Abstract

Pendekatan lean adalah metode pendekatan sistemik yang digunakan untuk menghilangkanatau meminimalkan waste yang terjadi dalam pelayanan dan meningkatkan efesiensipelayanan di suatu instalasi. Tujuan penelitian ini untuk mengidentifikasi waste dan akarpenyebab waste yang terjadi pada Instalasi Farmasi Rawat Jalan (IFRJ) RSUD Tobelo. Metodepenelitian ini adalah penelitian non-esperimental dengan pendekatan kualitatif secaradeskriptif eksploratif melalui kuesioner, observasi dan wawancara mendalam pada responden.Prosedur penelitian yaitu observasi alur proses pelayanan yang dibuat dalam bentuk valuestream mapping, penyebaran kuisioner value pasien dan waste, melakukan wawancara denganmetode 5 why untuk mengetahui akar penyebab dan waste kritis, serta usulan terhadappemecahan masalah. Hasil penelitian menunjukan bahwa waste kritis berupa waste waitingyang diperoleh nilai sebesar 25%. Akar penyebab dari waste waiting adalah kekurangansumber daya manusia, serta pembagian kerja bagi tenaga kefarmasian belum maksimal,sehingga menyebabkan penumpukan resep obat. Usulan perbaikan waste waiting adalahpenambahan sumber daya manusia serta penerapan SOP dalam pelayanan, selain itu jugamenambah unit computer dan printer. Abstract

Background: Diabetes mellitus (DM) is a chronic degenerative disease whose prevalence continues to increase. Treatment of DM with pharmacological therapy is often expensive and causes side effects. This study explores the effectiveness of black turmeric (Curcuma caesia Roxb.) in reducing complications of diabetic nephropathy by reducing microalbumin and creatinine. The purpose of this study was to analyze the effectiveness of black turmeric rhizome extract (Curcuma caesia Roxb.) in reducing serum creatinine and microalbumin levels in diabetic nephropathy model rats. Methods: This study used an in vivo method with rats induced with diabetes using STZ-NA and testing was carried out for 45 days. Serum creatinine and microalbumin were measured using a spectrophotometer and immunoturbidimetry. Results: This study showed that black turmeric extract at a dose of 400 mg/kg BW had an effectiveness equivalent to the positive control in reducing serum creatinine and microalbumin levels. The decrease in serum creatinine reached 20.10%, while the decrease in microalbumin reached 11.88%. The antioxidant and polyphenol content in black turmeric plays a role in reducing oxidative stress and inhibiting apoptosis in ? cells, which has an impact on reducing blood sugar levels and improving kidney function. Conclusion: Black turmeric extract (Curcuma caesia Roxb.) is effective in reducing serum creatinine and microalbumin levels in diabetic nephropathy model rats, with an optimal dose of 400 mg/kg BW. This study supports the potential of black turmeric as a natural therapeutic agent for diabetic nephropathy complications.

Abstract

Older adults require a service known as Elderly Integrated Service Posts (Posyandu Lansia) to achieve better health status, independence, and to become healthy, active, and productive, as they often face various health issues, including degenerative and cognitive problems at this stage of life. The purpose of this study is to describe the effectiveness of the Elderly Program Services at the Integrated Service Post (Posyandu) Sumber Mulya. The author used a qualitative descriptive approach. The research method employed was qualitative descriptive techniques, with data collection through interviews, observation, and documentation. To gather research data, the researcher interviewed 13 informants as a sample. Data analysis techniques consisted of three stages: data reduction, data display, and conclusion or verification. The findings revealed that the services for the elderly at Posyandu Kurik 6 Sumber Mulya, Merauke District have not been effective when evaluated using three indicators according to Budiani. Regarding the program’s socialization, it has not been conducted regularly and clearly by the organizers. Additionally, the goal achievement has also been ineffective, both in terms of the implementation process and in phases, and monitoring indicators have not been properly conducted by the organizers. Furthermore, there is a lack of concern from volunteers and midwives for the elderly community members who are unable to attend the Posyandu Lansia program.

