Abstract

Latar Belakang. Penyakit diabetes mellitus tipe 2 merupakan penyakit kronis dan silent, dimana pankreas tidak cukup dapat menghasilkan insulin sesuai kebutuhan tubuh serta tanda dan gejala awalnya tidak disadari oleh partisipan. Kerusakan saraf tepi (neuropati), dapat dengan mudah sekali terjadinya luka yang tidak di rasakan oleh partisipan. Wound care modern dengan menggunakan madu dan chitosan adalah salah satu intervensi proses akselerasi penyembuhan pada perawatan luka diabetik.Objektif. Tujuan penelitian ini adalah untuk melihat efektifitas dari penggunaan madu atau chitosan pada diabetisi dengan luka wagner II tendon exposed.Metode. Metode penelitian ini menggunakan desain desain penelitian  kuantitatif-research menggunakan desain Quasi Eksperimen dengan pengamatan secara aplikatif pre dan post perawatan. Sampel yang terlibat yaitu para partisipan dengan ulkus diabetik wagner II sebanyak 18 partisipan.Hasil. Setelah dilakukan uji statistik dengan bantuan program spsss diperoleh nilai p-value = 0,007 dengan jumlah partisipan 18 sehingga p-value < α (0,007 < 0,05) artinya H0 di tolak dan H1 diterima, maka dapat diartikan bahwa ada lebih efektif penggunaan chitosan dalam perawatan luka modern terhadap akselerasi penyembuhan fase proliferasi pada partisipan diabetik wagner II (tendon exposed) di Klinik perawatan luka Bekasi.Implikasi Klinis. Implikasi klinis dari penelitian ini Chitosan dinilai sebagai Dressing Luka yang Efektif untuk Diabetes Melitus dimana penelitian ini menunjukkan bahwa chitosan merupakan dressing luka yang efektif untuk mempercepat penyembuhan luka pada pasien diabetes mellitus Wagner II (Tendon exposed).Kata Kunci. Wound Care Modern , Madu, Chitosan, Akselerasi

Abstract

ABSTRAKHIV/AIDS merupakan masalah kesehatan yang menyebabkan kekhawatiran di berbagai belahan dunia karena virus HIV menyerang sel darah putih manusia yang menyebabkan turunnya kekebalan tubuh sehingga mudah terserang penyakit. Pasien HIV/AIDS mendapatkan pengobatan Antiretroviral (ARV). Pasien HIV/AIDS harus patuh dalam menjalani pengobatan ARV, karena akan terjadi resistensi obat pada pasien HIV/AIDS yang tidak patuh dalam menjalani pengobatan. Kepatuhan dipengaruhi beberapa faktor, seperti pengetahuan dan dukungan keluarga. Oleh karena itu tujuan penelitian ini untuk mengetahui Hubungan Pengetahuan dan Dukungan Keluarga dengan Kepatuhan dalam Menjalankan Pengobatan Antiretroviral (ARV) pada pasien HIV/AIDS di Yayasan Pelita Ilmu Jakarta. Desain penelitian ini adalah penelitian deskriptif kuantitatif dengan desain cross-sectional. Populasi pada penelitian ini sebanyak 105 orang dan sample sebanyak 91 orang dengan teknik pengambilan sample yaitu purposive sampling. Alat ukur yang digunakan pada penelitian ini yaitu kuesioner. Uji statistik yang digunakan yaitu uji chi square dengan tingkat kemaknaan α=0.05. Hasil  penelitian ini menunjukan bahwa tidak ada hubungan antara pengetahuan dengan kepatuhan dalam menjalankan pengobatan ARV (p value=0.079) dan ada hubungan antara dukungan keluarga dengan kepatuhan dalam menjalankan pengobatan ARV (p value = 0.004). Diharapkan pelayanan kesehatan memberikan informasi dan edukasi tentang pengobatan ARV dengan melibatkan keluarga pasien.Kata kunci: Pengetahuan, dukungan keluarga, kepatuhan ARVABSTRACTHIV/AIDS is a health problem that causes concern in various parts of the world because the HIV virus attacks human white blood cells which causes a decrease in the body's immunity so that it is susceptible to disease. HIV/AIDS patients receive antiretroviral (ARV) treatment. HIV/AIDS patients must be compliant in undergoing ARV treatment, because there will be drug resistance in HIV/AIDS patients who are not compliant in undergoing treatment. Compliance is influenced by several factors, such as knowledge and family support. Therefore, the aim of this study was to determine the relationship between knowledge and family support and adherence in carrying out antiretroviral treatment (ARV) in HIV/AIDS patients at the Pelita Ilmu Jakarta Foundation. The research design is a quantitative descriptive study with a cross-sectional design. The population in this study were 105 people and a sample of 91 people with a sampling technique that is purposive sampling. Measuring tool used in this study is a questionnaire. The statistical test used is the chi square test with a significance level of α=0.05. The results of this study indicate that there is no relationship between knowledge and adherence in carrying out ARV treatment (p value = 0.079) and there is a relationship between family support and adherence in carrying out ARV treatment (p value = 0.004). It is hoped that health services will provide information and education about ARV treatment by involving the patient's family. Keywords: Knowledge, family support, family adherence

