Abstract

Abstract

One of the points of the HIV AIDS prevention strategy is to increase the availability and affordability of antiretroviral treatment in hospitals or health centers. The emptiness in antiretroviral drugs is a phenomenon that commonly occurs in hospitals or health centers at the district or city level, and Manggarai Regency is one of the districts that experienced this condition from 2013 to 2017. The aim of this study is to explain the situation and the causes of the absence of antiretroviral drugs with a logistic management approach. This type of research is qualitative research. The informants of this study amounted to 8 people from RSUD dr. Ben Mboi, NTT Provincial Health Office, KPAP NTT and KPAD Manggarai. Data collection will take place from February to March 2018. The results are: Antiretroviral drug planning using consumption methods; budgeting and procurement of antiretroviral drugs by the Indonesian Ministry of Health; the storage of antiretroviral drugs is carried out according to the current standards for the storage of medicines; distribution of antiretroviral drugs from the provinces with expeditions and from hospitals to PLWHA with individual prescriptions that are represented by one person; the elimination of antiretroviral drugs was never performed; control of antiretroviral drugs that are adapted to the schedule of taking medication; cause of the absence of antiretroviral drugs due to the lack of specialized staff administering antiretroviral drugs in hospital pharmacies. Management of antiretroviral drugs in Manggarai Regency runs accordingly the logistics management cycle, but there are still some issues that accompany this process. Issues in question include insufficient human resources in terms of quantity and quality, non-procedural implementation and a weak coordination system. It is recommended that improvements be made to the logistical management process of antiretroviral drugs in hospitals, as well as strengthening the coordination and evaluation system of all parties involved in the logistics management of antiretroviral drugs.

Abstract

The immunization program has a short-term goal, namely preventing the spread of disease. The achievement of 80 percent UCI in all districts is one of the benchmarks for the success of the immunization program. According to health profile, Kupang Regency in 2018, it has a coverage rate of 37.9%, with Batakte health center having the lowest coverage rate of 0.00 percent. This research is a descriptive qualitative research, took place in November 2021 at the Batakte Health Center. The head of the Puskesmas, immunization coordinator, and cadres were the three informants in this study. The stages of data analysis include identification of the research objective group, data management and interpretation, derivation, presentation, and verification or conclusion drawing. The findings reveal that there is no vaccine management officer in the input component, limiting the effectiveness of the immunization program. Planning, targeting, coverage, vaccination needs assessment, organizing, immunization services, vaccine chain management, documentation and reporting, and monitoring and evaluation are all part of the immunization process. The targeting data is not obtained directly from the Batakte Health Centers, but through the Health Office, ensuring that target data is different from the data collected in the field. It is intended that the Batakte Health Center will appoint personnel to oversee the vaccine chain and conduct immunization training at least twice a year in order to improve the quality of immunization services

Abstract

Exclusive breast milk is the process of providing breast milk within 6 months without any drinks or additional food for babies. Breastfeeding is exclusively beneficial in maintaining the health of the baby. Lack of breast milk intake causes unbalanced infant nutrition. The problem is that exclusive breastfeeding in Indonesia and the Oebobo Health Center have not reached the target. This research aims to find out what factors are related to exclusive breastfeeding at the Oebobo Health Center in 2023. This research is an analytical survey, with a cross sectional research design. The population in this study is mothers who have babies 6-7 months at the Oebobo Health Center. The sampenyal collection technique is simple random sampling and the number of sampenyal is 52 people. The data were analyzed using univariate and bivariate with chi square test. The results showed that there was a relationship between knowledge (ρ=0.002), attitude (ρ=0.002), work (ρ=0.003), family support (ρ=0.001), and health worker support (ρ=0.001). Health workers should provide a good explanation regarding exclusive breastfeeding to mothers and mothers' families since the mother checked for pregnancy, increase the frequency of counseling by utilizing various health promotion media, and guidance on breastfeeding counseling. It is hoped that mothers should also visit health facilities more regularly so that they can get information or counseling related to exclusive breastfeeding. Working mothers can seek exclusive breastfeeding by sucking milk before going to work.

Abstract

Tuberculosis (TB) is one of the most dangerous infectious diseases in the world and is among the top ten causes of death globally. This study aims to analyze the implementation of the pulmonary TB control program at the Sikumana Health Center, Kupang City, in 2022. This research employs a descriptive qualitative method through interviews selected based on purposive sampling. The results indicate that in terms of input, human resources and funding are sufficient, but infrastructure remains inadequate. Regarding the process, program planning is conducted by a special team in collaboration with the head of the health center and healthcare workers during a cross-sectoral mini-workshop. The Sikumana Health Center collaborates with sub-district heads, village heads, health cadres, PMOs, and the military (Babinsa) to monitor medication adherence and track TB contact patients. Case detection is carried out actively and passively, with diagnosis conducted through a Molecular Rapid Test (TCM). The treatment follows the 2016 pulmonary TB control guidelines, while recording and reporting activities are done both manually and electronically. The main challenges include treatment dropout due to patient mobility, low public awareness of TB control, limited cross-sectoral involvement, and an increased workload for analysts due to extensive contact tracing. In conclusion, while the program is running well, several obstacles need to be addressed to enhance the effectiveness of pulmonary TB control at the Sikumana Health Center.
