Abstract

Environmental sanitation encompasses housing, sewage disposal, and clean water supply, all crucial to health. A healthy environment sustains the ecosystem, while a poor one leads to sanitation issues and disease spread. The STBM program in Indonesia aims to empower communities to reduce open defecation, though challenges in improving sanitation access persist. This study explores the factors influencing open defecation and provides recommendations for enhancing sanitation and environmental health. The research, conducted in Kampung Dalam Village, Bilah Hulu Sub-district, Labuhan Batu Regency, North Sumatra, began in January 2023. An initial survey found that 96 households lacked healthy latrines. The study used a quantitative cross-sectional design with a sample of 50 households, determined using the Lameshow formula and selected through Simple Random Sampling (SRS). Independent variables included knowledge, attitude, economic status, and facilities (healthy restrooms), while the dependent variable was open defecation behavior. Data collection involved questionnaires and direct interviews with respondents, analyzed using univariate and bivariate methods and the Chi-Square test with a 95% confidence level. The Chi-Square test results showed significant relationships between open defecation behavior and knowledge (p=0.019), attitude (p=0.001), economic status (p=0.009), and facilities (p=0.002) with p-values ≤ 0.05. The study concluded that while knowledge and attitude are linked to open defecation behavior, they do not always directly influence it, suggesting other factors like environment and facilities also play a role. Lower economic status was more often associated with open defecation, and better facilities and access contributed to reduced open defecation behavior.

Abstract

Food consumption is one of the direct causes of nutritional status. The Ministry of National Development Planning in 2019 reported that almost half of the population or 45.7 percent of the population is very poor in energy allowances and 36.1 percent of the population is low in protein allowances. Meanwhile, the North Sumatra Provincial Health Office in 2019 presented the results of monitoring nutritional status which showed the three districts with the highest stunted of children under five, namely Gunung Sitoli, West Nias, and Samosir. The districts with the most underweight of children under five are West Nias, Central Tapanuli, and North Nias. The purpose of the study was to determine the relationship between food consumption and the nutritional status of children under five at Pasar Lahewa Village, Lahewa Sub-District, in North Nias District, which was selected purposively. This type of research is descriptive analytic, cross sectional design. The food consumption data of sixty children under five were collected using the 24-Hour Food Recall Method for three non-consecutive days, anthropometric nutritional status with Body Weight Index according to Height or Body Length. The results showed that the largest contribution of energy and protein samples came from rice, powdered milk, chicken, fish, eggs, tofu, tempeh, potatoes, noodles, and biscuits. Most of the samples were categorized as well consumption and normal nutritional status. The Chi-Square test produces a p-value of 0.003 (p<α (0.05) which means Ho is rejected so that it can be stated that there is a relationship between food consumption and the nutritional status of children under five at Pasar Lahewa Village, Lahewa District in North Nias District. The results are expected can be used by the leadership and staff of the Lahewa District Health Center to plan nutrition improvement programs. Similar research needs to be carried out in other villages within the working area of the Lahewa District Health Center, to obtain and enrich data in planning program to improve food consumption and nutritional status of children under five.

Abstract

Background: Type 2 Diabetes Mellitus (T2DM) is a global health challenge with a rising prevalence, particularly among the elderly. Its management requires a holistic approach, including medical nutrition therapy. Red dragon fruit (Hylocereus polyrhizus) is rich in fibre and antioxidants, which may help regulate glucose metabolism. This study aimed to evaluate the efficacy of red dragon fruit juice on reducing random blood glucose levels in elderly patients with T2DM. Methods: A quasi-experimental study with a pretest-posttest control group design was employed. Thirty-two elderly T2DM patients within the working area of Karo District General Hospital were selected via purposive sampling and allocated into an intervention group (n=16) and a control group (n=16). The intervention group consumed 250 ml of red dragon fruit juice once daily for seven consecutive days, while the control group received routine care only. Random blood glucose levels were measured using a glucometer before and after the intervention. Data were analysed using descriptive statistics and the Wilcoxon Signed Ranks Test. Results: At baseline, the mean random blood glucose was 231.88 ± 99.37 mg/dL in the intervention group and 250.31 ± 63.32 mg/dL in the control group. Post-intervention, the intervention group exhibited a significant decrease to 226.25 ± 96.99 mg/dL (p=0.017). Conversely, the control group showed a non-significant increase to 252.75 ± 63.66 mg/dL (p=0.027). Rank analysis indicated that 75% of subjects in the intervention group experienced a decrease in blood glucose, whereas 81.25% of subjects in the control group experienced an increase. Conclusion: The administration of 250 ml of red dragon fruit juice daily for seven days was effective as an adjuvant therapy in reducing random blood glucose levels in elderly patients with Type 2 Diabetes Mellitus. Red dragon fruit may be considered a safe and affordable complementary nutritional intervention for T2DM management in the elderly population.

Abstract

Background: Clean and Healthy Living Behavior (PHBS) in schools is a government program implemented through community health centers to improve public health status. Elementary school-aged children are a vulnerable group for various diseases associated with PHBS. This study aimed to identify factors associated with PHBS among students at public elementary schools in Banda Aceh City. Methods: A quantitative study with a cross-sectional design was conducted from November 2023 to January 2024. The population comprised 192 students, with a sample of 85 fourth and fifth grade students selected through purposive sampling. Data were collected using questionnaires and checklists and analyzed using the Chi-Square test. Results: Results showed that most respondents were male (51.8%) and aged 5-11 years (61.2%). More than half of respondents (56.5%) demonstrated poor PHBS, 65.9% had low knowledge levels, 45.9% reported unavailable PHBS facilities and infrastructure, and 60.0% perceived teacher roles as poor. Bivariate analysis revealed significant associations between knowledge level (p=0.024) and availability of facilities and infrastructure (p=0.004) with PHBS; however, no significant association was found between teacher role and PHBS (p=0.562). Conclusion: In conclusion, knowledge level and availability of facilities and infrastructure were significantly associated with PHBS among public elementary school students in Banda Aceh City. It is recommended that community health centers conduct regular PHBS education in schools and that schools complete necessary facilities while strengthening supervision of student PHBS implementation.