Abstract

One of the pharmacologically controlled blood sugar levels in T2DM patients is by giving the correct insulin injection including the dose, method, time, and location, the success parameters of insulin injection include the injection location, injection procedure, and insulin management where everything will depend on glycemic control. Complications that appear at the injection site include bruising, pain, and swelling that indicate the occurrence of Lipohypertrophy (LH) Lipodystrophy is a form of lipohypertrophy (LH) which is a common complication that often occurs in T2DM patients with insulin injection therapy. This lipodystrophy will also be one of the factors that cause other T2DM complications. Method: the design in this study is One-Group Pre-test-Post-test Design, with a sampling of 14 DM patients who receive insulin injection therapy. The research period is from May to July 2024. Education using injection pad sets and educational videos on insulin injection techniques, observation of skills, and signs of lipodystrophy using questionnaires. Results: The results showed a decrease in the average blood sugar level before and after the educational intervention with a P-value of <0.001, the average patient's skill insulin injection showed an increase with a P-value of <0.001, the incidence of lipodystrophy showed that out of three areas of insulin injection, there was one location where lipodystrophy was found, namely in the arm area. With a p-value of 0.041. Conclusion: in this study, modifications are needed in the use of media or techniques in educating DM patients on insulin injection. Education can be repeated in both new and old patients on insulin. Patients need to be explained how to independently check for signs of lipodystrophy or changes in the injection site area

Abstract

Aims: Chronic Kidney Disease (CKD) poses a significant global health challenge, with an increasing number of kidney transplantations in Indonesia. The success of kidney transplants hinges on adherence to immunosuppressant medications to prevent organ rejection. Non-adherence among post-transplant patients can lead to infections, graft loss, or a return to dialysis. This study evaluated the impact of the TEMAN SETIA program on adherence to immunosuppressant medications among post-kidney transplant patients at Dr. Cipto Mangunkusumo National Referral Hospital (RSCM) in Jakarta. Methods: A pre-post-test design with a control group was utilized. The sample included 28 post-transplant patients meeting inclusion criteria. The TEMAN SETIA program provided patient mentoring to improve adherence. Adherence was assessed before the intervention, one month after, and two weeks post-intervention using a validated questionnaire. Data were analysed using t-tests and repeated measures ANOVA. Results: The control and intervention groups were comparable in demographic and clinical characteristics, though the intervention group had more non-working participants. The program significantly improved adherence (Cohen’s d = 1.97) and knowledge (Cohen’s d = 1.99). While family support increased in both groups, differences were not significant (Cohen’s d = 1.02). A significant group-by-time interaction (F (1.246, 26) = 7.394, p = 0.007, η² = 0.221) demonstrated greater adherence improvements over time in the intervention group. Discussion: The findings demonstrate that the TEMAN SETIA program is an effective intervention for enhancing medication adherence and knowledge among post-transplant patients. The structured mentoring approach provided patients with the necessary support and education to improve their adherence behaviors. Although family support increased in both groups, the changes were not statistically significant, highlighting the need for further exploration of the role of familial involvement in adherence programs Conclusion: The TEMAN SETIA program effectively enhanced medication adherence and knowledge in post-transplant patients. This structured intervention offers a promising strategy to improve outcomes and reduce risks of organ rejection.