Abstract

Pharmaceutical services are integrated activities with the aim of identifying, preventing, solving drug problems, and health-related problems. This study aims to determine the quality of pharmacy services at public health center and the factors that influence it. This observational study was conducted by direct observation of research subjects with document review and interviews to explore the supporting and inhibiting factors in the implementation of pharmacy services at the public health centers in Cilacap Regency. The results showed that for the quality of drug management, of the 25 indicators, what was standardized were drug proposals, items suitability with disease patterns, drug storage, drug availability for one month, empty stock items, insufficient drug items, overstock items, and suitability of physical quantities. drug. For clinical pharmacy services, out of 18 indicators, the standards are service time, drug cost per prescription visit, items per prescription, generic preparations, antibiotics for non-specific diarrhea, ORS and zinc administration for diarrhea, antibiotics for non-pneumonia ISPA, and injection use. Factors of human resources, budget availability, status of puskesmas, and type of puskesmas have no effect on the quality of pharmaceutical services.Keywords: pharmacy; service quality; public health center ABSTRAK Pelayanan kefarmasian merupakan kegiatan yang terpadu dengan tujuan untuk mengidentifikasi, mencegah, menyelesaikan masalah obat, dan masalah yang berhubungan dengan kesehatan. Penelitian ini bertujuan untuk mengetahui mutu pelayanan kefarmasian puskesmas dan faktor-faktor yang mempengaruhinya. Penelitian observasional ini dilakukan dengan observasi langsung terhadap subjek penelitian dengan telaah dokumen dan wawancara untuk menggali faktor pendukung dan penghambat pelaksanaan pelayanan kefarmasian Puskesmas di Kabupaten Cilacap. Hasil penelitian menunjukkan bahwa untuk mutu pengelolaan obat, dari 25 indikator, yang sesuai standar adalah pengusulan obat, kesesuaian item dengan pola penyakit, penyimpanan obat, ketersediaan obat satu bulan, item stok kosong, item obat kurang, item stok berlebih, dan kesesuaian jumlah fisik obat. Untuk pelayanan farmasi klinik, dari 18 indikator, yang sesuai standar adalah waktu pelayanan, biaya obat per kunjungan resep, item per resep, sediaan generik, antibiotik pada diare non-spesifik, pemberian oralit dan zink untuk diare, antibiotik pada ISPA non-pneumonia, dan penggunaan injeksi. Faktor sumber daya manusia, ketersediaan anggaran, status puskesmas, dan tipe puskesmas tidak berpengaruh terhadap mutu pelayanan kefarmasian.Kata kunci: kefarmasian; mutu pelayanan; puskesmas

Abstract

Pharmaceutical service quality refers to degree of service perfection that increases patient satisfaction. Pharmacies are required to review several aspects in order to improve quality of pharmaceutical services which include tangibles, empathy, assurance, responsiveness and reliability. The research objective was to determine the correlation between quality of pharmaceutical services and degree of patient satisfaction with pharmaceutical services in pharmacy of X District Surakarta in 2022. This study is a descriptive involved 225 patients and 8 pharmacies in a survey. Pharmaceutical service data is concluded based on quality category of pharmaceutical services in pharmacies. Patient satisfaction was analyzed by comparing the weight of reality and expectations based on variables that affect patient satisfaction. Additionally, it was examined using the SPSS 23 Spearsman's Rho test to see if there was a correlation between caliber of pharmaceutical services and degree of patient satisfaction. In pharmacies of X District Surakarta, the quality of pharmaceutical services received an average score of 63,62% (sufficient) and an average percentage of patient satisfaction levels of 79.55% (satisfied) in 2022. The results of the Spearman's rho correlation test of 0.994 indicate that there is a correlation between quality of pharmaceutical services and level of patient satisfaction. Keywords: pharmaceutical services, patient satisfaction, pharmacy X district, Surakarta Abstrak Mutu pelayanan kefarmasian mengacu pada tingkat kesempurnaan suatu pelayanan untuk meningkatkan kepuasan pasien. Pihak apotek wajib meninjau aspek meliputi bukti fisik, empati, jaminan, daya tanggap dan reliabilitas. Tujuan penelitian ini adalah untuk mengetahui hubungan mutu pelayanan kefarmasian dengan tingkat kepuasan pasien terhadap pelayanan kefarmasian di apotek wilayah Kecamatan X Kota Surakarta tahun 2022. Penelitian ini merupakan penelitian deskriptif dengan metode survei menggunakan kuesioner terhadap 8 apotek dan 225 pasien. Data pelayanan kefarmasian disimpulkan berdasarkan kategori mutu pelayanan kefarmasian di apotek. Kepuasan pasien dianalisis dengan membandingkan bobot realita dan ekspektasi berdasarkan variabel yang mempengaruhi kepuasan pasien. Selanjutnya dianalisis untuk mengetahui ada atau tidaknya hubungan antara mutu pelayanan kefarmasian dengan tingkat kepuasan pasien menggunakan SPSS 23 uji spearsman’s rho. Mutu pelayanan kefarmasian di apotek wilayah Kecamatan X Kota Surakarta tahun 2022 memperoleh skor rata-rata sebesar 63,62% (cukup) dan rata- rata persentase tingkat kepuasan pasien sebesar 79,55 % (puas). Hasil uji korelasi spearman’s rho sebesar 0,994 hal tersebut menunjukkan bahwa terdapat hubungan antara mutu pelayanan kefarmasian dengan tingkat kepuasan pasien. Abstract

