile JKN helps in health services, using mobile JKN makes it easier and saves time as well as efficiently and effectively.