Abstract

Type 2 diabetes mellitus (T2DM) is often accompanied by sleep disturbances that can worsen glycemic control and increase the risk of complications. The combination of lavender aromatherapy and sleep hygiene is a non-pharmacological intervention that can potentially improve sleep quality in T2DM patients. This study aims to evaluate the effectiveness of lavender aromatherapy and sleep hygiene in improving sleep quality in T2DM patients. The study used a quasi-experimental design with a pre-test and post-test approach. The sample consisted of 15 inpatients with T2DM at PKU Muhammadiyah Gamping Hospital, who were selected using an accidental sampling method. Interventions included lavender aromatherapy inhalation and sleep hygiene implementation, including ablution, prayer, sleeping position, and lighting. Data were analyzed using paired t-tests to see changes in Pittsburgh Sleep Quality Index (PSQI) scores before and after the intervention. The mean PSQI score of the patients decreased significantly from 8.67 before the intervention to 5.40 after the intervention (p=0.001), indicating a significant improvement in sleep quality. The combination of lavender aromatherapy and sleep hygiene effectively improves sleep quality in T2DM patients. This intervention can be adopted in nursing practice to manage sleep disorders holistically.

Abstract

Kecemasan merupakan gejala yang sering muncul pada pasien Acute Coronary Syndrome yang akan menjalani Percutaneous Coronary Intervention. Kecemasan yang tidak tertangani dengan baik dapat menimbulkan perburukan kondisi dan penundaan prosedur kateterisasi jantung. Penelitian ini bertujuan untuk mengetahui pengaruh terapi murotal Al-qur'an terhadap kecemasan pasien yang akan menjalani PCI. Metode yang digunakan dalam studi melalui pendekatan quasi eksperimental. Jumlah sampel sebanyak 30 responden, yaitu pasien ACS yang akan mendapatkan terapi PCI. Teknik pengambilan sampel menggunakan purposive sampling sesuai dengan kriteria inklusi dan eksklusi yang telah ditetapkan. Kecemasan diukur menggunakan Visual Analogue Scale (1-10). Data dianalisis menggunakan uji Wilcoxon. Hasil didapatkan terdapat pengaruh pemberian terapi murotal Al-quran terhadap kecemasan (p<0.05). Adanya perbedaan yang signifikan pada skor kecemasan sebelum dan setelah diberikan intervensi. Kesimpulan, terapi murotal merupakan terapi yang mudah dilaksanakan serta secara signifikan dapat menurunkan tingkat kecemasan pada pasien yang akan menjalani PCI.

Abstract

Stress and fatigue are common complaints among hemodialysis patients, significantly affecting their quality of life during long-term therapy. This study aimed to evaluate the effectiveness of a modified Mindfulness-Based Stress Reduction (MBSR) intervention on stress and fatigue in patients undergoing hemodialysis. The study adopted a quasi-experimental pretest-posttest control group design and employed simple random sampling. A total of 32 respondents participated, with 16 in each group. Stress was measured using the Perceived Stress Scale (PSS-10) and fatigue using the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F 40). The modified MBSR intervention was conducted for 30 minutes per session. Data were analyzed using paired t-tests and independent t-tests. The results showed significant differences in stress (MD = -13.12, t = -4.748, p < 0.001) and fatigue levels (MD = 7.34, t = -3.865, p = 0.001) between the intervention and control groups. These findings indicate that the modified MBSR therapy was effective in reducing stress and fatigue, with a p-value < 0.05 indicating statistical significance. Therefore, this modified MBSR therapy is a promising non-pharmacological nursing intervention that can be integrated into nursing care for chronic kidney failure patients experiencing stress and fatigue during hemodialysis.