Asthma is a chronic respiratory inflammation that limits expiratory air flow. Black cumin reduces the infiltration of inflammatory cells in the respiratory tract comparable to class III antihistamines, cardamom suppresses the formation of a Th2 immune response and has the potential to be therapeutic for allergic asthma. This study aims to determine the profile of black cumin & cardamom essential oils using GC-MS, the effect of the combination of black cumin & cardamom in reducing the thickness of bronchial smooth muscle, bronchiolar epithelium, number of mast cells, and eosinophils in asthmatic mice. This research is an experimental Post-test Only Control Group Design, creating an OVA-induced asthma test animal model of 30 Balb/C mice divided into 6 normal groups (N), asthma control (N-), positive control (P+) and 3 treatment groups with dose variations (K1, K2, K3). Day 7 desensitization OVA 10μg+1 mg Al(OH)3 0.5mL NaCl intraperitoneally, day 14 re-desensitization OVA 20μg+2 mg Al(OH)3 1 mL NaCl. On the 21st, 23rd and 25th days, 1% OVA was inhaled. Days 26 to 39 (K1), (K2), (K3) were given a combination of black cumin oil & cardamom. On the 40th day, histopathology was terminated. The results of the study showed that in 3 groups of dose variations with (P+) the thickness of bronchial smooth muscle, bronchiolar epithelium, mast cells and eosinophils was significantly greater (p>0.05), meaning that there was no statistically significant difference as an anti-asthma, the combination of black cumin & cardamom was proven. reducing the thickness of bronchial smooth muscle, bronchiolar epithelium, number of mast cells and eosinophils in asthmatic mice with optimal doses of a combination of black cumin oil and cardamom oil in a ratio of 75:25 (K2) showed better antiasthmatic potential, compared to K1 (50:50) and K3 (25 :75).

Abstract

Antioksidan alami (berasal dari bahan alami / tanaman) dijadikan alternatif akibat adanya kekhawatiran akan kemungkinan efek samping dari antioksidan sintetik. Salah satu tanaman yang berpotensi sebagai antioksidan alami yaitu kawista (Limonia acidissima L.). Tujuan penelitian ini untuk membandingkan aktivitas antioksidan dari daun kawista dengan proses ekstraksi beberapa pelarut (metanol, etanol 70%, dan etanol 96%). Metode penelitian yang dilakukan meliputi maserasi, identifikasi senyawa, dan pengujian antioksidan dengan metode DPPH. Pengukuran antioksidan menggunakan spektrofotometer UV-Vis dinyatakan dengan nilai IC50 (Inhibitory Concentration 50) yang kemudian dikonversikan ke nilai AAI (Antioxidant Activity Index). Nilai aktivitas antioksidan yang diperoleh paling tinggi dari ekstrak metanol yaitu nilai rata – rata IC50 82,2034 ± 0,5733 dan nilai rata – rata AAI 1,2165 ± 0,00852 artinya mempunyai sifat antioksidan kuat. Aktivitas antioksidan dari ekstrak etanol 70% daun kawista dengan nilai rata – rata IC50 135,5030 ± 7,2238 dan nilai rata – rata AAI 0,7934 ± 0,0382 mempunyai sifat antioksidan sedang. Sedangkan ekstrak etanol 96% dengan nilai rata – rata IC50 123,5900 ± 2,9718 dan nilai rata – rata AAI 0,8094 ± 0,0195 mempunyai sifat antioksidan sedang. Kesimpulannya perbandingan hasil ekstraksi dengan berbagai pelarut menunujukkan aktivitas antioksidan yang berbeda signifikan. Antioksidan tertinggi terdapat pada ekstrak metanol dengan nilai IC50 82,20 ppm dan AAI sebesar 1,22.