Abstract

ABSTRACT Coronary Heart Disease (CHD) is a health condition that is not caused by infectious factors and is a serious concern in society, with the number of cases continuing to increase both globally and in Indonesia. Someone who experiences stress in a CHD condition tends to experience various symptoms such as anxiety, disappointment, difficulty feeling calm after experiencing irritation, and is susceptible to feeling angry or offended by small things. People with CHD who experience stress also need a longer time to feel relaxed or calm again, the impact is very detrimental to heart health. From a behavioral perspective, high levels of stress hormones also make individuals with CHD experience difficulty sleeping. This study aims to apply Evidence-Based Nursing Practice (PKBB) to understand the impact of applying Mind-body Therapy on anxiety levels in CHD patients. Use descriptive tests for continuous variables such as age, while binomial tests are used for nominal and ordinal variables such as gender and education level. To calculate the number of respondents, researchers used G*Power Version 3.1 software. In G*Power, researchers use family tests: t-test, statistical tests: the average difference between two dependent means (matched pairs), type of power analysis: a priori, calculating the required sample size - keeping in mind alpha, power, and effect size, with two-tailed input parameters, total effect size of 0.78, alpha error probability of 0.05, and power of 0.80. The results showed 15 respondents. Mind-body therapy has been proven to be successful in reducing anxiety levels in CHD patients. Mind-Body Therapy is carried out for 8 weeks, 6 times a week, with a duration of 30 minutes in a quiet environment. Keywords: Evidence Based Practice Mind-Body Therapy, Mindfullness Based Stress Reduction, Ansietas, Coronary Heart Disease (CHD).  ABSTRAK Penyakit Jantung Koroner (PJK) merupakan suatu kondisi kesehatan yang bukan disebabkan oleh faktor penularan dan menjadi perhatian serius dalam masyarakat, dengan jumlah kasus yang terus meningkat baik secara global maupun di Indonesia. Seseorang yang mengalami stres dalam kondisi PJK cenderung mengalami berbagai gejala seperti kecemasan, kekecewaan, sulit merasa tenang setelah mengalami kekesalan, serta rentan terhadap perasaan marah atau tersinggung oleh hal-hal kecil. Orang dengan PJK yang mengalami stres juga memerlukan waktu yang lebih lama untuk dapat kembali merasa rileks atau tenang, dampaknya sangat merugikan bagi kesehatan jantung. Dari segi perilaku, tingginya hormon stres juga membuat individu dengan PJK mengalami kesulitan dalam tidur. Penelitian ini bertujuan menerapkan Praktik Keperawatan Berbasis Bukti (PKBB) guna memahami dampak penerapan Terapi Mind-body terhadap tingkat kecemasan pada pasien PJK.  Menggunakan uji deskriptif untuk variabel kontinu seperti usia, sementara uji binomial digunakan untuk variabel nominal dan ordinal seperti jenis kelamin dan tingkat pendidikan. Untuk menghitung jumlah responden, peneliti memanfaatkan perangkat lunak G*Power Versi 3.1. Pada G*Power, peneliti menggunakan uji keluarga: t-test, uji statistik: perbedaan rata-rata antara dua rata-rata tergantung (matched pairs), jenis analisis daya: a priori, menghitung ukuran sampel yang diperlukan - dengan mengingat alpha, daya, dan ukuran efek, dengan parameter input berupa dua ekor, ukuran efek total 0,78, alpha error probabilitas 0,05, dan daya 0,80. Hasilnya menunjukkan 15 responden. Terapi Mind-body terbukti berhasil mengurangi tingkat kecemasan pada pasien PJK. Pelaksanaan Terapi Mind-Body dilakukan selama 8 minggu, 6 kali seminggu, dengan durasi 30 menit di lingkungan yang hening. Kata Kunci: Praktek Berbasis Bukti Terapi Pikiran-Tubuh, Pengurangan Stres Berbasis Perhatian, Ansietas, Penyakit Jantung Koroner (PJK).