Abstract

Black turmeric (Curcuma caesia) influences the wound healing process in male white Wistar rats suffering from diabetes with a focus on the anti-inflammatory content it contains. This study aimed to determine the activity of varying concentrations of black turmeric extract (Curcuma caesia) in healing diabetic wounds in a male white rat model involving 6 treatment groups, namely the no treatment group, the positive control group (bioplacenton gel), the negative control group (Na-CMC), groups varying the dose of black turmeric extract, namely 10%, 15%, 20%. The incision is applied and then smeared with the test preparation for up to 14 days in the morning and evening. Wound healing is measured by monitoring healing time and reduction in wound size. Data were analyzed statistically using the One Way ANOVA method. The study results showed that black turmeric extract could heal diabetic wounds in male white rats with an increase in the concentration of black turmeric, namely black turmeric extract containing 20%.

Abstract

Micro, Small and Medium Enterprises (MSMEs) have a strategic role in the Indonesian economy. In the face of increasingly fierce competition, "Kedai Rasa" is faced with various challenges. One of the main challenges is creating a positive consumer perception of the taste of the product while building strong branding. UMKM "Kedai Rasa" is one of the main targets of Community Service (PKM) activities. This research uses descriptive methods with quantitative and qualitative approaches. Overall, Kedai Rasa has succeeded in meeting customer expectations with taste, innovation and adequate portions. To increase satisfaction, Kedai Rasa can continue to innovate its menu, maintain taste consistency, and adapt its offerings to a wider range of customer preferences.

Abstract

Siklus manajemen obat mencakup empat tahap yaitu: 1) selection (seleksi), 2) procurement (pengadaan), 3) distribution (distribusi), dan 4) use (penggunaan). Tujuan penelitian ini adalah menganalisis tahap seleksi, pengadaan dan distribusi di Instalasi Farmasi Dinas Kesehatan Kota Surakarta tahun 2021 agar manajemen pengelolaan obat yang dikelola tidak hanya efektif dan efisien, tetapi juga tepat dan baik yang berkelanjutan sesuai standar Kemenkes RI. Penelitian ini menggunakan rancangan deskriptif non eksperimental dan analisis data deskriptif dengan indikator Kemenkes RI dan JICA 2010. Data dikumpulkan secara retrospektif berupa data sekunder. Pengambilan berupa data rekap laporan persediaan obat, data pengadaan obat, data obat Expired date dan data Lembar Pemakaian dan Lembar Permintaan Obat (LPLPO) tahun 2021. Pengelolaan obat berdasarkan indikator Kemenkes RI dan JICA ialah kesesuaian item obat yang tersedia dengan DOEN sebesar 78%; ketepatan perencanaan obat sebesar 466%; ketersediaan obat sesuai dengan kebutuhan sebesar 90%; pengadaan obat generik sebesar 95%; persentase dan nilai obat kedaluwarsa sebesar 15,3%; persentase dan nilai obat rusak sebesar 0%; ketepatan distribusi obat sebesar 88,2%; tingkat ketersediaan obat sebesar 100%; dan rata-rata waktu kekosongan obat sebesar 0%. Kesimpulan penelitian ini adalah pengelolaan obat di Instalasi Farmasi Dinas Kesehatan Kota Surakarta tahun 2021 pada tahap seleksi dan perencanaan menggunakan metode konsumsi. Tahap pengadaan menggunakan cara e-katalog dan katalog dengan menggunakan metode e-purchasing, sedangkan obat yang tidak ada di katalog dapat dipesan langsung ke PBF dan dengan cara Tender. sedangkan tahap distribusi menggunakan metode bottom-up. Pengelolaan obat pada tahap seleksi di Instalasi Farmasi Dinas Kesehatan Surakarta belum sesuai standar, sedangkan pengelolaan obat pada tahap pengadaan dan distribusi telah sesuai standar berdasarkan indikator pengelolaan obat Kemenkes RI dan JICA 2010.