Abstract

ABSTRACT The incidence of stroke throughout the world is currently becoming increasingly worrying, not only as the main cause of death, but also ranked second as a cause of global disability with more than 13 million new cases each year. A number of intervention measures have been carried out to overcome the physical limitations of the upper extremities in post-stroke patients, such as training the paralyzed arm, carrying out arm exercises that focus on the disorder experienced, applying functional stimulation using electricity, carrying out rehabilitation assisted by robots, and undergoing bilateral arm training. However, all of these interventions require a high level of intensity as well as “1 to 1” interaction with a therapist over several weeks, which makes it difficult for patients to maintain an intensive level of therapy. To carry out mirror therapy with the aim of increasing muscle strength in parts of the body that experience weakness due to stroke, especially upper extremity hemiparesis. This research is a cross-sectional study. The study population consisted of stroke patients with upper extremity hemiparesis who were undergoing inpatient treatment on the seventh floor of the National Brain Center Hospital. The sample size was calculated taking into account the effect size of 0.67, significance level of 0.05, power of 80%, and risk of dropping out of 20%. Thus, the number of participants in this study was 24 patients. Mirror therapy has been proven to be successful in increasing muscle strength in stroke patients with upper limb hemiparesis at PON Hospital. This therapy is carried out for 30 minutes, five days a week, for three weeks (8-31 May 2023). Mirror therapy can be carried out easily by nurses and involves the patient's family. Mirrors are available in each treatment room to make carrying out therapy easier. The study findings showed an increase in muscle strength in the upper extremities, confirming the effectiveness of mirror therapy in stroke patients with hemiparesis. Keywords Evidance Based Practice Mirror Therapy, Extremity Motor Function, Stroke Patients.  ABSTRAK Kejadian stroke di seluruh dunia saat ini menjadi semakin mengkhawatirkan, tidak hanya sebagai penyebab utama kematian, tetapi juga menduduki peringkat kedua sebagai penyebab kecacatan global dengan lebih dari 13 juta kasus baru setiap tahunnya. Sejumlah tindakan intervensi telah dilakukan untuk mengatasi keterbatasan fisik ekstremitas atas pada pasien paska stroke, seperti melatih lengan yang mengalami paralisis, melakukan latihan lengan yang difokuskan pada gangguan yang dialami, menerapkan stimulasi fungsi menggunakan listrik, melakukan rehabilitasi dibantu oleh robot, dan menjalani pelatihan lengan bilateral. Namun, semua intervensi tersebut memerlukan tingkat intensitas yang tinggi serta interaksi "1 to 1" dengan terapis selama beberapa minggu, yang membuat tingkat intensif dari terapi yang dapat dijalankan oleh pasien menjadi sulit. Untuk melaksanakan terapi cermin dengan tujuan meningkatkan kekuatan otot pada bagian tubuh yang mengalami kelemahan akibat stroke, khususnya pada hemiparesis ekstremitas atas. Penelitian ini merupakan studi potong lintang. Populasi penelitian terdiri dari pasien stroke dengan hemiparesis ekstremitas atas yang sedang menjalani perawatan rawat inap di lantai tujuh Rumah Sakit Pusat Otak Nasional. Jumlah sampel dihitung dengan memperhatikan effect size 0,67, tingkat signifikansi 0,05, power 80%, dan risiko drop out sebesar 20%. Sehingga, jumlah partisipan dalam penelitian ini adalah sebanyak 24 pasien. Terapi cermin terbukti berhasil meningkatkan kekuatan otot pada pasien stroke dengan hemiparesis ekstremitas atas di RS PON. Terapi ini dilakukan selama 30 menit, lima hari seminggu, selama tiga minggu (8-31 Mei 2023). Pelaksanaan terapi cermin dapat dilakukan dengan mudah oleh perawat dan melibatkan keluarga pasien. Cermin tersedia di setiap ruang perawatan untuk mempermudah pelaksanaan terapi. Temuan penelitian menunjukkan adanya peningkatan kekuatan otot pada ekstremitas atas, menegaskan efektivitas terapi cermin pada pasien stroke dengan hemiparesis. Kata Kunci: Terapi Cermin Praktek Berbasis Bukti, Fungsi Motorik Ekstremitas, Pasien Stroke
